Risk Factors for Lymphedema Patients (림프부종 환자에서의 위험 인자)

  • Purpose: Lymphedema is edema developing mainly in the arms and legs due to an abnormal lymphatic system, over one hundred millions of patient worldwide suffer from it. At present, prevention is the best treatment. Thus, It is important to know which patient are more pone to develop it in order to prevent it. By evaluating the risk factor for lymphedema, we intended to find the effective prevention. Methods: We have investigated that outpatients who chiefly complained of lymphedema visited the lymphedema clinic at one university hospital from September 1 in 2003 to August 31 in 2005. We evaluated the risk factor for lymphedema by questionnaires. Questionnaire consists of demographic data, disease factor, treatment factor, posttreatment factor. We evaluated the correlation between lymphedema with each factor. Results: The total number of patients was 50, 19 patients with breast cancer and 31 patients with cervical cancer. In terms of stages of edema, 12 patients were below stage 1 and 38 patients were above stage 2. The incidence of lymphangitis was more frequent, more obese and the impairment of the site of edema site more severe, the stage of cancer was higher in the patients higher than stage 2 than in the patients below stage 1 (P>0.05). Conclusion: Lymphedema is more severe when the cancer stage in higher, accompanying more frequent infections, more obesity and more impairment at the site of edema site. Above all, cancer stage and the impairment of edema site are impossible to correct but obesity and infection may be corrected. Therefore we speculate that we are able to protect the aggravation of edema by weight reduction and infection control.

Establishment of Total Sugar Reference Value for Koreans (한국인 총당류 섭취기준 설정)

  • Sugars are a ubiquitous component of our food supply and are consumed as a naturally occurring component of many foods and as additions to foods during processing, preparation, or at the table. Most fruits and dairy products are high in sugars and thus naturally occurring sugars are consumed as part of a healthy diet. Some countries developed recommended daily intake figures(daily values : DVs or guideline daily amounts: GDA) for nutrients, and some countries, but not most have developed DV/GDA for total sugars. Dietary Reference Intakes for Koreans established by the Korean Nutrition Society in 2005, did not include the reference values for total sugar or added sugar. The committee on Dietary Reference Intakes for sugar was constituted in 2006 and discussed whether to special added sugars or total sugar. Although added sugars are not chemically or physiologically different from naturally occurring sugars, many foods and beverages that are major sources of added sugars have lower micronutrient densities compared with foods and beverages that are major sources of naturally occuring sugars. But it was so hard to calculate a dietary intake of added sugar for Korean people, because there was insufficient information about contents of added sugar during processing or preparation of Korean food. Currently Korean or US food labels contain information on total sugars per serving but do not distinguish between sugars naturally present in food and added sugars. Therefore the committee decided to set the reference value for total sugar for Koreans. According to the recommended diet pattern for Koreans suggested by the Korean Nutrition Society, estimated sugar intake from the sugar containing food based on 2,000 kilocalories is 67 g or 13% of total energy. Based on the data available on risk of obesity, hypertension, hyperlipidemia, insulin resistance, and metabolic syndrome from the analysis of Korean NHANES, it was insufficient evidence to set a UL for total sugar, but tended to increase serum LDL cholesterol and obesity at over 20-25% of energy from total sugar when consumed with high carbohydrates. Therefore the committee on Dietary Reference Intakes for sugar set the Acceptable Macronutrient Distribution Range for total sugar as 10-20% of total energy intake.

Associations of Eating Habits with Obesity and Nutrition Knowledge for Middle and High School Adolescents in Shanghai and Heze China (중국 상하이·허쩌 중·고등학생의 식습관과 비만도 및 영양지식과의 관련성 연구)

