전산 시스템이 도입된 일개 종합병원 간호단위의 간호시간 산정에 관한 연구 (A Study on the Nursing Time in Nursing Units in Hospital to Applied Computer System)

  • This study was done to determine direct and in-direct nursing time in nursing units in hospital to ap-plied computer system and expect that those results contribute to measure efficiency of nursing practice and planning of nursing manpower. The design for study was a descriptive study. The study results are as follows. 1. Patients admitted to the Internal Medicine and General Surgery units were grouped into four. Group Ⅰ, 32.9% of the total patients, consisted patients whose condition was considered minor Group Ⅱ, 26.1%, was of those whose condition was considered moderate, Group Ⅲ, 41. 8%, moderate severe and Group Ⅳ, 29.2% the most severe. 2. Nursing intervention times by care type were as follows four minutes spent for suction eight minutes, for simple position change, ten minutes, for sheet change, seven minutes for a hot or cool compress, six minutes for dressing change, four minutes for Ⅰ.M. injection, six minutes for patient health education and five minutes for body temperature check. 3. Direct care time by patient group revealed the following : Group Ⅰ rquired 191.4 minutes, Group Ⅱ required 331.1 minutes, Group Ⅲ rquired 499.4 minutes, and Group Ⅳ rquired 1328.0 minutes. 4. The ratio of time for adequate nursing care and direct care time in the Internal Medicine and Goneral Surgery units was 67.4%-83.4% and 94.7%-99.3% in the Intensie Care Unit. 5. Average daily direct care time per patient was 5.5 hours in the Internal Medicine unit and 11.5 hours in the Intensive Care Unit. 6. Time spent in indirect care was 48.3 minutes for computer recording,34.8 minutes for giving and receiving patient information for shift duty, 28.0 minutes for eating and resting time, 26.6 minutes for transfering and identifying patients, 25.6 minutes for identifying Doctor's order, 23.9 minutes for recording vital signs. 7. Time spent in indirect care was 282.2 minutes by head nurses (charge nurses), 258.7 minutes by nurses and 261.6 minutes by nurse aids. 8. The average nurse's workload was 9.3 hours and daily indirect nursing time required 46.3%-50. 5% of above mentioned workload time. .9. The average daily indirect care time per patient was expected to be 57.7 minutes in the Internal Medicine unit and 3.3 hours in the Intensive Care Unit.

  • 인터넷은 더 이상 특정그룹만을 위한 네트워크가 아닌 일상적인 ?에서 이루어지는 광범위한 통신수단이 되었다. 사람들은 이메일을 사용해서 사적인 메시지를 교환하고 학생들은 웹을 통해서 교육적인 도움을 받으며, 사이버 쇼핑몰을 통해 다양한 상품을 구입하기도 한다. 또한 여러 기업들은 인터넷 상에서의 업무를 실시하고 있다. 최근 들어, 이 같은 인터넷 트래픽의 폭발적인 성장은 트래픽의 양, 트래픽의 특성, 다양한 서비스 성능의 필수 요건 등의 관점에서 계속적으로 변화하는 사용자들의 요구를 수용하는 방법에 큰 관심을 올렸다. 망 자원의 Over provisioning 방식은 단순한 해결책이 될 수 있지만 너무 비싸고 비효율적인 단점을 가진다. 따라서, 이 당면한 난제를 해결하기 위해 많은 신기술이 최근 소개되고 있다. 그러나 어떤 특정 한가지의 해결책은 불충분하므로 다양한 해결방안을 신중하게 고려한 통합 솔루션이 요구되어 진다. 본 논문에서는 이 같은 문제에 대한 해결책으로써 policy 기반 인터넷 QoS provisioning, 트래픽 엔지니어링과 성능관리 시스템을 통합한 해결책을 제안한다. 본 해결책은 신속하고 자동화된 품질 서비스 provisioning 기능, 실제 망의 성능 정보들에 기반한 보다 현실적인 서비스와 망 자원의 정책 제어 기능 및 이기종의 다양한 IP 망의 중앙 집중식 트래픽 엔지니어링 기능을 제공한다.

