당뇨병환자의 메뉴개발 요구도 및 보리이용 당뇨식단 개발 (Need for Development of a List of Meals for Diabetic Patients and Development of Barley-Based Diabetic Meals)

  • The aim of this study was to develop and apply a list of meals and standard recipes using barley for diabetic patients. The degree of interest and requirements of diabetic patients were investigated for the development of meals. The ingredients of the meals were selected through the diabetic literature and previous research. While developing a list of meals, dietetic therapies for diabetic patients were considered. After developing 15 kinds of meals and modifying them through sensory evaluation, a standard recipe was completed. In the standard recipe, the menu name, the ingredients, quantity, and recipe were stated and the nutritional components were indicated. Photographs of the meals were included. The calorie prescription for the diabetic patients was aimed at elderly women, that is, those 65 years old or above, based on research showing this to be the average age of diabetics. The prescribed calories were 1,500 kcal based on the food exchange list. Weekly lists of meals including the developed dishes were made for diabetic patients. The list were modified after consultation with a clinical nutritionist. When completed, one meal item was selected for each day and cooked. A photograph was taken and presented diabetic patients. This article presents the standardized recipes of the developed list of meals and applies them to modifying the diabetic diet, with an aim to be of service to diabetic patients attempting to meet their dietetic therapy goals. We also provide basic data on institutional food services for diabetic patients and nutrition education.

당뇨병성 족부궤양의 재발과 연관된 위험인자: 심리사회적 위험인자를 포함한 후향적 연구 (The Risk Factors Associated with Foot Re-Ulceration in Diabetes: A Retrospective Study Including Psychosocial Risk Factors)

  • Purpose: To evaluate several risk factors related to re-ulceration of diabetic foot including psychosocial aspects such as familial support and degree of independence of patients' activity. Materials and Methods: We reviewed medical records and performed telephone interview with eighty-five patients who had a history of hospitalization in our hospital due to diabetic foot ulceration from year 2002 to 2010. Based on the collected data, we analyzed several factors such as age, gender, prevalence duration, accompanying diseases, HbA1c level, degree of independence and familial support. Results: The mean age was 61.4 years and most common in the 4th decade. There were 57 cases (67%) of recurrence, predominance of male. Eleven patients with recurrent diabetic foot ulceration had undergone major amputations. Psychosocial problems such as depression, insufficient familial support and mortality were more frequently observed in recurrent group. Conclusion: This study shows that psychosocial factor such as familial support for patient with diabetic foot could be important to reduce the recurrence rate of diabetic foot ulceration. Therefore, we should pay attention to strategic plans for prevention, screening, treatment, and aftercare through the prospective studies including psychosocial risk factor in diabetic foot ulceration.

A Study on Eating Habits, Life Styles and Nutrition Care of Diabetic Outpatients

  • The purpose of this survey was to investigate eating habits, life styles and nutritional care of diabetic outpatients and to provide basic data for developing individualized nutritional care and diabetic education programs. This survey was carried out by nutritional counseling with a questionnaire and checking medical record. Information about the general characteristics of the subjects, eating habits, health-related life styles and attitude and perception of subjects to diet therapy was gathered from 200 randomly-sampled diabetic outpatients at a University Hospital located in Inchon. All data were analyzed by Statistical Analysis System(SAS) software. The results are summarized as follows : Female subjects were 63.5% of total subjects and 65.5% of total subjects were 50 years or more. The average Body Mas Index(BMI) of male and female subjects were 23.06kg/㎡ and 25.02kg/㎡ respectively and 44% of all subjects wee obese. Among subjects, type II diabetic patients were 81.0% and 82.5% of subjects had suffered from diabetes for more than one year. Also 41% subjects had diabetic history in their family. More than half of the subjects had nutrition education concerning diabetes. Also 75.5% of them thought that nutrition education was effective. The most important guideline in diet therapy was to eat cooked rice with dietary fiber-rich grains. Therefore, it might be necessary to develop nutrition education program adjusted according to diabetic patient's needs and life styles, which may increase feasibility of self-care and implementation of management guidelines.

