수복재와 이장재에 따른 응력과 온도 분포의 유한 요소 분석 (FINITE ELEMENT ANALYSIS OF STRESS AND TEMPERATURE DISTRIBUTION AFFECTED BY VARIOUS RESTORATIVE AND BASE MATERIAL)

  • Dental caries, one of the most frequent dental disease, become larger because it can be thought as a simple disease. Further more, it can progress to unexpected root canal therapy with fabrication of crown that needs reduction of tooth structure. Base is required in a large caries and ZOE, ZPC, glass ionomer are used frequently as base material. They, with restorative material, can affect the longevity of the restoration. In this study, we assume that the mandibular 1st molar has deep class I cavity. So, installing the 3 base material, 3 kinds of fillings were restored over the base as follows; 1) amalgam only, 2) amalgam with ZPC, 3) amalgam with ZOE, 4) amalgam with GI cement, 5) gold inlay with ZPC, 6) gold inlay with GI cement, 7) composite resin only, 8) composite resin with GI cement. After develop the 3-dimensional model for finite element analysis, we observe the distribution of stress and temperature with force of 500N to apical direction at 3 point on occlusal surface and temperature of 55 degree, 15 degree on entire surface. The analyzed results were as follow : 1. Principal stress produced at the interface of base, dentin, cavity wall was smallest in case of using GI cement as base material under the amalgam. 2. Principal stress produced at the interface of base, dentin, cavity wall was smaller in case of using GI cement as a base material than ZPC under gold inlay. 3. Composite resin-filled tooth showed stress distributed over entire tooth structure. In other words, there was little concentration of stress. 4. ZOE was the most effective base material against hot stimuli under the amalgam and GI cement was the next. In case of gold inlay, GI cement was more effective than ZPC. 5. Composite resin has the small coefficient of thermal conductivity. So, composite resin filling is the most effective insulating material.

유동성 복합레진을 이용한 유구치 II급 수복의 생존율 (Survival Rates of Class II Restoration in Primary Molar with Flowable Resin Composite)

  • 이 연구의 목적은 소아 환자의 유구치 인접면 우식증 치료에 유동성 복합레진을 이용한 II급 와동 수복물의 생존율을 기성금속관과 비교하여 후향적으로 평가하는 것이다. 2015년 6월부터 2019년 8월까지 인접면 우식증으로 진단되어 유동성 복합레진과 기성금속관 수복 치료를 시행한 590명의 1,504개의 유구치에 대한 전자의무기록과 방사선사진을 조사하였다. 수집된 자료는 Kaplan-Meier method를 이용해 생존 분석을 하였다. 유동성 복합레진 수복된 치아의 1년 생존율은 98.5%, 3년 생존율은 87.7%, 평균 생존기간은 39개월이었으며, 기성금속관과 비교할 때 통계적으로 유의한 차이는 없었다(p = 0.896). 현 연구의 한계 내에서 유동성 복합레진을 이용한 II급 수복은 유구치 인접면 우식증 치료의 예지력 있는 선택지가 될 수 있다고 사료된다.

수종(數種) Composite Resin의 색소침투(色素浸透)에 관(關)한 실험적(實驗的) 연구(硏究) (AN EXPERIMENTAL STUDY ON PENETRATION OF DYE IN COMPOSITE RESIN)

  • The purpose of this study was to measure penetration of dye stuff (2% Methylene blue, 2% Hematoxylin, 2% crystal violet and 2% safranin-O) on unfilled resin (Lang Dental MFG Co.) Hipol (Boopyung Dental Chemical Co. Macrocomposite resin) Durafill (Kulzer, Co. Microfilled Composite resin) and Heliosit (Vivadent Co. Microfilled Composite resin) The unfilled resin with dough stage was inserted into plastic tuble (5mm in diameter and 4mm in height) with condensation force of 1000 gr, 2000 gr and without condensation force. Hipol mixed on the mixed pad was inserted into the plastic tube by the same method as the unfilled resin. The microfilled resins which were Durafill and Heliosit were polymerized for 60 seconds with the visible light on each surface of the plastic tube which was upper and lower, under condensation force of 1000 gr, 2000 gr and without condensation force. All specimens were stored in the air for 24 hours, then specimens were immersed in the various kind of dye solution for different period of time (1 hour and 24 hours). These dye-treated specimens were polished horizontally until removing 0.5mm of each surface on the emery paper (#1000), and the dye penetration in the polished surface was measured under the digital microscope (Japan Fosuh). Following results were obtained 1. The penetration of dyes was the most excessive in Durafill and was not influenced on the condensation force and the period of immersion time. 2. All dyes were penetrated into Hipol, and Crystal violet was penetrated most excessively in all dyes. 3. The penetration of dye in all resins was not influenced by the period of immersion time and condensation force. 4. There was no evidence of dye penetration in unfilled resin.

컴포머에 대한 복합레진의 전단결합강도에 관한 연구 (A STUDY ON THE RELATIVE SHEAR BOND STRENGTH OF COMPOSITE RESIN TO COMPOMERS)