고유동 일반강도 콘크리트의 요변성 부여에 따른 거푸집 누출 저감 성능 분석 (Assessment of Formwork-Seepage Minimization in High Fluidity, Normal Strength Concrete Utilizing Thixotropic Properties)

  • 본 연구의 목적은 고유동성이 부여된 일반강도 배합 콘크리트에 대하여 요변성을 부여하여 시공정밀도가 낮은 거푸집 틈새를 통해 누출될 수 있는 모르타르나 시멘트 페이스트의 양을 저감하는 것이다. 본 연구에서는 콘크리트에 요변성을 부여하기 위해 PVA와 붕사를 사용하였다. 이에 PVA와 붕사를 사용하는 방법에 대하여 각각의 수용액을 추가하고 치환하는 방법에 따라 요변성을 포함하는 콘크리트의 성능의 변화와 거푸집의 틈새를 모사한 누출 저감 성능을 평가하였다. 본 연구에서 진행된 실험의 조건에서는 PVA와 붕사를 추가하는 방법보다는 치환하는 방법이 추가적인 물의 사용을 억제하여 재료분리 저항성과 응결 지연에서 더 양호한 콘크리트 물성을 나타내었으며 요변성의 발현 및 거푸집 누출 방지에도 효과적인 것으로 나타났다. 특히, PVA 수용액 6%를 치환하였을 때 의미 있는 수준의 거푸집 누출 감소 효과를 얻을 수 있었다. 이를 통해 PVA와 붕사를 이용하여 콘크리트 배합에 대해 요변성을 부여할 수 있다고 판단하였다. 또한 거푸집의 시공정밀도가 다소 낮은 경우에도 PVA와 붕사를 사용하여 콘크리트에 요변성을 부여함으로써 시공 품질을 확보할 수 있을 것으로 기대한다.

요양병원 간호사의 간호행위 위임정도, 간호전문직관 및 이직의도와의 관계 (A study on delegation level of nursing activities, nursing professionalism and turnover intention for long-term care hospital nurses)

  • 본 연구는 요양병원 간호사의 간호행위 위임정도, 간호전문직관 및 이직의도를 파악하고 이들간의 관계를 확인함으로써 향후 간호행위 위임에 필요한 정책적 지침 및 절차에 관한 기초자료를 제공하고자 시도되었다. 본 연구는 서술적 조사연구로 구조화된 설문지를 이용하여 2개 광역도 소재 6개 요양병원의 간호사 146명을 대상으로 실시하였으며, 자료수집기간은 2016년 7월부터 8월까지이었다. 수집된 자료는 SPSS 22.0을 이용하여 기술통계, t-test, ANOVA, Pearson 상관분석을 실시하였다. 연구결과, 요양병원 간호사의 간호행위는 위임에 대한 명확한 지침이나 법적 절차가 없는 상태에서 7개 행위를 제외한 대부분의 간호행위가 간호보조인력에게 위임되고 있었다. 요양병원 간호사의 간호행위 위임정도와 간호전문직관, 이직의도와의 유의한 관련성은 없었으나 간호전문직관과 이직의도는 유의한 음의 상관관계를 보였다. 따라서, 요양병원 간호사의 이직의도를 낮추기 위해서는 개인적인 측면으로 긍정적인 간호전문직관을 확고히 하는 방안이 모색되어야 하며, 정책적인 측면에서는 요양병원의 적절한 간호인력배치 기준을 마련하여 간호행위 위임지침을 개발해야 한다.