  • Objective: A Case Report on Control of Diabetes Using Nutritional Therapy Methods: The patient is a Korean woman aged 58 years. The patient has a history of cervical cancer surgery and is a diabetic patient. Results: Diabetes improved following nutritional therapy. Conclusion: Nutritional therapy can be beneficial in the treatment of diabetes.

만성 질병환자를 위한 CDSS를 적용한 PHR 시스템 (CDSS enabled PHR system for chronic disease patients)

  • With the advance of Information Technology (IT) and dynamic requirements, diverse application services have been provided for end users. With huge volume of these services and information, users are required to acquire customized services that provide personalized information and decision at particular extent of time. The case is more appealing in healthcare, where patients wish to have access to their medical record where they have control and provided with recommendation on the medical information. PHR (Personal Health Record) is most prevailing initiative that gives secure access on patient record at anytime and anywhere. PHR should also incorporate decision support to help patients in self-management of their diseases. Available PHR system incorporates basic recommendations based on patient routine data. We have proposed decision support service called "Smart CDSS" that provides recommendations on PHR data for diabetic patients. Smart CDSS follows HL7 vMR (Virtual Medical Record) to help in integration with diverse application including PHR. PHR shares patient data with Smart CDSS through standard interfaces that pass through Adaptability Engine (AE). AE transforms the PHR CCR/CCD (Continuity of Care Record/Document) into standard HL7 vMR format. Smart CDSS produces recommendation on PHR datasets based on diabetic knowledge base represented in shareable HL7 Arden Syntax format. The Smart CDSS service is deployed on public cloud over MS Azure environment and PHR is maintaining on private cloud. The system has been evaluated for recommendation for 100 diabetic patients from Saint's Mary Hospital. The recommendations were compared with physicians' guidelines which complement the self-management of the patient.

Current perception threshold in diabetic sensory polyneuropathy with normal routine nerve conduction study

  • Background: Routine nerve conduction study (NCS) can only be used to evaluate the function of large fibers, and the results of NCS are often normal in patients with distal sensory polyneuropathy. The measurement of the current perception threshold (CPT) has been reported to represent a variety of peripheral nerve fiber functions. This study was performed to investigate the value of measuring CPT in patients with diabetic sensory polyneuropathy who have no abnormalities in routine NCS. Methods: Twenty-seven diabetic patients with sensory polyneuropathy and normal routine NCS and 18 age-matched control subjects participated in this study. The CPT was measured on the unilateral index finger and great toe of each subject at frequencies of 5 Hz, 250 Hz, and 2,000 Hz. Results: CPT values were significantly higher in the patient group than in the control group, especially with stimuli at the lowest frequency of 5 Hz (p < 0.05). There were significant correlations between the CPT values obtained at three different frequencies in the patient group, whereas the correlation was only significant in the pair of 250 Hz/5 Hz (both in the hands and feet), and in the pair of 2,000 Hz/250 Hz (in the feet) for the control group. Conclusions: Our data suggest that the CPT test, especially at a stimuli frequency of 5 Hz, may be a useful screening tool for diabetic polyneuropathy in patients who show no abnormalities in routine NCS.

간호대학생의 당뇨환자 시뮬레이션기반 간호교육의 효과 (The Effects of Simulation-based Nursing Education on Diabetic Patient in Nursing Students)

  • 본 연구는 당뇨환자의 시뮬레이션기반 간호교육이 간호대학생의 당뇨지식, 임상수행능력, 실습 만족도 및 의사소통능력에 미치는 효과를 확인하기 위해 시도되었다. 이 연구는 시뮬레이션 교육 전과 후에 당뇨지식, 임상수행능력, 실습만족도 및 의사소통능력을 파악하기 위해 2013년 10월 21부터 11월 1까지 자료수집을 하였으며, 간호대학생 3학년 75명이 연구대상자로 참여하였다. 연구의 결과는 시뮬레이션 교육을 받은 후 교육 전에 비해 당뇨지식(t=-9.684, p<.001), 임상수행능력(t=-4.173, p<.001), 의사소통 능력(t=-7.685, p<.001) 점수가 높게 나타나 통계적으로 유의하게 나타났으나, 실습만족도(t=-1.144, p=.256)의 변화는 통계적으로 유의하지 않았다. 이상의 연구결과를 토대로 시뮬레이션기반 교육은 대상자의 간호지식, 임상수행능력과 의사소통능력을 강화할 수 있으며, 다양한 교육과정에도 적용될 수 있을 것이다.

