Clinical applcation of water laser (Er,Cr:YSGG) (물방울 레이저의 다양한 임상 적용)

  • Laser means "Light amplification by stimulated emission of radiation". Laser have unique characteristics according to wavelength. Wavelenth of Waterlase is 2780nm and it can be absorbed to water and hydroxyapatite. When laser is applied to some material, its temperature goes up due to laser's energy. But in dental treatment high temperature is not good for teeth. High temperature can make dental pulp and bone necrosis. Waterlase can be absorbed to water droplet, so when it burst, it can cut soft and hard tissue without raising temperature. so it is so proper to dental treatment.

  • Various sorts of dental devices for the dental treatment have been introduced and adopted during past several years. Dental Laser, among all devices, seems to be possible for applying to over the whole treatments and widely employed. Furthermore, this article is to introduce that Dental Laser is intended to care patients secured with the effectual treatment against Conventional procedures based upon its studies. The advantages of Dental Laser treatment-along with LLLT (Low Level Laser Therapy) effect, biostimulation effect, and minimal invasive technique - make the patients reduce their fear of the operation, lighten the inconvenience of post operation, and shorten the treatment period. In particular, it is worth considering that the use of laser-assisted therapies is associated with a marked reduction in the use of analgesics and anti-inflammatory medications compared with conventional procedures. This article is to state advantages and differences of Dental Laser treatment compared with Conventional procedures, and to emphasize to become well-acquainted with the precautions for safety and effective Dental Laser treatment. In case of operating Dental Laser with lack of the instructions, it will cause the unpredicted fatal results; therefore this treatment requires special care in its operation. Henceforward, it is anticipated that infinite treatment protocols will be introduced by applying Dental Laser, and this is to address the utilization of Dental Laser.

  • LASER application has many advantages in the field of dentistry, however, it is not easy to apply dental LASER in general practice. Various LASER systems are in the market and it is little bit confused which LASER systems are useful. Most of all, it is important to select the appropriate LASER system to their own usage. In the present article, I introduce several LASER system such as $CO_2$, Diode, Nd:YAG, Er:YG, Er,Cr:YSGG, and its application according to specific disease criteria.

  • Currently dental laser is used at a variety of dental treatment and many favorable clinical case reports are presented. We performed this study for the presentation of the validity of dental laser which are available and supportable in the oral and maxillofacial surgery. From October, 1998 to March, 1999, dental laser was used in 70 cases. Male patients were 38, female 32. The age ranged from 5 to 77, with a mean of 45.4. Indicated cases were 18 chronic periodontitis, 11 infections, 6 atypical pain, malignant or benign tumor, precancerous lesion, abnormal frenum, TMJ disorder, mucocele, and so forth. $CO_2$ laser(Sharplan 15F, Sharplan Co.) was used in 40 cases, Ho-YAG laser(Duopulse 2000, Excel Co.) 11, Nd-YAG(Duopulse 2000, Excel Co.) 18. We present and discuss about the objectiveness, availability, and future application of laser with case reports.

  • Objectives : This study was progressed a research with the aim of using it as basic data necessary for a plan in order to increase qualitatively medical level for patients who feel anxiety about dental treatment, and of offering basic data for improving the whole medical services by grasping conditions and satisfaction level with laser therapy in patients who visited dental clinic. Methods : The statistical analysis was carried out t-test and ANOVA by using SPSS(PASW statistics) 18.0. When there is statistically significant difference in analysis of variance, post-test was carried out by using Duncan multiple range test. Results : In therapy kinds of using laser, gingival treatment was the largest with 37.0%. It was the highest in likely seeming to be not painful with 32.9% as for a decisive factor, which came to receive laser therapy, and in having been enough to endure even if having been slightly tingling as for pain level. Conclusions : This study can be considered to have significance in changing recognition of medical cure by surveying patients satisfaction with using laser as a method for slightly solving elements of fear and anxiety about dental treatment and to have its value as basic data for seeking improvement plan for diverse medical services in the future.