Blended Learning(BL) 전략을 활용한 실천적 문제 중심 가정과 교수 학습 과정안 개발 - '청소년과 소비생활' 단원을 중심으로 - (Development of Practical Problem-Based Home Economics Teaching.Learning Process Plans by Blended Learning Strategy - Focusing on a Unit 'the Youth and Consumer Life' -)

  • 본 연구의 목적은 중학교 2학년 기술 가정 교과의 '청소년과 소비생활' 단원을 중심으로 Blended Learning(BL: 혼합형 학습)전략을 활용한 실천적 문제 중심 가정과 수업을 위한 교수 학습 과정안을 개발하고 평가하는데 있다. 이러한 연구목적을 달성하기 위하여 ADDIE 교수설계 모형에 따라 분석(Analysis) 단계, 설계(Design) 및 개발(Development) 단계, 실행(Implement) 단계, 평가(Evaluation) 단계를 수행하였다. 분석 단계에서는 청소년과 소비생활에 관련된 선행연구와 문헌, 제7차 기술 가정 교육과정에서 제시하고 있는 교과 교육의 목표, 기술 가정 10종 교과서의 '소비자 교육' 관련 단원의 내용, 소비자 교육 내용에 대한 요구도 및 지식 정보화 사회에서 요구되는 소비자 교육 내용을 분석하여 청소년 소비자 특성을 파악하고 중학교 2학년 소비자 교육에 적합한 학습 목표와 내용을 추출하였다. 설계 및 개발 단계에서는 분석 결과를 토대로 '청소년과 소비생활' 단원을 실천적 문제 중심으로 재구성하고, BL전략을 활용한 실천적 문제 중심 수업의 구체적인 진행 과정을 설계하였다. 에듀넷을 이용하여 수업에 필요한 온라인 학습 환경으로 "BL전략을 활용한 실천적 문제 중심 가정과 수업( http://community.edunet4u.net/${\sim}$consumer2)" 홈페이지를 개설하였다. 그리고 BL전략을 활용한 실천적 문제 중심 수업을 위한 총 8차시의 교수 학습 과정안을 개발하였으며, 수업에 활용할 수 있는 학생용 학습 활동지 및 교사용 자료를 개발하였다. 실행 단계에서는 개발된 8차시 교수 학습 과정안 중에서 소단위 실천적 문제 '현명한 소비자가 되기 위해 나는 무엇을 준비해야 하는가?'의 1차시 교수 학습 과정안을 학교 수업에 적용하였다. 적용 대상은 충청북도 청주시 소재의 중학교 2학년 두 개 학급과 대전광역시 소재의 중학교 2학년 한 개 학급, 총115명을 대상으로 하였다. 이 수업은 2주 동안 두 차례의 온라인 수업과 세 차례의 오프라인 수업으로 실행하였다. 두 차례의 온라인 수업, 즉 교실에서 행해진 수업의 첫번째에는 이 수업에 대한 전체적인 소개를 하는 예비수업을 하였고 두 번째에는 1차시에 해당하는 본시 수업을 하였다. 컴퓨터로 이루어지는 온라인 학습은 1차시의 본시 수업에 해당하는 내용으로 세 차례(수업 전의 학습 활동지 작성, 수업 후의 행동계획 작성, 실천 후 성찰일기 작성) 진행하였다. 비록 온라인 학습을 세 차례나 하였으나 이 학습이 1차시 본시 수업에 해당하는 내용이고 온라인 학습을 차시로 규정하는 것이 애매하기에 차시의 합계에 포함시키지 않았다. 평가 단계에서는 모든 수업이 끝나고 수업에 참여한 학습자를 대상으로 수업에 대한 전반적인 의견과 수업의 효과, 수업에 대한 제언점 등에 대하여 자유응답식 개방형 질문지로 물어 분석하였다. 본 연구 결과를 교수 학습 과정안 개발과 이 수업에 대한 학생들의 평가 측면으로 살펴보면 다음과 같다. 