호스피스 자원봉사자의 존엄사에 대한 태도요인 (Attitudes of Hospice Volunteers towards Death with Dignity)

  • The purpose of this study was analyzed the factors of influencing toward attitude to death with dignity to hospice volunteers. The data was collected for 21 days from 14 March to 3 April 2010. Among a total of 220 cases of the questionaries, only 195 cases were used. To data were analyzed by factor analysis, independent t-test, one-way anova and logistic regression using PASW statistics 18.0. The results were as follows; The attitudes towards death with dignity according to general characteristics was high in those with will to agreed to the passive euthanasia than those opposite to the attitude factors, namely, acceptive, the right to decide, negative, and dereliction of duty attitude factors. Significant variables for effects of death with dignity were gender, acceptive attitude factor and dereliction of duty attitude factors. Given that main provider of human organs is the brain-dead and we don't have enough organ donation, death with dignity should be linked with activating policy of organ donation, while solving donation shortage problem. This way, constructing social implementation and sharing consciousness on organ donation, would be diluting the bio-ethic controversies.

Determination of Methamphetamine and its Metabolite Amphetamine in Biological Fluids from 11 Fatal Gases

  • Gas chromoatography with flame ionization detector (FID) along with mass spectrometry (GC/MS) were used for the screening and quantification of methamphetamine (MA) and its major metabolite, amphetamine (AM0, in blood and urine in eleven fatal cases in which MA abuse was suspected. Postmortem blood MA varied from $0.5-30.2\;\mu{g/ml}$, while Am levels ranged from none detected (6 of 11 cases) to 4.8 .mu.g/ml. Additionally, distribution studies were performed in three of these cases in which tissue smaples were available for evaluation. Liver contained the highest ocncentration of MA among the tissu samples. In eight of the eleven cases, when no other direct cause of death was evident (i.e. 3 cases of traumatic dath0, either no blood AM was found or the ratio of MA/AM was 3.4 or greater. These data are consistent with acute MA use followed by death due to acute drug intoxication or by the occurrence of hypersensitivity and reverse seen in cases of chronic drug abuse.

개심술 39례의 임상적 고찰 (Clinical Analysis of 39 Cases of Open Heart Surgery)

  • Over a period from July 1984 to June 1985, 39 cases of open heart surgery were done under cardiopulmonary bypass. There were 23 male and 16 female patients, ranging in age from 18 months to 58 years. Thirty cases were valvular disease. The most common of congenital heart disease is VSD. Among the 9 cases of acquired valvular diseae,1 MAP, 5 MVR, 2 MVR+TAP, and 1 MVR+AVR were done. There were 1 operative death[11%] which was done MVR+TAP. Operative mortality in 30 congenital heart disease was 7%; 1 death [5%] in 22 acyanotic and 1[13%] in 8 cyanotic cases. Finally, overall mortality for 39 cases was 8%; 7% for congenital heart disease and 11% for acquired heart disease.

폐쇄성 하지 동맥 경화증의 임상적 고찰 (A Clinical Analysis of Atherosclerosis Obliterance in the Lower Extremity)

  • aortoiliac pattern, Group II; femoropopliteal pattern and Group g; tibioperoneal pattern. A majority of patients belonged to group I [27 cases], 8 patients came under group II .and none in group g. Thirty patients underwent bypass operation with autogenous saphenous vein or synthetic graft with or without concomitant lumbar sympathectomy. Remaining 5 patients were operated on with sympathectomy only, Bypass procedures were anatomic bypass in 22 cases: aortoiliac artery bypass in 11 cases, femoropopliteal artery bypass in 10 cases, sequential femoropopliteal artery bypass in one case and extra-anatomic bypass in 8 cases, axillary-bifemoral artery bypass in one case and femorofemoral artery bypass in 7 cases. Postoperative complications which mainly composed of superficial wound infection[5 cases] which were treated without any significant sequel in all cases and thrombosis[2 cases]. Three patients died whose causes of death were acute renal failure in 2 cases and myocardial infarction in other, The overall patency, rate was 70Zo in 5 years. In conclusion, the clinical pattern and operative outcome were similar to he western pattern and all cases of death did not related to operative procedures and ischemic symptoms were relieved by bypass operations except several cases. I think and recommend that all patients suffering chronic arterial insufficiency by atherosclerosis obliterans ought to be managed with urgent and adequate operative procedure.

Palliative Care for Patients with Gynecologic Cancer in Japan: A Japan Society of Gynecologic Palliative Medicine (JSGPM) Survey

  • Purpose: To evaluate palliative care for patients with gynecologic cancer in Japan. Materials and Method: A questionnaire asking facility characteristics, systems to coordinate palliative care, current status of end-of-life care, provision of symptom relief, palliative radiation therapy and chemotherapy, and cases of death from gynecological cancer, was mailed to facilities treating gynecologic cancer. Results: A total of 115 facilities (29.3% of the total) responded to the questionnaire. Of these, 33.0 (29.0%) had a palliative care ward. End-of-life care was managed by obstetricians and gynecologists in 72.0% of the facilities. The site where end-of-life care was provided was most often a ward in the department where the respondent worked. The waiting period for transfer to a hospice was 2 weeks or more in 52% of facilities. Before the start of primary treatment, pain control was managed by obstetrians and gynecologists in 98.0% of facilities. Palliative radiation therapy or chemotherapy was administered at 93.9% and 92.0% of facilities, respectively. Of the 115 facilities, 34.0 (29.6%) reported cases of death from gynecological cancer. There were 1,134 cases of death. The median time between the last cycle of chemotherapy and death was 85 days for all gynecological cancers. The proportion of patients receiving chemotherapy in the last 30 and 14 days of life were 17.4% and 7.1%, respectively. Conclusions: This large-scale survey showed characteristics of palliative care given to patients with gynecologic cancer in Japan. Assessment of death cases showed that the median time between the last cycle of chemotherapy and death was relatively short.

