  • Many previous studies based on respiratory characteristics of Idiopathic Parkinson's Diseases (IPD) patients have not controlled related factors appropriately. Accordingly, these studies produced discordant results. Furthermore, there is currently a lack of studies that can provide precise explanations on the characteristics of respiration and phonation. This study included a total of 40 subjects: 20 mixed gender de novo IPD patients ranging in age from 50 to 80 (Hoehn & Yahr stage 1~3), and 20 normal subjects with similar matches for age and gender. All participants were controlled based on their gender, age, height, weight, vocal fold function, cognitive abilities, and depression factors. K-MMSE (Korean-Mini Mental State Examination), nVHI-10 (new Voice Handicap Index), and KGDS (Korean Form of Geriatric Depression Scale) were evaluated to select this study subjects. In order to compare respiratory functions between the two groups, FVC, FEV1, and FEV1/FVC were measured using microQuark, a PC-based spirometer. CSL was used by measure MPT and PAS was used to measure MFR. To investigate the characteristics of phonation ability, CSL was used to measure jitter and shimmer, while PAS was used to measure Psub. In order to compare the respiratory function averages and phonation ability between the two groups, statistical analysis was conducted using SPSS (version 12.0). The results of this study showed that most de novo IPD patients were included in the normal average range of respiratory and phonatory ability. But the respiratory and phonatory ability of de novo IPD patients showed lower tendency as compared with the normal group. When the average of respiratory and phonatory ability among the gender was compared, the difference of males was greater than the difference of females.

Role of Three-dimensional Computed Tomography Angiography in the Follow-up of Patients with Aneurysm Clips

  • Objective : The purpose of this study is to assess the usefulness of three-dimensional computed tomography angiography [3D-CTA] as a postoperative follow-up examination after intracranial aneurysms have been clipped. Methods : Between January 2002 and June 2005, 522 consecutive patients received treatment for intracranial aneurysms. A retrospective analysis of 310 patients with postoperative 3D-CTAs was performed to evaluate aneurysmal remnants and de novo aneurysms. This study was conducted in 271 patients with at least immediate and 6-month routine 3D-CT As for postoperative clipped aneurysm and 39 patients with 3D-CTAs for clipped aneurysm before 2002 when there was no 3D-CTA in our hospital. Results : Eight patients had abnormal CT angiographic findings. Aneurysm remnants were revealed in 4 patients and de novo aneurysms were discovered in 5 patients. Two patients were found at the postoperative 6-month 3D-CTA performed routinely. In 1 patient, the aneurysm was demonstrated on the way to the examination of syncope. In 2 patients, the author recommended 3D-CTA although there was no symptom because the patients had visited our institute long time ago [5.1, 4.5 years]. Of the 8 patients, 2 remnants and 1 de novo aneurysm were treated by endovascular treatment. Three de novo aneurysms at the middle cerebral artery and 1 pericallosal artery aneurysm were treated by direct clipping because these aneurysms were not suitable for the endovascular treatment in point of anatomical configuration. One patient with both remnant and de novo aneurysm was treated conservatively. Conclusion : 3D-CTA is an available, non-invasive diagnostic tool for the postoperative follow-up examination of aneurysmal state in patients after clipping.

생체 부분 간이식에서 De Novo Hepatitis B에 대한 B형 간염 면역글로불린의 예방적 효과 (Efficacy of Hepatitis B Immune Globulin for Prevention of De Novo Hepatitis B in Living-related Liver Transplantation)