첫째, 본 교수 학습 과정안은 온라인 학습과 오프라인 교실 수업을 연계하고 다양한 학습 매체와 학습방법을 혼합하는 BL전략을 가정과의 실천적 문제 중심 수업에 도입하여 구체적인 수업 진행 과정을 설계하고, '청소년과 소비생활'단원을 실천적 문제로 재구성하여 개발된 것이 특징이다. 특히, 이 교수 학습 과정안을 개발하는 과정에서 수업 전 온라인 학습, 오프라인 본시수업, 수업 후 온라인 학습으로 진행되는 BL전략의 도입으로 교실 현장에서 실천적 문제 중심 수업으로 수업을 진행하였을 때의 한계점인 시간이나 시수 부족의 문제, 참고할 학습 자료 부족의 문제, 많은 학생 수와 공간부족으로 인한 상호작용 부족의 문제 등을 해결하는데 역점을 두었다. 이 교수 학습 과정안의 내용은 중학교 2학년의 '청소년과 소비생활' 단원명을 '나와 가족과 사회의 구성원으로서 바람직한 소비생활을 위해 나는 무엇을 해야 하는가?'라는 큰 단위의 실천적 문제를 해결하기 위해서 4개의 작은 단위의 실천적 문제, 즉 '현명한 소비자가 되기 위해 나는 무엇을 해야 하는가?' '합리적으로 물품과 서비스를 구매하는 태도를 기르기 위해 나는 무엇을 해야 하는가?' '소비자의 권리와 책임을 알고 소비 문제를 적극적으로 해결하기 위해 나는 무엇을 해야 하는가?' '환경을 생각하는 지속가능한 소비생활을 위해 나는 무엇을 해야 하는가?'로 정한 후에 총 8차시의 분량을 개발하였다. 학습 자료 내용은 학생들이 주로 실생활에서 접하는 소비생활과 직접적으로 관련이 있는 동영상, 신문자료, 인터넷 정보 등을 바탕으로 구성하였다. 둘째, 수업에 참여한 학생들은 BL전략을 활용한 실천적 문제 중심 학습이 다양한 학습매체 사용과 정보 공개 공유가 가능하고, 정보 탐색이 용이하며, 자기 주도적 학습과 협동학습을 할 수 있을 뿐만 아니라 활발한 상호작용이 가능하고, 학습한 내용을 실천하고 반성할 수 있고, 수업에 흥미가 있어 수업의 참여가 촉진되며, 수업 내용을 이해하는 데 도움이 될 뿐만 아니라 가정과수업에 만족을 주어 그들의 소비생활 태도에 긍정적인 변화를 주는 것으로 평가하였다. 하지만 부족한 수업시간에 비해 학습 자료량이 너무 많고, 수업을 위한 물리적 환경 시설이 미비하며, 에듀넷 회원 가입이 어렵고, 자신들의 정보 소양 능력이 미숙하고, 학습 방법을 이해하지 못한 점을 단점으로 평가하였다. 본 연구의 결과 BL전략을 활용한 실천적 문제 중심 수업이 교실에서만 수행되던 학습의 공간을 온라인의 사이버 학습 공간으로 확장하고, 실천적 문제 중심 수업의 한계점이었던 학습 정보량 부족의 문제와 상호작용 부족의 문제를 보완할 수 있었으며, 다양한 활동으로 학생들의 흥미와 수업 참여도를 증대시킨 점에서 효과가 있는 수업이라는 것이 밝혀졌다. 하지만 앞으로 BL전략을 활용한 실천적 문제 중심 수업이 학교 현장에서 좀 더 잘 수행되기 위해서는 이 수업에 참여한 학생과 교사들이 지적한 문제들, 즉 학생들의 정보 소양 능력 및 토론능력, 교사의 수업에 대한 이해, 학생들의 수업 참여 방안, 물리적 환경 확충 등을 체계적으로 해결하는 방안이 마련되어야 할 것이다.

메타데이터 상호운용성을 위한 기록관리 메타데이터 표준 분석 5W1H와 태스크 모델의 관점에서 (Analysis of Metadata Standards of Record Management for Metadata Interoperability From the viewpoint of the Task model and 5W1H)

