The majority of people diagnosed with diabetes mellitus are in the working age group in developing countries. The interrelationship of diabetes and work, that is, diabetes affecting work and work affecting diabetes, becomes an important issue for these people. Therapeutic options for the diabetic worker have been developed, and currently include various insulins, insulin sensitizers and secretagogues, incretin mimetics and enhancers, and alpha glucosidase inhibitors. Hypoglycemia and hypoglycaemic unawareness are important and unwanted treatment side effects. The risk they pose with respect to cognitive impairment can have safety implications. The understanding of the therapeutic options in the management of diabetic workers, blood glucose awareness training, and self-monitoring blood glucose will help to mitigate this risk. Employment decisions must also take into account the extent to which the jobs performed by the worker are safety sensitive. A risk assessment matrix, based on the extent to which a job is considered safety sensitive and based on the severity of the hypoglycaemia, may assist in determining one's fitness to work. Support at the workplace, such as a provision of healthy food options and arrangements for affected workers will be helpful for such workers. Arrangements include permission to carry and consume emergency sugar, flexible meal times, selfmonitoring blood glucose when required, storage/disposal facilities for medicine such as insulin and needles, time off for medical appointments, and structured self-help programs.
Purpose: The purpose of this study was to assess the difference in knowledge and learning needs of the coronary artery disease according to the general characteristics of the patients with type 2 diabetes mellitus. Method: The participants were 188 patients who had diabetes mellitus. Data were obtained between January and April 2006 by asking the knowledge and learning needs of the coronary artery disease. Data were analyzed using SAS program. Results: Participants' knowledge level was high in the cause and prevention, but the level was low in the symptom and occurrence of pain. Learning need for "Influence of smoking on heart disease" and "The structure and functions of the heart" was great, but learning need for "Complete diagnosis" and "Management of pain and pressure on the sternum" was little. No significant differences were found in the knowledge level of the coronary artery disease according to the general characteristics. Learning needs were greater in participants under 60 years of age (p=0.011) and in those with low education level (p=0.049). There was a significant correlation between knowledge and learning needs of the coronary artery disease (p=0.003). Conclusion: In planing the education programs, the general characteristics of the patients should be considered.
Purpose: The purpose of this study was to investigate the sexual dysfunction of diabetic women in order to provide basic data contributed in nursing intervention. Method: A convenience sample consisted of 67 subjects with diabetes who attended outpatient department of 2 university hospitals. After verbal consent was obtained, subjects were asked to complete a questionnaire including BISF-W developed by Taylor et al. Data were collected from April 27 to May 22, 2006, and were analyzed by descriptive statistics, t-test, oneway ANOVA using SPSS 12.0 program. Results: This study discovered that the degree of sexual dysfunction of women with diabetes was high, and in particular that most of the women with diabetes did not have satisfactory sex life because of lack of vaginal lubrication, lower sexual desire, lower orgasm, and sexual pain. The test of differences in sexual dysfunction according to demographic and disease-related characteristics revealed that significant differences existed only with regard to menopause and age. Conclusion: As the sexual function of women varies widely among individuals, we would like to emphasize the importance of sexual counseling and education programs to improve the quality of life of diabetic women to prevent or relieve their sexual dysfunction.
Purpose: To investigate the level of depression and self management behavior in patients with diabetes. Method: From May to August 2004, 124 diabetic patients 18 years or older were surveyed using the Korean version of the BDI, the self-management scale by Toobert(2000) and hospital lab data for HbAl, FBS and Blood Sugar-2hr. Results: 1. Mean score on the BDI for the patients was 10.85 points. The score on the BDI was significantly higher in hyperglycemic patients than in normoglycemic patients. 2. The level of self management for the past week showed care on an average of 4.2 days. According to level of depression, patients in the normal group and patients in the mild group had significantly higher levels of self management than those in the moderate group. 3. According to level of depression, patients in the normal group had significantly lower levels of blood glucose than those in the moderate group, and for HbAlc and FBS those in the mild group had lower levels compared to those in the moderate group. 4. The factors significantly associated with level of self management were depression (25.66%) and duration of illness. Conclusion: Programs should be developed to combat depression in patients with diabetes in order to increase compliance through self care management and thus increase of blood glucose levels.
Purpose: The purpose of the present study is to examine the factors influencing intention of dementia prevention behavior in adults with hypertension or diabetes. Methods: A total of 113 community-dwelling adults with hypertension or diabetes agreed to participate in this study. Data were analyzed using descriptive statistics, t-test, ANOVA, correlation, and multiple regression. Results: Self-efficacy and cues to action toward dementia prevention behavior were factors influencing intention of dementia prevention behavior of participations. Self-efficacy toward dementia prevention behavior was the most influential factor. These factors accounted for 22% of intention of dementia prevention behavior. Conclusion: These results suggest a need for strengthening self-efficacy to increase intention of dementia preventive behaviors. And dementia prevention programs should be developed in consideration of strategies to reinforce cues to action.
