Microsurgical Training using Preserved Saphenous Vein (혈관문합 연습을 위한 보존된 복재정맥의 활용)

  • Purpose: Given that the critical nature of the microvascular anastomosis to what is often a long and difficult reconstructive operation, trainees need to have a high level of microsurgical competence before being allowed to perform microsurgery on patients. Some artificial substitutes and dead or live animal models have been used to improve manual dexterity under the operating microscope. Yet, most surgeons are not equipped with such models, so search for easy available and appropriate microsurgical practice model have been an issue. Umbilical artery, placental vessels and gastroepiploic arteries have been previously suggested as a microsurgical training model, which involves other surgical departments. The purpose of this article is to introduce that saphenous vein specimen obtained from varicose vein surgery is useful and has many advantages as training model for the practice of microvascular anastomosis. Methods: The conventional technique using perforation/inversion method with a metallic stripper is widely performed for varicose vein patients. The stripper is inserted through disconnected safeno-femoral junction and retrieved at the knee or the medial side of ankle. The length of saphenous vein specimens removed is about that of one's leg and inversed from inside out. Obtained saphenous vein specimens are re-inversed and cleansed with normal saline, to be readily available for microsurgical practice. Preserved in a squeezed wet saline gauze and refrigerated, frozen or glycerated specimens were investigated into their comparative quality for microsurgical practice. Results: Varicose vein surgery remains one of the common operations performed in the field of plastic surgery. Convenient informed consent regarding the vessel donation can be easily signed. The diameter of the obtained saphenous vein is as variable as 1.5 to 6 mm, which is already stripped, and is in sufficient length corresponding to that of patient's leg. Vessels specimens were available for microsurgical practice within 1 week period when preserved with squeezed wet saline gauze, and the preservation period could be extended monthly by freezing it. Conclusion: Saphenous vein obtained from varicose vein patients provide with variable size of vessel lumen with sufficient length. The practice can be cost effective and does not require microsurgical laboratory. Additionally there is no need of involving other surgical departments in acquiring vessel specimens. Furthermore, simple preservation method of refrigerating for a week or freezing with squeezed wet saline gauze for a month period, allow the saphenous vein obtained after varicose vein surgery as an excellent model for the microsurgical practice.

Long-term Survival after Coronary Artery bypass Surgery (관상동맥 우회술의 장기 생존율)

  • Background: There were very few reports on long-term survival after coronary artery bypass graft (CABG) in this country. The aim of this study is to investigate the long-term result in patients undergoing CABG in the early period in this hospital. Material and Method: One-hundred and fourteen patients (male/female, 79/35) who had undergone CABG from December 1990 to December 1995 were identified. Most of the patients had undergone CABG using left internal thoracic artery and vein grafts under cardiopulmonary bypass and cardiopulmonary arrest, and the proximal and distal anastomoses of the grafts were performed during the single aortic cross clamping period. Result: During the mean follow-up period of $135.5\pm17.9$ months, 37 patients $(32.5\%)$ were dead and only 10 patients $(27\%)$ of them died of cardiac cause. Risk-unadjusted survival after CABG was $95.6\%,\;85.1\%,\;71.8\%,$ and $57.9\%$ at 1, 5, 10, and 13 years, respectively, and cardiac death-free survival was $97.4\%,\;94.5\%,\;92.1\%$, and $81.3\%$ at 1, 5, 10, and 13 years, respectively. Predictable factors of long-term survival were sex and age. Predictable factors of postoperative coronary angiography and intervention were hypertension, diabetes, and dyslipidemia. Conclusion: Long-term survival after CABG in the early operative period was comparable to the previous outcomes, and females showed the better long-term survival. Postoperative coronary intervention was more common in patients with preoperative dyslipidemia.

Encapsulation of Agro-Probiotics for Promoting Viable Cell Activity (생균력 증진을 위한 농업용 미생물제 미세캡슐화)

  • In this work, to develop soil inoculant which maintains stable viable cells and normalized quality, studies on micro-encapsulation with bacteria and yeast cells were performed by investigating materials and methods for micro-encapsulation as well as variation and stability of encapsulated cells. Preparation of capsule was conducted by application of extrusion system using micro-nozzle and peristaltic pump. K-carragenan and Na-alginate were selected as best carrier for gelation among K-carageenan, Na-alginate, locust bean gum, cellulose acetate phthalate (CAP), chitosan and gelatin tested. Comparing the gels prepared with Bacillus sp. KSIA-9 and carriers of 1.5% concentration, although viable cell of K-carragenan and Na-alginate was six times higher than those of other, Na-alginate was finally selected as carrier for gelation because it is seven times cheaper than K-carragenan. The gel of 1.5% Na-alginate was also observed to have the best morphology with circular hardness polymatrix and highest viable cell. When investigating the stability of encapsulated cells and the stabilizer effect, free cells were almost dead within 30 or 40 days whereas encapsulated cells decreased in 10% after 30 days and 15-30% even after 120 days. As stabilizer for maintaining viable cell, both 1% starch and zeolite appeared to possess the level of 70-80% cell for bacteria and yeast until after 120 days.

