갑상선 낭종으로 오인된 갑상설관 낭종 1예 (A Case of Thyroglossal Duct Cyst Presenting as Thyroid Cyst)

  • Thyroglossal duct cyst is the most common congenital neck mass. It develops from remnants of precursors of thyroid gland left behind during embryologic descent form the foramen cecum at the tongue base into the anterior neck during fetal development. An anterior midline neck mass presenting before the age of twenty and displaying vertical movement with tongue protrusion and swallowing is characteristic of this lesion. In this paper, we report on a case of TGDC without remnant duct that is presenting as thyroid cyst.

Eosinophilic granuloma in the anterior mandible mimicking radicular cyst

  • Eosinophilic granuloma is a common expression of Langerhans cell histiocytosis and corresponds with typical bone lesions. The radiographic appearance of eosinophilic granuloma in the jaw is variable and not specific. It may resemble periodontitis, radicular cyst, or malignancies. The purpose of this report is to describe the characteristic radiographic features of eosinophilic granuloma of a 39-year-old male. The lesion in the anterior mandible was first diagnosed as radicular cyst because the radiographic findings were ovoid radiolucent lesion with well-defined border. However, careful interpretation revealed a non-corticated border and floating tooth appearance that were the characteristic radiographic features for the differential diagnosis. Early clinical signs of eosinophilic granuloma can occur in the jaw and a bony destructive lesion might be mistaken for periodontitis or an odontogenic cystic lesion; therefore, careful interpretation of radiographs should be emphasized.

소아에서 발생하는 선천성 두경부 기형 (Congenital Anomalies of Head and Neck in Children)

  • Congenital anomalies of the head and neck region such as preauricular sinus and skin tag, thyroglossal duct cyst, branchial anomaly, cystic hygroma and dermoid cyst are common in pediatric population. It is important for pediatricians and pediatric surgeons to be familiar with the embryology and the anatomical characteristic of these lesions in order to diagnose and treat them properly. Three hundred and nineteen patients with congenital head and neck anomalies treated at Hanyang University Hospital between 1980 and 1999 were reviewed to determine the relative frequency of the anomalies and to analyze the method of management. Eight-four (25.1 %) of 335 lesions were preauricular sinus and skin tag, 81 (24.2 %) were thyroglossal duct cyst, 81 (24.2 %) branchial anomaly, 58 (17.3 %) cystic hygroma and 31 were (9.2 %) dermoid cyst. The male-to-female ratio was 1.4:1. Thyroglossal duct cyst most commonly present at 3-5years, however branchial anomalies commonly are diagnosed in children younger than 1 year. Preauricular sinus showed familial tendency in three patients and was bilateral is 33.8 %. Most head and neck anomalies in children have specific clinical and anatomical characterics. A careful history and physical examination is very useful for diagnosis and proper management. Experienced pediatric surgeons should do the initial surgery since the recurrence rate after incomplete surgical excision can be high.

Mxi1 influences cyst formation in three-dimensional cell culture

  • Cyst formation is a major characteristic of ADPKD and is caused by the abnormal proliferation of epithelial cells. Renal cyst formation disrupts renal function and induces diverse complications. The mechanism of cyst formation is unclear. mIMCD-3 cells were established to develop simple epithelial cell cysts in 3-D culture. We confirmed previously that Mxi1 plays a role in cyst formation in Mxi1-deficient mice. Cysts in Mxi1 transfectanted cells were showed by collagen or mebiol gels in 3-D cell culture system. Causative genes of ADPKD were measured by q RT-PCR. Herein, Mxi1 transfectants rarely formed a simple epithelial cyst and induced cell death. Overexpression of Mxi1 resulted in a decrease in the PKD1, PKD2 and c-myc mRNA relating to the pathway of cyst formation. These data indicate that Mxi1 influences cyst formation of mIMCD-3 cells in 3-D culture and that Mxi1 may control the mechanism of renal cyst formation.

