TREATMENT FOR IDIOPATHIC MULTIPLE DENTIGEROUS CYSTS: CASE REPORT (양악 편측에 발생한 다발성 함치성 낭종의 치험례)

  • Dentigerous cysts generally encompass the crown of an unerupted tooth. These cysts are usually solitary. They are the second most common odontogenic type of cysts following radicular cysts, and are frequently associated with impacted mandibular third molars or maxillary canines. Most multiple cysts found in the jaw are odontogenic keratocysts associated with the nevoid basal cell carcinoma syndrome, mucopolysaccharidoses and cleidocranial dysplasia. Although a single dentigerous cyst is well documented in the medical literature, including the prevalence, treatment and prognosis, multiple dentigerous cysts without any systemic symptoms is unusual. Furthermore, cases involving both the maxilla and mandible are especially rare. We present the case of an 11-year-old boy with nonsyndromic multiple dentigerous cysts associated with a mandibular second premolar and a maxillary canine. The treatment was conservative and included marsupialization and eruption guidance. Further follow up is planned to rule out additional problems and the possible identification of a syndrome.

Disorders of Secondary Neurulation : Mainly Focused on Pathoembryogenesis

  • Recent advancements in basic research on the process of secondary neurulation and increased clinical experience with caudal spinal anomalies with associated abnormalities in the surrounding and distal structures shed light on further understanding of the pathoembryogenesis of the lesions and led to the new classification of these dysraphic entities. We summarized the changing concepts of lesions developed from the disordered secondary neurulation shown during the last decade. In addition, we suggested our new pathoembryogenetic explanations for a few entities based on the literature and the data from our previous animal research. Disordered secondary neurulation at each phase of development may cause corresponding lesions, such as failed junction with the primary neural tube (junctional neural tube defect and segmental spinal dysgenesis), dysgenesis or duplication of the caudal cell mass associated with disturbed activity of caudal mesenchymal tissue (caudal agenesis and caudal duplication syndrome), failed ingression of the primitive streak to the caudal cell mass (myelomeningocele), focal limited dorsal neuro-cutaneous nondisjunction (limited dorsal myeloschisis and congenital dermal sinus), neuro-mesenchymal adhesion (lumbosacral lipomatous malformation), and regression failure spectrum of the medullary cord (thickened filum and filar cyst, low-lying conus, retained medullary cord, terminal myelocele and terminal myelocystocele). It seems that almost every anomalous entity of the primary neural tube may occur in the area of secondary neurulation. Furthermore, the close association with the activity of caudal mesenchymal tissue in secondary neurulation involves a wider range of surrounding structures than in primary neurulation. Although the majority of the data are from animals, not from humans and many theories are still conjectural, these changing concepts of normal and disordered secondary neurulation will provoke further advancements in our management strategies as well as in the pathoembryogenetic understanding of anomalous lesions in this area.

Treatment of Benign Bone Lesions with Autologous Bone Marrow Stromal Cell Transplantation (자가 골수 기질 세포 이식을 이용한 장관골 양성 골 병변의 치료)

  • Purpose: We analyzed the result of autologous bone marrow stromal cell transplantation with or without cancellous chip bone allograft for benign long bone lesions. Materials and methods: Since July 1996, eight benign bone lesions treated by curettage, cancellous chip bone allograft and bone marrow or marrow stromal cell transplantation were observed for resolution of clinical symptoms, new bone formation and consolidation. There were 6 males and 2 females. Average age was 24 (range 8 to 47) years old. Histologic diagnoses were 5 fibrous dysplasia, 2 simple bone cysts and one chondroblastoma and fibrous cortical defect each. Mean follow-up period was 16.3 (range 3 to 84) months. Results: In all four symptomatic patients, the pain was subsided in two weeks after surgery. New bone formation in the lesion was observed at 4 weeks, which incorporated into surrounding normal bone around 8 weeks. There were one pathologic fracture through the lesion at 3 weeks and one recurrence of simple bone cyst at 5 months postoperatively. Conclusion: Bone marrow or marrow stromal cell transplantation for bone defects from curettage of benign bone lesions, with or without cancellous chip bone allograft revealed rapid healing. Though it was the result of short-term follow up, it supports that bone marrow stromal cell transplantation will be very useful for the treatment of benign long bone cysts or other lesions. The complete curettage of inner cystic wall is important to prevent later recurrence, and the rigid internal fixation is also needed in selected high risk lesions of fracture.