  • 목 적: HBcAb 양성인 공여자로부터 생체 부분 간이식을 시행할 때 HBsAg 음성인 수혜자가 B형 간염 바이러스에 감염될 가능성이 높다. 본 연구에서는 삼성서울병원의 소아 생체 부분 간이식에서 능동 면역을 주로 사용하였던 초창기와 Hepatitis Bimmunoglobulin (HBIg) 단독 요법을 사용하였던 후기를 비교 분석하여 간이식 후 de novo hepatitis B발생에 대한 HBIg 단독 요법의 예방 효과에 대해 조사해 보고자 하였다. 방 법: 1996년 5월부터 2002년 6월까지 시행한 생체 부분 간이식에서 공여자가 HBcAb 양성이었으며 수혜자가 HBsAg 음성인 소아는 15명이었다. 다른 이유로 사망한 2명을 제외한 13명 중 11명은 HBsAb 양성, 2명은 naive (HBsAb 음성, HBcAb 음성)였다. 모든 환자는 간이식 전 B형 간염 바이러스 예방 접종을 실시하였다. 초기 단계에는(1997년 1월~1997년 11월, 3명) 수혜자가 HBsAb 양성인 경우 간이식 후 B형 간염 추가접종을 실시하였다. 후기 단계에서는(1997년 12월 이후, 10명) 간이식 전후에 모두 B형 간염 예방 접종을 실시한 이후 항체 양성인 수혜자에게 HBsAb 항체가를 200 IU/L 이상 유지하기 위해 HBIg를 단독 유지요법으로 사용하였다. HBIg의 심한 부작용으로 인해 1명의 경우 Lamivudine을 사용하였다. De novo hepatitis B의 예방효과를 병력 고찰을 통하여 후향적으로 분석하였다. 결 과: 13명 중 3명(23.1%)에서 de novo hepatitis B가 발생하였다. 능동 면역만을 시행한 초기 단계에서 3명 중 3명 모두 7~19개월에 HBsAg 양성으로 혈청 변환을 하였다. 1명은 간이식 전 naive 혈청 소견이었고 2명은 HBsAb 양성인 상태였다. HBIg를 사용한 후기 단계에서는 10명 모두 관찰 기간 7~55개월 동안 HBsAg 음성으로 남아 있다. 결 론: HBIg 단독 요법은 HBcAb 양성인 공여자의 간을 이식받은 HBsAg 음성 수혜자에서 de novo hepatitis B를 예방하는데 효과적이다.

Celecoxib-mediated activation of endoplasmic reticulum stress induces de novo ceramide biosynthesis and apoptosis in hepatoma HepG2 cells

  • Ceramides are the major sphingolipid metabolites involved in cell survival and apoptosis. When HepG2 hepatoma cells were treated with celecoxib, the expression of the genes in de novo sphingolipid biosynthesis and sphingomyelinase pathway was upregulated and cellular ceramide was elevated. In addition, celecoxib induced endoplasmic reticulum (ER) stress in a time-dependent manner. SPTLC2, a subunit of serine palmitoyltransferase, was overexpressed by adenovirus. Adenoviral overexpression of SPTLC2 (AdSPTLC2) decreased cell viability of HEK293 and HepG2 cells. In addition, AdSPTLC2 induced apoptosis via the caspase-dependent apoptotic pathway and elevated cellular ceramide, sphingoid bases, and dihydroceramide. However, overexpression of SPTLC2 did not induce ER stress. Collectively, celecoxib activates de novo sphingolipid biosynthesis and the combined effects of elevated ceramide and transcriptional activation of ER stress induce apoptosis. However, activation of de novo sphingolipid biosynthesis does not activate ER stress in hepatoma cells and is distinct from the celecoxib-mediated activation of ER stress.

국내 도시 쓰레기 소각로에서의 운전조건과 다이옥신 배출량과의 상관관계에 관한 연구 (PCDD/PCDFs Emission and Operating Conditions of Domestic MSW Incinerators)

  • In order to minimize emission of polychlorinated dibenzo dioxins and polychlorinated dibenzo furans (PCDD/PCDFs) from municipal solid waste incinerators, it is important to maintain optimized operating conditions along with the system modification/improvement. Operating conditions of MSW incinerator make very complicated influence on formation of PCDD/PCDFs in each unit apparatus. For revealing these influences, concentrations of PCDD/PCDFs are measured from the stack and from the fly ash, while monitoring the plant operating conditions. The effects are grouped into 3 main categories, combustion conditions, de Novo synthesis effects, and adsorption/destruction effects in the flue gas treatment system. Interpretation of the results showed that de Novo synthesis effect, reformation by metalic catalyst, especially Cu in fly ash in the temperature range of $250{\sim}500^{\circ}C$, is found to influence most dominantly on the concentration of PCDD/PCDFs. A plausible mathmatical model for predicting concentration of PCDD/PCDFs is proposed, and discussed.

Functional annotation of de novo variants from healthy individuals

  • The implications of germline de novo variants (DNVs) in diseases are well documented. Despite extensive research, inconsistencies between studies remain a challenge, and the distribution and genetic characteristics of DNVs need to be precisely evaluated. To address this issue at the whole-genome scale, a large number of DNVs identified from the whole-genome sequencing of 1,902 healthy trios (i.e., parents and progeny) from the Simons Foundation for Autism Research Initiative study and 20 healthy Korean trios were analyzed. These apparently nonpathogenic DNVs were enriched in functional elements of the genome but relatively depleted in regions of common copy number variants, implying their potential function as triggers of evolution even in healthy groups. No strong mutational hotspots were identified. The pathogenicity of the DNVs was not strongly elevated, reflecting the health status of the cohort. The mutational signatures were consistent with previous studies. This study will serve as a reference for future DNV studies.

