Objectives : The purpose of this study is to investigate the characteristics of patients referred for delirium and depressive disorder and to find direction of improvement of consultation-liason psychiatry in general hospital. Methods : We performed a retrospective computed chart review of the 4,966 inpatients hospitalized at Konkuk University Hospital who were referred to the Department of Psychiatry from August 1, 2005 to December 31, 2011. Results : Depressive disorder shows the order of frequency of consultation type Parallel-Complementary-Mending. Delirium shows the order of frequency of consultation type Mending-Parallel-Complementary. When comparing 'follow up consultation' and 'without follow up consultation' group within the depressive disorder, the proportion of men in the 'follow up consultation' group was higher. In the analysis of the consultation type, the 'follow up consultation' group showed the order of consultation type Parallel-Mending- Complementary, and type Parallel-Complementary-Mending in the 'without follow up consultation' group. When comparing 'follow up consultation' and 'without follow up consultation' group within the delirium, the proportion of the surgical field in the 'follow up consultation' group was higher. In the analysis of the consultation type, both group showed the order of consultation type Mending-Parallel- Complementary. Conclusions : Doctors in each department and psychiatrists should pay attention to delirium symptoms that may occur in surgical inpatients and preventive measures should be taken. Screening tests should be conducted in medical patients to properly evaluate coexisting psychiatric diseases. Risk factors of Delirium and Depressive disorder should be identified from the time of hospitalization, and actively discussing treatment plans and early interventions could improve the quality of medical services.
Purpose: Sleep onset association disorder (SOAD) is a form of behavioral insomnia observed in children that is caused by inappropriate sleep training. SOAD typically disturbs the sleep of not only infants and children but also their parents. We investigated levels of depression and marital intimacy among parents of infants with typical SOAD, to understand the influence of SOAD on family dynamics, as well as examine ways for improving depression and marital intimacy through behavioral training. Methods: Depression and marital intimacy were assessed using the Beck Depression Inventory (BDI) and Waring Intimacy Questionnaire (WIQ). These measures were administered to 65 parents of infants (n=50) diagnosed with SOAD. We conducted sleep education and behavioral training for the parents and compared levels of depression and marital intimacy after 2-6 weeks of training. Results: The 65 parents consisted of 50 mothers and 15 fathers. Depressive symptoms were higher among mothers than fathers (P=0.007). Marital intimacy was negatively correlated with depressive symptoms. Twenty-six parents were assessed again after sleep training. We found that mothers' depressive symptoms and marital intimacy improved post training. Conclusion: SOAD can be detrimental to both infants and parents, especially for parents who sleep with their infants. For instance, disruption of sleep patterns in such parents can reduce marital intimacy. However, behavioral modification is an effective treatment for infants with frequent nighttime waking, as well as for diminishing the depressive symptoms of sleep-deprived parents.

Objectives Restless legs syndrome (RLS) is a sleep disorder characterized by uncomfortable and unpleasant sensations in the legs and an urge to move the legs, usually at night. The aim of this study is to investigate the incidence of RLS in patients with late life depression and its influence on various clinical outcomes such as severity of depression, sleep quality, cognitive function, and quality of life and accordingly, to elucidate the clinical significance of RLS in patients with late life depression (LLD). Methods This study enlisted 170 depressive patients aged 65 years or older from an outpatient clinic. Structured diagnostic interviews were performed using the Korean version of the Mini-International Neuropsychiatric Interview. All patients completed the questionnaires, including the International RLS Severity Scale, the Korean version of Short-Form 36-Item Health Survey (SF-36), and the Pittsburgh Sleep Quality Index (PSQI). The severity of depression was evaluated by the Korean form of the Geriatric Depression Scale (KGDS) and the level of global cognition was assessed by the Mini-Mental State Examination in the Korean version of The Consortium to Establish a Registry for Alzheimer's Disease Assessment Packet (MMSE-KC). Results The incidence of RLS was 17.6% in LLD patients. RLS was more prevalent among the subjects with major depressive disorder (MDD) than those with minor depressive disorder or subsyndromal depressive disorder. The RLS group showed higher score in the KGDS than the Non-RLS group but the difference did not reach the statistical significance (p = 0.095, Student t-test). The mean PSQI score was significantly higher in the RLS group than in the Non-RLS group (p = 0.001, Student t-test). The MMSE-KC score was also lower in the RLS group than in Non-RLS group (p = 0.009, analysis of covariance). But, there was no difference in the score of SF-36 between the RLS group and the Non-RLS group. Conclusions RLS is common in LLD patients, especially in the patients with MDD and is associated with poor sleep quality and cognitive dysfunction, indicating that RLS is clinically significant in patients with LLD. Therefore, RLS should be considered as an important clinical issue in the management of LLD.

