Background: Breast cancer is the most frequent cancer of women in Pakistan with the majority presenting with stage III or IV lesionsat initial diagnosis. Patient and health system related factors are well known determinants of delay in presentation and diagnosis. Additionally, breast cancer being a heterogeneous disease, the various molecular subtypes featuring different aggressiveness also need to be considered. The present study evaluated the association of stage at initial diagnosis of breast cancer with these two factors in local women at a tertiary level health care facility in Lahore, Pakistan. Materials and Methods: Patient and tumor features were recorded separately during diagnostic workup in Breast Clinics at INMOL and at Services Hospital, Lahore. Data were entered in MS Excel and analyzed by descriptive statistics and Chi-Square test. Results: Among the 261 patients, 64% were staged as late breast cancer (LBC), the mean age was 46.8 with standard deviation of 13 years. Some 92% had invasive ductal carcinoma (IDC), 61% had luminal types (LT) of non-aggressive tumor while 39% had the non-luminal types (NLT) of of HER2-enriched or basal aggressive tumors. While 70% of patients presented within one year of symptomatic disease (early report group "ERG"), 30% reported after a mean delay of 4 years with a standard deviation of 3.75 years. The stage distribution among ERG patients was not statistically different from those reporting late (P=0.123). Statistically larger proportion of patients with NLT presented as LBC as compared to the LT (P =0.034). Among the ERG, statistically different stage distribution of disease was observed for the NLT versus LT (P=0.047). Among those presenting late, this difference was insignificant (P=0.416). Conclusions: Breast cancer is a distinct disease in Pakistan with a high frequency of aggressive molecular types affecting younger women, with the majority presenting as LBC. Association of NLT with higher stage at diagnosis is statistically significant whereas time delay in diagnosis is not. Further research is required to define the risk profile and features in local patients. The burden of LBC can be reduced by promoting breast health awareness and by establishing easily accessible dedicated breast care set ups in the hospitals.
In this paper the performance of a closed-loop power control scheme using variable step size decision method for DS/CDMA based-low earth orbit(LEO) mobile satellite systems in which the long round trip delay is a dominant performance degradation factor is evaluated. Because there are fundamental differences in the characteristics between the LEO mobile satellite channel and terrestrial mobile channel, such as long round trip delay and different elevation angle, these factors are considered in channel modeling based on the European Space Agency(ESA) measurement data. Since the round trip delay (from the mobile terminal to the gateway station via satellite) is typically 10∼20ms in low altitude satellite channels, closed-loop power control is much less effective than it is on a terrestrial channel. Thus, the adaptive power control scheme using a variable step size control is essential for overcoming the long round trip delay and fading due to the elevation angle. It is shown that the standard deviation of signal to interference ratio(SIR) adopting a variable step size closed-loop power control scheme is much less than that of a fixed step size closed-loop power control. Furthermore, we have driven the conclusion that the measurement interval of power control commands is optimal choice when it is twice the round trip delay.

Sonar bearing accuracy is the correspondence between the target orientation predicted by sonar and actual target orientation, and is obtained from measurements. However, when measuring sonar bearing accuracy, many errors are included in the results because they are made at sea, where complex and diverse environmental factors are applied. In particular, parallax error caused by the difference between the position of the GPS receiver and the sonar sensor, and the time delay error generated between the speed of underwater sound waves and the speed of electromagnetic waves in the air have a great influence on the accuracy. Correcting these parallax errors and time delay errors without an automated tool is a laborious task. Therefore, in this study, we propose a sonar bearing accuracy measurement equipment with parallax error and time delay error correction. The tests were carried out through simulation data and real data. As a result of the test it was confirmed that the parallax error and time delay error were systematically corrected so that 51.7% for simulation data and more than 18.5% for real data. The proposed method is expected to improve the efficiency and accuracy of sonar system detection performance verification in the future.
Digital transformation and the COVID-19 pandemic have facilitated the rapid development and dissemination of non-face-to-face technologies such as self-service technologies (SSTs). This research investigates how motivation factors affect consumers' negative responses to digital shadow work (DSW) in SSTs which decreases their continuous use intention of SSTs. Also, we examine whether the grafication delay ability moderates the relationship between consumers' negative responses to DSW and their continuous use intention of SSTs. By an analysis of usable 450 user respondents via SmartPLS 4.0, perceived benefits was found to significantly influence consumers' negative responses to DSW. Also, consumers' negative responses to DSW can significantly decrease their intention to continue using SSTs. Further, the effect of negative responses to DSW on continuous use intention of SSTs is stronger in case of consumers with low gratification delay ability than in case of consumers with high gratification delay ability. The study findings contribute to providing some strategies for companies operating SSTs by examining the effects of consumer's responses to DSW and gratification delay ability on the continuous usage intention of SSTs.