  • The aim of this study was to investigate the relationships between eating habits and health among adolescents in Shanghai and Heze, China. A cross-sectional study was conducted in 2013 on 2,089 adolescents; 1,089 students were from Shanghai and 999 students from Heze region. Eating habits, weight, height, and nutritional knowledge were assessed using a self-administered questionnaire. Eating habits score was classified into two categories: healthy eating habits and unhealthy eating habits, based on "Korean Youth Risk Behavior Web-based Survey", for statistical data analysis. Associations between eating habits, BMI, and nutritional knowledge were examined using a general linear model with adjustment of potential confounding factors such as region, gender, age, parents' education level, and pocket money. Statistical analyses were performed using the SAS (version 9.3) program. Proportions of healthy eating habits group were 90.0% for breakfast (3-7 times/wk), 29.1% for fruit (${\geq}once/d$), 12.5% for vegetable (${\geq}3times/d$), 7.3% for milk (${\geq}2times/d$), 90.0% for fast food (<3 times/wk) consumption, respectively. The average BMI score was 20.1 (Shanghai 20.5 Heze 19.6), which is in the range of normal weight. Rates of obesity and overweight were 16.5% and 8.3% in Shanghai and Heze, respectively. There were significant negative correlations between intake frequencies of breakfast, fast food, biscuits, sugar, chocolate, and BMI score. Eating habits and nutritional knowledge score showed a significant positive correlation. These results showed better eating habits regarding eating regularity and consumption of fruits and soft drinks in Chinese adolescents compared with Korean adolescents, although cultural differences were not fully considered. This study demonstrated significant associations of BMI and nutritional knowledge with dietary behavior in Chinese adolescents in two regions of China. Further studies on Chinese adolescents from other regions in China should be considered.

The Combined Effect of Subjective Body Image and Body Mass Index (Distorted Body Weight Perception) on Suicidal Ideation

  • Objectives: Mental health disorders and suicide are an important and growing public health concern in Korea. Evidence has shown that both globally and in Korea, obesity is associated with an increased risk of developing some psychiatric disorders. Therefore, we examined the association between distorted body weight perception (BWP) and suicidal ideation. Methods: Data were obtained from the 2007-2012 Korea National Health and Nutritional Evaluation Survey (KNHANES), an annual cross-sectional nationwide survey that included 14 276 men and 19 428 women. Multiple logistic regression analyses were conducted to investigate the associations between nine BWP categories, which combined body image (BI) and body mass index (BMI) categories, and suicidal ideation. Moreover, the fitness of our models was verified using the Akaike information criterion. Results: Consistent with previous studies, suicidal ideation was associated with marital status, household income, education level, and perceived health status in both genders. Only women were significantly more likely to have distorted BWP; there was no relationship among men. In category B1 (low BMI and normal BI), women (odds ratio [OR], 2.25; 95% confidence interval [CI], 1.48 to 3.42) were more likely to express suicidal ideation than women in category B2 (normal BMI and normal BI) were. Women in overweight BWP category C2 (normal BMI and fat BI) also had an increased OR for suicidal ideation (OR, 2.25; 95% CI, 1.48 to 3.42). Those in normal BWP categories were not likely to have suicidal ideation. Among women in the underweight BWP categories, only the OR for those in category A2 (normal BMI and thin BI) was significant (OR, 1.34; 95% CI, 1.13 to 1.59). Conclusions: Distorted BWP should be considered an important factor in the prevention of suicide and for the improvement of mental health among Korean adults, especially Korean women with distorted BWPs.

Repression of Cathepsin D Expression in Adipocytes by MicroRNA-145 (지방세포에서 microRNA-145에 의한 Cathepsin D의 발현 제어)

  • Cathepsin D (CtsD), an aspartyl peptidase, is involved in apoptosis, resulting in the release of cytochrome C from mitochondria in cells. Here, we investigated microRNA regulation of CtsD expression in 3T3-L1 cells First, we observed the expression of CtsD in cells in response to doxorubicin (Dox). As expected, the level of CtsD mRNA was increased in 3T3-L1 cells exposed to Dox in a dose-dependent manner. Cellular viability of ectopically expressed CtsD cells was also decreased. Next, we used the miRanda program to search for particular microRNA targeting CtsD. MiR-145 was selected as a putative controller for CtsD because miR-145 had a high mirSVR score. In a reporter assay, the luciferase activity of cells containing the CtsD 3'-UTR region was decreased in cells transfected with miR-145 mimic compared to that of a control. The level of CtsD expression was down-regulated in preadipocytes ectopically expressing miR-145 and up-regulated by an miR-145 inhibitor. Cells also suppressed miR-145 expression when exposed to Dox. The miR-145 inhibitor reduced the cellular viability of 3T3-L1 cells. Taken together, these data suggest that miR-145 regulates CtsD-mediated cell death in adipocytes. These findings may have valuable implications concerning the molecular mechanism of CtsD-mediated cell death in obesity, suggesting that CtaD could be a useful therapeutic tool for the prevention and treatment of obesity by regulating fat cell numbers.

