Inhibitory Effect of a Decoction of Eucommiae ulmides OLIVER and Dipsacus asperoides C. Y. Cheng et T.M.Ai on Collagen II-induced Arthritis Mice (Collagen II-induced Arthritis 생쥐에 대한 두충(杜仲).속단(續斷) 배합약물의 관절염 억제 효과)

  • Objectives : The object of this study was to verify the inhibitory effect of a decoction of Eucommiae ulmides OLIVER (EU) and Dipsacus asperoides C. Y. Cheng et T.M.Ai (DA) on Collagen II-induced Arthritis Mice (CIA mice). Methods : DBA/1OlaHsd mice were immunized with bovine type II collagen. Boostnig same collagen 21 days later, arthritis was induced and then administrated orally the extract of EU+DA (200 or 50 mg/kg) once a day for 4 weeks and compared with that of methotrexate (MTX, 0.3 mg/kg) as a positive control. Results : Administration of EU+DA suppressed the inflammatory progression of CIA mice and the results were 1. Arthritis index of CIA mice was decreased. 2. EU+DA decreased the production of TNF-$\alpha$, IL-6, IL-$1{\beta}$ in the serum of CIA mice. 3. EU+DA decreased the level of IgM. 4. EU+DAincreasaed $CD3^+$, $CD4^+$, $CD4^+$/CD25 but decreased $CD19^+$, $CD3^+/CD49b^+$(NKT), $CD3^-/CD49b^+$(NK), $B220^+/CD23^+$ in PBMC of CIA mice. 5. EU+DA decreased $CD3^+$, $CD4^+$, $CD3^+/CD69^+$ of paw joint in CIA mice. 6. EU+DA decreased subsynovial inflammation. Conclusions : This results demonstrated that extract of EU+DA suppressed the inflammatory progression of CIA mice and supported further studies are required to clarify a mechanism of therapeutic role.

Quantitative Analysis of the Eleven Marker Components in Traditional Korean Formula, Jakyakgamcho-Tang Decoction Using an Ultra-Performance Liquid Chromatography Coupled to Electrospray Ionization Tandem Mass Spectrometry (UPLC-MS/MS를 이용한 작약감초탕 물 추출물 중 11종 성분의 함량분석)

  • Jakyakgamcho-tang is a well-known traditional herbal medicine and has been used for the treatment of mainly pains in oriental medicine. In this study, analytical method for the quantitative determination of the eleven marker components, gallic acid (1), oxypaeoniflorin (2), paeoniflorin (3), albiflorin (4), liquiritin (5), isoliquiritin (6), ononin (7), liquiritigenin (8), benzoylpaeoniflorin (9), paeonol (10), and glycyrrhizin (11) in Jakyakgamcho-tang decoction was performed using an ultra-performance liquid chromatography-electrospray ionization-mass spectrometer. The analytical column for separation of the compounds 1~11 was used an UPLC BEH $C_{18}$ ($100{\times}2.1mm$, $1.7{\mu}m$) column and column oven temperature was maintained at $45^{\circ}C$. The mobile phase consisted of 0.1% (v/v) aqueous formic acid (A) and acetonitrile (B) by gradient elution. The flow rate was 0.3 ml/min and injection volume was $2.0{\mu}l$. Correlation coefficient in the calibration curves of the compounds 1~11 were showed a good linearity with more than 0.99. The limit of detection and limit of quantification values of the compounds 1~13 were detected in the ranges 0.06~18.43 ng/ml and 0.18~58.29 ng/ml, respectively. Among the compounds 1~11, the compounds 10 were not detected in this sample, while the ten compounds, 1~9 and 11, were detected $44.05{\sim}19,289.05{\mu}g/g$ in Jakyakgamcho-tang extract.

Survey on Records about Magnetism in Written Korean History (Records Regarding the Applications of Magnet for Oriental Medicine) (한국사 속의 자기(磁氣) 관련 기록에 대한 조사 연구 (자석의 한방 응용 기록을 중심으로))

  • Until the discovery of universal gravitation, a magnet had been considered to be a unique and magical stuff which exhibited an interacting force between two objects separated afar. Perhaps because of this belief, magnets were used widely as a medicinal stuff in East-Asia. In the present article, the records regarding the applications of a magnet for oriental medicine in written Korean history were surveyed. The records were categorized in terms of efficacy, prescription, and clinical case of taking-medicine. The records revealed that a magnet was used as an important component in the medicine for curing kidney filtering failure. Magnet was also prescribed to heal heart problems. Typical magnet-containing medicines included the SHINRYUKTANG, JIHWANGTANG, and INSAMBAEKHOTANG. Interestingly, it was revealed that some of the kings (Hyojong, Yungjo) in the era of Josun dynasty (1392-1910) believed the efficacy of the magnet-containing medicine and took it for curing their health problems.

