Usefulness of Various Questionnaires in the Assessment of Excessive Daytime Sleepiness and Circadian Rhythm (수면의학(睡眠醫學)에 있어서 지필(紙筆) 척도(尺度)의 활용(活用) -과도한 주간 졸음과 일주기리듬의 평가를 중심으로-)

  • Recently excessive daytime sleepiness was found to have relations with various social, occupational, and health problems. This condition is common symptom of several sleep disorders, among which sleep apnea syndrome is most contributive. It is essential to assess daytime sleepiness exactly for the diagnosis of such sleep disorders. Multiple sleep latency test which is a valid and objective measurement technique of sleepiness is time and cost consuming, and so there is increasing need of scales measuring general level of daytime sleepiness which are quick and simple to perform for clinical and research purpose. And also, there have been a lot of sleep researches viewing sleep as a chronobiological process, especially in the study of circadian type of shift workers. In these studies they used various techniques of multiple demensions to assess sleepiness or circadian rhythm which concerns various psychological variables. Of these measurement techniques circadian type questionnaires might have some problems in their psychometric properties. So some of these morningness-eveningness questionnaires have been revised and more valid scales are being suggested by different authors. The author briefly reviewed various measurement techniques of sleepiness and circadian rhythm and introduced recently developed scales which are more valid allegedly, and finally discussed psychometric properties of these morningness-eveningness questionnaires.

The Characteristics of Sleep in Headache Patients

  • Purpose: To investigate the relationship between headache and sleep by evaluating sleep quality, daytime sleepiness, and specific features related to sleep-disordered breathing (SDB). Methods: One hundred one subjects with headache and 118 healthy controls were enrolled. To collect various information on headache attacks, headache group completed self-reported questionnaire about the characteristics of headache attacks and the migraine disability assessment (MIDAS) questionnaire. The subjective quality of sleep was evaluated in all of the subjects using the Pittsburgh sleep quality index (PSQI) and Epworth sleepiness scale (ESS). In addition, the following specific features of sleep were evaluated in 28 subjects selected randomly from each group: apnea-hypopnea index (AHI), prevalence of SDB, nocturnal oxygen saturation (SaO2), and oxygen desaturation index (ODI) as measured using a portable monitoring device. Results: The global PSQI and the prevalence of poor sleeping (global PSQI >5), ESS scores and the prevalence of daytime sleepiness (ESS score >10) were significantly higher in the headache group (both p<0.0001, respectively). The mean scores on the numerical rating scale and the MIDAS were significantly higher in the poor-sleeper group than in the good-sleeper group (p=0.0347 and p=0.0016, respectively). The global PQSI and prevalence of daytime sleepiness were significantly higher in the chronic-headache group than in the acute-headache group (p=0.0003 and p=0.0312, respectively). Conclusions: There is a significant association between headache and sleep. Especially, severity and chronicity of headache were significantly associated with sleep quality and daytime sleepiness.

A Study on Perspirations(汗出) in Daily Time Cycle (하루 중 시간변화(時間變化)에 따른 한출(汗出)의 기전(機轉)에 대한 연구(硏究))

  • This thesis intend to help the eastern medical doctor to understand body condition from interpretation of perspirations(汗出) in daily time cycle. The conclusion is followed. 1. In most Eastern Medical classic and clinic literatures, the time of fever and perspirations are described as a result of disease's position at human body. Following this description, in daytime the perspirations must come from the Gi phase and night time the perspirations must come from the blood phase. Because in daytime the skin pores are opening and the defensive Gi is going out to the superficial portion of the body. In night time the skin pores are shutting and the defensive Gi is going in to the five solid organs. So a sweat in daytime comes out from the Gi phase and superficial portion of the body. And in night time comes out from the blood phase and five solid organs. But in recent real clinic cases, in daytime, there are so many perspirations from the five solid organs. Comparatively, the perspirations from the superficial portion of body are very little. And in same daytime perspirations, when the heat pathogens mixed with moist, the symptom revelation time delay to the afternoon. Therefore it can be concluded that the time of perspirations are combination of disease's Gi or blood phase and characteristics of pathogens. The position of disease at human body cannot simply judge the symptom revelation time. 2. The exchange of climate following time cycle of a day effect to the condition of human body. At same time it activates or not activates the pathogens in human body. So we can consider the kinds and characteristics of pathogens by distinguishing the symptom revelation time. In general differentiation of syndromes[辨證] pathogen's kinds and location are generally judged. By understanding the characteristics of pathogen, doctor can devise more correct and delicate prescription.

