철도차량에서의 지체장애인 피난 실험 (Experiments on Egress of Persons with Mobile Disability in Train Car)

  • 철도차량의 화재안전성능을 향상시키기 위해 최근 성능평가 방법의 도입을 추진하고 있으며, 피난도 큰 부분을 차지하고 있다. 그런데 철도는 공공교통수단이므로 평가에 교통약자를 포함해야 하지만, 일반적으로 피난에 관한 평가를 수행할 경우 대부분의 경우가 교통약자들을 고려하고 있지 않은 것이 현실이다. 장애인중 보행이 어려운 지체장애인의 화재 시 철도차량에서의 피난에 대한 실험을 수행해보았다. 철도차량으로는 현재 운행 중인 새마을과 무궁화를 대상으로 하였다.

  • Objectives : This study was designed to investigate the effect of SAAM-Acupuncture and GamigungguitangGaNokyong(GMGGTN) on Ankle Fracture. Methods : SAAM-Acupuncture was administered once daily. The following points were selected: $SP_3$(太白), $LU_9$(太淵), $LI_{11}$(曲池). After short leg casting, GMGGTN was taken 3 times per a day from March 28th to April 30th. We evaluated the patient using the Visual Analogue Scale(VAS) and Ankle Range of Motion(ROM). Results : After about 3 weeks of treatment, the patient's short leg cast was removed a week earlier than expected and the patient could walk without crutches and physical therapy. Their VAS changed from 10 to 1. And after added treatment for 2 weeks, there was no pain and limitation of ROM. Conclusions : According to the results, SAAM-Acupuncture and GMGGTN may have rapid effects on ankle fractures. But further studies are required to confirm this effect.

장애인을 위한 의복디자인 (I) -부목 .목발 및 휠체어 사용자를 중심으로- (Clothing for the Handicapped: Brace.Crutch & Wheelchair User)

  • The purpose of my study is to provide some information on clothing for the handicapped with special needs. So I have designed clothes for the handicapped to provide some solutions for their clothing problems. These clothing designs for crutch and wheelchair users were based on solutions proposed by many researchers. The number of handicapped has been increasing gradually for years, especially the physically handicapped. Although they make use of many instruments for ease of movements and other physical conditions, most of the handicapped generally use crutches and wheelchairs. So I designed clothing for the crutch and wheelchair users. In order to ease problems in dressing and undressing, when using the crutch and wheelchair, various physical attributes of clothing should be considered: (1) Selection of fabric, (2) construction and location of the opening, (3) type and location of fastenings, and (4) design of garments for comfort and ease of movement. Clothing should also be able to satisfy psychological needs related to attractive appearance. My designs have proposed seven items of clothing. These include a cape-coat, over-blouse, one-piece dress and an pants for the crutch users. Also, a shirts, pants, and wheelchair wrap for wheelchair users will be displayed.

Searching for Hidden, Painful Osteochondral Lesions of the Ankle in Patients with Chronic Lower Limb Pain - Two Case Reports -

  • It is easy to overlook osteochondral lesions (OCLs) of the ankle in patients with chronic lower limb pain, such as complex regional pain syndrome (CRPS) or thromboangiitis obliterans (TAO, Buerger's disease). A 57-year-old woman diagnosed with type 1 CRPS, and a 58-year-old man, diagnosed with TAO, complained of tactile and cold allodynia in their lower legs. After neurolytic lumbar sympathethic ganglion block and titration of medications for neuropathic pain, each subject could walk without the aid of crutches. However, they both complained of constant pain on the left ankle during walking. Focal tenderness was noted; subsequent imaging studies revealed OCLs of her talus and his distal tibia, respectively. Immediately after percutaneous osteoplasties, the patients could walk without ankle pain. It is important to consider the presence of a hidden OCL in chronic pain patients that develop weight-bearing pain and complain of localized tenderness on the ankle.

  • Arthritis of hip joints deteriorates the quality of life in ankylosing spondylitis (AS) patients. Secondary to the articular inflammatory process, the shortened hip-girdle muscles contribute to the decreased joint mobility which may lead to the functional impairment. As the limitation of range of motion (ROM) usually progress slowly, clinicians regard it as a chronic condition and prescribe long-term therapy. However, by short-term intensive multimodal treatment, a 20-year-old man diagnosed as AS with severely limited hip joint ROM who relied on crutches doubled the joint angle and could walk independently only within 2 weeks. The combination included intra-articular steroid injection, electrical twitch obtaining intramuscular stimulation, extracorporeal shock wave therapy, heat, manual therapy, and stretching exercises. The management focused on the relaxation of hip-girdle muscles as well as the direct control of intra-articular inflammation. Hereby, we emphasize the effectiveness of intensive multimodal treatment in improving the function even within a short period.

