A Case of Chronic Diarrhea with Symptoms Named 'San (疝)' Every Fall (매 해 가을에 반복하는 설사를 주증으로 내원한 산증환자(疝症患者) 치험 1례)

  • Objective: The aim of this clinical study was to describe the case of a patient who experienced chronic diarrhea related to seasonal changes that improved with herbal medicine treatment. Methods: A patient with diarrhea and globus hystericus was treated following treatment with Hoehyangbanchong-san-hap-Ukgan-san and Gamisachil-tang. A defecation and urination diary, the Bristol Stool Form Scale, and the Visual Analog Scale (VAS) were used to evaluate the effectiveness of this treatment. Results: During treatment, the patient's Bristol Stool Form Scale score changed from Type 7 to Type 4, and the VAS score was significantly decreased. The symptoms of diarrhea and globus hystericus almost subsided. Conclusion: In this case, Korean traditional herbal medicine reduced the clinical symptoms of diarrhea and globus hystericus.

Surgical Correction of Congenital Type III Atresia ani with Rectovaginal Fistula in a Cat (고양이의 직장질루가 병발한 Type III 선천성 항문무형성증의 수술적 교정)

  • A four-week-old female Scottish Fold cat weighting 0.6 kg was admitted for vaccination. During the physical examination, the liquid feces were observed from the vulva and the anus was imperforate. The location of a narrow fistula and distended colon were identified on the contrast radiography. Definitive diagnosis was made as type III atresia ani with rectovaginal fistula. Anal reconstruction and ligation of the fistula were successfully undertaken to treat atresia ani. After surgery, the cat was treated with lactulose and a special diet consisting of high fiber was fed to increase digestibility. The cat was able to control defecation after 2 weeks post-operation. There was no complication for 8 months after surgery.

A Case Study of Constipation in a Chronic Kidney Disease Soyang Patient Treated with Hyeongbangjihwang-tanggamibang (형방지황탕가미방으로 호전된 만성 신질환 소양인 환자의 변비 치험례)

  • Objectives This study was aimed to report a significant improvement of a CKD patient's constipation, diagnosed as 'Soyang type yin depletion pattern' based on Sasang constitutional medicine. Methods The patient had treatment with Sasang constitutional medicine. The treatment was executed three times everyday from January 6th to February 21st of 2023. Clinical improvement was evaluated with Bristol Stool Form Scale, the number of defecation per week, average stool weight per day, the degree of abdominal discomfort and abdomen x-ray. Results The symptoms of constipation, abdominal discomfort, appetite and delirium were improved. Conclusion After the treatment with Sasang constitutional medicine, constipation, abdominal discomfort, appetite, delirium and the general condition of the patient also had improved.

Surgical Correction of Congenital Type IV Atresia Ani with a URethrorectal Fistula in a Dog (개의 선천성 제4형 항문폐쇄증과 요도직장루의 외과 교정술)

  • A 4-week-old 0.5 kg male Shih Tzu with history of congenital abnormality, abnormality, was referred to Veterinary Teaching Hospital, Chungbuk National University for further evaluation and treatment. During physical examination, the dog revealed mild depression and dyschezia. In plain radiographs, a digital thermometer put in the anus and grasped blind end of the rectum. In contrast radiographs, a urethrorectal fistula was confirmed. Urine specimens were collected with cystocentesis. Bacteria of the urine were detected using an auto microorganism analyzer. According to history taking, physical examination, radiographic signs and urinalysis, it was diagnosed as type IV atresia ani with a urethrorectal fistula. The dog was treated by fistulectomy and anoplasty, and discharged with instruction. Three days after operation, mild dehiscence was appeared. Wound was left to heal by second intention. During the follow-up of eight weeks, wound showed it to be healed and defecation was normal.