Relationships between sleep habits, daytime sleepiness and problem behaviors among adolescents (청소년의 수면습관, 주간졸음과 문제행동과의 관계)

  • This study examined the relationships sleep habits, daytime sleepiness, and problem behaviors. A cross-sectional survey design was used. Participants were 276 adolescents. Sleep habits, daytime sleepiness and problem behavior were measured by Adolescent Sleep Hygiene Scale (ASHS), Epworth Sleepiness Scale (ESS), and Korea-Youth Self Report (K-YSR), respectively. Data were collected from July to August, 2014 using self-reported questionnaires. Participants were middle school students, perceived their sleep time as enough and their sleep quality as good, have no problem behavior during sleep, and have no caffeinated drink for last a week were more likely to have higher score of sleep hygiene. Sleep habits score was associated negatively with daytime sleepiness and problem behaviors. Sleep impacts on physical growth, behavior, and emotional development of adolescent. Therefore, it is necessary to develop a sleep enhancement program for the good sleep habits of the adolescents, and to make efforts at home and school.

An Analysis on Noise Reduction Effects of Two-Layer Low Noise Pavements using Statistical Methods (통계적 방법을 이용한 복층 저소음포장의 소음저감효과 분석)

  • PURPOSES : The purpose of this study is to compare noise reduction quantities between before/after two-layer low noise pavement implementation using equivalent noise level analysis and to analyze the noise reduction effects of the two layer low noise pavement with a statistical method such as the Anderson-Darling Test. METHODS : In order to compare and to analyze noise reduction effects between before/after two-layer low noise pavement implementation, data acquisition as noise levels on a roadside and an apartment rooftop was conducted in the study area. The equivalent noise level was estimated in order to compare noise reduction quantities and the Anderson-Darling Test was carried out for estimating noise reduction effects of the two-layer low noise pavement. RESULTS : The equivalent noise levels of before/after two-layer low noise pavement implementation for the roadside during the daytime are 65.355 dB and 63.520 dB and during the nighttime are 62.463 dB and 59.088 dB. The equivalent noise levels for the apartment rooftop during daytime are 57.301 dB and 59.088 dB and during the nighttime are 54.616 dB and 52.464 dB. Also two-layer low noise pavement decreased the noise reduction effects estimated with the statistical method as the Anderson-Darling test for the roadside during the daytime by around 66.68% and decreased noise reduction effects on the roadside during the nighttime by 0.70%. Moreover it reduced noise reduction effects in the apartment rooftop during the daytime and nighttime by 0% and 96.32%, respectively. CONCLUSIONS : Based on the result of this study, two-layer low noise pavement can positively affect noise reduction during both the daytime and nighttime according to the results of estimating the equivalent noise levels and the Anderson-Darling test.

  • This case study is to report the effectiveness of Hyungbangdojeock-san in the treatment of daytime sleepiness. The patient was diagnosed as a Soyangin, one of the 4 constitution types in Korean medicine, and treated with herbal medicine, Hyungbangdojeock-san and acupuncture treatment for 14 days. The severity of symptom was assessed using Epworth Sleepiness Scale (ESS). After treatment, the severity of daytime sleepiness was reduced. ESS score was decreased from 11 to 2. As a result this case showed the effect of Hyungbangdojeock-san on daytime sleepiness.

Do depression and its associated factors differ in women daytime and shift workers?: an analysis of the Korea National Health and Nutrition Examination Survey 2018

  • Purpose: This study examined health behaviors, use of health services, and depression among women who perform daytime and shift work in Korea, as well as factors related to depression. Methods: We conducted a secondary analysis using data from the 2018 data of the 7th Korea National Health and Nutrition Examination Survey. Data on women, 1,493 regular daytime workers and 322 shift workers, were analyzed. Results: Women shift workers (χ2=43.97, p<.001), had a lower education level (χ2=45.56, p<.001), and lower personal income (χ2=16.85, p=.030) than their daytime counterparts. A higher proportion of shift workers were unmarried (χ2=37.47, p<.001) and they typically worked fewer than 40 hours per week (χ2=69.94, p<.001). The depression score of shift workers was higher than that of daytime workers (t=2.85, p=.005). A higher proportion of shift workers also drank alcohol (χ2=6.49, p=.032) and smoked (χ2=30.79, p<.001). Over 8% of shift workers typically slept fewer than 5 hours per night (χ2=14.17, p=.024). It was confirmed that depression in women shift workers was affected by age, personal income, marital status, health status, and smoking status, in addition to cancer screening participation, unmet medical care needs, and unmet dental care needs. Conclusion: More attention should be given to the health needs of women working shifts. Health promotion programs specific for women shift workers are needed to improve their physical and mental health, encourage use of medical care services, and improve public health policies and systems.