Efficacy of Baycox(Bay Vi 9142) Suspension against Coccidium Isospora suis in piglets (신생자돈의 콕시듐증에 대한 Baycox(Bay Vi 9142) 현탁액의 효능)

  • I suis cocciosis in piglets seems to occur in the majority of Korean sow herds. Cocci야osis is characterized by a pasty diarrhea in piglets appearing at 5 to 11 days old. Morbidity is variable, Mortality is usually low but piglets growth is always retarded. And, the principal source of infection is the environment because of the high resistance of oocysts. Control of coccidiosis is usually frustrated since the majority of treatment have not constant results. Mundt et al(1990) developed a new strategy by administrating toltrazuril as an oral suspension. This new anticoccidial drug reduced clinical signs and mortality. Therefore, we will try to medicate piglets with Baycox(Bay Vi 9142) suspension in a sow herd, having a problem of I suis coccidiosis. The incidences of coccidiosis in suckling piglets in C and D bleeding stock farm were from 28.6 to 42.3 and 21.1~57.9 % during 3 to 21 day-old. The body weights of suckling piglets in treated groups were higher than those of control groups at 11 and 18 day-old. Also, those of C breeding stock firm were higher than those of D farm. The body weight gains of suckling piglet of treated groups were higher than those of control groups. Also, those of C breeding stock firm were higher than those of D farm. There was no dead piglet in all experimental groups. Some atrophic piglets were done away with. In C breeding stock farm, the incidences of diarrhea in treatment groups were significantly lower an those of control groups. Also, the degrees of diarrhea in treated groups were significantly lower than those in control groups. In D breeding stock farm, the incidences of diarrhea in treated groups were a little lower than those in control groups. Also the degrees of diarrhea in treated groups were a little lower than those in control groups. In the treated groups, the incidences and degrees of diarrhea were reduced at the 4th day and increased at the 13th day after treatment. The number of litters excreted oocysts in treated groups wert lower than those in control groups after treatment. Also, the OPG in treated groups were lower than those in control groups. According to above results, the effect of Baycox suspension against swine coccidiosis was very good. If we will treat with Barcox suspension swine coccidiosis in swine stock farms, we can get good effects of the improvement of body weight gains and diarrhea and e reduce of excreted costs in faeces.

Assessing Biosecurity Practices on Commercial Pig Farms across Korea and Risk Factors for Porcine Circovirus Type 2 Infection (국내 양돈장의 차단방역 수준 평가 및 돼지써코바이러스 2형 감염의 위험요인에 관한 연구)

  • The primary aim of this study was to investigate biosecurity practices in pig farms and to determine the major risk factors associated with PCV2 infection for a sampled swine population in Korea. To this end, we analyzed data from a cross-sectional study of 296 farrow-to-finish farms, which was conducted between March and September 2014 to explore the prevalence of swine disease at farm level. Face-to-face interviews by on-site visit of trained veterinarians were conducted with the farm owners or managers using a standardized questionnaires with information about basic demographical data and management practices. Farms were classified as negative or positive through the use of infection profiles that combined data on serological testing including PCR antigen test result, antibody titer and sero-conversion pattern at each age category taking into account vaccination status. Data were analyzed using multivariate ordinal logistic regression. Results from this study indicated that biosecurity level of the farms was considered not good given low compliance of the biosecurity programs and facilities in the farm: off-site removal of dead stocks (7%), off-site location of storage facility for incoming feeds (12.6%), off-site pick-up location for finishers (19.3%), restrictions on feed supplier vehicles for farm entrance (19.6%), restriction of finisher trucks entering the farm (22.4%), and restriction on manure disposal trucks entering the farm (26.4%). In the final model (n = 255), allowance of finisher truck driver to the pig unit had increased risk of infection (OR = 2.4, 95% CI 1.22-4.67) whereas farms with a sign forbidding the entrance had decreased risk of infection (OR = 0.19, 95% CI 0.10-0.58). Further comprehensive research with larger sample size is required to better understand the multifactorial characteristic that some predisposing risk factors that were not available in this study. To the best knowledge of the authors, this was the first study to use empirical data to report risk factors associated with PCV2 infection in the Korean pig farms. Results from the current study could be used to decide optimal biosecurity measures to reduce the impact of PCV2 infection to farmers and policy makers.