A Case Report on Opioid-Induced Constipation in a Patient with Cancer Treated by Jowiseunggi-tang (조위승기탕으로 호전된 암 환자의 마약성 진통제로 인한 변비 치험 1례)

  • Objective: Jowiseunggi-tang (JWSGT) is a traditional herbal medicine commonly used for purgative activity in constipation. This study evaluates JWSGT for the treatment of opioid-induced constipation (OIC), the most common and debilitating gastrointestinal effect of opioid use. Methods: A 64-year-old man with floor of mouth cancer was hospitalized for OIC, and JWSGT was administered orally twice a day for 10 days, along with acupuncture, moxibustion, and cupping. The primary outcome measures were defecation type according to the Bristol Stool Form Scale (BSFS) and the frequency of bowel movements. The Brief Fatigue Inventory (BFI) and the Functional Assessment of Cancer Therapy-General (FACT-G) were used as secondary measures. Results: After three days of JWSGT administration, spontaneous bowel movements were observed two to three times per week with improved BSFS from type 2 to 4. A reduction in BFI score (8.7 to 2.0) and an increase in FACT-G score (44.3 to 59.0) suggested an improvement in fatigue level and quality of life. Conclusion: This is the first report to assess the efficacy of JWSGT for the management of OIC in patients with cancer, and JWSGT may be an effective option to improve symptoms and quality of life in this group.

Hypersensitive Large Intestine Syndrome in Hyungsang Medicine (과민성대장증후군의 형상의학적 고찰 -동의보감(東醫寶鑑)을 중심으로-)

  • The writer reports the conclusions gained from study about the cause of the hypersensitive large intestine syndrome with Dongeuibogam as the central figure through researching the disharmony among Body Essence, Vital Energy, Mentality, and Blood, mutual action of five viscera and six bowels, and external shapes. The hypersensitive large intestine syndrome is generally chronic and recurred in many cases, so it is more efficacious than symptomatic to treat according to find the contradictions of individual shapes. The shapes and cases suffering frequently the hypersensitive large intestine syndrome are Gi-kwa and Sin-kwa, having a long nose, having a bruised spot on Triple warmer, man with inclined mouth, Taeeum type, man with congested fluids, man with colic symptoms. The hypersensitive large intestine syndrome in Oriental medicine is recognized of diarrhea, constipation, abdominal pain, abdominal distention and fullness caused by seven emotions. In Dongeuibogam it can be found out the similarity in depressive symptoms due to disorder of Gi, stagnation of Gi, dysphasia due to disorder of Gi, diarrhea due to disorder of Gi, fullness of due to Gi, diarrhea due to phlegm-retention, retention of undigested food, immoderate drinking, hypo-function of the spleen, or deficiency, abdominal pain from colic symptom, and difficulty in defecation and urination, internal injury, diarrhea due to weakness and fatigue. If the Jung, Gi, Sin, and Hyul composed the human body is broken harmony, the function of large intestinal transmission would be fallen, so similar symptoms like the hypersensitive large intestine syndrome are gotten. Especially Gi-kwa suffers diarrhea, constipation abdominal pain, and abdominal distention and fullness due to depressive symptoms from disorder of Seven emotions or Seven Gi. And Sin-kwa suffers from the hypersensitive large intestine syndrome due to emotional restlessness having an influence on rhythmic movement of abdomen. Examining between five viscera and six bowels and the hypersensitive large intestine syndrome, Liver cannot disperse well having influence on mutual relation of Liver-Large intestine, Heart reduces the function of defecation and urination not to control the seven emotions, Lung having exterior and interior relation with intestine has an influence on primordial energy and let the main symptoms occur, Spleen circulating the body fluid let the main symptoms occur due to malfunction of circulation, Kidney locating in lower part of the body has deep connection with intestine, so let the disorder. Urinary bladder is connected with intestine in moisture metabolism, Stomach is connected in receive and transmission, Small intestine is connected in absorption and excretion, from small intestine pain disturbing the abdominal movement, Samcho managing the catharsis of lower heater if declined its function causes the hypersensitive large intestine syndrome. The colic symptoms of Front private parts which disorder in lower abdomen give rise to abdominal pains, difficulty in defecation and urination due to Cold are similar to the hypersensitive large intestine syndrome. The treatments of applying the shapes of colic syndrome advocated by Master Park can be efficacious cure in clinic. Researching after the clinical cases of Master Park advocating Hyungsang medicine, we came to know that plenty of prescriptions of internal injury are applied and take good effects.