Effects of Zizyphi Spinosae Extract on Cisplatin and t-Butylhydroperoxide Induced Acute Renal Failure in Rabbits (토끼에서 cisplatin에 의해 유도된 급성 신부전시 산조인 추출물의 효과)

Incidence and Related Factors of the Metabolic Syndrome in a University Hospital (대학병원 종합검진센터 수진자의 대사증후군 발생과 관련요인)

  • Objectives: The purpose of this research was to investigate incidence and related factors of the MS in a university hospital Methods: This survey was conducted from January in 1997 to March in 2008. This study is a cohort study. The baseline subjects were 529 men and 609 women without metabolic syndrome(MS) who visited the health promotion center of Chungnam National University Hospital for general health check-up. The MS was diagnosed according to the definition by the NCEP ATP III. The abdominal obesity guidelines for waist circumference applied the Korean Society for the Study of Obesity Guideline Results: The survey exhibited that baseline risk factors that mainly affect odds ratio(OR) of the MS at follow-up were sex, ages, heavy drinking, uric acid and transpeptidase(GGT) in women. Incidence of the MS was 10.6% in total cases, 18.1% in men and 4.0% in women. The risk of the MS was significantly lower in women than men(OR=0.4, 95% confidence interval(CI: 0.2-0.9), significantly higher 50-59 Ages than < 40 ages(OR=3.4, 95% CI: 1.9-6.4), significantly high in women with heavy drinkers(OR=14.3, 95% CI: 1.2-177.5), significantly high in whole, men and women with high uric acid group(whole, OR=2.6, 95% CI: 1.6-4.2. men, OR=2.1, 95% CI: 1.3-3.5. women, OR=14.2, 95% CI: 3.0-68.3) and significantly high in women with high GGT(OR=1.8, 95% CI: 2.4-12.6). Conclusion: Risk factors of incident MS were baseline sex, ages, heavy drinking in women, uric acid and GGT in women. These can be utilized as an important index to determine risk factors of incident MS and will be basic data in part of management, education, countermeasure and selection test of the MS.

Case-Control Study on Relationship of Dampness-phlegm to Blood Lipid Level in Stroke Patients (중풍(中風)환자의 습담(濕痰)변증과 혈중지질의 상관성에 관한 Case-Control 연구)

  • The purpose of this study was to investigate the relationship of Dampness-phlegm to blood lipid level and second reason of hyperlipidemia in acute stroke patients by case-control study. This study was done over 348 patients hospitalized in the Oriental Medical Hospital of Daejeon University of November 2006 to July 2008. Patients had been interviewed by residents and medical specialists who studied standard operation procedures in Fundamental Study for Syndrome of Oriental Medicine for Stroke. Study subjects consisted of 86 patients who distributed to Dampness-phlegm by medical specialist and discriminating program as the case I group, 157 patients who distributed to Dampness-phlegm by medical specialist or discriminating program as the case II group and 191 patients who distributed to Non-Dampness-phlegm by medical specialist and discriminating program as the control group. For the purpose of obtaining suitable result we analyzed blood lipid level of each group by univariate and multivariate logistic analysis. Dampness-phlegm was not significant correlated with increasing of Total cholesterol, Triglyceride and decreasing of HDL cholesterol. Dampness-phlegm was significant correlated with increasing LDL cholesterol and the independent predictors of hyperlipoproteinemia by multivariate logistic analysis. Dampness-phlegm was not significant correlated with diabetes melitus, liver disease, kidney disease, obesity and abdominal obesity. In this study, we demonstrated new relationship between Dampness-phlegm and LDL cholesterol. Based on these results, it is suggested that Dampness-phlegm would be the independent predictors of hyperlipoproteinemia. And more prospective studies are to be done with more clinical data.