Heat-treatment Effects of Agricultural Imports for Devitalization of Quarantine Weed Seeds (검역잡초종자 발아력 제거를 위한 수입농산물 열처리 효과)

  • Korea is carrying out weed quarantine by plant quarantine regulations including heat-treatment method to prevent the influx of exotic weeds. In order to confirm suitability for current heat-treatment criteria and find field-applicable methods which can completely devitalize quarantine weed seeds, the conditions for heat-treatment with/without 40% relative humidity were studied with 9 species among quarantine weed seeds and one weed species similar to quarantine weed. Dry heat-treatments had been tested under various temperatures and time conditions. All seeds were dead at the conditions of $95^{\circ}C$ for 48 hours, $100^{\circ}C$ for 36 hours, $110^{\circ}C$ for 24 hours, $121^{\circ}C$ for 4 hours and $130^{\circ}C$ for 30 minutes. Heat-treatments at 40% relative humidity resulted in complete seed death at the conditions of $85^{\circ}C$ for 36 hours and $90^{\circ}C$ for 16 hours. The above results show that current heat-treatment criteria are not suitable for several quarantine weeds and these conditions could be applied as a quarantine method to prevent the influx of quarantine weeds along with agricultural imports. More specific conditions for heat tolerant species such as Picris echioides and heat susceptible species such as Cuscuta spp. are necessary and will improve plant quarantine process in devitalizing quarantine weed seeds with different heat tolerances.

Bacterial Common Blight and Fuscous Blight of Small Red Bean caused by Xanthomonas axonopodis pv. phaseoli and X. axonopodis pv. phaseoli var. fuscans (Xanthomonas axonopodis pv. phaseoli와 X. axonopodis pv. phaseoli var. fuscans에 의한 팥의 세균성잎마름병)

  • A bacterial disease of small red bean (Phaseolus angularis) was observed on field-grown plants in Suwon in year 2003. Leaf symptoms initially appeared as water-soaked spots that gradually enlarged, became flaccid and necrotic and were often bordered by a small zone of lemon yellow tissue. In the case of severe infection, dead leaves were defoliated. Pod symptoms consisted of the lesions that were generally circular, slightly sunken and dark reddish brown. Isolation made from diseased leaves on yeast extract dextrose calcium carbonate agar yielded nearly pure cultures of a yellow-pigmented bacterium typical of a xanthomonad. Three bacterial strains were purified and used for further tests. Pathogenicity of strains was confirmed on 3-week-old small red bean plants sprayed with bacterial suspensions containing $10^8 cfu/ml$ of phosphate buffered saline. The representative Xanthomonas strains isolated from small red bean were compared with X. axonopodis pv. phaseoli and X. axonopodis pv. phaseoli var. fuscans type strains for fatty acid profiles, biochemical tests and metabolic fingerprints using Biolog GN2 microplate, showing that all outcomes were indistinguishable between our isolates and reference strains. Two of three strains produced a melanin-like brown pigment extracellularly on King's medium B agar. These results suggest that this new small red bean disease observed in Suwon is bacterial fuscous blight caused by X. axonopodis pv. phaseoli and X. axonopodis pv. phaseoli var. fuscans.

Toxicity Studies of DA-l25, an Anthracycline Antitumor Antibiotic : Intravenous Repeated Doses for 26 Weeks in Beagle Dogs (Anthracycline계 항암성 항생물질 DA-125의 Beagle dog에 대한 26주 반복정맥투여독성시험)