Background: Breast cancer is the most common cancer and the leading cause of cancer mortality among women of all ethnic and age groups in Malaysia. Delay in seeking help for breast cancer symptoms is preventable and by identifying possible factors for delayed diagnosis, patient prognosis and survival rates could be improved. Objectives: This narrative review aimed to understand and evaluate the level of in-depth breast cancer knowledge in terms of clinical breast examination and breast self-examination, and other important aspects such as side-effects and risk factors in Malaysian females. Since Malaysia is multicultural, this review assessed social perceptions, cultural beliefs and help-seeking behaviour in respect to breast cancer among different ethnic groups, since these may impinge on efforts to 'avoid' the disease. Materials and Methods: A comprehensive literature search of seven databases was performed from December 2015 to January 2015. Screening of relevant published journals was also undertaken to identify available information related to the knowledge, perception and help-seeking behaviour of Malaysian women in relation to breast cancer. Results: A total of 42 articles were appraised and included in this review. Generally, women in Malaysia had good awareness of breast cancer and its screening tools, particularly breast self-examination, but only superficial in-depth knowledge about the disease. Women in rural areas had lower levels of knowledge than those in urban areas. It was also shown that books, magazines, brochures and television were among the most common sources of breast cancer information. Delay in presentation was attributed mainly to a negative social perception of the disease, poverty, cultural and religion practices, and a strong influence of complementary and alternative medicine, rather than a lack of knowledge. Conclusions: This review highlighted the need for an intensive and in-depth breast cancer education campaigns using media and community health programmes, even with the existing good awareness of breast cancer. This is essential in order to avoid misconceptions and to frame the correct mind-set about breast cancer among women in Malaysia. Socio-cultural differences and religious practices should be taken into account by health care professionals when advising on breast cancer. Women need to be aware of the risk factors and symptoms of breast cancer so that early diagnosis can take place and the chances of survival improved.

Construction defect which can causes economic damage such as schedule delay, cost overrun is a considerably important factor in construction industry. In general, a construction defect features a difficulty to find out causes precisely because it occurs when several interrelated causes combine. Yet, studies have tried to understand the interrelationships between factors are limited. In addition, despite of a tremendous amount of construction data, it's not still enough to analyze them, but tends to depend on experience or know-how of practitioners. Thus, it is necessary to identify underlying causes in influential factors by utilizing related data. This paper analyses Interrelationships between causal factors using Association Rule Mining to discover root causes of construction defects. Confidence and Lift that can be used for presenting the interrelationships of the causes were extracted from 1241 cases in 30 projects in Korea. It is expected that this paper allows the construction managers to discover key factors and make right decisions to reduce occurrence of construction defects. Furthermore, analysis of interrelationships can improve understanding of structural patterns of construction defects.

