A comparative study on the current oral health conditions of the elderly at home and welfare facilities (재가노인과 시설노인의 구강건강실태 비교연구 -삼척시에 거주하는 일부노인을 대상으로-)

  • This study investigated the current oral health conditions of the elderly at home and welfare facilities in their age over 65 years around some rural areas in Gangwon province, which would expect the fewer medical benefits even with lower interest than urban areas, despite of relatively high ratio of elder populations, so that it could prepare a basic document necessary to determine certain planned quantification for the benefit of elder's oral healthcare. As of the end of December 2004 both 50 elders at home and 50 elders at welfare facilities were randomly sampled in their age over 65 years in Samcheok city. As a result of this study, it was found that the elders at welfare facilities scored 15 pts. in DMFT index level typical of oral health conditions, which was higher than the elders at home. In addition, the elders at welfare facilities scored 26.0% in the coexistence of immobile bridge and partial denture higher than the elders at home with regard to the presence of intraoral prosthetic appliance. The results of analyzing the difference in the one-year dental visiting experience of respondents hereof showed that the elders at home were relatively more in ratio(62.0%) than those at welfare facilities, while many of the former group(38.0%) had relatively more handicap in masticatory movement than the latter one with regard to the conditions of dental prosthesis in use. Besides, many of the elders at facilities(30% or more) felt subjective symptoms of periodontal disease including bleeding or swelling, which indicates higher ratio than the elders at home. Finally, the elders at home used to brush their teeth at more frequency on a daily basis than those at facilities, while the latter group suffered general body disease more than the former group. Summing up, it is concluded that a formulated oral healthcare system will become more needed in near future than now for the benefit of the elderly living in welfare facilities, while nationwide policy-level supports would be urgent for them in the aspect of national welfare.

  • Purpose: The laser processability of dental prosthesis is investigated using two ceramic composites, including 3M, Lava Ultimate and Ivoclar vivadent, IPS e.max. Materials and methods: The $CO_2$ laser, picosecond laser and femtosecond laser are used to assess the processing power of dental prosthetic materials Lava Ultimate and IPS e.max and the line processing shape was measured using a confocal microscope. Results: The brittleness, carbonization and micro crack of the ceramic composite were influenced by heat accumulation of the material and could be controlled by the laser power and pulse time. Conclusion: In the case of $CO_2$ lasers, micro crack and carbonation occurred immediately, and in the picosecond laser processing, the micro cracks are partially improved, but the carbonization occurs continuously. Finally, we confirmed the high efficiency of laser processing with femtosecond laser. In particular, Lava Ultimate, a ceramic resin composite material, showed the best processability when processed using a femtosecond laser.

  • In advanced stages of periodontal disease, a lot of the periodontal tissue support are usually lost. The tissue destruction around some teeth is progressed to a level which requires either spontaneous exfoliation or extraction of several teeth due to their excessive mobility. In such cases, a comprehensive treatment plan encompassing the adequate periodontal and maintenance therapy, as well as perio-prosthetic treatment involving occlusal equilibration, is needed in order to restore health, function, and esthetics. Cross-arch fixed partial dentures(CAFPDs), one of the perio-prosthetic treatments, are used to stabilize the teeth with severely reduced periodontal tissue support. Unfortunately, however, a little is known about the occlusal scheme and biomechanical concept of CAFPDs. This paper will demonstrate summaries of the trauma from occlusion(TFO), Ante's law revisited, the treatment principles, the role of occlusion, and the long-term consequences for CAFPDs, and the possibility of CAFPDs through a case presentation.

  • There are some similar aspects at histological and morphological characteristics between the peri-implant tissue and periodontal tissue and the direct attachment between the titanium and soft tissue around the implant called as "Functional ankylosis" can prevent the apical infiltration of inflammatory and bone resorption around implant. But, the repeated connection and disconnection of the abutment can destroy the mucosal barrier of soft tissue around the implant and can cause the marginal bone resorption. The amount of marginal bone resorption may reduced if the prosthetic abutment is placed at that time of surgery. Connection of the prosthetic abutment at surgery was limited because the low accuracy of conventional method, but by using of Cone Beam Computed Tomography(CBCT) and guide surgery, the 3-dimensional accuracy of implant placement became much higher than before and it became possible. This is a clinical case of immediate connection of prosthetic abutment and provisional restoration by using of precise CBCT diagnosis and pre-fabricated zirconia customized abutment at surgery and the alternative method is described in this article because of the clinically contentable results.

  • Ameloblastoma is a benign odontogenic epithelial tumor with high recurrence rate and requires extensive resection of the surrounding tissue and reconstruction of defect site. Because of the anatomical limitation of the reconstruction site, prosthetic treatment with implants is the first recommendation. This is a case of prosthetic restoration of the reconstruction site with implant fixed prosthesis in patient who underwent mandibular resection and iliac bone reconstruction due to ameloblastoma. However 14 months after completion of implant prosthesis, adjacent natural tooth erupted unexpectedly, resulting in 1mm infra-occlusion occurred including posterior implant prosthesis and anterior natural teeth. In adults, implant infra-occlusion may occur due to residual growth after placement of the maxillary anterior implant. But this case, hypo-occlusion of molar implant and open bite of anterior natural teeth is occurred due to extrusion of adjacent tooth, is rare. Thus we report the treatment process including orthodontic treatment with intrusion of the posterior tooth, and investigate the causes of sudden, unexpected tooth extrusion.