전두동을 침습한 안와부 피부모양기형낭의 치험례 (A Clinical Experience of Direct Extension to Frontal Sinus of Orbital Dermoid Cyst)

  • The dermoid cyst is the one of common space occupying orbital lesion. This lesion is regarded as a non-invasive tumor, but infrequently causes destruction of adjacent bony structure and displacement of adjacent tissue. We experienced a characteristic ovoid orbital dermoid cyst that occupied in the frontal sinus and causes displacement of the eyeball with well-defined lining. This 55-year-old male presented a mass in left orbit, which rapidly increased in size for past 12 months, and patient could not open left eyelid. We removed this tumor totally then reconstructed the orbital roof and frontal sinus with an iliac bone graft and polyethylene sheet(Medpor Newnan, USA). This patient was followed up for 12 months and patient obtained satisfactory result without any complication suck as recurrence or infection.

  • Skin cancer, which often occurs as a result of skin exposure to ultraviolet light radiation, usually presents with characteristic abnormal features, such as ulcerative lesions, irregular morphology, bleeding, and excessive growth. Therefore, skin cancer rarely resembles a benign tumor on visual inspection. Nonetheless, squamous cell carcinoma and basal cell carcinoma with nodular or polypoid features can have a similar appearance to that of benign tumors, meaning that they are sometimes misdiagnosed as benign. As benign and malignant tumors have some overlapping features, clinicians sometimes use additional imaging techniques such as ultrasonography to improve the accuracy of the diagnosis because even a malignant tumor that externally resembles a benign tumor generally has internal morphological features characteristic of malignancy, such as invasion and irregular borders. However, these imaging tools also have limitations, and punch or excisional biopsy can be needed if malignancy cannot be completely ruled out. Herein, we report a case of skin malignancy initially misdiagnosed as a benign epidermal cyst based on external visual inspection and ultrasonography.

개의 신낭종의 1례 (Renal Cyst in a Mongrel Dog)

  • Renal cyst was detected in a mongrel dog aged 3 months old. The most prominent clinical abnormalities were vomiting, loss of appetite, depression, abdominal swelling and lethargy. Gross appearances included ascites and bilaterally irregular capsular surfaces of the kidneys. Characteristic microscopical findings were various sized cysts formation, shrunken glomeruli with hypercellularity, connective tissue porliferation in the interstitum, tubular dilatation and sligth leukocytic infiltration. The cause. of the renal disorder in a mongrel dog could not be determined.

  • 기관지기원 물혹은 드물게 발생하는 선천성 질환으로 주로 종격동에 분포하며 기관지와의 소통이 발생하는 경우, 악성으로 전환된 경우를 제외하고는 크기가 갑자기 커지는 경우는 드문 일이다. 1년 전 검진에서 시행한 단순 흉부 촬영에서 이상이 없었던 젊은 남자가 흉부검진에서 종격동 덩어리가 발견되어 급격한 크기의 증가가 확인되었던 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

  • The spermatogenesis and chromosome number were investigated in the pupal testes of Helicouerpa assulta Guenee by light microscopy. During the spermatogenesis, each bundle of P8(256) sperms developed by 6 mitotic and 2 meiotic spermatogonial divisions. From the early stage of spermatogenesis, it was distinguishable between two kinds of sperm differentiation, eupyrene and apyrene spermatogenesis, which are characteristic in Lepidoptera, by the differences in nuclear shape and cell distribution in immature spermatocyst. Through the followed spermiogenesis, the spermatocysts were developed into two kinds of mature cyst, a streamline-shaped eupyrene cyst with nucleated sperms of thready head or a long spindle-shaped apyrene cyst with anucleated sperms of cylindrical head. As the results off chromosomal analysis at metaphase of the spermatogonial mitosis and spermatocytic meiosis, the chromosome number were 2n=6a/n=31, respectively, and no variation between individuals.

$^{99m}Tc-DISIDA$ 간담도 스캔으로 발견된 총담관낭종 1예 (A Case of Choledochal Cyst Detected by $^{99m}Tc-DISIDA$ Hepatoblilary Scan)

  • It is well known that $^{99m}Tc-DISIDA$ hepatobilliary scan is a noninvasive and accurate method for detecting choledochal cyst. In the case we are reporting, $^{99m}Tc-DISIDA$ scan of a 17 year old female patient showed the characteristic pattern of a choledochal cyst, confirmed by surgery.