Relationship between Sleep Insufficiency and Excessive Daytime Sleepiness (수면 부족과 과도한 주간졸림증의 관련성)

  • Objectives:Sleep loss and excessive daytime sleepiness may have serious consequences, including traffic and industrial accidents, decreased productivity, learning disabilities and interpersonal problems. Yet despite these adverse effects, there are few epidemiological studies on sleep loss and daytime sleepiness in the general population of Korea. This study investigates the number of people who suffer from sleep insufficiency, how much recovery sleep occurs on weekends, and the relationship between the amount of recovery sleep and daytime sleepiness. Methods:A total 164 volunteers, aged 20 and over, were recruited by advertisement. The subjects were workers and college students living in Seoul, Korea. Subjects were excluded if they were aged over 60;if they had medical, neurological, psychiatric or sleep disorders that could cause insomnia or daytime sleepiness;if they were not following a regular sleep schedule;if they traveled abroad during the study;or if they did not leave home to work or were shift workers. They were interviewed and given a sleep log to complete on each of 14 consecutive mornings. They also completed the Epworth Sleepiness Scale (ESS) at noontime on the last day of the second week. All statistical data were analyzed by t-test, $X^2$-test or ANOVA, using SPSS/PC+. Results:The results showed that the subjects woke up at 6:50 (${\pm}1$:16) on weekdays, 7:09 (${\pm}1$:29) on Saturdays, and 8:12 (${\pm}1$:39) on Sundays and holidays. They took more frequent and longer naps on Sundays than on weekdays and Saturdays. The mean sleep duration was 6h 35 min. on week nights, with a mean increase of about 1h on weekends. Only 9.1% of the subjects spent more than 8h in bed on week nights, with 67% spending less than 7h, and 49.4% reported recovery sleep of more than 1h on Sundays. The subjects who reported recovery sleep of more than 2h on Sundays, showed significantly more excessive daytime sleepiness than those who reported less than 30 min (F=2.62, p<.05). Conclusions:These findings suggest that sleep insufficiency and excessive daytime sleepiness are relatively common in Korea, and that the people who get insufficient sleep on weekdays try to compensate for sleep loss with oversleeping and daytime napping on Sundays and holidays. It appeared that daily sleep insufficiency had a cumulative effect and increased daytime sleepiness.

Characterization of Size distribution of Anion Species in Atmospheric Aerosols (대기에어로졸중 음이온성분에 대한 입경분포의 변화특성)

  • Aerosol size distribution was determined using Anderson sampler for the anions( sulfate nitrate and chloride ) and TSP. Ionic species concentration and size distribution have been investigated in the daytime and nightime individually. Size classified samples were extracted with distilled water and analyzed for C $l^{[-10]}$ , N $O_3$$^{[-10]}$ and S $O_4$$^{2-}$, by ion chromatography. The size distribution of these ions and TSP was analyzed to investigate the seasonal and diurnal variation of concentrations as follows: (1)Size distribution of TSP showed bi- modal type in the daytime, but indicated tri-mode distribution in the nightime without any seasonal variation. (2)Sulfate concentrations were higher in fine- mode both in the daytime and nightime but fraction of sulfate was higher in coarse-mode during the Yellow Sand Period. (3)Nitrate and Chloride ions are dominant in fine-mode in winter while dominant in coarse-mode in the summer.

Acoustical Survey for Estimating Fish Biomass at Chilam Bay, Korea

  • This acoustic experiment noted that fish species in Chilam-Gijang marine ranching area were more densely distributed in the pelagic zone during nighttime than daytime. In each season, the gill nets caught 15 different fish species and the estimated average target strengths were -44.0 dB and -44.4 dB for autumn and winter surveys, respectively. The estimated autumn fish biomass were 7.7 tons and 26.0 tons during daytime and nighttime, respectively. Winter biomass was 2.27 tons and 30.97 tons during daytime and nighttime, respectively. Different fish species form schools that exhibit different movements and behaviors, and thereby occupy varying water layers. These results explained the estimated fish biomass, and variation with seasons and time of the surveys around artificial reefs in Chilam Bay, Korea.
