Analysis on the development necessity of a safety health knowledge test for young children (유아 안전보건지식 측정도구 개발 필요성에 대한 분석)

  • The purpose of this study is to emphasize the importance of the safety and health ability to safely protect body in a risky situation in infancy, but due to the lack of a valid test tool for measuring infant safety knowledge, measure the safety and health knowledge of infants. In order to achieve the purpose of the study, opinions of education experts and early childhood education experts were collected, and suitable factors for measuring safety and health knowledge of children aged 3-5 years were extracted. The measurement questions developed based on this were surveyed on 186 children aged 3-5 years old to verify the relevance of the question composition and the reliability of the contents. In the distribution of the item response, values of 0.8 or less were deleted, and items with a correlation of r<.40 or less were also deleted, and the reliability test showed more than .70. After exploratory factor analysis, it was developed into 23 questions in 3 areas. The total score of safety and health knowledge for the final question and the degree of correlation were above .80. The tool for measuring child safety and health knowledge developed in this study was confirmed to be appropriate for item discrimination, item reliability, and content validity, and thus will be used as basic data for measuring child safety and health knowledge. After the early childhood safety and health education program is implemented, Further research should verify the effectiveness of the early childhood safety and health program.

PREVALENCE AND RISK FACTORS OF SEVERE EARLY CHILDHOOD CARIES IN PRESCHOOL CHILDREN IN IKSAN CITY (익산시 취학전 어린이의 중증 유아기 우식증 유병률과 위험요인)

  • Preschool children in Iksan city, 3-5 years old, were examined for their prevalence of severe early childhood caries. The case definition was 1 or more cavitated, filled, or missing (due to caries) smooth surfaces in primary maxillary anterior teeth. Feeding behavior and oral hygiene behavior were investigated by a questionnaire. The prevalence was 20.8% for 36-47 months, 23.6% for 48-59 months, and 30.8% for 60-71 months. Going to sleep with bottle beyond 1 year (P<0.05), immediate removal of bottle after child fell on sleep (P<0.01), beginning time of toothbrushing (P<0.01) had significant relationship with the prevalence of severe early childhood caries. Toothbrushing frequency of primary care provider and use of oral cleaning aids by primary care provider had a tendency to be related with the prevalence(P<0.10).

  • Purpose. Infant oral health care forms the basis of lifelong health, and the role of child care teachers in contributing to the quality level in the formation of correct habits at this time throughout the infant's health care and education is very important. The purpose of this study was to examine the relationship between the perceived oral health status of nursery teachers and the necessity of oral health education, and to emphasize that the oral health of teachers is closely related to the oral health of infants and young children. Methods. This study was conducted from May to June 2018 by surveying private nursery teachers in some parts of Gyeongsangbuk-do province by using the self-entry method. The research tools consisted of the general characteristics of subjects, subjective oral health status, oral health practice, oral health education awareness, and oral health education needs. We used shceffe as a post hoc test to compare the differences between groups after frequency analysis, independent t-test, and one-way ANOVA analysis. The linear regression analysis was conducted to determine the relationship between perceived oral health status, oral health practice and awareness, and oral health education needs. The statistical level was .05. Results. According to the results of this study, the oral health related practice according to the demographic characteristics is at the final education level, and the oral health related awareness is age, marital status and oral health education needs. The results showed statistically significant differences in final educational attainment and cohabitation. Regarding the relationship between perceived oral health status, oral health related practice and awareness, and oral health education needs There were significant differences in oral health practice(β =.329) and oral health education needs (β =.158). Conclusion. Based on the above results, the child care facility can emphasize the importance of the role of the child care teacher in the oral health care of infants, and the basic data for the establishment of the child care management program in the future by reflecting the role of the teacher in the development of the child care management program of the child care facility. A Study on the Motivation to Write Signing Advance Medical Directives (사전연명의료의향서 작성 동기에 관한 연구)