당뇨병성 족부질환자의 직접의료비용 분석 (Analysis of Direct Service Costs about Diabetic Foot Patients)

  • Purpose: The objective of this study was to analyze diabetic foot patients' direct service costs until the cure of the disease. Materials Methods: The subjects of this study were randomly sampled 60 patients who had been treated for diabetic foot at one of two tertiary hospitals and cured of the disease during from January 2008 to December 2009, and whose diagnostic code was E11.5 or E14.5. Data were collected from medical records and direct service costs were analyzed using data on the payments of individual service charges. Direct service costs spent at other medical institutions for the same disease were excluded. Collected data were analyzed using descriptive statistics. Results: The subjects' mean hospital stay was 29 days, and mean period until cure was 132 days. The inpatient cost per patient was 10,844,648 won, outpatient cost was 715,751 won, and home care services cost was 641,854 won, so total direct service cost per patient was 11,913,419 won. The total direct service cost in patients who had their foot amputated was 12,769,822 won, 1.3 times higher than without amputation, who had vascular intervention was 16,219,477 won, 1.9 times higher than non-vascular intervention, who had both infection and artery occlusion was 17,522,435 won, 2.0 times higher than either infection or artery occlusion. Conclusion: In diabetic foot patients, the direct service cost was highest as 17,522,435 won in patients accompanied with both infection and occlusion of lower extremity artery.

혈액은행 제조의 혈소판 농축액을 이용한 당뇨족부궤양의 치료 경험 (Clinical Experience in Treatment of Diabetic Foot Ulcers Using Platelet Concentrates from Blood Bank)

  • Many clinical trials have shown the effectiveness of the platelet releasate or the platelet gel on chronic wounds. However, the patient's own blood had to be aspirated and processed to make the platelet releasate or a platelet gel. The purpose of this study was to assess the effects of platelet concentrates from the blood bank for the treatment of diabetic foot ulcers. To obtain the basic data of the PDGF-BB content in platelet concentrates supplied from the blood bank, enzyme-linked immunosorbent assay quantification was performed. On average, 8.5 pg of the PDGF-BB was released per 1 million platelets. Sixteen patients with diabetic foot ulcers ranging from 1.0 to $18.0cm^2$(mean, $6.1cm^2$) in size were treated. The platelet concentrates was centrifuged and the precipitantte was mixed with 1 ml of fibrinogen. The platelets and fibrinogen mixture was dispersed on to the ulcer lesions. The liquid platelet and fibrinogen mixture was then sealed using 0.3-1.0 ml of thrombin and moisture dressing was performed. The procedure was repeated every one or two weeks until wound closure. Time required for complete healing ranged from 3 to 12 weeks after treatment (mean, 7.3 weeks). Patient satisfaction was also very positive. In this study, the use of platelet concentrates from the blood bank was found to be effective in treating diabetic foot ulcers.

봉약침 치료를 중심으로 한 한방 복합치료로 호전된 당뇨병성 신경병증 환자 1례 (A Case Report on Diabetic Neuropathy Treated with Bee Venom Acupuncture and Combined Korean Medicine)

  • Objective: This study investigated the effectiveness of bee venom acupuncture for a diabetic neuropathy patient. Methods: A diabetic neuropathy patient received twenty-one bee venom acupuncture treatments for a total of 99 days. The Neuropathy Pain Scale (NPS) was used to evaluate the weekly peak pain during the treatment period. Results: After the treatments, the NPS scores improved, and the frequencies of peak pain decreased. Conclusions: This clinical case indicates that bee venom acupuncture combined with Korean medicine may be effective for treating diabetic neuropathy patients.