The long-term evaluation of the prognosis of implants with acid-etched surfaces sandblasted with alumina: a retrospective clinical study

  • Background: The aim of this study was to evaluate the long-term clinical stability of implants with acid-etched surfaces sandblasted with alumina using retrospective analyses of the survival rate, success rate, primary and secondary stability, complications, and marginal bone loss of the implants. Methods: Patients who had implants placed (TS III SA, SS II SA, SS III SA, and U III SA) with SA surfaces from Osstem (Osstem Implant Co., Busan, Korea) at the Seoul National University Bundang Hospital, from January 2008 to December 2010 were selected for the study. Patients' medical records and radiographs (panorama, periapical view) were retrospectively analyzed to investigate sex, age, location of implantation, diameter, and length of the implants, initial and secondary stability, presence of bone grafting, types of bone grafting and membranes, early and delayed complications, marginal bone loss, and implant survival rate. Results: Ninety-six implants were placed in 45 patients. Five implants were removed during the follow-up period for a total survival rate of 94.8%. There were 14 cases of complications, including 6 cases of early complications and 8 cases of delayed complications. All five implants that failed to survive were included in the early complications. The survival of implants was significantly associated with the occurrence of complications and the absorption of bone greater than 1 mm within 1 year after prosthetic completion. In addition, the absorption of bone greater than 1 mm within 1 year after prosthetic completion was significantly associated with the occurrence of complications, primary stability, and implant placement method. Five cases that failed to survive were all included in the early complications criteria such as infection, failure of initial osseointegration, and early exposure of the fixture. Conclusions: Of the 96 cases, 5 implants failed resulting in a 94.8% survival rate. The failed implants were all cases of early complications such as infection, failure of initial osseointegration, and early exposure of the fixtures. Periimplantitis was mostly addressed through conservative and/or surgical treatment and resulted in very low prosthetic complications. Therefore, if preventive measures are taken to minimize initial complications, the results can be very stable.

Evaluation of sleep quality and stress response in patients requiring dental prosthetic treatment (치과보철치료를 요하는 환자에서의 수면질과 스트레스 반응 평가)

  • Purpose: The purpose of this study is to investigate the stress response and sleep state of the new patients who visit the hospital for prosthodontic treatment due to tooth loss to make reference to further treatment direction. Materials and methods: Examinations, consultations, and surveys were conducted on 236 patients (94 males, 142 females, and 24-86 years old), who newly registered at the department of prosthodontics at Pusan national university hospital, for 2 years from 2018. The number of remaining teeth, edentulous arch, possibility of mastication, and future prosthetic treatment directions were recorded. The visual analogue scale (VAS), stress response inventory (SRI) and Insomnia severity index (ISI) were used as tools to measure patient's subjective discomfort, stress response, and sleep status. Results: Among the entire prosthodontic patient group, the average value of 'fatigue' was significantly higher among the 7 items of SRI, and the value of 'aggression' was significantly lower (P<.001). There are a significant difference between the edentulous and full dentition in the oral discomfort value measured by VAS (P=.004). In cases where less than 20 teeth remain, mastication is impossible, and patients who are planning treatment with removable prostheses, VAS value and 'depression' and 'fatigue' values in SRI were higher than those in the reverse case (P<.05). The patients showed severe level in VAS value have high value of 'tension', 'somatization', 'depression', 'fatigue'and 'frustration'in SRI than non-serious patients (P<.05). Clinical insomnia occurred in 11.4% of all patients, and women had significantly lower sleep quality than men (P=.044). Patients with insomnia showed significantly higher scores on the VAS value and all 7 SRI items than those of normal sleepers (P<.05). Conclusion: The new prosthetics had high 'fatigue' value among the 7 items of SRI. The oral condition, number of residual teeth and mastication function and clinical insomnia affected oral discomfort and the stress response.

Effect of scaling and root planing on the expression of anti-inflammatory cytokines (IL-4, IL-9, IL-10, and IL-13) in the gingival crevicular fluid of electronic cigarette users and non-smokers with moderate chronic periodontitis