  • This study examined the patient's advantage and respect self-decision to protect human dignity and values, who are on the pathway of the hospice palliative care and death process. The study subjects were the elderly who had signed the advance medical directives at C Christian religious facilities in S region. The survey period was started on July 1st, 2019 and lasted for ten days. The study resulted in four topics and eight sub-topics on the motivation for preparing advanced medical directives. The four topics were 'for children', 'fear of pain', 'want to clear my life', 'felt the necessity', and the sub-topics were 'don't want to make a burden to children', 'don't want to make a worry of medical expenses', 'fear of pain', 'have experience of taking caring of painful death process', 'relaxed mind', 'importance of self-decision', 'have known it before but now decided', and 'it is the new information and decided'. This research is meaningful in that it can form the basis for improving well-dying education programs for the good death of the elderly and supplementing effective systems for preparing advance medical directives.

Study on validity of the National Examination based on smart device based test(SBT) by 7 types of healthcare occupation (보건의료직종별 SBT 국가시험체제에 대한 타당성 연구)

  • Purpose: The purposes of this research were to study the validity of the National Examination based on smart based test(SBT) by 7 types of healthcare occupation. Methods: We conducted a questionnaires survey among 1,219 people including 210 professors for all 7 types of healthcare occupation and 573 clinical specialists ; 436 students from random sampling participated and the reliability of the questionnaire was tested using Cronbach's alpha. Results: Professors from 7 types of healthcare occupation were investigated for their opinions on the introduction of the SBT system ; 65.9% of the professors answered positively on the introduction of the SBT National Examination System, while 40.1% of the clinical experts answered likewise. In particular, professors and the clinical experts recognized altogether that the multimedia questionnaire, being effective for the evaluation of clinical and practical competence of the subjects, enabling the evaluation of various areas and strengthening the school education in clinical practices, the 42.0% of the students from 7 departments answered that the future direction of the national test is in the SBT test system with significant difference (p=.000) found in students majoring in order of Emergency Medical Services, Radiology and Physical therapy, compared to students from other majors, showing the positive response to the SBT system. Conclusion: The results of this study comprehensively evaluated the positive recognition of development of multimedia items and SBT system in national examination of 7 types of healthcare occupation in professors, field specialists and students.

A study on the change in the perception of funeral culture among the elderly (노인의 장례문화 인식변화에 대한 연구)

  • The purpose of this study is to suggest the direction of the funeral culture industry suitable for the age of longevity and well-aging through a study on changes in the perception of funeral culture among the elderly. The survey of this paper was conducted by Gallup Korea, and the survey method was a 1:1 individual interview using a structured questionnaire. The survey was conducted from October 1 to October 25, 2021, targeting 127 elderly people over the age of 65 living in Seoul, Incheon, and Gyeonggi-do. The number of valid subjects was 110, and the sampling method used was allocation by sex/age/income level/religion. As a result of the study, as for the change in the perception of funeral culture among the elderly, women preferred cremation to men (p=0.034). It was investigated that the level of income (p=0.004) and religious status (p=0.020) had an effect on funerals while alive. According to the presence or absence of thoughts about death, there was a significant difference in the membership of the mutual aid product (p=0.008) and the intention to return to a religious organization (p=0.004) when a memorial service was held. The results of this study are expected to be usefully utilized in presenting the direction and policy of the funeral culture industry suitable for the age of well-aging in the future.

A Convergence Study on the Meaningful Activity Experience of the Elderly (노인의 의미 있는 활동에 관한 융합 연구)

  • The purpose of this study, a self-written questionnaire was conducted on 110 people aged 65 or older to analyze the factors of meaningful activities in elderly. As a research methods were student t-test and one-way ANOVA were conducted using the SPSS statistical program, and hierarchical regression analysis was performed to identify factors affecting meaningful activities of the elderly. As a result of the study, the highest score was 3.95±0.64 in "I think my work (activity) with my family is rewarding." As a result of hierarchical regression analysis, the factors affecting the meaningful activities of the elderly were 'resident' in both stages 1(β=-.308, p=.002), 2(β=-.330, p=<.001), and 3(β=-.281, p=<.001), and 'age(β=-.215, p=.026)' in the second stage, indicating that the factors affecting the meaningful activities of the elderly were 'resident' and 'age'. In conclusion, it's necessary to develop and apply a systematic program that prioritizes conversation and communication while working with families for the younger elderly(65-74). In the future, it will be necessary to systematically apply various programs for meaningful activities in old age.

