Evaluation of the periodontal and pulpal healing of replanted rat molars with doxycycline root conditioning

  • Purpose: This study aimed to evaluate periodontal and pulpal healing in replanted rat teeth, preserved under different storage conditions, with or without root conditioning using doxycycline. Methods: A total of 40 maxillary first molars extracted from 20 Sprague-Dawley rats were stored for different durations under different conditions (5 minutes in dry storage and 60 minutes in Hank's balanced salt solution [HBSS]) and subsequently replanted. The rats were divided into 2 groups based on the use of root surface treatment: the doxycycline group (root surface treated with doxycycline) and the control group (no doxycycline treatment). Eight weeks after replantation, the animals were sacrificed, and the teeth were evaluated using micro-computed tomography (micro-CT) and histomorphometric analysis. Results: In the micro-CT analysis, the doxycycline group showed the same rate of occurrence of periapical radiolucency as was observed in the control group, but a lower degree of root resorption in teeth replanted after 60 minutes of storage in HBSS (P<0.05). In the histomorphometric analysis, the doxycycline group exhibited no improvement in either pulpal or periodontal healing of the replanted tooth after 5 minutes of dry storage, but showed a lower grade of surface root resorption ($1.37{\pm}0.77$) and inflammatory resorption in the teeth stored for 60 minutes in HBSS ($1.33{\pm}0.71$). Conclusions: In conclusion, doxycycline improved the periodontal healing of replanted teeth stored for 60 minutes in HBSS, whereas doxycycline did not improve periodontal healing of replanted tooth after 5 minutes of dry storage. Within the limits of this study, doxycycline showed more favorable periodontal healing despite delayed replantation.

병원 시설 파견·용역 근로자들의 주관적 구강 증상에 영향을 미치는 직무 위험 요인에 관한 연구 (Occupational risk factors influencing subjective oral symptoms in hospital facility Temp·Contract Workers)

  • 본 연구목적은 병원 시설 근로자들의 구강 증상에 영향을 미치는 업무 위험요인을 살펴보고자 한다. 연구 방법은 수도권 지역의 대학병원 시설 관리 파견·용역 근로자 627명을 대상으로 2020년 11월 17일~2021년 5월 20일까지 설문조사를 실시하였다. 연구결과 근골격계 질환과 상해 위험이 높은 근로자들에게서 구강 증상이 높게 나타났다. 또한 직무 불안정과 조직 불공정성의 영역에서 구강점막, 악관절 장애, 구강건조증 증상이 높게 나타났으며, 이직 의도와 질병 출근 요인에서 구강건조증 위험도가 가장 높게 나타났다. 근로자들의 업무 위험 요인과 직무스트레스가 증가할수록 구강 증상 위험도가 증가하였다. 추후 병원 시설 근로자들의 구강건강 증진을 위한 구강보건정책 개선 기초 자료로 활용될 것이다.

Successful Carapace Puncture Wound Repair with Polymethyl Methacrylate (PMMA) in an Amur Softshell Turtle (Pelodiscus maackii)

  • An Amur softshell turtle with multiple shell injuries was admitted to the Seoul Wildlife Center on 19 May 2021. The most severe lesion was a puncture wound requiring urgent closure. In addition to routine supportive therapy, the damaged shell was patched with biocompatible polymethyl methacrylate (PMMA) materials (bone cement and dental acrylic) and fiberglass. Despite a few methods to repair the carapace or plastron of hard-shelled turtles, shell repair in the Amur softshell turtle has rarely been reported. This paper reports the repair process of a puncture wound in the carapace of a softshell turtle using polymethyl methacrylate (PMMA). PMMA is a biocompatible acrylic polymer that forms a tight structure that holds the implant against tissue defects, such as skin, bones, and dentures. Fiberglass, a preferred fiber in various medical fields, was used with PMMA to provide extra strength and waterproof capability. After the procedure, there were no signs of edema, inflammation, bleeding, skin discoloration, or any other complications. Accordingly, this can be a method of choice in softshell turtles using biocompatible materials to cover the lesion in the carapace and provide appropriate wound management, supportive therapy, and a suitable course of antibiotics considering all other circumstances.

