Antimicrobial surfaces for craniofacial implants: state of the art

  • In an attempt to regain function and aesthetics in the craniofacial region, different biomaterials, including titanium, hydroxyapatite, biodegradable polymers and composites, have been widely used as a result of the loss of craniofacial bone. Although these materials presented favorable success rates, osseointegration and antibacterial properties are often hard to achieve. Although bone-implant interactions are highly dependent on the implant's surface characteristics, infections following traumatic craniofacial injuries are common. As such, poor osseointegration and infections are two of the many causes of implant failure. Further, as increasingly complex dental repairs are attempted, the likelihood of infection in these implants has also been on the rise. For these reasons, the treatment of craniofacial bone defects and dental repairs for long-term success remains a challenge. Various approaches to reduce the rate of infection and improve osseointegration have been investigated. Furthermore, recent and planned tissue engineering developments are aimed at improving the implants' physical and biological properties by improving their surfaces in order to develop craniofacial bone substitutes that will restore, maintain and improve tissue function. In this review, the commonly used biomaterials for craniofacial bone restoration and dental repair, as well as surface modification techniques, antibacterial surfaces and coatings are discussed.

Acute Fascial Space Abscess upon Dental Implantation to Patients with Diabetes Mellitus

  • As popularity of dental implantation is increasing, the number of cases associated with complications also increase. Evaluation on diabetes mellitus is often neglected due to the disease's irrelevance to implantability. However, patients with diabetes mellitus are susceptible to infection due to impaired bactericidal ability of neutrophils, cellular immunity and activity of complements. Due to this established connection between diabetes mellitus and infection, a couple of cases were selected to present patients with diabetes mellitus with glycemic incontrollability, suffering from post-implantation dentigerous inter-fascial space abscess.

감염 조절용 차단막의 두께가 광중합기의 중합광에 미치는 영향 (Effect of infection control barrier thickness on light curing units)

  • 연구목적: 본 연구는 감염 조절용 차단막을 여러 겹으로 사용했을 때 광중합기의 광강도와 파장, light diffusion 등에 미치는 영향에 대해 조사하였다. 연구 재료 및 방법: 감염 조절용 차단막은 투명 랩 (크린랩)을 사용하였고 광중합기는 할로겐 광중합기 (Optilux 360)와 LED 광중합기 (Elipar FreeLight 2)를 사용하였다. 차단막을 1겹, 2겹, 4겹, 8겹으로 광중합기의 광섬유말단을 감싸고 휴대용 광강도 측정기 (Cure Rite)로 광중합기의 광강도를 측정하였다. 광중합기를 주문제작한 optical breadboard에 고정시킨 후 휴대용 spectroradiometer (CS-1000)를 이용하여 광중합기의 파장을 측정하였고, DSLR (Nikon D70s)을 이용하여 광중합기의 light diffusion을 사진 촬영하였다. 결과: 광강도 측정 결과는 차단막의 두께가 증가할수록 광강도가 유의하게 감소하였으나 할로겐 광중합기에서 1겹과 2겹 사이에는 유의차가 없었으며, 4겹 이상의 차단막을 투과할 때 광강도가 더 많이 감소하였다. 여러 겹의 차단막을 투과한 광중합기의 전체적인 파장 형태와 peak wavelength의 변화는 관찰되지 않았다. Light diffusion 사진 촬영 시, LED 광중합기에서는 차단막의 두께가 미치는 영향이 없었으나 할로겐 광중합기에서는 차단막을 4겹 사용했을 때부터 중합광이 조사되는 각도가 감소하기 시작하여 8겹 사용했을 때 통계적으로 유의하게 감소하는 것을 볼 수 있었다 (p < 0.05). 결론: 광중합형 복합레진을 광중합할 경우 감염 조절용 차단막이 찢어지는 경우를 대비하여 1겹으로 사용하기 보다는 2겹으로 사용하는 것이 환자간의 교차감염을 예방하는데 유리할 것으로 사료된다.