The influence of accumulated clinical experience with Soyang-type patients on the conceptualization of the Soyang pathology (소양인(少陽人) 치험례(治驗例)가 소양인(少陽人) 병리관(病理觀) 형성과정에 미친 영향에 대한 고찰)

  • 1. Objective: This literary review investigates Lee Jema's clinical experiences with Soyang-type patients and their influence on his conceptualization of the Soyang physiology and pathology. 2. Methods: 1) The case reports in "Soyang constitutional type: Spleen Cold-based Exterior-Cold disease" were compared before and after the Sinchuk revision to explore the temporal change in the Sasang medical concepts. 2) The texts in Donguisusebowon (Gabo edition) and Donguisusebowon (Sasangchobongwon) were analyzed to investigate the pathological concepts appearing before the completion of the Donguisusebowon (Gabo edition). 3) The texts in Donguisusebowon (Sinchuk edition) was analyzed to investigate the pathological concepts formed between the Gabo and Sinchuk editions of Donguisusebowon. 3. Results and Conclusions: 1) Gabo edition : The Gabo edition divided the External-origin Exterior disease into Cold-dominant (Heat-moderate) and Heat-dominant (Cold-moderate) patterns and differentiated the severity of Exterior-based Exterior disease and Interior-based Interior disease into mild, moderate, severe, and critical conditions. Cold-damage Delirium disease pattern was categorized as an Interior-based Interior disease, and the treatment protocol using Baekho-tang (Baihu-tang) was established. The stool condition and bowel movement reflecting the patient's defecation habits, the prime indicator of health in the Soyang constitutional type, were emphasized on their importance. 2) Sinchuk edition: The Delirium disease pattern was moved into the Exterior-cold disease, and the treatment protocol applying Jihwangbaekho-tang (Dihuangbaihu-tang) and Hyeongbangsabaek-san (Jingfangxiebai-san), with variational usage of Gypsum, was newly established. The Seong-Jeong and basal disease patterns were suggested as important factors in treating the patients. Also, it was proposed that the symptoms and signs reflecting the condition of the life-preserving energy be assessed to understand the patient's current condition. The importance of post-acute rehabilitation and aftercare as well as the most appropriate acute-stage treatment were emphasized.

  • 1. Objectives The purpose of this study was to search the descriptions of feces in the text of Lee, Je-Ma for the physiological feces and pathological feces in constitutional medicine. 2. Methods We searched the descriptions of feces mentioned in the text of $\ulcorner$Dongyisusebowonsasangchobongwon$\lrcorner$, $\ulcorner$Dongyisusebowongabobon$\lrcorner$, and $\ulcorner$Dongyisusebowonsinchukbon$\lrcorner$ and classified the feces into physiological feces and pathological feces by the solidity, color, impurities of feces, and the frequency of defecation. 3. Results For Soyangin and Taeyangin, there are formal descriptions of feces related to the constipation in Dongyisusebowon sasang chobongwon, but for Soeumin and Taeumin, there is not formal description of feces related to the constipation in $\ulcorner$Dongyisusebowonsasangchobongwon$\lrcorner$ and for all 4 Type Constitution, there are various formal descriptions of feces related to the diarrhea in $\ulcorner$Dongyisusebowonsasangchobongwon$\lrcorner$. For all 4 Type Constitution, there are various formal descriptions of feces related to the constipation in $\ulcorner$Dongyisusebowongabobon$\lrcorner$, except Taeumin. and for all 4 Type Constitution, yhere are various formal descriptions of feces related to the diarrhea in $\ulcorner$Dongyisusebowongabobon$\lrcorner$. For Soeumin, especially there are the formal descriptions of color and impurities of feces. For all 4 Type Constitution, there are various formal descriptions of feces related to the constipation and the diarrhea in $\ulcorner$Dongyisusebowonsinchukbon$\lrcorner$. For Soeumin, especially there are formal descriptions of color and impurities of feces. 4. Conclusions The diarrhea in Soeumin is serious illness rather than the constipation, and the constipation in Soyangin is serious illness rather than diarrhea. Especially In Yin Exhausted Syndrome in Soyangin, the diarrhea is the important standard of diagnosis in Soyangin's constitutional symptom. The description of feces in Taeumin is not systematic, compared with Soeumin's and Soyangin's. The description of feces in Taeyangin is not yet scientifically established compared with Soeumin's, Soyangin's, Taeumin's.