  • De novo variants (DNVs) can arise during parental germ cell formation, fertilization, and the processes of embryogenesis. It is estimated that each individual carries 60-100 such spontaneous variants in the genome, most of them benign. However, a number of recent studies suggested that DNVs contribute to the pathogenesis of a variety of human diseases. Applications of DNVs include aiding in clinical diagnosis and identifying disease-causing genetic factors in patients with atypical symptoms. Therefore, understanding the roles of DNVs in a trio, with healthy parents and an affected offspring, would be crucial in elucidating the genetic mechanism of disease pathogenesis in a personalized manner.

  • Accumulation of ceramide mass in MCF-7 cells by the anti-cancer agent. paclitaxel. was found to occur primarily due to activation of the de novo synthesis pathway. Morever. the addition of paclitaxel resulted in the accumulation of ceramide, which was followed by a prolonged arachidonic acid release. Participation of ceramide de novo pathway in arachidonate signaling was detected since L-cycloserine, an inhibitor of de novo synthesis, was able to inhibit the paclitaxel-induced AA release and cytotoxicity. Occurrence of De Novo Dural Arteriovenous Fistula after Transvenous Embolization of Dural Arteriovenous Fistula : Case Reports of Two Patients

  • Development of de novo dural arteriovenous fistula (DAVF) at a different site after resolution of an initial DAVF, is rare. Here we report two cases, which we encountered in our hospital. A 68-year-old woman presented with pulsatile tinnitus on the left side. Cerebral angiography demonstrated a left anterior condylar confluence (ACC) DVAF and she underwent transvenous embolization. Four years after this treatment, she presented with tinnitus on the left side, and cerebral angiography revealed a right DAVF around the sinus of the lesser sphenoid wing. Another 69-year-old woman presented with left-sided orbital bruits, chemosis, and conjunctival hyperemia. Cerebral angiography showed left cavernous sinus (CS) DAVF, for which she underwent transvenous embolization for CS DAVF. One year later, she developed a left ACC and transverse-sigmoid sinus (TSS) DAVF.

Risk of Treatment Related Death and Febrile Neutropaenia with First Line Palliative Chemotherapy for De Novo Metastatic Breast Cancer in Clinical Practice in a Middle Resource Country

  • Background: The risk of febrile neutropaenia (FN) and treatment related death (TRD) with first line palliative chemotherapy for de novo metastatic breast cancer (MBC) remains unknown outside of a clinical trial setting despite its widespread usage. This study aimed to determine rates in a large cohort of patients treated in the University of Malaya Medical Centre (UMMC). Materials and Methods: Patients who were treated with first line palliative chemotherapy for de novo MBC from 2002-2011 in UMMC were identified from the UMMC Breast Cancer Registry. Information collected included patient demographics, histopathological features, treatment received, including the different chemotherapy regimens, and presence of FN and TRD. FN was defined as an oral temperature > $38.5^{\circ}C$ or two consecutive readings of > $38.0^{\circ}C$ for 2 hours and an absolute neutrophil count < $0.5{\times}10^9/L$, or expected to fall below $0.5{\times}10^9/L$ (de Naurois et al, 2010). TRD was defined as death occurring during or within 30 days of the last chemotherapy treatment, as a consequence of the chemotherapy treatment. Statistical analysis was performed using the SPSS version 18.0 software. Survival probabilities were estimated using the Kaplan-Meier method and differences in survival compared using log-rank test. Results: Between $1^{st}$ January 2002 and $31^{st}$ December 2011, 424 patients with MBC were treated in UMMC. A total of 186 out of 221 patients with de novo MBC who received first line palliative chemotherapy were analyzed. The mean age of patients in this study was 49.5 years (range 24 to 74 years). Biologically, ER status was negative in 54.4% of patients and Her-2 status was positive in 31.1%. A 5-flourouracil, epirubicin and cyclophosphamide (FEC) chemotherapy regimen was chosen for 86.6% of the cases. Most patients had multiple metastatic sites (58.6%). The main result of this study showed a FN rate of 5.9% and TRD rate of 3.2%. The median survival (MS) for the entire cohort was 19 months. For those with multiple metastatic sites, liver only, lung only, bone only and brain only metastatic sites, the MS was 18, 24, 19, 24 and 8 months respectively (p-value= 0.319). Conclusions: In conclusion, we surmise that FEC is a safe regimen with acceptable FN and TRD rates for de novo MBC.