  • 메타데이터 표준규격(이하 메타데이터 표준으로 기재)은 디지털 자원(Digital resource)의 장기보존 및 디지털 아카이브를 위해 필요한 기본 요소 중 하나로, 이는 현대 정보사회에서 중요한 요소로 잘 알려져 있다. 자원(Resource)의 기록관리와 아카이브, 장기보존을 위한 메타데이터 표준은 다양하며, AGRkMS, EAD, ISAD(G), OAIS, PREMIS5) 등이 이용되고 있다. 우리는 아카이브 시스템의 메타데이터 표준을 디자인하기 위해 목적에 따른 메타데이터 표준을 선택하고 맞춤화(Customization)하지 않으면 안 된다. 한편으로, 다른 시스템의 메타데이터 스키마와의 상호운용성(Interoperability)에 대한 고찰도 실시하지 않으면 안 된다. 이전 연구에서, 우리는 기록 생애 주기(Records lifecycle)라는 관점으로부터 메타데이터 표준의 특성에 대해 분석을 실시하였다. 이로 인해, 각 메타데이터 표준 요소가 해당하는 기록 생애 주기의 처음 단계를 확인할 수 있었고, 아카이브 혹은 보존을 위해서는 하나의 메타데이터 표준만으로 기록 생애 주기 전체를 포괄할 수 없다는 것을 보여 주었다. 우리는 이 분석을 통해서 기록 생애 주기의 단계와 메타데이터 표준간의 관계, 기록 생애 주기 전체에서의 메타데이터 특성은 볼 수 있었으나, 보다 상세한 분석을 실시하는 것은 앞으로의 과제로 남겨두었다. 지금까지의 연구에 근거하여, 본 논문은 기록 생애 주기의 관점에서 디지털 아카이브와 보존, 기록관리를 위한 메타데이터 표준의 특징 분석을 위해, 기록 생애 주기 안에서 실행되는 업무의 관점으로부터 메타데이터 스키마를 재 파악하고 분석하였다. 지금까지 메타데이터 스키마는 기술대상이 되는 자원을 중심으로 정의되었기 때문에 기록 생애 주기 전체와 생애 주기 안의 각 단계에서 이용되는 메타데이터 표준간의 매핑을 위한 적절한 방법이 없었다. 이에 본 논문에서는 각기 다른 메타데이터 표준의 기술 요소를, 기록 생애 주기에 포함시키는 업무와 연결시키는 것으로 메타데이터 표준간의 매핑 방법을 제안한다. 본 연구에서는 메타데이터 표준 분석을 위한 프레임워크(Framework)로, 기록 생애 주기를 이용하여 작성한 자원의 업무중심 모델, 즉 태스크 모델(Task Model)을 제안한다. 태스크 모델을 이용함에 있어서 업무를 실행하는 '이벤트(Event)'의 관점을 보다 명확하게 할 수 있다. 한편, 업무를 중심으로 기술 요소간의 매핑을 보다 효율적으로 실시하기 위해서는 요소를 카테고리 화하여 매핑의 대상 범위를 좁히는 것이 중요하다. 이를 위해 우리는 범용성을 가진 5W1H모델(Who, What, Why, When, Where, How)을 이용하여, 기술 요소를 카테고리 화 하는 것을 제안한다. 그리고 태스크 모델과 5W1H 모델을 이용하여 메타데이터 표준 요소에 특징을 부여하고, 요소 간의 매핑을 실시하여 표준 간 관계를 확인하였다. 태스크 모델은 기록 생애 주기 전체에 업무의 관점을 반영한 것으로, 이 모델을 이용함으로써 기록 생애 주기와 그 안의 각 업무에 대한 메타데이터 표준의 사용, 그리고 특징 분석을 위해 실시되는 기술 요소간의 매핑이 가능할 수 있었다. 또한, 5W1H 카테고리를 이용하여 업무와 자원에 관련되는 기술 요소간의 관계를 살펴보는 것으로, 관계가 명확해지는 것과 함께 조사대상을 좁히는 것이 가능하게 되었다. 이 프레임워크의 제안과 이용으로 우리는 특징 분석을 위해 실시되는 매핑 혹은 분류가 단순한 일반적인 매핑이 아닌 의미적인 분류를 할 수 있었다. 본 연구에서는 이 모델들을 이용하여, 메타데이터 표준간의 크로스워크를 정의하였다. 그리고 태스크 모델의 각 단계의 문맥 내에서 메타데이터 기술 요소의 특성을 매핑 예로 확인하고, 이를 바탕으로 하여 프레임워크에 대해 고찰하였다.