  • This study was performed to investigate the toxicity of DA-125 in beagle dogs, an anthracycline antitumor antibiotic. The dogs were administered DA-125 i.v. at 0.0023, 0.0375, 0.15 and 0.6 mg/kg/day, 6 days/week for 26 weeks. At 0.6 mg/kg, all male and female dogs were either sacrificed moribundly or dead during the 26-week treatment. The dogs revealed inactivity, salivation, dark bloody discharge, swelling of the subcutaneous injection site, abscess, and ulceration in the abdominal wall and legs. At 0.15 mg/kg, anorexia, salivation, and swelling of the injection site were observed. The food consumption was decreased with a statistical significance at 6 and 12 weeks treatment in males of 7.6 mg/kg. At 0.0375, 0.15 and 0.6 mg/kg, body weights were decreased significantly in a dose-related fashion after 17 weeks treatment. Total white blood cell counts for male dogs at 0.6 mg/kg were lower than those of control dogs after 13 weeks treatment, which appeared mainly due to decreased neutrophils. At 0.15 mg/kg, testicular atrophy was found in all males by gross pathology and the testicular weights were significantly decreased when compared to those of control males. Microscopically, the testis showed moderate atrophy of the seminiferous tubules and marked decrease in number of spermatozoa in the epididymal tubules. At 0.6 mg/kg, petechia or echymotic hemorrhage was observed in gastrointestinal tract, heart, lungs, and other organs at the necropsy, Marked atrophy of thymus were observed in both males and females. In addition, severe testicular atrophy was noted in all males. Microscopically, gastrointestinal tract showed hemorrhage, epithelial denudation, hypermucus secretion, and atrophy of intestinal villi. Seminiferous tubules of the atrophic testis were lined with Sertoli cells only and devoid of germ cells. Severe oligospermia or aspermia was present in the epididymal tubules. Bone marrow showed marked depletion of hemopoietic cells. In addition, marked atrophy was found in the lymphoid tissue of gastrointestinal tract, various Iymph nodes, and thymus. Injection sites showed marked inflammatory response with necrosis, necrotizing vasculitis, thrombus formation, and ulceration in the skin. According to the present results, no observed effect level appeared to be 0.0375 mg/kg. At 0.15 mg/kg, testis was a target organ, while at 0.6 mg/kg hemopoietic tissue, gastrointestinal tract, and testis were considered to be target organs. At 0.6 mg/kg the test compound seems to inflict a damage on the blood vessels causing hemorrhage in the various organs and tissues.

Quarantine Pest Occurrence in Exporting Pear Fruits (Pyrus pyrifolia Nakai cv. Whangkeumbae) during Cold Storag (수출용 황금배의 저온저장 중 검역병해충 발생조사)

  • National plant quarantine inspector of exporting country should inspect agricultural products according to the quarantine requisites of counterpart country when the commodities are shipped. Export of pear fruits was held because quarantine pests including Conogethes punctiferalis were found at the point of entry in Canada. In order to examine the problems in detail, the inspected pear fruits were stored at a low temperature for 43 days, almost the same period necessary for fruit sorting, quarantine procedures, and transportation by ship. Dead larvae of fruit moths, C. punctiferalis and Grapholita molesta were found during cold storage. Quarantine pathogens were not found, but non-quarantine ones such as Penicillium sp. was infected through wounds by bruises and stabs resulted from sorting procedures. Because of the wounding problem during export procedures, pear fruits with thin exocarp, such as fruits of P. pyrifolia Nakai cv. Whangkeumbae, are required more careful handling during fruit sorting and conveyance.

Quantitative Analysis of Feline Calicivirus Inactivation using Real-time RT-PCR (Real-time RT-PCR을 이용한 Feline Calicivirus 불활성화의 정량적 분석)

  • Norovirus causes acute gastroenteritis in all age groups and its food poisoning outbreaks are rapidly increasing in Korea. Reverse transcription-polymerase chain reaction (RT-PCR) is most widely used for the rapid detection of foodborne viruses due to high sensitivity. However, the false positive results of RT-PCR obtained against already inactivated viruses could be a serious drawbacks in food safety area. In this study, we investigated a method to yield true positive RT-PCR results only with alive viruses. To decompose the RNA genes from dead viruses, the enzymatic treatments composed of proteinse K and Ribonuclease A were applied to the sanitized and inactivated virus particles. Another aim of this study was to quantify the efficiencies of several major sanitizing treatments using real-time RT-PCR. Feline calicivirus (FCV) that belongs to the same Caliciviridae family with norovirus was used as a surrogate model for norovirus. The initial level of virus in control suspension was approximately $10^4$ PFU/mL. Most of inactivated viruses treated with the enzymatic treatment for 30 min at $37^{\circ}C$ were not detected in RT-PCR, Quantification results to verify the inactivation efficiencies of sanitizing treatments using real-time RT-PCR showed no false positive in most cases. We could successfully develope a numerical quantification process for the inactivated viruses after major sanitizing treatments using real-time RT-PCR. The results obtained in this study could provide a novel basis of rapid virus quantification in food safety area.