This was an open trial to evaluate the efficacy and safety of moclobemide twice daily for treatment of Korean patients with major depressive disorder(DSM-III-R). The duration of the trial was 6 weeks with the initial dose of moclobemide being fixed lor the first two weeks at 300mg/day(150mg twice daily, each token after morning and evening meals). Thereafter, when necessary, the dose was allowed to increase to 600mg/day or decrease to 150mg/day according to the seventy of the depression and/or the tolerability of the drug. Hypnotics and/or sedatives from a benzodiazepine group could be concomitantly administered at usual dosage. Patients were assessed at baseline and at days 14, 28 and 42. Efficacy was primarily judged on the Hamilton Rating Scale for Depression(HAM-D) and Beck Depression Inventory(BDI). Patients had to score at least 17 respectively an both scales to enter the trial. Secondary efficacy parameters included Clinical Global Impression(CGI) for severity of illness and improvement. Safety and tolerability were judged on reported adverse events, vital signs and laboratory parameters. In addition, there was a series of questions and assessments for the psychiatrists and patients to complete at the end of the trial Twenty nine patients completing trial were included in the analysis of efficacy : of thirty one patients participating in the safety and tolerability analysis, those who withdraw voluntarily without particular reasons or violated the treatment schedule were not included. The efficacies as determined by HAM-D, BDI or CGI were found to be significant compared to baseline. The number of responders defined as patients with a total score of 10 or less or with a total score of 50% or less of the baseline score on HAM-D and BDI were 17(59%) and 18(62%) respectively. Regarding safety and tolerability, nine patients(29%) reported mild adverse events probably related to moclobemide : of these one patient dropped out because of poor tolerability : however, there were no appreciable changes in blood pressure, pulse rate, body weight or laboratory parameters for all patients over the trial period. Furthermore, the physicians' and patients' opinions at final evaluation showed that moclabemide has a good antidepressant effect as well as a favorable tolerability. In conclusion, a twice-daily dosage schedule with maclobemide is recommendable for the treatment of Korean patients with major depressive disorder since its efficacy and safety were demonstrated in this study.
The debate about whether depressive disorders should be divided into categories or arrayed along a continuum has gone for decade, without resolution. In our review, there is more evidence consistent with the spectrum concept than there is with the idea that depressive disorders constitute discrete clusters marked by relatively discontinuous boundaries. First, "depression spectrum", "is there a common genetic factors in bipolar and unipolar affective disorder", "threshold model of depression" and "bipolar spectrum disorder" are reviewed. And, a new subtype of depression is so called SeCA depression that is a stressor-precipitated, cortisol-induced, serotonin-related, anxiety/aggression-driven depression. SeCA depression is discussed. But, there is with the idea that depressive disorders constitute discrete subtypes marked by relatively discontinuous boundaries. This subtypes of depressive disorder were reviewed from a variety of theoretical frames of reference. The following issues are discussed ; Dexamethasone suppression test(DST), TRH stimulation test, MHPG, Temperament Character Inventory(TCI), and heart rate variability(HRV).

The use of natural environments to change lifestyle and health has been long recognized. In particular, forests, trees, and open space have been shown to promote mental health. In this study, we examined the effectiveness of the structured psychotherapeutic program using forest environment ("forest activity program") to improve the symptoms of nine patients with Major Depressive Disorder (MDD) who were taking variable doses of antidepressants. We assessed the depressive symptoms, quality of life, and autonomic nerve regulation among the MDD patients. Hamilton Rating Scales for Depression (HRSD) scores significantly decreased after the forest therapy (13.56 vs. 5.56, p=0.003), and some subscores of Short Form 36 health survey questionnaire (SF-36) and heart rhythm coherence are improved as well. Combined with antidepressant pharmacotherapy, the structured psychotherapeutic program using forest environment showed an improved health status for MDD patients and thus has potential as an adjuvant treatment for MDD, especially for rehabilitation and relapse prevention.
Objectives:Body image is closely related to self-esteem and weight-control related behaviors. In particular, relationship between two factors would be stronger in female adolescents. False recognition on body image and weight can be a risk factor of eating disorder, depression, and suicidal ideation. This study aimed to examine the effects of body image and eating disorders on developing depressive symptoms and suicidal ideation in female adolescents. Methods:Two hundred thirty nine students of a Girls' Commercial High School in Seoul were recruited. Eating Attitude Test for Korean Adolescents, Self-Esteem Scales, Impulsiveness Scale, Beck's Depression Inventory and Beck's Suicidal Ideation Scale were used to measure eating attitude and severity of psychiatric symptoms. Results:Among 239 subjects, the estimated risk group of eating disorders was 10%(n=24). They experienced more depressive symptoms than the control group. The bigger discrepancy in current and ideal body mass index was significantly related with higher depressive mood, suicidal idea, abnormal eating habits and lower self-esteem. Discrepancy between current and idea BMI was the most meaningful predictive factor about depression and suicidal thoughts by linear regression analysis. Conclusion:In spite of normal weight range of enrolled subjects, they experienced significant depressive mood, suicide thoughts and lower self-esteem associated with the discrepancy of their own subjective body image and current body mass index. Hence educational approach regarding normal body image and healthy weight control is needed for their mental health and preventing eating disorder.