Clinical Characeristics of Intracordal Cysts (성대낭종의 임상적 특성)

  • Background and Objectives : The intracordal cysts are more increasingly diagnosed and treated due to advanced laryngeal stroboscopy and laryngeal microsurgical technique. The intracordal cysts are frequently misdiagnosed as vocal polyp or nodule The purpose of this study is to evaluate clinical features of intracordal cysts. Materials and Methods : In the present series, 83 cases of the intracordal cysts treated with laryngeal microsurgery are reported. The intracordal cysts are diagnosed preoperatively with indirect laryngoscopy, laryngeal endoscopy, laryngeal stroboscopy and confirmed with laryngeal microsurgical findings and biopsies. Results : Intracordal cysts are 83 of 1900 patients treated with laryngeal microsurgery(4.4%)-ductal cysts are 56 cases and epidermoid cysts are 27 cases. Intracordal cysts are more frequent in women, forties and the frequent site is an anterior third of the true vocal cord. With the indirect laryngoscopic examination, the ductal cysts are frequently misdiagnosed as vocal polyps or nodules but the epidermoid cysts are relatively easily diagnosed. The etiologic factors of the intracordal cysts are suspected as voice abuse and upper respiratory infection. The degree of postoperative voice satisfaction is similar to that of the vocal polyps. Conclusion : Intracordal cysts are frequently misdiagnosed as polyps or nodules, therefore preoperative stroboscopic findings and laryngeal microsurgical findings is important. An ideal treatment is to enucleate the cysts avoiding rupture of cyst and injury of lamina propria of the vocal cord.

  • Radiation-induced life shortening, carcinogenesis and other pathological changes were investigated in NIH(GP) mice after $^{60}Co-{\gamma}$ ray irradiation(100~700rads). The results were summarized as follows: 1. There were little difference in body weights, hematological examination and other clinical findings between normal and irradiated groups. 2. Mean survival time of the mice after irradiation were decreased dose-dependently. 3. Main gross findings of the mice irradiated were appeared as enlargement of spleen, thymus and lymph nodes, tumorous nodules of lung and cyst of ovary. Especially, enlargement of thymus was promineort in high dose group. 4. Microscopically, there were various findings including myeloid leukemia, thymic lymphoma, lung adenoma, adenosquamous cell carcinoma of pancreas, pneumonia and other pathological changes. Especially thymic lymphoma was highly frequent in the 700 rads group.

A Study on the Mixed Jaw Lesions Associated with Teeth (치아와 관련되어 나타나는 악골의 혼합병소에 관한 연구)

  • Purpose : 1. Retrospectively evaluate the accuracy of tentative diagnosis or impression from the clinico-radio-graphic materials of jaw lesions which showed mixed lesions associated with teeth. 2. To observe the diagnostic importance of the calcified part of the lesions which appear as radiopaque areas. Materials and Methods: 14 cases of jaw lesions which showed mixed lesions associated with teeth were reviewed. These lesions were mostly diagnosed as adenomatoid odontogenic tumors (6 cases) or calcifying odontogenic cysts with (4 cases) or without odontomas (4 cases). The calcified elements of the lesions which demonstrated various sizes and patterns of radiopaque shadows resembled odontoid tissues in some cases but could not be defined in some other cases radiographically. Results : The final histopathologic diagnosis confirmed adenomatoid odontogenic tumors in 4 of the 6 cases. The remaining 2 cases turned out to be odontoma and ameloblastic fibroodontoma. The 4 cases of calcifying odontogenic cysts with odontomas were correct in 3 cases but remaining 1 case was just odontoma. The 4 cases of calcifying odontogenic cysts were proved to be odontogenic keratocyst, calcified peripheral fibroma, unicystic ameloblastoma and squamous cell carcinoma. Conclusion : The diagnostic accuracy of the adenomatoid odontogenic tumors and calcifying odontogenic cysts were high when the lesions show typical appearance. The calcifications which show radiopaque areas could be odontomas or dystrophic calficifations or remnants of bone fragments from resorption.