A comparison of bone bed preparation with laser and conventional drill on the relationship between implant stability quotient (ISQ) values and implant insertion variables

  • PURPOSE. The aim of this study was to investigate a comparison of implant bone bed preparation with Er,Cr:YSGG laser and conventional drills on the relationship between implant stability quotient (ISQ) values and implant insertion variables. MATERIALS AND METHODS. Forty implants were inserted into two different types of pig rib bone. One group was prepared with conventional drills and a total of 20 implants were inserted into type I and type II bone. The other group was prepared with a Er,Cr:YSGG laser and a total of 20 implants were inserted into type I and type II bone. ISQ, maximum insertion torque, angular momentum, and insertion torque energy values were measured. RESULTS. The mean values for variables were significantly higher in type I bone than in type II bone (P < .01). In type I bone, the ISQ values in the drill group were significantly higher than in the laser group (P < .05). In type II bone, the ISQ values in the laser group were significantly higher than in the drill group (P < .01). In both type I and type II bone, the maximum insertion torque, total energy, and total angular momentum values between the drill and laser groups did not differ significantly (P ${\geq}$ .05). The ISQ values were correlated with maximum insertion torque (P < .01, r = .731), total energy (P < .01, r = .696), and angular momentum (P < .01, r = .696). CONCLUSION. Within the limitations of this study, the effects of bone bed preparation with Er,Cr:YSGG laser on the relationship between implant stability quotient (ISQ) values and implant insertion variables were comparable to those of drilling.

Nuclear Transfer by Using a Laser-Assisted Zona Pellucida Piercing Technique in Mice

  • Somatic cells nuclear transfer (SCNT) is a useful tool in studies of developmental biology and animal cloning. However, SCNT experiments only are allowed to skilled technical experts. In this experiment, laser-assisted zona pellucida piercing tool (LASER) was applied in murine SCNT. LASER minimized the use of piezo-driven micromanipulator (PIEZO), reducing chances of problems caused by PIEZO pulses. LASER reduced time that took to pierce zona pellucida in removal of nucleus from oocyte and somatic cell injection, which might have taken longer time with PIEZO. Time and difficulties that took researcher of equivalent skilled for their experiments were decreased with LASER, and this might affect the improvement of embryonic development. (LASER, 6.2% versus PIEZO, 2.9%; P<0.05). Thus, these data support that the use of LASER can be used for zona pellucida piercing in murine SCNT program as an alternative to PIEZO.

Various clinical use of laser in esthetic treatment (심미치료에 레이저를 활용한 다양한 임상적용)

  • The most valuable site of laser treatment is analgesic effect and fast healing process. If we understand an use this advantage correctly, it will helpful in relationship with patients and hospital management. So this study reported clinical cases about laser used in esthetic treatment with literature review. Periodontal treatment using laser was taken for patient who complain about gingival swelling during orthodontic treatment. And, esthetic plastic treatment using laser for alveoar reduction was taken for patient who complained about esthetic problem of maxillary gingiva. The treatment using laser can shorten the inconvenience and complicated treatment process and minimize patient's fear and discomfort during treatment procedure. So patients and clinicians can expect competent result in clinical cases. The laser for esthetic treatment was valuable.

  • Pulsed Nd : YAG LASER has been applied to various fields in clinical dentistry including the treatment of peri-implantitis. However, LASER can affect properties of matter of dental implants which are important to maintaining the health of peri-implant tissue and can raise its temperature during lasing. So there have been warings of using LASER to treat peri-implantitis. But, the effects of laser on dental implants itself are not certain yet. So we measured the temperature rising, examined matter of properties by SEM and EDX before and after pulsed Nd : YAG lasing various intensity. 7 TPS implants and 7 HPS implants were used and pulsed Nd : YAG LASER was used in 0.3W, 1.0W, and 2.0W. 1. 2.0W LASER made polished neck portion of HPS implants reach $39.2^{\circ}C$ after 5 seconds lasing. 2. LASER made crater-like defects on plasma sprayed surface and surfaces were melted and divided by fragments after lasing. 3. There was no specific evidence of element change after lasing.

  • Purpose: The laser processability of dental prosthesis is investigated using two ceramic composites, including 3M, Lava Ultimate and Ivoclar vivadent, IPS e.max. Materials and methods: The $CO_2$ laser, picosecond laser and femtosecond laser are used to assess the processing power of dental prosthetic materials Lava Ultimate and IPS e.max and the line processing shape was measured using a confocal microscope. Results: The brittleness, carbonization and micro crack of the ceramic composite were influenced by heat accumulation of the material and could be controlled by the laser power and pulse time. Conclusion: In the case of $CO_2$ lasers, micro crack and carbonation occurred immediately, and in the picosecond laser processing, the micro cracks are partially improved, but the carbonization occurs continuously. Finally, we confirmed the high efficiency of laser processing with femtosecond laser. In particular, Lava Ultimate, a ceramic resin composite material, showed the best processability when processed using a femtosecond laser.
