The Relationship between Left Ventricular Hypertrophy by Transthoracic Echocardiography and Dampness-Phlegm Diagnosis in Cerebral Infarction Patients (경흉부 심초음파를 이용한 뇌경색 환자의 좌심실 비대와 습담변증(濕痰辨證)의 관련성 연구)

  • Object : The aim of this study was to assess the relationship between left ventricular hypertrophy and Dampness-Phlegm diagnosis in cerebral infarction patients. Methods : Among 227 of the total recruited patients, 59 patients were diagnosed as left ventricular hypertrophy. We assessed their general characteristics, risk factors, lab findings and Korean medical diagnosis. We compared the assessed variables between left ventricular hypertrophy group and non left ventricular group. We analyzed the relationship between left ventricular hypertrophy and risk factors. And we also analyzed the relationship between left ventricular hypertrophy and dampness-phlegm diagnosis. Results : 1. The rate of left ventricular hypertrophy in female patients was larger than the rate of male patients. 2. There were more patients finally diagnosed hypertension in left ventricular hypertrophy group. 3. According to the analysis about the rate of Dampness-phlegm related Index for Pattern Identification by left ventricular hypertrophy, Sallow complexion and obesity were significantly higher in the left ventricular hypertrophy than in the non left ventricular hypertrophy group. 4. In multivariate analysis, Dampness-phlegm group showed close relationship with left ventricular hypertrophy. Conclusions : According to the analysis, significance between dampness-phlegm diagnosis diagnosed group and left ventricular hypertrophy were clarified. These results can be utilized in the future as a basic material to be used for diagnosis and management of dampness-phlegm diagnosis on cardiovascular diseases.

Interpretation of Eum-Yang' Deficiency, Excess and Exuberance which was described in "Somun.Jogyeongron(素問.調經論)" ("소문(素問).조경론(調經論)"의 음양허성(陰陽虛盛)에 대(對)한 연구(硏究))

  • It was described in "Somun Jogyeongron" that Eum(Yin)-Yang' deficiency, excess and exuberance. According to "Somun Jogyeongron", It was known that the syndrome of Yang deficiency and exuberance is belong to external contraction, Eum deficiency and exuberance is belong to internal damage. The syndrome of Yang deficiency is belong to Gyejitang[contraction of wind] which constitutional weakness as the main etiological factor of deficiency conditions. The syndrome of Yang exuberance is belong to Mahwangtang[cold damage] which constitutional strong as the main etiological factor of exuberance conditions. Eum deficiency is so dysfunction of the spleen and stomach that Eum fluid and essence is not engender, distributed. So the dysfunction of spleen and stomach makes dampness-heat obstruction and then makes Internal heat at last. The syndrome of Eum deficiency is applicable to bojungikgitang. As contrasted with Eum deficiency, Eum exuberance is occurred cold-dampness obstruction, which we call 'Eum exuberance'. The syndrome of Yang exuberance is applicable to Ijungtang. In the light of "Somun Jogyongron", We explain that the syndrome of Yin deficiency and Yin exuberance, which was caused by dampness-heat, cold-dampness obstruction and internal damage based on disorder of the spleen and stomach.

Study on the Endogenous Dampness Caused by Gi Deficiency of the Spleen and Sagunja-tang (비기허(脾氣虛)로 인한 내습(內濕)의 발생과 사군자탕(四君子湯))

  • The spleen is the source of gi, blood, body fluid and plays a vital role in maintaining life. The function of the spleen is to transform food nutrients and water, and to transport them to the heart and the lung. The movement of splenic gi is marked by elevation. The spleen governs the activity of elevating the lucid. The function of transportation and transformation is usually disturbed in the state of Gi deficiency of the spleen. The main clinical manifestations of gi deficiency of the spleen can be divided into as followers: anorexia, loose stool if the digesting and absorbing functions are disturbed; phlegm and edema if dampness and water are retained due to unhealthy water metabolism. Sagunja-tang can be applied for gi deficiency syndrome of the spleen. Ingredient bakchul(Rhizoma Atractylodis Macrocephalae) and bokryeung(Poria) can be used as monarch drug to eliminate dampness and strengthen the spleen.

