  • In ancient East Asia surgical methods existed even before the modern surgical methods were introduced. These surgical methods were mainly used to treat tumor, bruises, and dermatosis. The methods include many techniques such as making incisions in the skin, tell when to operate, and the aftercare following an operation. The traditional ways are not practiced any more due to its being replaced by the anesthesia techniques originating from the West, but the ancient methods can still be found in documents and these methods can be reproduced through such documents. There is a possibility that this ancient method could replace the present day method because of its nature-friendly characteristic as well as its post-operation management.

  • Background: Atopic dermatitis is chronic eczematous dermatosis characterized by severe itching, localized eruption and chronic relapsing progress and has family history of atopic diseases and hereditary disposition. This Study is attempted in order to estimate the efficacy of herb medication treatment on atopic dermatitis. Objective: We studied 35 patients with atopic dermatitis from October 2001 to September 2002. Methods: We treated them with herb-medication and used the SCORAD index to assess the severity of atopic dermatitis. Results: Of the 35 patients that were treated, the SCORAD index was lowered to $30.5{\pm}20(3.5-94)$ than before of which the SCORAD index was $46.8{\pm}18.1(14.9-97.5)$. Conclusion: On the basis of this study, It should be considered that treatment of atopic dermatitis should be focused on the gastrointestinal system. Moreover a balanced diet should be considered

  • A 66-year-old woman was transferred to our hospital due to her right breast cancer. Preoperative breast MRI shows 1.9 cm malignancy on her right breast (cT1N0M0) and incidentally found osteosclerotic change of left coststernoclavicular region. Bone scintigraphy showed hot uptake and the possibility of bone metastasis was not excluded. However, because the bone metastasis is not common in early stage cancer and the costosternoclavicular region is not common site, other possibility should be considered. SAPHO syndrome can be diagnosed even in the absence of dermatosis when there is an axial or appendicular osteitis and hyperostosis, especially in costosternoclavicular region. Though breast imaging specialists are not accustomed to this disease entity, awareness and diagnosis of the SAPHO syndrome can help differentiate bone metastasis.

  • Mycosis fungoides is an uncommon, chronic fatal disease of lymphoreticular system associated with primary ski3 involvement for many years and terminating as a malignant lymphoma with involvement of lymph nodes and viscerae. On occasion it simulates numerous other nonspecific benign skin lesions, thus it may be impossible to decide whether the infiltrate represents early mycosis fungoides or nonspecific on the histopathologic ground alone. A case of mycosis fungoides was confirmed by electron microscopy and reported here. The patient was 69-years-old male who had suffered from erythematous scaly eruption on the whole body since 10 years. Skin biopsies of 4 times showed focal ulceration with chronic nonspecific inflammation and polymorphic cell infiltration in lower dermis, thus possibility of mycosis fungoides could not be completely ruled out. Electron microscopically several atypical lymphoid cells, which had a large cerebriform nucleus with peripheral condensation of dense chromatin and scant cytoplasm, were noted in the upper dermis. Intraepidermal infiltration of these atypical cells was also seen. It was thought that the electron microscopic study may be very helpful to differentiate equivocal mycosis fungoides from the nonspecific dermatosis.

  • Atopic Dermatitis is chronic eczematous dermatosis which is related strongly to a family history of atopic disease and hereditary disposition. Its threshold of itching is low so that severe itching occurs and causes secondary eczema. The purpose of this study was to estimate the efficiency of oriental medical treatment and management on atopic Dermatitis. We studied 2 patients who visited Wonkwang University Kwangju Oriental Hospital Dept. of Dermatol. with atopic dermatitis from April 2004 to February 2005. We treated him with herb-medication, acupuncture, negative therapy, full spectrum visible ray therapy(by carbon arc). We used the ADSI(Atopic Dermatitis Severity Index) to assess the severity of atopic dermatitis. The grade of the severity of atopic dermatitis was lower only by the traditional chinese medical treatments and management. Considering the above result, more study is needed the factors of increasing severity of atopic dermatitis, and the clinical interchange or atopic Dermatitis between oriental medicine and western medicine.

  • A 8 kg, 3-year-old male West Highland white terrier dog with a 1.5-year history of chronic, severely pruritic, seborrheic skin disorder was referred to the Veterinary Medical Teaching Hospital of the Tokyo University of Agriculture and Technology. On physical examination, lesions were observed on entire cutaneous surface, except for face, dorsum of body, and footpads. Skin lesions were characterized by diffuse erythema, erythematous papules, severe alopecia, hyperpigmentation, and lichenification. Tape strip tests of skin lesions revealed cocci and Malassezia infections. The intradermal allergy tests revealed positive reactions to Japanese cedar pollen, but the non-seasonal clinical signs were not compatible with atopic dermatitis caused by this pollen. Results of hematological examination, serum chemistry and thyroid gland profile were normal. Examination of skin biopsy exhibited hyperplastic superficial perivascular dermatosis with severe acanthosis, excessive keratinocyte mitoses, patchy or diffuse mild spongiosis, and lymphocytic exocytosis in epidermis. Perivascular to interstitial mononuclear cells infiltration was seen in the superficial dermis. Based on the results of examination described above, epidermal dysplasia was diagnosed. Treatments with administration of antibiotics, etretinate, and prednisolone orally combined with topical ketoconazole cream and antiseborreic shampoos had no good results. Following treatment with long-term oral itraconazole at 10 mg/kg daily and chlorhexidine shampoos was successful. However, when itraconazole therapy was stopped, the condition worsened twice within 2 or 3 months. Readministration of itraconazole produced improvement within 4 weeks. This dog has now been controlled periodical itraconazole therapy.

  • Psoriasis is a chronic inflammatory disorder characterized by an erythematous scaly plaque of the skin and is occasionally accompanied by systemic complications. In the psoriatic lesions, an increased number of cytokine-producing dendritic cells and activated T cells are observed, which indicate that psoriasis is a prototype of an immune-mediated dermatosis. During the last decade, emerging studies demonstrate novel roles for the dendritic cell subsets in the process of disease initiation and maintenance of psoriasis. In addition, recently discovered anti-psoriatic therapies, which specifically target inflammatory cytokines produced by lesional dendritic cells, bring much better clinical improvement compared to conventional treatments. These new therapies implicate the crucial importance of dendritic cells in psoriasis pathogenesis. This review will summarize and discuss the dendritic cell subsets of the human skin and their pathophysiological involvement in psoriasis based on mouse- and patient-oriented studies.

  • Three compounds including 7,7-bis(3-indolyl)-p-cresol (1), cyclo-(S-Pro-R-Leu) (2) and cyclo-(S-Pro-R-Val) (3) were isolated from the strain of Bacillus megaterium LC derived from the marine sponge Haliclona oculata. All the isolated compounds showed antimicrobial activity at MIC values ranging from 0.005 to $5{\mu}g/mL$ against Gram-negative bacteria Vibrio vulnificus and V. parahaemolyticus, gram-positive bacteria Bacillus cereus and Micrococcus luteus, and the dermatophyte Trichophyton mentagrophytes. The results suggested that these compounds might have potential to be developed as agents treating dermatosis and controlling vibriosis in aquaculture.
