  • The purpose of this study was to obtain the guides of establishing vertical dimension by analyzing the facial and oral landmarks of Korean adults. The following conclusions were obtained from this study. 1. The ratio of bizygomatic width to dental arch width was 3.26 : 1 in male and 3.21 : 1 in female, and the ratio of vertical dimension to dental arch length was 2.49 : 1 in male and 2.39 : 1 in female. 2. It was obtained by analysis of multiple regression that the corelative formula, vertical dimension$=23.37+0.24{\times}bizygomatic$ $width+0.29{\times}dental$ arch length. 3. There was no statistically significant difference between the results of vertical dimension acquired by means of Hayakawa's prediction($68.04{\pm}3.16mm$ in male, $64.38{\pm}3.00mm$ in female) and that of vertical dimension by this study.

정상교합자의 치열궁 형태 (THE DENTAL ARCH FORM IN NORMAL OCCLUSION)

  • 이 연구는 bracket이 붙여지는 위치에서의 이상적인 협측 및 설측 치열궁 형태를 그려내기 위하여 $17\~26$세까지의 정상교합자 63명의 상하악 모헝을 채득하여 계측점과 설정, 통계처리하여 다음과 같은 결과를 얻었다. 치열궁 형태를 나타내는 선계측 항목에서 Bicanine width, Bipremolar width, Bimolar width에서 남자가 여자보다 더 크게 나타났다. NEC 9801 V-M computer를 이용한 graphic 입력을 분석하여 협설측 치열궁 형태를 구하였다. 협측 및 설측 치열궁 형태를 나타내는 포물선식의 상수와 결정계수를 구하였다.

Orthodontic treatment in a patient with Moebius syndrome: A case report

  • Moebius syndrome (MBS) is a congenital neurologic disorder that causes cranio-facial abnormalities. It involves paralysis of the VI and VII cranial nerves and causes bilateral or unilateral facial paralysis, eye movement disorder, and deformation of the upper and lower limbs. The orofacial dysfunctions include microstomia, micrognathia, hypotonic mimetic and lip muscles, dental enamel hypoplasia, tongue deformity, open bite or deep overbite, maxillary hypoplasia, high arched palate, mandibular hyperplasia or features indicating mandibular hypoplasia. This case report presents a 7-year-old male patient who was diagnosed with MBS at the age 2 years. The patient displayed typical clinical symptoms and was diagnosed with Class II malocclusion with a large overjet/overbite, tongue deformity and motion limitation, and lip closure incompetency. Treatment was initiated using a removable appliance for left scissor bite correction. After permanent tooth eruption, fixed appliance treatment was performed for correction of the arch width discrepancy and deep overbite. A self-ligation system and wide-width arch form wire were used during the treatment to expand the arch width. After 30 months of phase II treatment, the alignment of the dental arch and stable molar occlusion was achieved. Function and occlusion remained stable with a Class I canine and molar relationship, and a normal overjet/overbite was maintained after 9.4 years of retainer use. In MBS patients, it is important to achieve an accurate early diagnosis, and implement a multidisciplinary treatment approach and long-term retention and follow-up.

치열궁의 성장 변화 (GROWTH AND DEVELOPMENT OF ARCH FORM)

  • 치열궁의 성장 변화에 대한 연구는 교정 진단과 치료 계획 수립시와 결과의 분석에 있어서 중요한 자료가 될 수 있으며, 이러한 치열궁의 형태는 자연 인류학과 치의학 특히 보철학과 교정학 분야에서 더욱 중요한 의미가 있다. 특히 교정학 분야 에서는 상,하악 치아의 기능적인 면과 치료후의 치열과 교합의 안정성 유지에 대한 정보를 얻기 위하여 일찍 부터 연구가 진행 되어 왔다. 이에 대한 연구로는 이미 많은 선학들의 연구가 보고 되어 오고 있다. 초기에는 두개골을 직접 측정하여 치궁의 성장 변화를 설명 하였고,그 후 방사선이 소개 됨으로 방사선 사진을 이용한 계측과 또한 경석고 토형을 사용한 선계측 방법도 많이 시도 되어 왔다. 그 방법으로는 치열궁의 폭경, 치열궁 장경 및 주위경 등을 계측하여 치열궁의 성장 변화를 연구 하여 왔다. 본 연구에서는 강원도 지역과 서울 지역에 거주하는 교정 치료를 받지 않고, 건강한 심신과 정상적인 성장 발육을 하고 있는 정상 교합자 3세 에서 12세 까지의 남,녀 아동 600여명을 대상으로 하였으며, 각각 두부 측면 방사선 사진, 파노라마 사진, 그리고 치열궁 모형을 두해 연속해서 채득을 한 이후 계속 follow up 하고 있으며 현재까지 follow up 된 두해에 걸친 각각 200여 쌍, 총 400여 쌍의 모형을 채택하여 본 연구를 하였다. 본 연구에서는 치아의 근원심 폭경, 견치간 폭경, 구치간 폭경, 견치 치열궁 장경, 구치 치열궁 장경, 그리고 치열궁 주위경을 측정하여 연령별 성별 평균과 표준 편차를 내고 도표로 표시 하여 다음과 같은 결론을 얻었다. 1. 견치간 폭경은 계속 완만히 증가하다가 10세 이후 12세까지는 증가하지 않았다. 2. 제 1 대구치간 폭경에 대해서는 상악은 완만한 증가를 보이지만 하악의 경우는 유의성있는 변화를 보이지 않다가 9세 이후 증가하는 양상을 보였다. 3. 견치 치열궁 장경은 6세 이후 비교적 급격한 증가를 보였고 하악의 경우 더욱 뚜렷하다. 4. 구치 치열궁 장경은 상.하악 모두 서서히 증가하다가 10세 이후 부터는 감소하는 양상을 보였다. 이러한 감소 경향은 하악에서 더욱 두드러진다. 5. 치열궁 주위경은 서서히 증가하는 경향을 보이다가 그 증가량이 점차 완만해지고 10세이후에는 다시 감소하는 경향을 보이는데 이러한 경향은 하악에서 약간 크고 빠르게 나타났다.