  • The lymphangioleiomyomatosis(LAM) is a rare disorder, which afflicts mainly young woman of childbearing age, characterized by proliferation of immature smooth muscle cell in the lymphatics. We experienced a case of LAM in 26-years-old pregnant woman, confirmed pathologically by inguinal lymph node biopsy. She has suffered from exertonal dyspnea and dry coughing. The symptoms and chest X-ray were aggravated with pregnancy, but improved after delivery with two times of pregnancy. The chest X-ray showed diffuse reticulonodular infiltration and chest HRCT showed diffuse scattered tiny thin-walled cyst of lung parenchyma. We noted chylous ascites of which triglyceride level is 396 mg/dl. After delivery, the symptoms were getting better. We treated with medroxyprogesterone and planned close observation and follow-up.

Histological Changes of Rat Preputial Gland during Normal Sexual Cycle and Sex Hormone Treatment (성주기(性週期) 및 성(性)호르몬 주사(注射)에 따른 Rat 포피선(包皮腺)의 조직학적변화(組織學的變化))

  • In order to know the histological changes of rat preputial gland during normal sexual cycle and sex hormone treatment, matured Wistar rats (B.W.about 200g) were used for the experiment. Rats were subcutaneously given $2{\mu}g$ 17-${\beta}$-estradiol (Sigma) and 2.5g progesterone (Nakarai Chem., Japan) daily in 0.5ml propylen glycol for ten days respectively. The results obtained are as follow: At the stage of estrus and metestrus, the eosinophilic crystalloid granules and large vacuoles in the acinar cells appeared numerously, and the excretory ducts were severely extended. The developed connective tissue between the acinus were also found. At the stage of proestrus and diestrus, however, the small vacuoles and a few eosinophilic crystalloid granules appeared in the acinar cell. In the estradiol treatment, on the other hand, severely extended excretory ducts and a small number of the eosinophilic crystalloid granules compared with the progesterone treatment were found. The cyst-like structure was found, and in that the acinus disappeared completely. In the progesterone treatment, remarkably extended excretory ducts and nomerous appearance of oval formed eosinophilic granules in the acinar cells were found. As these findings, it could be suggested that the secretion of rat preputial gland was active at the estrus and metestrus stage, and estrogen may concerned in the secretion, land progesterone in the formation of secretory products.

Periosteal Ganglion of the Distal Fibula - A Case Report - (원위 비골에서 발생한 골막하 결절종 - 증례 보고 -)

  • In contrast to ganglion of the soft tissue, periosteal ganglion occurring within or beneath the periosteum is a rare disorder. The differential diagnosis includes periosteal chondroma, lipoma, giant cell tumor of tendon sheath and periosteal osteosarcoma. Most common location for periosteal ganglion is the tibia, followed by radius, femur and ulna. To our knowledge, only 1 case of periosteal ganglion of the fibula has been reported in the literature. We report a case of periosteal ganglion of the distal fibula in a thirty-year-old woman treated with excision of the cyst and the adjacent periosteum.

Infection by an Ichthyophonus sp. Fungus in Rainbow Trout Salmo gairdneri (무지개송어에 기생하는 Ichthyophonus sp.에 대하여)

  • Ichthyophonus disease broke out among rainbow trout (Salmon gairdneri) fry in November 1979, and after that a fish group containing diseased fish was kept for one year. The authors examined histopathologically the diseased fish 3 times, at 6 momths intervals. Diseased fish showed markedly stunted growth and darkish coloration, and anatomically the liver with small white nodules, the heart with red nodules, the spleen with granular nodules and the markedly tumefied kidney. Ichthyophonus invaded various tissues in the host and took the shape of multinucleate spherical or hyphal body. The multinucleate-spherical bodies contained many nuclei with a large nucleolus and granular cytoplasm. Histopathologically systemic dissemination and systemic proliferation by Ichthyophonus sp. wire indentified. In addition to a single multinucleate-spherical body appeared in the various tissue, large granulomas containing two to several generations of Ichthyophonus sp. were formed especially in the liver, kidey and spleen. The inflammatory response against Ichthyophonus was characterized by mononuclear-celluar proliferation with giant cell formation and fibrobasts proliferation.