  • 컴포머(compomer) 충전물이 파절될 경우 즉, 컴포머 수복이 실패했을 때 기존 컴포머의 파절된 부분(aged compomer)을 복합레진으로 수복해야 하는 경우를 고려해야 한다. 본 연구에서는 컴포머의 표면처리 방법에 따른 수리(repair)를 위한 복합레진과의 전단결합강도 변화를 알아보기 위해 두 개의 mold를 만들어서 Dyract AP, F2000를 컴포머 수복재료로, Z250을 수리용 복합레진 재료로 사용하여 컴포머와 레진을 부착시킨 시편을 제작하였다. 컴포머와 복합레진의 종류 조합, 본딩레진 사용유무, 열순환 처리 유무, 표면거칠기 부여 유무, 기존 수복물의 시효(aging)에 따라 10개의 군으로 나누어, 컴포머와 복합레진간의 전단결합강도를 측정하고 그 파절양상을 관찰하여 다음과 같은 결과를 얻었다. 1. 컴포머의 복합레진에 대한 전단결합강도를 측정한 결과, Dyract AP가 F2000 보다는 높은 전단강도 값을 보였으나 통계적으로 유의한 차이는 없었으며(p>0.05), fresh 컴포머군이 aged 컴포머군보다 결합강도가 높았다(p<0.05). 2. 각 군간의 전단결합강도를 측정한 결과 Dyract AP에 접착제를 도포하고 Z250을 접착시킨 제 5군이 가장 높았으며, Dyract AP를 충전하고 1주일간 물에 보관한 후 건조시켜 표면을 roughening 한 후 Z250을 충전시킨 제 9군에서 가장 낮았다(p<0.05). 3. 열순환을 시행한 군이 시행하지 않은 군보다 전단결합강도가 낮게 나타났으나 통계적으로 유의한 차이는 없었다. 4. 각 재료들의 결합실패가 일어난 파절단면을 조직학적으로 관찰한 결과, 전반적으로 접착파괴를 보였으나, 접착제를 도포한 군에서는 응집파괴가 우세하게 나타났다. 이상의 결과 fresh 컴포머 위에 접착제를 도포한 다음 수리용 복합레진 재료를 사용함으로써 컴포머와 복합레진간의 결합력을 증가시킬 수 있음을 알 수 있었다. 그리고 fresh 컴포머 위에 복합레진을 즉시 충전한 경우가 높은 결합력을 얻을 수 있었다. Aged 컴포머와 복합레진간의 성공적인 결합력을 얻으려면 aged 컴포머의 표면을 mechanically roughening하고 접착제를 도포한후 복합레진을 충전해야 할 것으로 사료된다.

How does duration of curing affect the radiopacity of dental materials?

  • Purpose : Clinicians commonly encounter cases in which it is difficult to determine whether adjacent radiopacities are normal or pathologic. The ideal radiopacity of composite resin is equal to or higher than that of the same thickness of aluminum. We aimed to investigate the possible effects of different curing times on the post-24-hour radiopacity of composite resins on digital radiographs. Materials and Methods : One mm thick samples of Filtek P60 and Clearfil resin composites were prepared and cured with three regimens of continuous 400 mW/$cm^2$ irradiance for 10, 20 and 30 seconds. Along with a 12-step aluminum step wedge, digital radiographs were captured and the radiopacities were transformed to the equivalent aluminum thicknesses. Data were compared by a general linear model and repeated-measures of ANOVA. Results : Overall, the calculated equivalent aluminum thicknesses of composite resins were increased significantly by doubling and tripling the curing times (F(2,8)=8.94, p=0.002). Notably, Bonferroni post-hoc tests confirmed that the radiopacity of the cured Filtek P60 was significantly higher at 30 seconds compared with 10 seconds (p=0.04). Although the higher radiopacity was observed by increasing the time, other comparisons showed no statistical significance (p>0.05). Conclusion : These results supported the hypothesis that the radiopacity of resin composites might be related to the duration of light curing. In addition to the current standards for radiopacity of digital images, defining a standard protocol for curing of dental materials should be considered, and it is suggested that they should be added to the current requirements for dental material.

수종 복합레진의 치수반응에 관한 실험저 연구 (Experimental Study on the Pulpal Response to Various Composite Resins)

  • The Author studied hisopathologically on the pulpal changes to the 3 kinds of Composite resin restorations in dog's teeth. The experimented materials were Blendant Adaptic and Composite. The results were as follows. 1. Inflammatory changes has been observed in all of the experimented materials. 2. Adaptic showed no cases of severe pulpal response such as highly destruction of odontoblastic layer or abscess formatoin in pulp tissues. 3. Composite induced slightly more cases of severe pulpal reaponse than other composite resins.

Erosive Effect of Salad Dressing on Flowable Composite Resin Surfaces

  • Background: The purpose of this study was to investigate the effect of salad dressings on the flowable composite resin surface and the erosion-inhibitory effect of calcium. Methods: The experiment included six groups: oriental dressing, balsamic dressing, lemon-garlic dressing, lemon-garlic dressing supplemented with 3% calcium, mineral water as a negative control group, and orange juice as a positive control group. pH and titratable acidity were measured. The prepared specimens were immersed in the experimental solutions for 1, 3, 5, 15, and 30 minutes. The surface microhardness was measured using the Vickers hardness number before and after the treatment, and the surface of the specimens was observed using a scanning electron microscope (SEM). Results: The pH values of the experimental groups in increasing order were as follows: lemon-garlic dressing (2.49±0.03), balsamic dressing (3.12±0.06), lemon-garlic dressing +3% calcium (3.27±0.09), oriental dressing (3.75±0.03), orange juice (3.82±0.02), and mineral water (7.32±0.16). The largest surface hardness reduction value was shown in lemon-garlic dressing (-9.61±1.16), followed by balsamic dressing (-9.17±1.63), oriental dressing (-8.62±1.09), orange juice (-8.19±1.36), lemon-garlic dressing +3% calcium (-6.76±1.23), and mineral water (-1.63±2.47). According to the SEM findings, the experimental and positive control groups showed rough surfaces and micropores, whereas the negative control group showed a smooth surface. Moreover, the lemon-garlic dressing with +3% calcium showed fewer micropores and a smoother surface than the lemon-garlic dressing. Conclusion: The intake of salad dressings at a low pH could weaken the surface microhardness of the flowable composite resin. However, adding calcium to these salad dressings can reduce the risk of microhardness reduction on the flowable composite resin surface.