  • Purpose: The aim of this cross-sectional study was to investigate the effect of scaling and root planing (SRP) on the expression of anti-inflammatory cytokines (interleukin [IL]-4, IL-9, IL-10, and IL-13) in the gingival crevicular fluid (GCF) of electronic cigarette users and non-smokers with moderate chronic periodontitis (CP). Methods: Electronic cigarette users and non-smokers with CP were included in the study. Full-mouth plaque and gingival indices, probing depth (PD), clinical attachment loss (CAL), and marginal bone loss (MBL) were assessed. The GCF was collected, and its volume and levels of IL-4, IL-9, IL-10, and IL-13 were assessed. These parameters were evaluated at baseline and 3 months after SRP. The sample size was estimated, and comparisons between groups were performed. P<0.05 was considered to indicate statistical significance. Results: Thirty-six electronic cigarette users (47.7±5.8 years old) and 35 non-smokers (46.5±3.4 years old) with CP were included. At baseline, there were no differences in plaque index (PI), PD, CAL, MBL, and GCF IL-4, IL-9, IL-10, and IL-13 between electronic cigarette users and nonsmokers. At the 3-month follow-up, there were no significant differences in PI, gingival index (GI), PD, CAL, and MBL in electronic cigarette users compared to baseline, while there were significant reductions in PI, GI, and PD among non-smokers. At the 3-month follow-up, GCF IL-4, IL-9, IL-10, and IL-13 levels were significantly elevated in both groups (P<0.05) compared to baseline. The increases in GCF IL-4, IL-9, IL-10, and IL-13 levels were significantly higher in non-smokers (P<0.05) than in electronic cigarette users at the 3-month follow-up. Conclusions: Levels of GCF IL-4, IL-9, IL-10, and IL-13 increased after SRP in electronic cigarette users and non-smokers with CP; however, the anti-inflammatory effect of SRP was more profound in non-smokers than in electronic cigarette users.

Study on Acknowledge and State of Clinical Experience for 3-years Dental Technology Department (3년제 치기공과 임상실습에 대한 인식 및 실태조사 - 일부 치과기공소 소장을 중심으로 -)

  • This study was conducted to collect and analyze previous information in order to manage efficience, improve experience effect and promote employment rate. The questionnaire interview with 27 chief of dental Laboratory refered clinical experience in technology department about clinical experience in 14 Jumior colleges were also investigated. The results were summarried as follows : The portion of age of 35-39 among chief of dental Laboratory was 40.7% which was the highest, that of male was 96.3%, that of junior college graduate was 97.5%, that of 10years experience was 92.6% and that of ceramic technician was 85.2%, 63.0% dental laboratory for clinical experience was a bore space of 30pyong. Aspect of dental laboratory management, manufacturing all part of prosthetic restoration was 29.6%, othodontic appliance and ceramic restoration was 7.4%, 3.8%, each. The percentage of 40.7 was having connection with 30-3a dental clinics and referring case per day was 10-19 cases(40.7%), manufacturing time of referred prosthetic restoration was 3-4 days(77.8%), places preparing seminar room for education was 29.6%, above a place of 40pyong was 11.1% 30-34 pyong and 35-39 pyong was 7.4% each. During training of 2 years education course student, 18.5% was rack of thorough occupational career. While 44.4% will want the more salary among 3years education course student, 74.1% will expect the more dental techmicians would engaged in their field, 51.9% will hope improve of their theory and practice, 29.6% be expected better skill and 14.8% be expected better theory. Attitude of clinical experience places was distributed by 59.3% of offering only experience chance, 25.9% of wasting time and 29.0% of annoying. The big emphasis of climical experience was thorough occupational career(44.4%). The clinical experience places of our college were selected after direct visiting, so their condition of management was not that bad but most of dental laboratory were poor in management state and working environment. Therefore it is difficult to choose appropriate places and dental Laboratory are also limited manpower and time as suppliers. So that it recommended to induce flexible management of experience period by interval and rotation of experience places among college and to applicate intern-system for employment ant industry-college cooperation aspect.

Association between oral health status and oral health impact profile(OHIP-14) among the community elderlies (노인의 객관적 구강건강상태와 주관적 구강건강수준간의 관련성)

  • Objectives : This study was attempted in order to grasp oral health level according to socio-demographic characteristics in elders in some communities, and to evaluate oral health status and its association. Methods : The subjects in this study were performed with 235 people, who were over 65 years and resided in Daejeon Province, from June 20 to July 10, 2011. An individual interview was held, and they got a dental checkup. As for data analysis, chi-square test, t-test, one-way ANOVA, pearson correlation were utilized. Results : The older age in the whole research subjects and the lower educational level led to the less remaining teeth and the larger missing teeth index. The decayed missing filled teeth index and the decayed missing filled teeth rate were higher in more women and older age and in the lower educational level. Tooth mortality rate was higher in the older age, the lower educational level, and the group of living together with spouse. The maxillary-mandibular fixed-bridge status in the mouth was indicated to be the highest in the full-denture mounting ratio as for elders in over 80 years old. Oral Health Impact Profile(OHIP-14) average score was $56.05{\pm}11.64$ in the whole research subjects The decayed missing filled teeth index and the decayed missing filled teeth rate showed significantly positive correlation with the decayed missing filled teeth rate, tooth mortality rate and showed significantly negative correlation with OHIP-14. Tooth mortality rate showed significantly negative correlation with OHIP-14 Oral Health Impact Profile(OHIP-14) showed significantly positive correlation with its factors. Conclusions : Accordingly, the policy effort is considered to be necessary that implements in elders in order to spend active senescence, and that elders' health and oral-health behavior can be implemented continuously and preventively through classification according to elders' physical function.
