Some Toxic Heavy Metals Transferring Rates to Decoction of Five Most Generally Adopting Herbal Medicinal Prescriptions (다섯 가지 다용한약처방의 전탕 전후 유해 중금속 이행률 조사)

  • Objective : Five most generally adopting basic oriental prescriptions (yookmijihwangtang, ojeoksan, sipjeondaebotang, samultang, bojoongikgitang) samples were collected from 10 oriental hospitals nationwide and the transferring ratio to aqueous phase (TRAP) were calculated. Method : The samples are grounded evenly and the decocted at $120^{\circ}C$ for 12 min on hot plate and then filtered. The concentrations of As, Cd, Co, Cr and Pb in before and after decoction were measure by ICP/MS and the TRAPs were calculated. Results : The concentrations of As, Cr and Pb in the ground samples before decoction were all below criteria level, and TRAP values were also very low (<10%). For Cd, although some ground samples before decoction showed relatively high concentration level compare to the criteria level, because of their low TRAP (~3%), the actual concentrations in the decoction were quite low compare the criteria level. The Co's concentrations in the ground samples before decoction were all relatively low level. However, the TRAP of Co was astonishingly high value of around 40% average. Conclusion : The toxic heavy metal concentrations in the decoction were very low compare to the criteria level. The TRAPs are less than <10% except Co. The toxic heavy metals in the prescriptions are mainly remained in the residue and the only very small fraction may transferred to the decoctions. Therefore the toxicity of the oriental herbal medicine about the toxic heavy metal may be overstressed. The Co's TRAP turned out to be quite high value (40% average). Since the criteria level wes not established yet, the safety of decoctions according to Co was not fully confirmed.

Inhibitory Effect of Ten Kinds of Herbal Medicine Including Ephedrae Herba on Nitric Oxide in LPS-induced Murine Peritoneal Macrophage (LPS로 활성화된 복강 대식세포에서 마황(麻黃)이 배오(配伍)된 10종(種) 처방의 NO 억제 효과)

  • Objective : The purpose of this study is to find out whether Herba Ephedrae, solely used, and Herbal medicine in which this was included, have inhibitory effects of Nitric Oxide(NO). Methods : We tested the inhibitory effects of Nitric Oxide(NO) with Herba Ephedrae and ten kinds of Herbal medicine combined with Herba Ephedrae(Worlbikachul-Tang, 越婢加朮湯; Mahaengkamsuk-Tang, 麻杏甘石湯; Shinbi-Tang, 神秘湯; Mahwangbujaseshin-Tang, 麻黃附子細辛湯; Euiin-Tang, 薏苡仁湯; Galgeun-Tang, 葛根湯; Mahaengeuigam-Tang, 麻杏薏甘湯; Mahwang-Tang, 麻黃湯; Socheongryong-Tang, 小靑龍湯; Gaemagakban-Tang, 桂麻各半湯) on RAW264.7 cells. Results and Conclusions : 1. We carried out MTT assay on Herba Ephedrae and those decoctions including this in order to determine whether they accommodate cotytoxicity. The results were that Worlbikachul-Tang, Mahaengkamsuk-Tang, Mahwangbujaseshin-Tang, Mahaengeuigam-Tang, Mahwang-Tang, Socheongryong-Tang and Gaemagakban-Tang showed no cytotoxicity on RAW264.7 with 0.1mg/ml and 0.5mg/ml dosages of decoctions but displayed cytotoxicity on the cell with 1mg/ml. Solely used Herba Ephedrae, Shinbi-Tang, Euiin-Tang and Galgeun-Tang exhibited cytotoxicity beyond the concentration of 0.5mg/ml. 2. Worlbikachul-Tang, Mahaengkamsuk-Tang, Shinbi-Tang, Mahwangbujaseshin-Tang, Euiin-Tang, Galgeun-Tang, Mahaengeuigam-Tang, Mahwang-Tang and Socheongryong-Tang showed inhibition of NO production but solely used Herba Ephedrae and Gaemagakban-Tang did not exhibit such reaction.