Anti-aging and Anti-Oxidative Effect of SYG (SipYiMiGwanJungTang) in aged Rats (십이미관중탕(十二味寬中湯)의 노화(老化) 억제 효과에 대한 실험적(實驗的) 연구(硏究))

  • The purpose of this study is to investigate the anti-aging and anti-oxidative effects of oral administration with SYG (SipYiMiGwanJungTang) decoction in early aged rats. The results were as follows : 1. At the 52 week of the SYG group, the body weight gain was decreased. 2. At the 52 week of the SYG group, the activity of the SOD in the liver was significantly increased than the 10, 22 and 40 week. 3. At the 52 week of the SYG group, the level of the glutathione in the liver was significantly increased than the normal group. 4. At the 68 week of the SYG group, the activity of the catalase in the liver was significantly increased than the normal and control group. 5. At the 52 week of the normal group, the level of the NO in the liver was significantly increased than the 22 and 40 weeks. But, the level of the NO of the SYG group was not changed by the aging. 6. At the 52 and 68 weeks of the SYG group, the level of the MDA in the liver was significantly decreased than the normal and control groups. These results suggest that oral administration of SYG (SipYiMiGwanJungTang) decoction has anti-oxidative and anti-aging effects in early aged rats.

Metal Concentrations in Rats and Fetuses Treated by Herbal Decoction, Samultang (사물탕투여가 임신랫드의 모체 및 태자에 미치는 영향 -유해금속(As, Cd, Pb, Hg)과 양-반응관계를 중심으로-)

  • Purpose : To evaluate the effects of Samultang(SMT) on the metals in the dams and fetuses Method : SMT(Decoction) was administered orally using different concentrations to pregnant Sprague-Dawley rats for 20days after being assigned randomly to 5 groups. Each group has 11 dams. Pregnant rats were sacrificed at the 20th day of gestation and metal concentration in the dams and fetuses was observed using a ICP-MS and mercury analyzer. Results : Hg concentration in the blood of dams was lower in the $SMT{\times}1$, $SMT{\times}2$, $SMT{\times}4$group than VC(Vehicle Control) and Control group. Hg concentration in the organs of dams was lower in the $SMT{\times}1$, $SMT{\times}2$, $SMT{\times}4$ group than VC, Control group. Whereas Cd concentration in the organs of fetuses was getting lower as increasing SMT dose, Hg concentration in the organs of fetuses was getting higher as increasing SMT dose. Discussion and Conclusion : There were not any significant harmful differences of metal concentration in the dams and fetuses treated by SMT. Particularly, SMT showed the possibility of efficacy to reduce the concentration of Hg in dams but to increase the concentration of Hg in fetuses. The mechanism of reduction is not yet proven. A hypothesis says there is a possibility of the chelation effect between the essential elements of herbs and heavy metals. Another hypothesis is the possibility of Hg transmission from dam to fetus. Further study should be conducted to make guidelines for the safe use of herbal medicine.

  • Guillain-Barre syndrome, or acute inflammatory demyelinating Polyradiculoneuropathy, is frequently accompanied by cardiac and autonomic dysfunction. We report a patient who had tachycardia, orthostatic hypotension, hypertension, pronounced blood pressure fluctuations, abnormal sweating, constipation and urinary frequency as well as qudriparesis. We thought that the GBS was incurred by Damp-Heat, used Heat-Clearing and Dampness-Transforming decoction(Chongjoo-tang) in the early stage. In the later stage, fortifying the Spleen and Boosting Qi plus Supplementing the Kidney decoction(Palmultang+chongawon) was used to remove low back pain and boost recovery. The patient reached the nadir 14 days after onset. He became bed-bound and autonomic dysfunction was very severe. From 3rd week, abnormalities of autonomic function and paresis impoved gradually and he could walk above 5m without walker or equivalent support at the 5th week after onset.

A Patient with Moderate Insomnia Improved after Treatment with Yukmijihwang-tang (Liuwei Dihuang tang), Hwangryunhaedok-tang (Huanglian Jiedu tang), and Electroacupuncture: A Case Report (중등도의 불면환자를 육미지황탕(六味地黃湯)(Liuwei Dihuang decoction)과 황련해독탕(黃連解毒湯)(Huanglian Jiedu Decoction) 병용 투여 및 배수혈 전침치료로 호전시킨 1례 : 증례보고)

  • Objectives: Insomnia is a common condition that causes deterioration of cognitive function and physical function and has a negative impact on quality of life. Insomnia is experienced by more than one-third of the population of the world. Although sleeping pills are currently used as therapeutic agents, they have side effects, so safer treatment methods are needed. Therefore, we report a case of an outpatient who sleeps only one hour a day due to insomnia. Methods: The patient visited Korean Medicine Hospital 10 times for four weeks and was treated with Korean medicine therapies, including electroacupuncture on the back-Shu points and herbal medicine (Yukmijihwang-tang and Hwangnyeonhaedok-tang). Treatment progress was assessed using the Pittsburgh Sleep Quality Index (PSQI) and the Insomnia Severity Index (ISI). Results: After 29 days of treatment, the PSQI score decreased from 20 points and 14 points. The ISI score also decreased to decreased to 22 points from 26 points. The average daily sleeping time increased from 60 minutes to 197.1 minutes after treatment. No significant side effects were observed. Conclusions: Yukmijihwang-tang and Hwangnyeonhaedok-tang with electroacupuncture might be a recommended therapeutic option for insomnia patients.