Non-comparative Study for the Observation of Clinical Efficacy and Safety of CheungYeolYiSeup-tang and Hwangbaek External dressings on Dampness-Heat Pattern Atopic Dermatitis

  • Objective : To observe the efficacy and safety of CheungYeolYiSeup-tang and Hwangbaek external dressings on dampness-heat pattern atopic dermatitis(AD) in a non-comparative study. Methods : 10 patients with AD were included for 4 weeks of treatment. Efficacy and safety assessment included the scoring atopic dermatitis index(SCORAD), typical signs and symptoms of AD, results of some laboratory tests related to toxicity, and the incidence of adverse events. Results : Improvements in efficacy parameters were observed and produced no significant changes in laboratory tests related to toxicity in these patients. Their SCORAD results significantly decreased after 4weeks(P value<.01, according to the Wilcoxon sum of ranks test). Similarly, significant reductions from baseline in subjective pruritus scores and (P value<.05 by the Wilcoxon sum of ranks test) and the mean average of individual signs and symptoms of AD were reported after 4 weeks(P value<.05, P<0.01 by the Wilcoxon sum of ranks test). There were no significant changes in eosinophil, neutrophil, lymphocyte, immunoglobulin E and ESR in blood serum by paired ttest. Conclusion :CheungYeolYiSeup-tang administration and Hwangbaek external dressings are an effective and safe treatment for the management of dampness-heat pattern atopic dermatitis.

A Study On the Development of the Pi-Yin Theory (비음(脾陰) 이론의 형성에 대한 고찰)

  • Objectives : To examine the developmental process of the Pi-Yin theory since the Huangdineijing to Miu Xiyong. Methods : Related contents since the Huangdineijing to Miu Xiyong was searched to examine the development of the Pi-Yin theory. Results & Conclusions : In the Huangdineijing and Shanghanzabinglun, the origin of the Pi's body fluids that nurtures everywhere is explained to be the Wei, which limited the development of the Pi-Yin theory. Liu Wansu understood tonification of the Pi by means of moistening medicinals to mean adding Pi-Yin based on the manifestation of dryness in the case of dampness deficiency which is the main Qi of the Pi and Wei. Zhu Danxi understood adding Pi-Yin as accomplished by supplementing Blood. The understanding of the nature of Pi to be 'likes dryness and hates dampness' leading to thinking that drying dampness tonifies the Pi was the reason why the Pi-Yin theory could not develop fully. Miu Xiyong accepted theories of both Li Dongyuan and Zhu Danxi, and constructed the Pi-Yin theory to bring caution to the unwanted effects of using Baizhu wrongly. Through examination of the developmental process of the Pi-Yin theory, it could be understood that rather than focusing on the physiological function of the Pi-Yin and ways of maintaining its proper functioning, the theory was developed to bring caution to using medicinals with dry and hot properties that could dry the Pi-Yin.

A Study on the Gagamjeong-gisan in the Onbyeongjobyeon - In Comparison with the Gwakhyangjeong-gisan - (『온병조변(溫病條辨)』 에 나타난 가감정기산(加減正氣散)에 대한 고찰 - 곽향정기산(藿香正氣散)과의 비교를 중심으로 -)

  • Objectives : The purpose and application method of the five kinds of Gagamjeong-gisan in the Onbyeongjobyeon were analyzed and their significance was examined. Methods : First, contents within medical texts on the Gwakhyangjeong-gisan, the original formula for the Gagamjeong-gisan were analyzed. Next, application, symptoms and pathogenic mechanism, treatment principle, composition of the Gagamjeong-gisan as written in the Onbyeongjobyeon were analyzed. Characteristics of each formula as deduced from this process were compared and examined. Results : The Gwakhyangjeong-gisan covers both external contraction and internal damage, applied regardless of external or internal symptoms. It restores the Yang qi of the Spleen and Stomach, balancing the Zheng qi to respond to the external qi. The Gagamjeong-gisan takes after the Gwakhyangjeong-gisan, reflecting such properties. However, the latter differs in that it excludes ingredients unfit for treating dampness-warmth, and uses different application methods according to the level of discharge of dampness pathogen. Conclusions : The Gagamjeong-gisan is modified from the Gwakhyangjeong-gisan, which is applied to cold damage and internal damage to better treat warm disease. It can be said to have acted as a bridge for warm disease scholars in composing formulas that deal with dampness-heat disease.