승모판막질환의 재수술에 대한 임상적 고찰 (Clinical Evaulation of Reoperation for Mitral Vavular Disease)

  • Total 20 cases of reoperation for mitral vavular disease, which had been performed during the period from May 1983 till October 1991, were reviewed. This study contained 1 case of previous balloon valvuloplasty and 1 case of death with bleeding from right ventricle during sternal reenrty. The average time intervals between reoperation and previous operation was 19 years in closed mitral commissurotomy[n=4], 7 years and 2 months in mitral valve replacement[n=10], 1 year and 8 months in mitral valvuloplasty[n=4], 3 years and 10 months in open mitral commissurotomy [n=2]. The cause of reoperation in closed mitral commissurotomy was progression of the disease, and residual stenosis with progression was the cause in open mitral commissurotomy cases. Technical failure might be the cause in the cases of valvuloplasty. In prosthetic valve replacement group the causes of reoperation were primary failure. Also two cases of suggested valve thrombosis and one case of failure of tricuspid annuloplasty was noted in prosthetic valve replacement group. The used valves for reoperation were Ionescue-Shiley in 3 cases, Bjork-Shiley in 6 cases, St. Jude Medical in 2 cases and CarboMedics in 8 cases. The mortality rate was 20%[n=4] and the causes of death were low output syndrome in 1 case, multiple organ failure in 2 cases and bleeding in 1 case during sternal reentry. During follow up 1 case of sudden death was observed.

삼첨 판막이식 8례 보고 (Tricuspid Valve Replacement: A Report of 8 Cases)

  • Between April 1976 and March 1978, six cases of tricuspid valve replacement were done in the Department of Thoracic Surgery, Seoul National University Hospital. There were 4 men and 2 women and the age of the patients ranged from 17 years of the youngest to 48 years of the oldest. Most of them had characteristic symptoms of tricuspid valve disease, such as a systolic murmur audible over the lower sternum and varying with respiration, pulsatile and distended neck vein, and an enlarged and pulsatile liver. Preoperative functional levels according to NYHA Calcification were class III in 4 cases, and class IV in 2 eases. Most of the cases showed moderate to severe cardiomegaly in chest films and elevated right atrial pressure on preoperative right heart catheterization. Five of them underwent concomittent mitral valve replacement and one pulmonary valvotomy. All of them showed tricuspid insufficiency resulted from massive dilatation of annulus, destructive lesions of valve structure, or both anomalies. One postoperative hospital death was encountered and the cause of death was low out-put syndrome. All survivors showed clinical improvement and cardiomegaly regressed and left hospital in a good condition . *Attendum; Recently 2 more cases of tricuspid valve replacement with mitral valve replacement were done after this review.

만성 농흉에 대한 Modified Eloesser's Operation 과 그 의의 (Modified Eloesser's Operation and Its Result In Chronic Empuema with Poor General Condition or Chachecic State)

  • During the past 3 years, 15 patients who could not be anesthetized generally because of poor general condition or cachecic state, of 111 patients with empyema, have been treated with modified Eloesser`s operation under the local anesthesia with 2% procaine. There were 13 males and 2 females ranging from 21 years to 61 years of age. The etiology was tuberculosis [6 cases], pyogenic pneumonia [5 cases], lung abscess [1 case], post-trauma [2 cases] and malignancy[1 case]. " The over-all mortality rate was 6.7%[1 case] and cause of its death was poor oral feeding because of post-traumatic psychosis. Modified Eloesser`s operation was performed after closed tube thoracostomy and irrigation with 1% zephanon solution for over 2 weeks. And then the other operation was not performed and all patients except 1 case appeared good progression (Complete healing; 4 cases, Progressive healing 10 cases, death: 1 case).

The COVID-19 Pandemic and Instability of Stock Markets: An Empirical Analysis Using Panel Vector Error Correction Model

  • The objective of this research is to examine the influence of the COVID-19 pandemic on stock markets in a few developing and developed countries. This study uses daily data from January 2020 to May 2021 and obtained from World Health Organization and Thomson Reuters. The secondary data was evaluated through panel econometric methodology that includes different unit root tests, and to analyze the long-run relationship between variables, panel cointegration techniques were applied. The long-run causality among variables was examined through Panel Vector Error Correction Model. The overall findings of this study suggest a long-run association exists between several cases and death with the stock returns of the GCC and other stock markets. Furthermore, the VECM model also identified a long-run causality running from COVID cases and death towards the stock rerun of both sets of stock markets. However, a subsequent Wald test yielded mixed results, indicating no short-run causality between cases and deaths and stock returns in both groups; however, in the case of GCC, several COVID-19 cases are having a causal impact on stock markets, which is notable in light of the fact that the death rate in GCC is significantly lower than in many developed and developing countries.

  • Cardiopulmonary arrest has long been accepted as an unquestionable definition of death. An advent of cardiopulmonary resuscitation and artificial ventilation along with the development of organ transplantation has prompted the emergence of the concept of brain death. The criteria for brain death are based mainly on the clinical examination of coma, apnea and total loss of brain stem function. Although organ transplantation by donor brain death has increased in Korea over recent years, there is still a substantial shortage of donor organs compared to the demand. Improvement of government policies and changes of social culture for organ donation are needed for the activation of organ transplantation by donor brain death. Pediatricians have an important role for the search of potential donors in cases of brain death and optimal medical care for successful organ transplantation.