Objective : The purpose of this study was to develop the Anger Response Scale(ARS), and then to use the scale in clinical practice. Methods First, a preliminary survey was conducted for 123 healthy adults to obtain 16 response items. Second, a preliminary questionnaire was completed by 258 healthy subjects. Third, a comparison was made regarding anger responses among 189 patients(59 with anxiety disorder, 72 with depressive disorder and 58 with somatoform disorder) and 258 healthy subjects. Results : Factor analysis yielded 4 subscales : aggression, irritability, avoidance and anger suppression. Reliability was computed by administering the ARS to 53 healthy subjects during a 2-week interval. Test-retest reliability for 4 subscales and the total score was significantly high, ranging between .53-.71. Cronbach's ${\alpha}$ for 4 subscales ranged between .62-.72, and .76 for the total score. Convergent validity was computed by correlating the 4 subscales and the total score with the total score of Aggression Questionnaire, State-Trait Anger Expression Inventory, anger and aggression subscale of Stress Response Inventory and hostility subscale of Symptom Checklist-90-Revised. The disorder group was significantly higher than normal group in scores of the avoidance and anger suppression subacale. The depressive disorder and somatoform disorder groups scored significantly higher on the avoidance subscale than the normal group. Conclusion : These results indicate that the ARS is highly reliable and valid. In addition, avoidance response is likely to be a characteristic anger response of the depressive disorder and somatoform disorder groups.

From a view point of Hyungsang medicine a study is made on depressive syndromes through 'Donguibogam(東醫寶鑑)' ,'Clinical lectures by Mr. Jeesan(芝山先生 臨床學特講)' and other literature. Depressed energy(?) originally meant the disorder of vital energy in the circulation of nature with the lapse of time. In later, it has been changed to mean the disease of man, especially related to the mind. Depressive syndromes come from the stagnation of the Ki or the abnormal circulation(rising, falling, coming and going), which happens when man cannot adjust oneself to the circumstances due to the disharmony between internal and external conditions. Depressive syndrome easily attacks the following types of persons; Dam type person with excessive Ki and deficient Hyung(形), manly woman, womanly man, Bird type person who shows a great variety of emotion, Turtle type person declined to melancholy, Ki type person apt to be depressive, and Shin type person with upward gush of the fire. It also attacks the persons who have smudgy face with the scar between eyebrows, those whose pulses are mixture of two different kinds and those whose pulses extend on two ranges in Jeesan's diagram. Depressive syndromes recorded in 'Donguibogam' are globus hystericus, chest congestion, gastric discomfort, cough and asthma caused by depressed Ki, abdominal mass, edema, and tympanites. The most effective prescriptions for depressive syndromes are Eajintang(二陳湯), Guibitang(歸脾湯), Hyangsosan(香蘇散), and Gamisachiltang(加味四七湯).
Objectives : N100 amplitude slope(the intensity dependence of the cortical auditory evoked potentials) is widely considered as an indirect indicator of central serotonergic neurotransmission. However, there are only a few studies about N100 amplitude slopes of major psychiatric disorders. In this study, we examined N100 amplitude slope differences among major depressive disorder(MDD), bipolar disorder(BD), schizophrenia (SCZ) and normal controls(NC). Methods : We measured the N100 amplitude slopes of 35 patients with MDD, 33 patients with BD, 27 patients with SCZ and 35 NC subjects. Amplitude differences from N1 to P2 at the five different sound intensities(55, 65, 75, 85 and 95dB) were examined at Cz electrode. The N100 amplitude slope was calculated as the linear regression of five N1/P2 peak-to-peak amplitudes across stimulus intensities. Results : BD patients showed significantly reduced N100 amplitude slope compared with NC(0.54${\pm}$0.70 vs. 0.96${\pm}$0.72, p=0.035). N100 amplitude slope of SCZ patients was significantly reduced compared with NC(0.50${\pm}$0.47 vs. 0.96${\pm}$0.72, p=0.027). N100 amplitude slope of BD patients was significantly lower than that of MDD patients(0.54${\pm}$0.70 vs. 0.94${\pm}$0.60, p=0.046). SCZ patients also showed significant reduction of N100 amplitude slope compared with MDD patients(0.50${\pm}$0.47 vs. 0.94${\pm}$0.60, p=0.036). There was no significant difference of N100 amplitude slope between MDD patients and NC(0.94${\pm}$0.60 vs. 0.96${\pm}$0.72, p=1.000). Conclusion : Interestingly, the N100 amplitude slopes of BD and SCZ were reduced compared to NC and MDD patients. Our results suggest the predictive use of N100 amplitude slope in making differential diagnoses of major psychiatric disorders. Clinical implications of N100 amplitude slope in major psychiatric disorders were discussed.