Literature Study on Jaundice of Kum-Won Sa Dae Ga (금원사대가(金元四大家)의 황달(黃疸)에 대한 문헌적 고찰)

  • The aim of this study was to establish opinions of Kum Won Sa dea ga on jaundice and find out clinical significance. Contents dealing with jaundice were searched manually in works of Kum Won Sa dea ga. Those were translated into Korean and studied. Yoo wan so(劉完素) had opinion that jaundice is induced by dampness with heatness(濕熱) or dryness with heatness(濕熱) and should be treated with diuretics and laxatives. Jang jong jung(張從政) had opinion that jaundice is induced by spleen(脾) mainly and should be treated with diaphoretics, emetics, laxatives. Lee dong won(李東垣) had opinion that jaundice is induced by mistaken diaphoretics, dysfunction of spleen(脾), heatness(熱) and treatments should depend on six meridian pathways(六經). Ju jin hyeong(朱震亨) had opinion that jaundice is induced by dampness with heatness(濕熱) and coldness with dampness(濕熱) and should be treated with diuretics and detoxicant. Kum Won Sa dae ga thought jaundice in induced by dampness with heatness(濕熱) and explained pathology by the five viscera and the six entrails(五臟六腑). And they treated patients with jaundice according to etiologic source.

  • Objective : The purpose of this case study is to report the effectiveness of Sojongyuryang-tang gagambang in respect of spleen dampness on 3 psoriasis patients who had unhealthy dietary habit. Methods : PASI(psoriasis area and severity) score was used to check the prognosis of 3 psoriasis patients, who have been treated with Sojongyuryang-tang gagambang and acupuncture. Results : PASI score decreased as a result of Sojongyuryang-tang gagambang treatment in 3 psoriasis patients. Conclusion : Sojongyuryang-tang gagambang can be effective treatment for the unhealthy diet related psoriasis when diagnosed as spleen dampness.

  • Low Back Pain Prescriptions of Jin, sa tak are peculiar to using Atractylodis Macrocephalae Rhizoma(AMR). All low back pain prescriptions which are enlisted in Byunjeunggimoon, Byunjeungrok and Byunjeungokham include AMR. Whereas low back pain prescriptions which is enlisted in Seoksilbirok are 7 of 10. Preexistence of low back pain prescriptions are not necessarily used AMR. But Jin, sa tak who lived at Ming and Ching era presented AMR in low back pain treatment. AMR is able to get rid of dampness between the kidney functional area and umbilicus. The results are as follows : It is made much of the malicious dampness which is in the kidney. There are not used cold but warm and eliminating dampness herbs to invigorate kidney. It shows that Jin, sa tak who was a Taoist used invigorating and warming kidney herbs. Atractylodis Macrocephalae Rhizoma is mainly used in treating low back pain from Jin, sa tak. Jin, sa tak shows concrete prognosis to treat a disease.

Exposure Assessment of Biological Agents in Indoor Environments (실내환경에서 생물학적 인자에 대한 노출평가)

  • The Institute of Medicine of the National Academies of Science in the United States concluded in its 2004 report that excessive indoor dampness is a public health hazard and that its prevention should be a public health goal. Water damage in buildings, such as leaks from roofs, walls, or windows, may increase indoor moisture levels. Excessive dampness may promote microbial proliferation in indoor environments, increase occupants' exposure to microbial agents, and eventually produce adverse health effects in building occupants. Epidemiological studies to demonstrate the causal association between exposure to indoor microbial agents and health effects require reliable exposure assessment tools. In this review, I discuss various sampling and analytical methods to assess human exposure to biological agents in indoor environments, their strengths and weaknesses, and recent trends in research and practice in the USA.