Protocol for Systematic Review of Controlled Trials of Korean and Chinese Herbal Treatments for Stroke

  • Background: Many stroke patients receive traditional medical care in Korea and China. Stroke patients comprise the largest proportion of inpatients in traditional Korean medicine and traditional Chinese medicine hospitals. The purpose of this study is to identify the types and effects of Chinese medicine widely used in China and Korea, and to apply them to future studies of stroke. Methods: Nine Korean and Chinese databases will be surveyed for clinical studies of herbal medicines for stroke, published between the inception of the database and August 2017. Clinical studies of decoctions or modified decoctions will be included without restriction on study type. The frequencies and patterns of formulas or single herb usage and the any type of herbal medicine utilization will be analyzed. Dissemination: The results of the systematic review will be published in a peer-reviewed journal and disseminated electronically and in print. Updates of the review will be conducted to inform and guide healthcare practices.

Herbal Medicine Therapeutic Attempt for Diarrhea Induced by Carcinoid Symptoms of Rectosigmoid Neuroendocrine Tumor with Liver Metastasis Patient : a Case Report (간전이가 있는 직장구불결장의 신경내분비 종양 환자의 카르시노이드 증후에 의해 발생한 설사에 대한 한약 치료 시도 : 증례 보고)

  • This study aims to report a case of rectosigmoid neuroendocrine tumor patient with liver metastasis whose outstanding carcinoid symptom was mucosal diarrhea for more than 10 times a day. Three different herbal decoctions were administered orally for the treatment of diarrhea which was among carcinoid symptoms from diffuse metastatic liver lesion. The herbal decoctions given were modified Lijung-tang, modified Sambaek-tang and modified Jeokseokjiuyeoryang-tang. The administration period was 5, 14 and 9 days respectively. Frequency of patient's mucosal diarrhea had increased consistently during hospitalization. It had almost doubled compared to pre-hospitalization up to 27 times/24 hrs. But during the period of modified Sambaek-tang administration, frequency of diarrhea was maintained between 9 to 15 times/24hrs. There was no difference of liver tumors between Nov 16th 2013 and Dec 9th 2013 in computed tomography images. Three conventional established herbal prescriptions administered in this case didn't show any favorable effect on improving mucosal diarrhea induced by carcinoid symptoms of a rectosigmoid neuroendocrine tumor with liver metastasis. Even if modified Sambaek-tang showed partial effect on frequency of diarrhea, we concluded that it was not sufficient to be a therapeutic method of the diarrhea as carcinoid symptom. Furthermore, it is necessary to conduct additional studies about the diarrhea of neuroendocrine tumor with liver metastasis.

Quantitative analysis of the marker compounds in the decoctions of Coptis chinensis-Scutellaria baicalensis at different proportion produced by 'Mixed decoction' and 'Single decoction mixture' (배합 비율에 따른 황련과 황금의 혼합 전탕액 및 개별 전탕 혼합액 내 성분 함량 분석)

  • Objective : The present study aimed to evaluate the change of the content of 7 active components in decoctions produced by various proportional pairs of Coptis chinensis Franch and Scutellaria baicalensis Georgi in 'Mixed decoction (MD)' and 'Single decoction mixture (SDM)'. Methods : The samples of MDs were prepared by decocting C. chinensis : S. baicalensis with the ratios of 10 g:10 g, 10 g:20 g, and 20 g:10 g. Those of SDMs were prepared by blending each single decoction from C. chinensis and S. Baicalensis with the ratios of 1:1, 1:2, and 2:1. The samples were evaluated by high-performance liquid chromatography with statistical analyses. Results : The analytical methods, which were optimized and validated, were reliably applied to present research. The content of all components in both MDs and SDMs at C. chinensis : S. baicalensis = 1:1 ratio were reduced compared with single herb decoction. The components from each compositional herb in MDs were proportionally increased with the ratio of original herb increased, but inversely proportional to paired herb. The contents of components in MDs were significantly lower than those in SDMs at all ratios, except for high content of baicalin at C. chinensis : S. baicalensis = 2:1. Conclusion : It was concluded that MDs and SDMs as well as the proportions of herbs could affect the contents of the components from original herbal medicines. These results provide the information for the quality control of herbal medicine combined C. chinensis with S. baicalensis.