Purpose: This study aimed to investigate the prevalence and risk factors of diabetic retinopathy (DR) in people with diabetes mellitus (DM) using Korean National Health and Nutrition Examination Survey VII (2017~2018). Methods: DM was defined as in two ways; 1) doctor's diagnosis (Group 1, n=549), 2) one of doctor's diagnosis, medication, or hyperglycemia (Group 2, n=849). The DR prevalence was measured as the prevalence proportion (%). Risk factors for developing DR were analyzed using multiple logistic regression, and odds ratios (ORs) and 95% confidence intervals (CIs) were calculated. Results: The prevalence of DR was 25.87% in Group 1 and 20.14% in Group 2. Risk factors for DR were identified as insulin therapy (Group 1: OR=5.31, Group 2: OR=5.27), DM duration ≥10 years (Group 1: OR=2.20, Group 2: OR=3.10), and systolic blood pressure ≥140 mmHg (Group 1: OR=2.26, Group 2: OR=2.23) for both groups. Conclusion: Considering the DR prevalence, eye examinations education is highly recommended as part of a diabetes management programs in the community. It is also proposed to shorten the eye examination cycle for people with risk factors and establish a referral system to link between screening to treatment.
This study was performed to investigate the conditions of physical activity and exercise programs in Public Health Center. For this study 244 Public Health Centers in Seoul and all of the countries were surveyed from October 15 1999 to June 30 2000. The study was done by telephone, e-mail, fax and questionnaire. First, the 25 Public Health Centers in Seoul were investigated, and then 107 Public Health Centers in other cities were investigated. Public Health Centers in Seoul and Public Health Centers in other cities are very different in exercise program and equipment, budgets, personnel. There were many kinds of exercise program for people who have health problems, but a few exercise programs for healthy people. Exercise programs for people who have health problems were to prevent hypertension, arthritis. obesity, diabetes and back pain. Physical activity and exercise programs for healthy people were stretching for pregnancy and elderly. There were $24(96.0\%)$ Public Health Centers in Seoul which had physical activity and exercise program, $80(36.5\%)$ Public Health Centers in other cities which had physical activity and exercise program.
Objectives: This study performed to enhance the competency of health education specialist on population-based program to prevent chronic disease in public health organizations. In addition, it will provide the basic data to enhance specialized competency for health education specialist. Method: The current operating strategy and demanded competency related to chronic disease programs were analyzed by reviewing the Korean Health Plan 2020, the guideline of integrated health promotion programs, and the job description of health education specialist. Results: Preventive programs for chronic disease provide with healthy lifestyle programs in integrated health promotion service. First, health education specialist should learn to professional knowledge on health risk factor and chronic disease. Second, they should cultivate the integrated competency to manage operations on obesity, hypertension, diabetes, cardiovascular disease, and cancer prevention programs. Third, they get strengthened the-job training to implement health education, public relations, campaigns, media advocacy, and utilization of multi-media. Conclusion: It should implement the preventive programs for chronic disease in various health promotion services through coordination with multiple sectors. To identify of the job in preventive program for that, it would be required empowerment of health education specialists on disease prevention science and practical life skill.
본 연구는 제2형 당뇨병 환자의 자기관리지식, 스트레스, 당뇨관리 자기효능감의 정도와 상관관계를 파악하고, 당뇨관리 자기효능감의 영향을 미치는 요인을 설명하는 서술적 조사연구이다. 본 연구의 참가자는 2015년 9월 17일부터 10월 15일까지 국내 K시 소재 1차 의료기관을 방문한 당뇨병 환자 150명으로 구성되었다. 데이터는 기술 통계량, t-검정, 분산 분석, Pearson 상관 계수 및 SPSS 18.0의 다중 회귀 분석을 사용하여 분석되었다. 일반적 특성에 따른 당뇨관리 자기효능감에서는 연령과 교육에서 유의한 차이가 나타났다. 자기관리지식과 당뇨관리 자기효능감은 양의 상관관계가 있는 것으로 나타났다. 당뇨관리 자기효능감과 스트레스, 자기관리지식과 스트레스는 음의 상관관계가 있는 것으로 나타났다. 당뇨관리 자기효능감에 영향을 미치는 중요한 요인은 스트레스와 자기관리지식이었다. 본 연구결과 자기관리지식을 향상시키고 제2형 당뇨병 환자의 스트레스를 줄이기 위한 적절한 당뇨관리 자기효능감 프로그램이 제공되어야 한다는 것을 시사한다. 이를 통해 당뇨관리 자기효능감 증진을 위한 효과적인 교육과 중재 개발의 기초자료를 제공하기 위함이다.
Purpose: The purpose of this study was to examine the mediating effect of regimen distress in the relationship between medication adherence and glycemic control in men with type 2 diabetes. Methods: A total of 116 patients with type 2 diabetes was recruited for the cross-sectional survey design. Data were analyzed using descriptive statistics, independent t-test, ANOVA, Pearson's correlation coefficient, and multiple regression using Baron and Kenny steps for mediation. Results: The mean score for medication adherence was 6.32, and the mean glycemic control level (HbA1c) was 7.47%. The mean score was 2.37 for regimen distress. There were significant correlations among medication adherence, regimen distress, and HbA1c. Regimen distress had a partial mediating effect (${\beta}$=.22, p=.005) in the relationship between medication adherence and HbA1c (Sobel test: Z=2.47, p=.013). Conclusion: Regimen distress was found to be associated with glycemic control in men with type 2 diabetes. Based on the findings of this study, nursing intervention programs focusing on decreasing regimen distress are highly recommended to improve level of glycemic control in patients with type 2 diabetes.