Recently, a curtain wall construction is a exterior finishing components which is most used for shortening time in high-rise building as well as the class of key management factors in cost and schedule control. Also, it is recognized that an effective management for curtain wall process is a major subject to accomplish the project successfully. However, as the current management for curtain wall construction is focused on the construction stage, it makes problems such as errors in business performance, rework by mistakes and duplications, errors and omissions by ineffective information management and there has never been any efficient management from a view of the entire Curtain Wall Life-cycle process. Therefore, the aim of this study is to suggest a stage check point for process improvement in the curtain wall Life-cycle process through current curtain wall process analysis, and then to investigate the cause of waste factors using the Muda method from the Toyota Production System and extract the weighted effects of the waste factors using the analytical hierarchy process method. According to the result, Most of the inefficient factors happened in architectural design stage of the entire curtain wall Life-cycle process and my research identified that detail factors of them are a delay of decision making and an approval in changes, a deficit of engineering capacity and a delay of approval in architectural design drawings by owner, etc.
Background: Breast cancer is known to be one of the most prevalent cancers among women in both developing and developed countries. The incidence of breast cancer in Pakistan has increased dramatically within the last few years and is the second country after Israel in Asia to have highest proportional cases of breast cancer. However, there are limited data for breast cancer available in the literature from Pakistan. Objectives: The study was conducted to bring to light the common clinical presentation of breast cancer and to evaluate the frequency of established risk factors in breast carcinoma patients and furthermore to compare the findings between premenopausal and postmenopausal women in Pakistan. Materials and Methods: A 6 months (from July 2012 to Dec 2012) cross sectional survey was conducted in Surgical and Oncology Units of Civil Hospital, Karachi. Data were collected though a well developed questionnaire from 105 female patients diagnosed with carcinoma of breast and analyzed using SPSS version 17. Institutional ethical approval was obtained prior to data collection. Results: Out of 105 patients, 43 were premenopausal and 62 were postmenopausal, 99 being married. Mean age at diagnosis was $47.8{\pm}12.4years$. A painless lump was the most frequent symptom, notived by 77.1%(n=81). Some 55.2% (n=58) patients had a lump in the right breast and 44.8%(n=47) in the left breast. In the majority of cases, the lump was present in upper outer quadrant 41.9% (n=44). Mean period of delay from appearance of symptoms to consulting a doctor was $5.13{\pm}4.8months$, from the shortest 1 month to the longest 36 months. Long delay (> 3 months) was the most frequent figure 41.9%. Considering overall risk factors most frequent were first pregnancy after 20 years of age (41%), physical breast trauma (28.6%), lack of breast feeding(21.9%), and early menarche <11 years (19%), followed by null parity (16.2%), consumption of high fat diet (15.2%), family history of breast cancer or any other cancer in first degree relatives (9.5% and 13.3%, respectively). Some of the less common factors were late menopause >54 years (8.6%), use of oral contraceptive pills (10.5%), use of hormone replacement therapy (4.7%),smoking (4.7%) and radiation (0.96%). Significant differences (p<0.005) were observed between pre and post menopausal women regarding history of physical breast trauma, practice of breast feeding and parity. Conclusions: A painless lump was the most frequent clinical presentation noted. Overall age at first child > 20 years, physical breast trauma, lack of breast feeding, early menarche <11 were the most frequent risk factors. Physical breast trauma, lower parity, a trend for less breast feeding had more significant associations with pre-menopausal than post-menopausal onset. Increase opportunity of disease prevention can be obtained through better understanding of clinical presentation and risk factors important in the etiology of breast cancer.

This paper is concerned with flow shop scheduling problems having the common due date. V-shape property is used for algorithms with early and tardy penalty cost. The objective of this paper is developing efficient heuristic scheduling algorithms for minimizing total penalty cost function and determining the optimal common due date. The between job delay and the work in process are considered for developing algorithms as penalty cost. Algorithms is simulated to analyze interrelated factors. A numerical example is given for illustrating the proposed algorithms.
This paper focuses on development of simulation model for SAE J1850 and performance evaluation of a J1850 network. The simulation model has been developed by using discrete event simulation language. SAE J1850 is one of Class B network protocol for general data sharing applications. Through numerous simulation experiments, several important performance factors such as the probability of a successful transmission, average queue delay, and throughput have been evaluated.