Fusobacterium nucleatum infection induces CSF3 expression through p38 MAPK and JNK signaling pathways in oral squamous cell carcinoma cells

  • Oral bacterial infections substantially affect the development of various periodontal diseases and oral cancers. However, the molecular mechanisms underlying the association between Fusobacterium nucleatum (F. nucleatum ), a major periodontitis (PT)-associated pathogen, and these diseases require extensive research. Previously, our RNA-sequencing analysis identified a few hundred differentially expressed genes in patients with PT and peri-implantitis (PI) than in healthy controls. Thus, in the present study using oral squamous cell carcinoma (OSCC) cells, we aimed to evaluate the effect of F. nucleatum infection on genes that are differentially regulated in patients with PT and PI. Human oral squamous cell carcinoma cell lines OSC-2O, HSC-4, and HN22 were used. These cells were infected with F. nucleatum at a multiplicity of infection of 100 for 3 hours at 37℃ in 5% CO2. Gene expression was then measured using reverse-transcription polymerase chain reaction. Among 18 genes tested, the expression of CSF3, an inflammation-related cytokine, was increased by F. nucleatum infection. Additionally, F. nucleatum infection increased the phosphorylation of AKT, p38 MAPK, and JNK in OSC-20 cells. Treatment with p38 MAPK (SB202190) and JNK (SP600125) inhibitors reduced the enhanced CSF3 expression induced by F. nucleatum infection. Overall, this study demonstrated that F. nucleatum promotes CSF3 expression in OSCC cells through p38 MAPK and JNK signaling pathways, suggesting that p38 MAPK and JNK inhibitors may help treat F. nucleatum-related periodontal diseases by suppressing CSF3 expression.

경부심부감염의 임상적 고찰 (A Clinical Study of Deep Neck Infection)

  • Background and Objectives: Deep neck infections, which affect soft tissues and fascial compartments of the head and neck and their contents, have decreased after the develop ment of chemotherapeutic agents and antibiotics. However they may still result in significant morbidity and mortality despite the use of chemotherapeutic agents and antibiotics. Materials and Methods : A retrospective study was performed on 66 deep neck infections in patients admitted for diagnosis and treatment at Asan medical center from June 1994 to December 2000. Results : Age of the patients varied from 1 to 86-year-old and sex ratio of male to female was 1.2:1. Most frequently involved site was submandibular space (21.2%). Most common cause of infection was dental disease (28.8%). The isolated pathogenic organisms were Streptococcus species in 19 cases, Staphylococcus species in 7 cases, Klebsiella in 5 cases, mixed infection of Staphylococcus and Klebsiella in 3 cases and a case of Corynebacterium. 51 cases were treated surgically, 15 cases were medically. Mean duration of admission was 9.6 days in cases of single space infection, 17.5 days in multiple spaces, 8.1 days when the infection resulted in cellulitis, 13.4 days in abscess, 7.9 days when the infection treated medically and 13.4 days when treated surgically. Conclusion Early diagnosis and treatment is important to manage deep neck infection and the duration of admission was increased when the infection involved multiple spaces.

장애아동 전신마취에 대한 Critical Pathway적용 (CRITICAL PATHWAY ON GENERAL ANESTHESIA FOR DISABLED CHILDREN)

  • A critical pathway (CP) defines the optimal care process, sequencing and timing of intervention by multi-disciplinary health care teams for a particular diagnosis and procedure. It plays an important role as a cost-effective health care delivery system and a tool for quality control of medical and dental services by means of standardizing medical practices. The aim of this study is to investigate the satisfaction of patients and medical/dental staff after implementation of a critical pathway for dental treatment of disabled children under general anesthesia and its cost effectiveness. Ten patients who underwent dental treatment under general anesthesia were included in the CP group between August and December 2006. The pre-CP group included 20 patients who underwent the same procedure from February 2003. The satisfaction of parent of child patient and medical staff members were compared between two groups. The parents' satisfaction was significantly improved after the implementation of CP and medical/dental staff members were highly satisfied with the usefulness of the critical pathway. In conclusion, the critical pathway for the dental treatment of disabled treatment under general anesthesia can highly improve the satisfaction of parents and medical/ dental staff members.

일부 치위생과 학생들의 교육수혜 및 임상실습에 따른 B형간염에 대한 이론적 지식의 관련성 (Relationship between Knowledge about Hepatitis B among Dental Hygiene Students Received Education and Clinical Practice)