  • Hwanggeumjackyak-tang (HJT) composed of Scatellaria baicalensis Georgi, Paeonia Lactiflora and Glycyrrhizae uralensis Fischer is a traditional Korean herbal medicine widely used for acute enteritis. In order to develop the tablet formulation of HJT, evaluation of the flow properties, thickness, diameter, hardness, friability and disintegration was carried out on four HJT granules according to mixed content of seven additives. Simultaneous analysis used HPLC method was performed of HJT tablet and was determined of the seven marker components; Albiflorin, Paeoniflorin, Liquiritin, Baicalin, Baicalein, Glycyrrhizic acid and Wogonin. The biological activities were examined the effect of HJT on anti-oxidation and pro-inflammation mediated by LPS-stimulation. We confirmed that both of HJT-Decoction (HJT-D) and HJT-Formulation (HJT-F) have the similar contents on total polyphenol and flavonoid and inhibited the secretion of nitro oxide (NO), interleukin $(IL)-1{\beta}$,interleukin (IL)-6, tumor necrosis factor $(TNF)-{\alpha}$ and the expression of nitric oxide synthase (iNOS), $IL-1{\beta}$, IL-6, $TNF-{\alpha}$. Therefore, the developed formulation for tablet of HJT would provide chemically and biologically the same effect compared with decoction of HJT.

Stimultaneous Determination of Ephedrine Alkaloids in Ephedra sinica and Wolbigachul-tang by High Performance Liquid Chromatography (고성능 액체 크로마토그래피를 이용한 마황 및 월비가출탕 전탕액에서 에페드린류의 동시분석)

  • Objectives: Ephedra sinica and Wolbigachul-tang which contains Ephedra sinica are used to treat obesity in Korean medicine. The aim of this study was to analyze the quantities of ephedrine alkaloids by high-performance liquid chromatography. Methods: The analysis was performed using a YMC-Triat C18 column with operating at 25℃, and UV detection at 210 nm. The mobile phase used a gradient flow with 0.1% H3PO4 in water and acetonitrile. Specificity, linearity, precision, accuracy, limit of detection, and limit of quantification were measured for validation anaylsis. This method was applied to analyze the quantities of ephedrine alkaloids in Ephedra sinica and Wolbigachul-tang. Results: The concentration per Ephedra sinica (gram) of ephedrine and pseudoephedrine in Ephedra sinica decoction are 4.74±0.22 mg and 2.19±0.10 mg, respectively and in Wolbigachul-tang decoction are 6.39±0.34 mg and 2.97±0.21 mg, respectively. The retention time of ephedrine was 23.6 min and that of pseudoephedrine was 25.8 min, and norephedrine and methylephedrine were not detected. Conclusions: In conclusion, analyzed the concentration of ephedrine alkaloids in Ephedra sinica and Wolbigachul-tang by the developed validation method.

Clinical Remission of Crohn's Disease Induced by Herbal Decoction (Jinmoo-tang): A Case Report (진무탕으로 임상적 관해가 유도된 크론병 환자: 증례보고)

  • Crohn's disease (CD) is an idiopathic inflammatory bowel disease (IBD) with unknown etiopathogenesis. Currently there are no definitive treatment modalities for CD. This case report presents clinical remission and its maintenance of CD induced by Korean herbal medicine treatment. A 56-year-old male patient diagnosed as CD by capsule endoscopy presented to a Korean Medicine clinic with severe abdominal pain as a chief complaint. Despite taking 5-aminosalicylic acid and immunosuppressant for 2 years, his abdominal pain was worsening at the time of visit. Treatment was initiated with Jinmoo-tang based on pattern identification and continued for approximately a year with some modifications according to patient's symptom changes. He was mainly given Jinmoo-Tang for a year and clinical remission was achieved; he could stop taking western medications. For 6 years since then, he took Korean medicine from time to time when he felt his condition went down and he is still maintaining remission. There were no adverse events reported. This case report suggests that individualized Korean herbal medicine treatment may have the potential for induction and maintenance of clinical remission of CD. Further research is warranted to establish an evidence-base for such approach.