Enhanced Antioxidative Potential by Silymarin Treatment through the Inductionof Nrf2/MAPK Mediated HO-1 Signaling Pathway in RAW 264.7 Cells (RAW 264.7 세포에서 Nrf2/MAPK 의 활성을 통한 HO-1 과발현에 의한 silymarin의 항산화 효과)

  • Silymarin, which is derived from dried Silybum marianum (milk thistle) seeds and fruits, possesses various beneficial properties, such as hepatoprotective, antioxidative, anti-inflammatory, and anticancer activity. This research aimed to explore the antioxidative activity of silymarin against oxidative stress and understand its molecular mechanism in RAW 264.7 cells. The study employed cell viability and reactive oxygen species (ROS) formation assays and western blot analysis. The results demonstrated that silymarin effectively reduced intracellular ROS levels induced by lipopolysaccharide (LPS) in a dose-dependent manner without causing any cytotoxic effects. Moreover, silymarin treatment significantly upregulated the expression of heme oxygenase (HO)-1, a phase II enzyme known for its potent antioxidative activity. Additionally, silymarin treatment significantly induced the expression of nuclear factor-erythroid 2 p45-related factor (Nrf) 2, a transcription factor responsible for regulating antioxidative enzymes, which was consistent with the upregulated HO-1 expression. To investigate the involvement of key signaling pathways in maintaining cellular redox homeostasis against oxidative stress, the phosphorylation status of mitogen-activated protein kinase (MAPK) and phosphoinositide 3-kinase (PI3K) was estimated by western blot analysis. The results showed that silymarin potently induced HO-1 expression, which was mediated by the phosphorylation of p38 MAPK. To further validate the antioxidative potential of silymarin-induced HO-1 expression, tert-butyl hydroperoxide (t-BHP)-induced oxidative damage was employed and attenuated by silymarin treatment, as identified by a selective inhibitor for each signaling molecule. In conclusion, silymarin robustly enhanced antioxidative activity by inducing HO-1 via the Nrf2/p38 MAPK signaling pathway in RAW 264.7 cells.

Comparison of electric and manual toothbrushes on periodontal health in fixed orthodontic patients (고정식 교정환자에서 치주건강 유지에 대한 전동치솔의 효과)

  • The purpose of this study was to evaluate the efficacy of an electric toothbrush, with a specially designed orthodontic brush head, compared to a manual toothbrush on the periodontal health of patients with fixed orthodontic appliances Forty patients were randomly divided into two groups. the electric and the manual toothbrush groups, 1 month after attachment of fixed orthodontic appliances. Periodontal status was measured using a plaque index, a gingival index. a bleeding index, a pocket depth and a relative attachment loss, at baseline and after 3 months. 6 months, 9 mouths. and 12 mouths. The Braun Oral-B Plak Control with Ortho OD 15-1 brush head was used as the electric toothbrush while the Butler G.U.M 124 was given as the manual toothbrush. In the manual toothbrush group, the giugival and bleeding indices showed no statistically significant differences, but the plaque index was significantly decreased (P<0.001) and pocket depth as well as relative attachment loss were significantly increased (p<0.01). In the electric toothbrush group. the bleeding index, pocket depth and relative attachment loss showed no statistically significant differences. but the plaque and gingival indices were significantly decreased (p<0.001). In the case of the plaque. giugival and bleeding indices, there were no statistically significant differences between the electric and the manual toothbrush groups. On the contrary, in the case of pocket depth and relative attachment loss, there were statistically significant differences between the electric and the manual toothbrush groups: an increase in the manual toothbrush groups unlike the electric toothbrush group which kept the same state (P<0.05) These findings suggest that an electric toothbrush is useful for orthodontic patients with fixed appliances.