가족계획과 모자보건 통합을 위한 조산원의 투입효과 분석 -서산지역의 개입연구 평가보고- (An Intervention Study on Integration of Family Planning and Maternal/Infant Care Services in Rural Korea)

  • This project was a service-cum-research effort with a quasi-experimental study design to examine the health benefits of an integrated Family Planning (FP)/Maternal & Child health (MCH) Service approach that provides crucial factors missing in the present on-going programs. The specific objectives were: 1) To test the effectiveness of trained nurse/midwives (MW) assigned as change agents in the Health Sub-Center (HSC) to bring about the changes in the eight FP/MCH indicators, namely; (i)FP/MCH contacts between field workers and their clients (ii) the use of effective FP methods, (iii) the inter-birth interval and/or open interval, (iv) prenatal care by medically qualified personnel, (v) medically supervised deliveries, (vi) the rate of induced abortion, (vii) maternal and infant morbidity, and (viii) preinatal & infant mortality. 2) To measure the integrative linkage (contacts) between MW & HSC workers and between HSC and clients. 3) To examine the organizational or administrative factors influencing integrative linkage between health workers. Study design; The above objectives called for quasi-experimental design setting up a study and control area with and without a midwife. An active intervention program (FP/MCH minimum 'package' program) was conducted for a 2 year period from June 1982-July 1984 in Seosan County and 'before and after' surveys were conducted to measure the change. Service input; This study was undertaken by the Soonchunhyang University in collaboration with WHO. After a baseline survery in 1981, trained nurses/midwives were introduced into two health sub-centers in a rural setting (Seosan county) for a 2 year period from 1982 to 1984. A major service input was the establishment of midwifery services in the existing health delivery system with emphasis on nurse/midwife's role as the link between health workers (nurse aids) and village health workers, and the referral of risk patients to the private physician (OBGY specialist). An evaluation survey was made in August 1984 to assess the effectiveness of this alternative integrated approach in the study areas in comparison with the control area which had normal government services. Method of evaluation; a. In this study, the primary objective was first to examine to what extent the FP/MCH package program brought about changes in the pre-determined eight indicators (outcome and impact measures) and the following relationship was first analyzed; b. Nevertheless, this project did not automatically accept the assumption that if two or more activities were integrated, the results would automatically be better than a non-integrated or categorical program. There is a need to assess the 'integration process' itself within the package program. The process of integration was measured in terms of interactive linkages, or the quantity & quality of contacts between workers & clients and among workers. Intergrative linkages were hypothesized to be influenced by organizational factors at the HSC clinic level including HSC goals, sltrurture, authority, leadership style, resources, and personal characteristics of HSC staff. The extent or degree of integration, as measured by the intensity of integrative linkages, was in turn presumed to influence programme performance. Thus as indicated diagrammatically below, organizational factors constituted the independent variables, integration as the intervening variable and programme performance with respect to family planning and health services as the dependent variable: Concerning organizational factors, however, due to the limited number of HSCs (2 in the study area and 3 in the control area), they were studied by participatory observation of an anthropologist who was independent of the project. In this observation, we examined whether the assumed integration process actually occurred or not. If not, what were the constraints in producing an effective integration process. Summary of Findings; A) Program effects and impact 1. Effects on FP use: During this 2 year action period, FP acceptance increased from 58% in 1981 to 78% in 1984 in both the study and control areas. This increase in both areas was mainly due to the new family planning campaign driven by the Government for the same study period. Therefore, there was no increment of FP acceptance rate due to additional input of MW to the on-going FP program. But in the study area, quality aspects of FP were somewhat improved, having a better continuation rate of IUDs & pills and more use of effective Contraceptive methods in comparison with the control area. 2. Effects of use of MCH services: Between the study and control areas, however, there was a significant difference in maternal and child health care. For example, the coverage of prenatal care was increased from 53% for 1981 birth cohort to 75% for 1984 birth cohort in the study area. In the control area, the same increased from 41% (1981) to 65% (1984). It is noteworthy that almost two thirds of the recent birth cohort received prenatal care even in the control area, indicating that there is a growing demand of MCH care as the size of family norm becomes smaller 3. There has been a substantive increase in delivery care by medical professions in the study area, with an annual increase rate of 10% due to midwives input in the study areas. The project had about two times greater effect on postnatal care (68% vs. 33%) at delivery care(45.2% vs. 26.1%). 4. The study area had better reproductive efficiency (wanted pregancies with FP practice & healthy live births survived by one year old) than the control area, especially among women under 30 (14.1% vs. 9.6%). The proportion of women who preferred the 1st trimester for their first prenatal care rose significantly in the study area as compared to the control area (24% vs 13%). B) Effects on Interactive Linkage 1. This project made a contribution in making several useful steps in the direction of service integration, namely; i) The health workers have become familiar with procedures on how to work together with each other (especially with a midwife) in carrying out their work in FP/MCH and, ii) The health workers have gotten a feeling of the usefulness of family health records (statistical integration) in identifying targets in their own work and their usefulness in caring for family health. 