교정적 치아초기배열단계시 발생하는 치근 변화에 관한 주사전자현미경적 연구 (ROOT SURFACE CHANCES DURING THE INITIAL LEVELING AND ALIGNMENT PHASE IN ORTHODONTICS: A SCANNING ELECTRON MICROSCOPIC STUDY)

  • 본 연구는 교정치료를 목적으로 발치가 예정된 치아를 대상으로 임상적으로 매우 중요한 치료과정 중의 한 부분인 초기치아배열단계에서 다양한 기간과 크기의 교정력을 가하여 치아배열기간, 교정력의 크기,치아이동방향에 따라 치근표면에 일어나는 흡수와 회복 양상에 대한 정보를 얻기 위하여 시행되었다. 치료받지 않은 13개의 치아를 대조군으로 하고 교정치료계획상 제1소구치를 발거하기로 예정된 37개의 치아를 실험군으로 하여 20-450gm의 초기교정력을 가한 후 주사전자현미경적 관찰을 통하여 다음과 같은 결과를 얻었다. 1. 실험군 37개 중 22개의 치아($59.5\%$)에서 치근흡수가 발견되었다. 치근흡수는 최소실험기간인 3주군부터 나타났으며 흡수부위의 회복은 3개월군부터 나타났다. 2. 교정력의 기간 증가에 따라 치근흡수의 발생빈도가 증가하는 경향을 보였다. 3. 교정력이 클수록 상아질을 포함하는 홉수가 증가하는 경향을 보였다. 4. 치근 흡수의 부위는 하중 받는 부위와 일치하는 경향을 보였다. 5. 치근 흡수 부위의 면적과 가장 상관성이 높은 변수는 교정력의 크기였다.

상악 유절치의 3가지 유형의 치근 흡수면에 관한 병리조직학적 관찰 (Histopathological Observation of Three Types of Root Resorption Surface in Maxillary Primary Incisors)

  • 전신적 또는 국소적 요소에 의하여 유치 치근이 병적으로 흡수될 수 있으며 치아 우식증이나 외상 등으로 염증성 치근 흡수가 발생할 수 있다. 유치의 병적 치근 흡수면은 생리적인 치근 흡수와 비교할 때 흡수면의 세포와 흡수와의 형태 등의 흡수 양상이 다르다. 이 연구는 생리적 치근 흡수, 외상, 염증성 치근 흡수면의 형태 및 인접한 세포를 주사전자현미경과 광학현미경으로 관찰하였다. 생리적 치근 흡수면의 흡수소와는 원형과 타원형이고 비교적 작고 균일하였다. 외상으로 인한 치근 흡수면과 염증성 치근 흡수면의 흡수소와는 생리적 치근 흡수면의 흡수소와에 비해 다각형이며 크고 불규칙하였다. 생리적 치근 흡수면과 외상으로 인한 치근 흡수면, 염증성 치근 흡수면에서 다핵거대세포와 단핵세포가 관찰되었으며 염증성 치근 흡수면에서는 간엽세포 및 염증세포가 많이 분포되어 있었다. 생리적 치근 흡수면과 외상으로 인한 치근 흡수면에서는 흡수 표면에 백악질양 조직이 침착되었으나, 염증성 치근 흡수면에서는 관찰되지 않았다.

Distribution of the lingual foramina in mandibular cortical bone in Koreans

  • Objectives: The interforminal region, between the mandibular foramen, is known as a relatively safe area that is free of anatomic structures, such as inferior alveolar nerve, submandibular fossa, and lingual side of the mandible is occasionally neglected for its low clinical importance. Even in the case of a severely constricted alveolus, perforation of the lingual cortical bone had been intended. However, anterior extension of the inferior alveolar canal, important anatomic structure, such as concavity of lingual bone, lingual foramina, and lingual canal, has recently been reported through various studies, and untypical bleeding by perforation of the lingual plate on implantation has also been reported. Therefore, in this study, we performed radiographic and statistical analysis on distribution and appearance frequencies of the lingual foramina that causes perforation of the mandibular lingual cortical bone to prevent complications, such as untypical bleeding, during surgical procedure. Materials and Methods: We measured the horizontal length from a midline of the mandible to the lingual foramina, as well as the horizontal length from the alveolar crest to the lingual foramina and from the lingual foramina to the mandibular border by multi-detector computed tomography of 187 patients, who visited Dankook University Dental Hospital for various reasons from January 1, 2008 to August 31, 2012. Results: From a total of 187 human mandibles, 110 (58.8%) mandibles had lingual foramina; 39 (20.9%) had bilateral lingual foramen; 34 (18.2%) had the only left lingual foramen; and 37 (19.8%) had the only right lingual foramen. Conclusion: When there is consistent bleeding during a surgical procedure, clinicians must consider damages on the branches of the sublingual artery, which penetrate the lingual foramina. Also, when there is a lingual foramina larger than 1 mm in diameter on a pre-implantation computed tomography, clinicians must beware of vessel damage. In order to prevent these complications and progress with a safe surgical procedure, a thorough radiographic examination before the surgery is indispensable. Further, clinicians should retract lingual flap definitely to confirm the shape of the lingual bone and existence of the lingual foramina.