백색광과 청색 LED 방식의 광학스캐너로 채득된 디지털 모형의 비교분석 (Comparative analysis on digital models obtained by white light and blue LED optical scanners)

  • Purpose: The purpose of this study was to analyze and compare the relative accuracy of digitized stone models of lower full arch, using two different scanning system. Methods: Replica stone models(N=20) were produced from lower arch acrylic model. Twenty digital models were made with the white light and blue LED($Medit^{(R)}$, Korea) scanner. Two-dimensional distance between the landmarks were measured on the Delcam $CopyCAD^{(R)}$(Delcam plc, UK). Independent samples t-test was applied for comparison of the groups. All statistical analyses were performed using the SPSS software package(Statistical Package for Social Sciences for Windows, version 12.0). Results: The absolute disagreement between measurements made directly on the two different scanner-based dental digital models was 0.02~0.04mm, and was not statistically significant(P>0.05). Conclusion: The precision of the blue LED optical scanner was comparable with the digitization device, and relative accuracy was similar. However, there still is room for improvement and further standardization of dental CAD technologies.

한국인 아동의 유치열기의 특성 - 치아 치열궁의 관계 (Spacing and crowding of the primary dentition in Korean children - relationship to tooth sizes and dental arch dimension)

  • 이 연구의 목적은 강화지역의 한국인 아동의 유치열기에서의 공극과 총생의 빈도를 조사하고, 치아 크기와 치열궁간의 관계를 조사하는 것이다. 102개의 취학전 아동(남자 57명, 여자 45명, $4{\sim}5$세)의 석고모형을 조사하여 치아의 크기, 견치간 폭경, 구치부 폭영, 치열궁 장경을 측정하였다. 유치열기의 공극 빈도는 남자에서 63.2%, 여자에서 57.8%로 나타났다. 공극의 빈도는 남자에서, 하악보다는 상악에서 더 높게 나타났다. 공극이 있는 경우보다 총생을 보이는 치열에서 치아 크기는 크고 치열궁 폭경은 더 작게 나타났다 (p<0.05). 전치부 공극의 존재 여부는 치아의 크기, 견치간 폭경과 관련이 있었다.

Management of flabby ridges using liquid supported denture: a case report

  • Flabby ridges commonly occur in edentulous patients. Inadequate retention and stability of a complete denture are the often encountered problems in these patients. A liquid supported denture due to its flexible tissue surface allows better distribution of stress and hence provides an alternate treatment modality in such cases. This case report presents the use of a liquid supported denture in a patient with completely edentulous maxillary arch with flabby tissue in anterior region opposing a partially edentulous mandibular arch.

  • A 9-year-old girl, in good health, presented a horizontal impaction of the upper left central incisor. History revealed that the patient had been involved in a trauma of the maxillary anterior portion during her childhood. This impacted tooth crown, which was exposed surgically, was bonded by plastic attachments of direct bonding system, and was carefully tried to induce on the dental arch. On the process of the orthodontic treatment the tooth was completely induced and in normal alignment on the arch. The treatment result was very satisfactory; color, vitality, and mobility were normal, periodontal support was good and the cosmetic result was excellent.

  • Transposition is defined as a dental anomaly manifested by a positional interchange of 2 adjacent teeth within the same quadrant of the dental arch. Maxillary canine-first premolar [Mx4-3] transposition is the most frequent tooth transposition reported in the literature. In this case report, an orthodontic correction of a transposition of the maxillary left canine and first premolar with the help of palatally located mini-implant anchorage is described. Esthetic and occlusal evaluations suggested alignment of the transposed teeth to their correct anatomic positions in the dental arch. The clinical result at the end of the treatment was satisfactory. Alignment was obtained, and intercuspation was adequate. Nevertheless, the maxillary canine showed facial recession, probably because it was initially positioned buccally. Supporting tissue was examined after treatment and no alveolar bone damage was observed.

  • Class I malocclusion is essentially a dental dysplasia. Rotations, individual tooth malpositions, missing teeth, tooth size discrepancies, etc., fall under this classification. There are two types of class I malocclusions. One is identified by and insufficient denture base to accommodate the teeth; the other has more denture base than tooth material, creating spaces in the arch. The tooth material-to denture base discrepancies may be slight, calling for only a little increase in arch length for alignment and the correction of minor rotations. Discrepancies may also be great, in which case it becomes necessary to reduce tooth material by extraction, so as to make the tooth material more in proportion to the size of the denture base. The author had attempted orthodontic treatment of a class I malocclusion case of 13-year old boy in which high canines and impacted mandibular second premolars were involved. The author obtained good results.