2. On the other hand, because of a lack of required organizational factors, complete linkage was not obtained as the project intended. i) In regards to the government health worker's activities in terms of home visiting there was not much difference between the study & control areas though the MW did more home visiting than Government health workers. ii) In assessing the service performance of MW & health workers, the midwives balanced their workload between 40% FP, 40% MCH & 20% other activities (mainly immunization). However, $85{\sim}90%$ of the services provided by the health workers were other than FP/MCH, mainly for immunizations such as the encephalitis campaign. In the control area, a similar pattern was observed. Over 75% of their service was other than FP/MCH. Therefore, the pattern shows the health workers are a long way from becoming multipurpose workers even though the government is pushing in this direction. 3. Villagers were much more likely to visit the health sub-center clinic in the study area than in the control area (58% vs.31%) and for more combined care (45% vs.23%). C) Organization factors (admistrative integrative issues) 1. When MW (new workers with higher qualification) were introduced to HSC, it was noted that there were conflicts between the existing HSC workers (Nurse aids with less qualification than MW) and the MW for the beginning period of the project. The cause of the conflict was studied by an anthropologist and it was pointed out that these functional integration problems stemmed from the structural inadequacies of the health subcenter organization as indicated below; i) There is still no general consensus about the objectives and goals of the project between the project staff and the existing health workers. ii) There is no formal linkage between the responsibility of each member's job in the health sub-center. iii) There is still little chance for midwives to play a catalytic role or to establish communicative networks between workers in order to link various knowledge and skills to provide better FP/MCH services in the health sub-center. 2. Based on the above findings the project recommended to the County Chief (who has power to control the administrative staff and the technical staff in his county) the following ; i) In order to solve the conflicts between the individual roles and functions in performing health care activities, there must be goals agreed upon by both. ii) The health sub·center must function as an autonomous organization to undertake the integration health project. In order to do that, it is necessary to support administrative considerations, and to establish a communication system for supervision and to control of the health sub-centers. iii) The administrative organization, tentatively, must be organized to bind the health worker's midwive's and director's jobs by an organic relationship in order to achieve the integrative system under the leadership of health sub-center director. After submitting this observation report, there has been better understanding from frequent meetings & communication between HW/MW in FP/MCH work as the program developed. Lessons learned from the Seosan Project (on issues of FP/MCH integration in Korea); 1) A majority or about 80% of the couples are now practicing FP. As indicated by the study, there is a growing demand from clients for the health system to provide more MCH services than FP in order to maintain the achieved small size of family through FP practice. It is fortunate to see that the government is now formulating a MCH policy for the year 2,000 and revising MCH laws and regulations to emphasize more MCH care for achieving a small size family through family planning practice. 2) Goal consensus in FP/MCH shouBd be made among the health workers It administrators, especially to emphasize the need of care of 'wanted' child. But there is a long way to go to realize the 'real' integration of FP into MCH in Korea, unless there is a structural integration FP/MCH because a categorical FP is still first priority to reduce the rate of population growth for economic reasons but not yet for health/welfare reasons in practice. 3) There should be more financial allocation: (i) a midwife should be made available to help to promote the MCH program and coordinate services, (in) there should be a health sub·center director who can provide leadership training for managing the integrated program. There is a need for 'organizational support', if the decision of integration is made to obtain benefit from both FP & MCH. In other words, costs should be paid equally to both FP/MCH. The integration slogan itself, without the commitment of paying such costs, is powerless to advocate it. 4) Need of management training for middle level health personnel is more acute as the Government has already constructed 90 MCH centers attached to the County Health Center but without adequate manpower, facilities, and guidelines for integrating the work of both FP and MCH. 5) The local government still considers these MCH centers only as delivery centers to take care only of those visiting maternity cases. The MCH center should be a center for the managment of all pregnancies occurring in the community and the promotion of FP with a systematic and effective linkage of resources available in the county such as i.e. Village Health Worker, Community Health Practitioner, Health Sub-center Physicians & Health workers, Doctors and Midwives in MCH center, OBGY Specialists in clinics & hospitals as practiced by the Seosan project at primary health care level.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.