Maxillofacial Trauma Trends at a Tertiary Care Hospital: A Retrospective Study

  • Purpose: Maxillofacial fractures are rapidly increasing from car accidents, industrial accidents, teenaged criminal activity, and sports injuries. Accurate assessment, appropriate diagnosis, and preparing individual treatment plans are necessary to reduce surgical complications. We investigated recent trends of facial bone fracture by period, cause, and type, with the objective of reducing surgical complications. Methods: To investigate time trends of maxillofacial fractures, we reviewed medical records from 2,196 patients with maxillofacial fractures in 1981~1987 (Group A), 1995~1999 (Group B), and 2008~2012 (Group C). We analyzed each group, comparing the number of patients, sex ratio, age, fracture site, and etiology. Results: The number of patients in each period was 418, 516, and 1,262 in Groups A to C. Of note is the increase in the number of patients from Group A to C. The sex ratios were 5.6:1, 3.5:1, and 3.8:1 in Groups A, B, and C. The most affected age group for fracture is 20~29 in all three groups. Traffic accidents are the most common cause in Groups A and B, while there were somewhat different causes of fracture in Group C. Sports-induced facial trauma was twice as high in Group C compared with Group A and B. Mandible fracture accounts for a large portion of facial bone fractures overall. Conclusion: We observed an increase in facial bone fracture patients at Kyungpook National University Dental Hospital over the years. Although facial injury caused by traffic accidents was still a major cause of facial bone fracture in all periods, the percentage decreased. In recent years, isolated mandible fracture increased but mandible and mid-facial complex fracture decreased, possibly because of a reduction in traffic accidents.

Lesch-Nyhan 증후군 환아의 자해에 의한 혀의 외상 (TONGUE INJURIES BY SELF MUTILATION IN LESCH-NYHAN SYNDROME PATIENT: A CASE REPORT)

  • Lesch-Nyhan 증후군은 드물게 나타나는 X 염색체 열성 유전 질환으로 purine의 대사 장애를 보이는 질환이다. Purine의 대사 장애는 대사에 관여하는 효소인 hypoxanthine guanine phosphoribosyl transferase의 결손 또는 활성 감소로 인하여 나타나며, 요산의 과다 생산에 의한 요산과다혈증, 요산과다뇨증 등의 신장계 증상과 정신지체, 무도증 등의 신경학적 장애를 보이고, 구강이나 손가락에 대한 자해를 나타낸다. 이 중 자해 행위는 입술과 혀, 손가락을 깨무는 경우가 흔하고, 심하면 혀와 손가락이 완전히 절단될 수도 있다. 이 경우 자해로 인한 통증뿐 아니라, 연조직 결손으로 인한 심미적인 문제와 상처 부위로의 2차 감염이 발생할 수 있다. 이러한 Lesch-Nyhan 증후군 환자에서 자해를 막기 위해 여러 방법이 사용되고 있는데, 이 중 치과적으로 접근하는 방법으로는 마우스 가드나 lip bumper 등의 장치를 이용하는 방법과 해당 치아들을 모두 발거하는 방법, 악교정 수술을 통해 개방 교합을 형성해 주는 방법 등이 있다. 본 증례는 13세 9개월의 Lesch-Nyhan 증후군 환아로서 자해에 의해 혀에 심각한 외상을 가한 경우로 마우스 가드를 이용한 보존적인 방법을 사용하였으나 계속적으로 혀에 외상을 가해 하악 전치 뿐 아니라 구치까지 발거하여 치료하였다.