  • Objectives : To satisfy the demand of good treatment of constipation Methods : we investigated the literatures of Oriental Medicine about Constipation. Results: 1. There are three categories of etimological factors of constipation, that is, endogenous, exogenous and non-exo-endogenous factor. The endogenous factor is caused by seven emotions, called depression of Ki and stagnation of Ki. The exogenous factor is six excessive atmospheric influences, for example, wind, cold, dampness, heat and dryness. And the non-exo-endogenous factors are overfatigue, improper diet, stagnated blood and deficiency of Ki and blood that comes from old age, long disease and after delivery. 2. Classification: According to cause of disease it is classified by constipation due to cold, heat, wind, dryness, retention of undigested and phlegm. According to Internal Organs there are constipation due to deficiency syndrome of the stomach, excess syndrome of the stomach, deficiency syndrome of kidney and splenic constipation. And Differentiation of syndromes according to Ki and blood, there are constipation of deficiency type and excess type. There are constipation due to stagnation and deficiency of Ki, deficiency of blood and stagnated blood. 3. Principles and Methods of treatment 1) Herbal Medicine (1) Excess type [1] Constipation due to heat : Seunggitang(承氣湯) and Majainwhan(麻子仁丸) [2] Constipation due to stagnation of Ki : Samatang(四磨湯) and Yukmatang(六磨湯) (2) Deficiency type [1] Constipation due to deficiency of Ki : Whanggitang(黃?湯) [2] Constipation due to deficiency of blood: Yunjangwhan(潤腸丸) [3] Constipation due to cold : Jechunjun(濟川煎) and Banyuwhan(半硫丸) 2) Enema therapy: It is a method to induce defecation by honey or pig's bile juice for weak people. 3) Acupuncture and Moxibustion: Acupoints used to treat constipation are BL25, ST25 and TE6. Moxibustion at CV8, CV6 is good for constipation due to cold. (4) Diet therapy: It is very important that we eat meals regularly and defecate on the same time even if you don't. And we have to eat food like fruits, vegetables and beans. (5) finger pressure: Finger pressing around these points like ST25, SP25, BL25, BL31, BL32, BL33 and BL34 is good for it. (6) Kigong therapy: Abdominal breathing (7) Old man' s constipation: Hip bath or diet therapy is commended. Laxation with lubricant like Supungyunjangwhan(搜風潤腸丸) is used. (8) Women' s constipation: After delivery, we have to administer tonifying prescription Sipjundaebotang(十全大補湯) and enema can be used. Conclusion : We have to examine the cause of disease and bowl movement carefully. After comprehensive analysis of the data gained by the four methods of diagnosis, we diagnose and treat patients on the base of overall of symptoms and signs.

Rectal Prolapse in Children (어린이 탈직장의 경화요법)

  • Because rectal prolapse in pediatric age was known to have a self-limitting natural history in weeks to years, this disease is prone to be regarded as a minor condition to the most of surgeons. But to the children and the parents who have to be suffered each time could be a heavy distress. Even though operative or nonoperative methods can be applicable for treatment, the main problem is in surgeon's side, whose preference is based on the experiences of adult patients. The authors have experienced 16 cases of ano-rectal prolapse for 9 years since 1986. Eleven of them were true rectal prolapses. In 7 cases of true type, injection therapy has been tried. One ml of five percent phenol in glycerine was injected into the submucosal layer of the ano-rectal angle level at both lateral and posterior sides. After first trial of each cases, 5 of them were cured completely so far. Recurrences were in two cases, but one of them was temporary to be subsided afterward. Complete bowel cleansing and adequate sedations were required as preoperative preparations. Two days' oral antibiotics and two weeks' laxatives for free of defecation straining were recommended after the procedure. The safety of sclerosis was supported by the experimental histology. In pediatric rectal prolapse, sclerosis seems to be a safe and effective treatment of choice without any significant morbidity.
