  • Obstructive sialadenitis, one of the diseases that most frequently causes swelling and pain in the salivary glands, is mainly caused by structural obstructions. Sialolithiasis is the most frequent cause of the disease, and other causes include calculus formation, duct strictures, foreign bodies, and anatomical variations. Although there is a possibility that facial fillers directly block the salivary ducts, no cases of obstructive sialadenitis associated with them have been reported yet. We report the case of a 34-year-old female patient who complained of recurrent swelling and pain in the left buccal mucosa. She had undergone facial filler injection procedures on her facial area for cosmetic purposes several years before. Based on the findings of magnetic resonance imaging (MRI) and MR sialography, she was diagnosed with obstructive sialadenitis due to facial fillers. Through this case, we should remember to obtain a thorough history including filler treatments in the case of parotid gland swelling. We also suggest proper utilization of advanced imaging such as MRI in evaluating the location of facial fillers.

  • The purpose of this research is to determine elements affecting the out-of-pocket cost of woman. The sample consisted of 1907 women living Iksan city. The survey was conducted by means of questionnaires. The model used in the analysis of out-of-pocket cost was the Andersen-Newman model, while the analysis techniques used were stepwise multiple regression and path analysis. The number of independent variables used in the analysis was 28 in total, ie 19 predisposing components, 6 enabling components, and 3 need components. In this study, the amount of variance by the model was 17 percent. Number of restricted activity days caused by oral disease, perceived susceptibility of dental disease, having a regular dental care, dental treatment costs, education level and income were found to have significant major effects on out-of-pocket cost. Number of restricted activity days caused by oral disease was the most important variable affecting out-of-pocket cost of woman. Also out-of-pocket cost shows larger effect due to enabling components than frequency of dental utilization.

  • Purpose. This study examined the predictive factors enabling access to children's oral health care at the level of financial barriers, beliefs, and the provider. Methods. In-depth interviews were conducted with 320 immigrant mothers of low-income families regarding their use of oral health services for children aged four to eight years old. Access to oral health care was measured with frequency of planned dental visits, continuity of care, and age at first visit to dentist. Results. The mother took her child to the dentist at a younger age if she received referrals to a dentist from pediatrician. Regular dental visits were significantly related to household income, provider availability on week-ends, and insurance coverage. The extended clinic hours in the evenings, and the belief in the importance of the child's regular dentist visits increased the likelihood of continuing care. The mothers perceiving a cost burden for the child's dental care were also less likely to return to the dentist. Conclusion. The available care delivery system, coordinated medical care, and health beliefs were among important predictors of the health service use. The study findings suggest need for culturally competent dental health interventions to enhance access to oral health care among particularly vulnerable populations such as low-income children in Korean communities.

  • The reconstruction of structurally compromised posterior teeth is a rather challenging procedure. The tendency of endodontically treated teeth (ETT) to fracture is considerably higher than vital teeth. Although posts and core build-ups followed by conventional crowns have been generally employed for the purpose of reconstruction, this procedure entails sacrificing a considerable amount of residual sound enamel and dentin. This has drawn the attention of researchers to fibre reinforcement. Fibrereinforced composite (FRC), designed to replace dentin, enables the biomimetic restoration of teeth. Besides improving the strength of the restoration, the incorporation of glass fibres into composite resins leads to favorable fracture patterns because the fibre layer acts as a stress breaker and stops crack propagation. The following case report presents a technique for reinforcing a badly broken-down ETT with biomimetic materials and FRC. The proper utilization of FRC in structurally compromised teeth can be considered to be an economical and practical measure that may obviate the use of extensive prosthetic treatment.

  • Toothaches are the main reason patients seek dental care, but not all pain in the orofacial region originates from the teeth. Some toothaches may stem from musculoskeletal, neuropathic, or neurovascular issues. Additionally, certain toothaches may not be tooth-related, but rather, referred from nearby orofacial structures, where the site and source of pain differ. In referred pain, the pain typically travels to the site from an ipsilateral source in the orofacial region. However, when the site and source of pain are on different parts of the body, but the pain is referred in a predictable manner, it is termed mirror-image pain. This case report illustrates mirror-image pain originating in the right mandibular tooth, referred from the contralateral superficial masseter muscle. A comprehensive history, thorough head and neck evaluation, an understanding of anatomy, and the utilization of diagnostic trigger-point injections were instrumental in reaching the correct diagnosis and the management of this non-odontogenic toothache, thus avoiding unnecessary dental interventions for managing the pain site.

  • The purposes of this study was to confirm the current issues of workforce policy of dental auxiliary personnel in Korea. The literature review and data collection were conducted in terms of the auxiliary personnel supply, regional distribution, legal works, working environment. Despite the rapid increase in the number of dental hygienists, the number of active dental hygienists is less than half of the total, and the dental clinics still experience the shortage. Considering the dental hygienist's work, the available dental auxiliary personnel for the dental assistance is limited. The proportion of dental hygienists is concentrated in large cities, and the ratio of dental clinics composed of only dental hygienists is high. Thus, it is essential to look for a plan to solve the disparity. The criterion for judging the legality of the works is ambiguous because a large portion of dental hygienist's work is not specified in the Medical Technicians Act. The job life is also shortened, and occupational profession and satisfaction are lowered. Thus, job burnout is accelerating. The difference between the legal works and the practiced works of dental hygienist and the conflict between the personnel might be due to the lack of utilization plans for the dental personnel at the government level. Therefore, the government should establish the roles of dental hygienist in accordance with the demands of the dental health service, institutionalize the necessary policies accordingly, and make efforts to establish plans to improve the relevant laws and utilize the manpower appropriately.

  • This study investigated the awareness and purchase status of the private dental insurance. Self-reported survey was conducted with patients over the age of twenties who visited dental institutions located in Seoul Metropolitan City and Gyeonggi Province. The demographic and health related characteristics of respondents were analyzed, and logistic regression was conducted to examine factors affecting the awareness and purchase of private dental insurance. Because only four years had been passed since the introduction of private dental insurance, the awareness and purchase rate was found to be low. However, the number of people considering subscription due to the economic burden of dental care service was relatively high. Factors affecting awareness were satisfaction of the national health insurance, purchase of private health insurance and private dental insurance, self-perceived dental health status, and smoking. The variables affecting purchase of dental insurance were age, awareness, purchase of private health insurance, smoking, number of visits to dental institution. Because qualitative and quantitative change would be made in the dental care utilization due to the rapidly growing dental insurance subscribers, further studies regarding the trend of purchase rate of private dental insurance and the effect of dental insurance on use of dental institution are needed.

  • This study was conducted to investigate the risk factors affecting dental caries in children by using the 2018 children's oral health survey data. The study was conducted on 20,235 children who were 12 years of age. The survey items examined general characteristics, dental dietary behavior, the oral condition, and the behavioral factors related to oral health. The results showed that gender, region, economic level, subjective oral health condition, dental dietary behavior, oral condition and oral health-related behavior were all risk factors. Those children with oral conditions particularly showed a higher risk of the dental caries symptoms of dental calculus, dental bleeding, tooth pain and white spot teeth. The oral health-related behaviors were found to be tooth brushing less than two times a day, the risk of not using a handle to hold dental floss and not using dental floss. Our results showed that countries or communities can diagnose and manage dental well-being early on for children with the highest sensitivity of dental health and they need to continue to establish a dental well-being management system for the oral health care of children. In addition, oral health education should be expanded, which can improve oral health care habits of children and adolescents. Further, an oral health policy system for improving community programs to prevent dental and community utilization is needed.