일부(一部) 농촌지역(農村地域) 주민(住民)의 이병(罹病) 및 진료실태(診療實態)에 관한 조사(調査) - 충남(忠南) 서산군(瑞山郡) 삼화의료보험조합(三和醫療保險組合) 대상지역(對象地域)- (A Study on the Status of Morbidity and Medicare in a Korean Rural Area - Area under Sam-Wha Medicare Insurance Union -)

  • To investigate the status of morbidity and medicare utilization during last 8 months from 1 st Oct. 1977 to 31th May, 1978 in the area under Sam-Wha Medicare Insurance Union, a study was carried out through analyzing the medicare records of patients who are enrolled. For the study, 3 doctors and one nurse were mobilized and the results are as follows: 1) The total number of the Medicare insurance Union members in the study area were 5,735 composed of 3,000 males(52.3%) and 2,735 females(47.7%). 2) The total number of patients were 1,405 composed of 783 males (55. 0%) and 622 females(45. 0%) and the incidence rate per 1,000 population was 245.0 of total(261.0 in males and 227.4 in females). 3) Five major diseases with 52, 7% of total patients were acute upper respiratory infection(20.7%), peptic ulcer(12.2%), bronchitis(5.5%), injuries(5.2%) and dental problems(5.1%). 4) The order of the incidence rate of age group per 1,000 population was the year group on 0-4(342.6), 25-44(312.7), 45-64(307.0), 65 and over(240.3), 15-24(178.8) and 5-14(164.8). 5) Of the 1,405 total patients, the out-patients were 1,661(96.9%) and the in-patients were 44(3.1%) and the ratio wae 30.9 : 1.0. 6) Among the out-patients 96.7% of them were cared in primary medicare facilities, 1.0,% in secondary care, and 2.3% in tertiary care. And among the in-patients 50. 5% of them were cared in primary medicare facilities, 4. 5% in secondary care, 45. 5% in tertiary care. 7) Duration of medicare was concentrated within a week in 84. 3% of total patients.

흉총창에 의한 심방파열 치험 2례

  • We have experienced 2 cases of the hunshot wound sof the chest involving cardiac injuries at department of the thoracic surgery, Capital Armed Forces General Hospital during I year from April I 1979 to Jan. 1980. In one case of two patients , he was a 22 years old man who was transported to this emergency room 4 hour 10 minutes after having gunshot wound of the left chest by helicopter. Physical examination showed small inlet in left 3rd ICS and left parasternal border, large outlet in left 8th ICS and left scapular line, no breath sound on left side and distant heart sound. chest roentgenography demonstrated marked pleural effusion in left side and mediastinum shifted to right. As soon as chest X-ray was taken, the bleeding through penetrating wound became profuse and cardiac arrest ensued. Closed chest cardiac massage was started and vigorous transfusion continued, but no effective cardiac activity could not be obtained. The patient was pronounced dead due to exsanguinating hemorrhage from wuwpected cardiac wounds. In this critically injured patient with evidence of intrathoracic hemorrhage and suspected cardiac penetration, only emergency thoracic exploration and immediate surgical control of bleeding points might offer the maximum possibility of survival. The other case was a 23 years old man who was transferred to the emergency room 4 hours 50 minutes after having kmultiple communicated fractures of sternum and linear fracture of right mandible by a missile. Examination revealed about 30% skin loss of the anterior chest wall, weak pulse of 96 beats/min., distant heart sound and decreased breath sounds bilaterally. finding on the chest X-ray films showed multiple sternal fractures, marked pericardial effusion indicating hemopericardium. So, the patient was moved immediately to the operation room where, after endotracheal tube inserted, a median sternotomy was performced. A hemorrhagic congestion of the right upper lobe and marked bulging pericardium were disclosed. The pericardium was opened anterior to right phrenic nerve and exsanguinating hemorrhage ensued from the 0.5cm lacerated wound in the auricle of right atrium. The rupture site of right atrium was occluded with non-crushing vascular clamps and then was over sewn with interrupted sutures. It was thought to be highly possible that he was alive long enough to have cardiorrhaphy because of cardiac tamponade, which prevented exsanguinating hemorrhage. He was taken closed reduction for linear fracture of right mandible 2 weeks after repair of ruptured right auricle in dental clinic. This patient's post-operative course was not eventful.