Acupuncture and Herbal Medicine Improved the Quality of life of Obese Women (한방치료를 받은 여성 비만환자의 삶의 질 연구)

  • To examine the difference of health-related quality of life (QoL) of obese patients between before and after treatment of Korean traditional medicine. This study was assessed in 18 obese-women (age 29.72${\pm}$7.38) treated by acupuncture and herbal medicine using SF-36 quality of life questionnaire between January and July 2006, a 36 item instrument yielding 8 dimensions (Social functioning, Role limitation-Physical, Role limitation-Emotion, Mental health, Vitality, Bodily pain, and General health) and a total score. Their weight was measured by Inbody 3.0 (Biospace co. Korea). QoL and body composition was assessed twice at baseline (B) and 4 weeks after treatment. Auricular acupuncture and electrical needle stimulation to abdomen, thigh, and arm fat was used twice a week and complex herb formula was prescribed according to their constitution and symptoms for 4 weeks. Herb medication was taken 2 times a day. Respective variables were analyzed with Wilcoxon signed ranks test and the level set for statistical significance was p <0.05. The mean of the body weight (P=0.000;B64,68${\pm}$6.86, 4 WKs 60.47${\pm}$5.69), fat percentage (P=0.000;B33.14${\pm}$4.86, 4 WKs 30.16${\pm}$5.34), body mass index (P=0.000;B25.18${\pm}$2.44, 4 WKs 23.46${\pm}$2.09) and fat weight (P=0.000;B21.66${\pm}$5.06, 4 WKs 18.41${\pm}$4.57) of some obese patients decreased significantly between before and after treatment for 4 weeks. Physical functioning (P=0.48;B27.06${\pm}$3.17, 4 WKs 28.00${\pm}$1.71), Mental health (P=0.01 ;B18.83${\pm}$5.25, 4 WKs 22.00${\pm}$3.73), Vitality (P=0.028;B13.89${\pm}$3.03, 4 WKs 15.44${\pm}$2.53), Bodily pain (P=.0014;B8.84${\pm}$1.75, 4 WKs 10.15${\pm}$1.78), and the total scores (P=0.001;B104.99${\pm}$12.60, 4 WKs 114.58${\pm}$11.11) of SF-36 were increased significantly after treatments. These data show some differences in QoL and BMI between before and after treatment on obesity and suggest that the treatment with acupuncture and herbal medicine have a positive impact on several domains of QoL of some obese patients. Further randomized clinical trials (RCTs) including follow-up are needed to examine whether acupuncture and herb medicine have a positive effect on QoL of treatment group compared with control group.

Influence on Enlisted Soldiers' Health behavior, Body-shape perception, and Weight control toward the BMI change (현역병의 건강행태, 주관적 체형인식, 체중조절 활동이 BMI 변화에 미치는 영향)

  • This study was conducted to identify the BMI of enlisted soldiers, to understand the difference between their BMI and that of other men in their age group, and to identify the influence factors of enlisted soldiers' BMI change. The survey has been conducted self-evaluation questionnaire from 23 Feb. 2009 to 31 Mar. 2009 from 4 different forces as methods. Independent-sample T-test, chi-square test and multiple linear regression analysis were used for statistical analysis from 301 collected data. As a result of surveying enlisted soldiers' BMI, their obesity rate 18.6% was lower than 22.1% of other men in their age group. In terms of health behavior, the underweight&normal weight group(2.39) showed higher diet score than the overweight group(2.13), showing that the underweight&normal weight group ate relatively slowly, less spicy and less sweet food compared to the overweight group. The overweight group(2.25) showed lower satisfaction with their body type than the underweight&normal weight group(2.98), while the overweight group(4.01) showed a significantly higher score than the underweight&normal weight group(3.37) for weight control activity. The influence factors of BMI change were diet habit, subjective perception of body type, and weight control activity. In order to improve of enlisted soldiers' BMI, it would be necessary to improve the food service and the snack bars for interventional control of food that influence obesity, rather than personal effort, in addition, education for right body-shape perception and encouraging on weight control activity.