LONGEVITY AND FAILURE ANALYSIS OF FIXED RESTORATIONS SERVICED IN KOREA (국내에서 제작된 고정성 보철물의 수명과 실패 요인 및 양상)

  • Statement of problem. Every effort has been continually made to obtain objectivity in measuring the longevity of fixed restorations, such as by establishing unified judgement standard for deciding success and adopting statistical method that analyzes the data of successful and failed cases at the same time. In Korea, however desired level of development has not to be made in this field yet. Purpose. This study, adopting California Dental Association (CDA) quality evaluation system, established objective standard for deciding success, and inferred the longevity of fixed restorations and their failure analysis through adopting Kaplan-Meier survival analysis. Material and method. In order to assess the longevity of flxed restorations serviced in Korea and causes of failure, a total of 1109 individuals (aged 15-74, 716 women and 393 men loaded with 2551 unit fixed restorations, and 1934 abutments) who lived in Kyung-In Province were examined and the findings were as follows : Results. 1. Length of service of fixed restorations serviced in Korea was 6.86$\pm$0.15 yr (mean), 5.5 yr (median), and the rate of success was 65.82% in 5 year survival, and 21.15% in 10 year survival. 2. When there was patient's need for replacing old prosthetics, longevity of fixed restorations was 7.51$\pm$0.27 yr (mean), 7 yr (median), and the rate of success was 61.08% in 5 year survival, and 17.57% in 10 year survival. 3. Longevity of fixed restorations was longest in the over-sixty age group(9.21$\pm$0.66) and that of the teen age group(3.39$\pm$0.28) was shortest (p<0.05). 4. Longevity of fixed restorations of women (7.38$\pm$0.18 years) was longer than that of men (6.00$\pm$0.26) (p<0.05). 5. As for the provider factor (such as unlicensed performers, university hospitals, and private clinic), there was no statistically significant difference in longevity of fixed restorations. 6. Defective margin (34.78%). periodontal disease (12.15%), periapical involvement (11.73%), was the most frequent causes of failure and poor esthetics group showed the longest life above all (p<0.05). Actual frequent causes of failure after removing old prosthetics were defective margin, periapical involvement, periodontal disease and uncemented restoration. In 75.67% of the cases, abutment state after removing old prosthetics was good enough for loading another prosthetics. 7. There was found to have statistically significant influence between longevity of single crown (6.35$\pm$0.20 yr) and that of 3 unit fixed restorations (7.60$\pm$0.30 y) (p<0.05). In each case the most frequent cause of failure was defective margin. 8. The number of cantilever pontic, pontic/abutment ratio, oral hygiene status were found to have no statistically significant influence on longevity of fixed restorations in all groups (p>0.05). 9. Longevity of fixed restorations made of non precious metal was longest (9.60$\pm$0.40 yr) semi precious and precious trailing behind(p<0.05). 10. Group function group (37.04%) and partial group function group (44.62%) were predominant in frequency but showed no correlation between them and among different types of occlusal plane and different types of occlusal surface (p>0.05). 11. Longevity of fixed restorations was longest in the centric interference group(9.35$\pm$0.62) (p<0.05) among different types of occlusal interference. Conclusion. We found that longevity of fixed restorations serviced in Korea is affected by age, gender and type of material, and that most frequent cause of failure is defective margin. In order to assess the accurate longevity of axed restorations, unified research design. overcoming inter-observer difference and establishing the objective research items are needed. Furthermore, it is thought that prospective approach through thorough study and regular follow-ups is needed just from the start of research. Nationwide detailed studies on length of service of fixed restorations manufactured in Korea are hoped to be conducted hereafter.