  • Objective: The purpose of this study was to obtain the basic data for the establishment of the curriculum from the analysis of knowledge about hepatitis B among dental hygiene students. Methods: Data was collected from the students of two colleges majoring in dental hygiene in Daegu, using questionnaire. They were the students of the 1st, 2nd, and 3 grade, in total 390 student. The analysis was made using t-test, (one-way ANOVA), and multiple linear regression analysis. These were conducted using spss 14.0 version Results: Hepatitis B in the theoretical knowledge 'yes' if the answer 'When one of the family member is contaminated with Hepatitis the other members have to take the physical examination and vaccination against Hepatitis' to 86.7 % was the highest. The educational experience(p<0.01) and hepatitis B vaccination(p<0.001) of the mean difference between the groups investigated were statistically significant. Third grade, the higher the knowledge of hepatitis B was found to be lower relationship showed a statistically significant negative(p<0.05), clinical experience, the more experience, the more hepatitis B vaccinations were higher statistical knowledge relationship showed a significant positive(p<0.01). Conclusion: Currently dental hygiene curriculum is offered as a regular dental infection control subject, but more specific and systematic infection prevention subjects are required through continuing education program. In addition, for the admission of the students majoring in dental hygiene hepatitis B vaccination plan should be mandatory.

  • Hyperventilation is defined as ventilation in excess of that required to maintain normal blood $PaO_2$ and $PaCO_2$. It is produced by several distinct causes: anxiety, respiratory alkalosis, increased blood catecholamine levels, and a decrease in the level of the ionized calcium in the blood. The dental fears about acute pain, needle, drill and dental surgery lead to the severe anxiety and increased blood catecholamine level. Therefore, the most dental patient should be cared gently as the stress reduction protocol. In spite of the gentle care, two cases of hyperventilation were occurred during local anesthesia for incision and drainage of acute odontogenic infectious lesions. We suggest that the dental patients with acute odontogenic infection must be attention for the manifestation of hyperventilation, especially in the medically compromised conditions.

사람 치은 섬유아세포에서의 Tannerella forsythia 전세균, 막단백질, 당지질에 의한 염증성 사이토카인 발현 (Pro-inflammatory cytokine expression in human gingival fibroblasts by Tannerella forsythia whole bacteria, membrane proteins, and lipopolysaccharide)

  • Purpose: The purpose of this study was to investigate induction of cytokine expression in human gingival fibroblasts (HGFs) by whole cell and the components of T. forsythia. Material and Methods: After HGFs were treated with lipopolysaccharide (LPS), membrane protein isolated from T. forsythia or culture media of T. forsythia, the induction of interleukin (IL)-1, IL-6 and IL-8 was examined with real-time PCR and ELISA. Their induction ability of cytokines was compared with whole bacteria. Result: The expression of IL-6 and IL-8 was significantly induced in HGFs by whole bacteria and membrane protein. The expression of IL-$1{\beta}$ was induced by membrane protein of T. forsythia, not by whole bacteria. LPS and condition media of T. forsythia slightly activated HGFs. Conclusion: The membrane protein of T. forsythia could be one of virulence factors.

유니폼의 감염인식, 세탁 규정 인식, 감염병 예방 의도에 관한 융합연구: 개인주의, 집단주의, 자아존중감 중심으로 (Convergence Research on Infection Awareness of Uniforms, Recognition of Laundry Rules, and Intention to Prevent Infectious Diseases: Focusing on Individualism, Collectivism, and Self-esteem)

  • 본 연구는 강원도 일개 대학 4년제 치위생학과 276명을 대상으로 2021년 11월 24일부터 11월 26일까지 구글 온라인설문을 통하여 연구하였다. 본 연구의 목적은 유니폼의 감염인식과 세탁 규정 인식, 감염병 예방 의도를 개인주의 및 집단주의 자아존중감을 통해 알아보는 것으로, 향후 감염병 시기에 학생의 감염 교육 방법 모색을 위해 시행되었다. 통계는 SPSS Statistics 24.0, AMOS 21.0을 사용하여 빈도 분석, 탐색적 요인분석, 확인적 요인분석, 신뢰도 분석, 구조방정식, ANOVA 분석을 시행하였다. 연구결과 유니폼 감염인식, 유니폼 세탁 규정 인식, 자아존중감, 개인주의, 집단주의 감염병 예방 의도의 모형이 적합함을 확인하였고(p<.001), 구조방정식 결과 자아존중감은 개인주의, 집단주의 감염병 예방 의도에 영향을 미치는 것으로 나타났다(p<.001). 집단주의는 유니폼 감염인식, 유니폼 세탁 규정 인식, 감염병 예방 의도에 영향을 미치는 것으로 나타나 감염예방을 위한 인식 변화에 자아존중감과 집단주의가 영향을 미치는 것을 확인하였다. 향후 치위생학과의 감염관리 교육시 집단주의를 고려한 유니폼 세탁 방법에서 활용이 가능할 것이다.