Survey on the Preference Formula for the Improvement of Oriental Herbal Medicine Insurance (한약건강보험에 대한 일반인의 한약제형 선호도 조사)

  • Objectives : This study aimed to provide basic data for future policies regarding the expansion of health insurance, through investigating the current status of Oriental herbal medicine insurance as perceived by the public. Methods : Questionnaire was developed through literature searches, in-depth interviews, item generation and item reduction. The questionnaires were further refined by a focus group. 221 surveys were retrieved from the general public. Results : The in-depth interviews, reveal that the public is aware of the various forms of Oriental herbal medicine formulas, except for the use of mixture of which are soluble granules covered by insurance. The public awareness of the soluble granules turned out to be so low that enhanced awareness of it is required. The public identified decoctions as the most preferred formula as well as the most effective formulas out of all Oriental herbal medical formulas. They also suggested that it should be the first to be included when the insurance policy expands in the future. The public had frequent experienced not choosing Oriental herbal prescription due to the high cost. Insurance coverage of Oriental herbal prescription is much needed. The public indicated that they will make frequent visits to the KMD if the insurance covers Oriental herbal prescriptions. Conclusions : The current status quo of Oriental herbal medicine insurance showed that the public identified decoctions as the most preferred, most effective and the top priority to be covered by insurance.

Trends in Clinical Research on Herbal Medicine Treatment in Pediatric Asthma - Focusing on Randomized Controlled Clinical Trials in Traditional Chinese Medicine (소아 천식의 한약 치료에 대한 임상연구 동향 - 중의학 무작위 대조군 임상시험을 중심으로)

  • Objective This study analyzed randomized clinical trials (RCTs) on pediatric asthma using traditional Chinese medicine (TCM) to determine its efficacy and safety. Methods We searched electronic databases in English, Chinese, Japanese, and Korean and evaluated 35 articles published up to December 28, 2022. Based on data from the literature, we analyzed treatments, results, composition of herbal medicine, frequency of medicinal herbs, and decoction. Results TCM for pediatric asthma was compared to supportive therapy, inhaled corticosteroids, and standardized treatments. Of the 35 studies, 32 showed that TCM treatments were effective in alleviating asthma symptoms. Furthermore, TCM treatment was considered safe compared to common asthma treatments. Among the TCM treatments for pediatric asthma, the most frequent decoctions were Gamisaganmahwangtang (加味射干麻黃湯) and Saganmahwangtang (射干麻黃湯). Ephedra Herba (麻黄), Armeniacae Semen (杏仁), Asiasari Radix et Rhizoma (細辛), Belamcandae Rhizoma (射干) were herbs that were frequently used for asthma in TCM decoctions. Conclusion This review showed that TCM is effective in treating pediatric asthma. More clinical RCTs are needed to confirm the efficacy and safety of TCM treatment.

Banhasasim-tang Administration for Treatment of Functional Gastrointestinal Disorders after Motor Vehicle Accidents: Case Series (교통사고 이후 발생한 기능성 위장관장애에 대한 반하사심탕 투여 : 연속증례)

  • Objectives: These cases report the benefits of administration of Banhasasim-tang extracts or decoctions to patients with functional gastrointestinal disorders after motor vehicle accidents. Methods: The patients were treated with Banhasasim-tang extract or decoction three times daily. History-taking and imaging tests were used to differentiate other diseases. We evaluated the patients using the Gastrointestinal Symptom Rating Scale (GSRS), which is used to assess overall gastrointestinal symptoms. Results: Treatment with herbal medicine resulted in a decrease in the patients' complaints of symptoms. The GSRS scale showed improvement in all four cases. Conclusions: Banhasasim-tang, regardless of its form, has beneficial effects in patients with functional gastrointestinal disorders after motor vehicle accidents.

Successful Treatment of Two Patients with Alcoholic Liver Disease Using Herbal Decoction: Case Report

  • Objectives: This case report is presented to introduce the effectiveness of herbal decoction on patients with alcoholic liver disease. Methods: We closely observed two patients who were admitted in the Department of Internal Medicine Department 1 of Kyung Hee Oriental Medicine Hospital due to alcoholic liver disease. We gave herbal decoction to these patients and then analyzed the changes in their general conditions as well as blood test results. Results and Conclusions: The treatments with herbal decoctions Cheongganhaeju-tang and Injincheonggan-tang were helpful for ALD patients. Conclusions: To provide more qualified medical service in the TKMH, they will be required to increase the proportion of non medical doctor employees, like Western medicine counterparts.