의료시설의 한국형 인명안전기준개발을 위한 피난용량 산정에 관한 연구 (A Study on the Calculation of Evacuation Capacity for the Development of Korean Life Safety Standards for Medical facilities)

  • In the case of medical facilities, the evacuation time is delayed due to the decrease in the number of people in the hallway and exits due to the increase in the width of evacuation by using mobile beds, wheelchairs, crutches, etc. Accordingly, it is considered to secure evacuation capacity to reduce evacuation time according to corridor width and exit width. Accordingly, we would like to compare the standards related to the evacuation capacity of medical facilities in Korea and NFPA, derive differences, and use evacuation simulations to compare evacuation times according to changes in corridor width and exit width. In Korea, it is calculated based on the floor area by use, but in the case of NFPA 101, the number of evacuation routes, stair width, corridor exit width, and two-way door width was stipulated depending on the number of people. Using evacuation simulation, efficient evacuation capacity is calculated according to the reduction of evacuation time by changing the width of the hallway, changing the width of the exit, the width of the corridor, and the width of the exit. The evacuation simulation is intended to be used to secure evacuation safety of domestic medical facilities by calculating the effective evacuation time reduction by changing the width of the hallway and exit.

이상적인 목발 길이와 목발 길이 추정법들간의 비교 (A Comparative Analysis between Several Crutch-Length-Estimation Techniques and Ideal Crutch Length)

  • The purpose of this study was to determine which of several crutch-fitting techniques best predicts ideal crutch length. Ideal crutch length is defined as the length of the crutch, including accessories, obtained during stance when the crutch tip is 6 inches (15.2 cm) lateral and 6 inches(15.2 cm) anterior to the fifth toe and the axillary pad is 2.5 inches(6.4 cm) below the axillary fold. Forty four volunteers were measured for crutches using each of the following methods:(1) 77% of actual height, (2) actual height minus 40.6 cm, (3) actual height minus 45.7 cm, (4) olecranon to opposite third finger tip, (5) olecranon to opposite fifth finger tip, (6) 77% of arm span, (7) arm span minus 40.6 cm, (8) anterior axillary fold to heel plus 5.1 cm, (9) anterior axillary fold to heel plus 10.2 cm, (10) ideal crutch length. Of the techniques studied, the two involving anterior axillary fold to heel were found to be good predictors: anterior axillary fold to heel plus 5.1 cm and anterior axillary fold to heel plus 10.2 cm. Finally, two additional length estimates were derived using linear regression analyses. These estimates provided the best overall predictors based on anterior axillary fold to heel and actual height.

노인의 보행보조기구 사용 보행시 보행패턴의 변화연구 (Biomechanical Analysis of the Elderly Gait with a Walking Assistive Device)

  • Walking is not only an essential component of the human mobility, but also is a good exercise. Inability to walk freely can reduce an individual's quality of life and independence substantially. Being a relatively low impact activity, walking is particularly good for the elderly and research has shown that regular walking in the elderly reduces the chance of fall-related injuries and mental diseases as well. In spite of the documented benefits of regular walking, it is still difficult to walk without the aid of assistive devices for the frail elderly who have lower extremity problems. Assistive walking devices(AWD), such as crutches, canes, hiking-poles, T-Poles and walkers, are often prescribed to the elderly to make their walking be safe and efficient. Many researchers have demonstrated the effects of AWDs such as reducing lower extremity loading, improved dynamic/gait stability, yet, no study has been done for gait pattern when the elderly gait with AWDs. Therefore, the purpose of this study was to examine whether T-Poles, one of the AWDs, change the elderly gait pattern. Eight community-dwelling female elderly participated in this study. Laboratory kinematics during walking with T-Poles(PW) and with out T-Poles(NPW) was assessed. PW showed significant increase in step width, stride length, gait velocity and decrease in swing time. No significances were found in lower body joint angles but meaningful trend and pattern were found. Maybe the reason was due to the participants. Our participants were healthy enough so that the effect of T-Poles was minimum. PW also showed typical gait phases which are no single support phase during a gait cycle. It indicates that walking with T-Poles may guarantee safe and confident walking to the frail elderly.

비골-가자미근 유리피판술을 이용한 족부의 골 및 연부 조직 결손 재건 (Fibula-Hemisoleus Osteomusculocutaneous Free Flap for Foot Reconstruction)

  • The injury on the dorsum of foot is usually manifested in the defect of bone and soft tissue, so its reconstruction requires composite tissue. Free flap satisfies this defect but its indication is determined by the defect size, recipient status and so on. Iliac crest bone and fibular bone are useful bone flap but in more than 8cm defect, fibular flap is more useful. The drawback of fibular free flap is the absence of soft-tissue coverage, so another local flap and myocutaneous flap must be added. Fibula-hemisoleus ostemusculocutaneous free flap has been used for the reconstruction of upper and lower extremity. Its advantages are one stage operation, one donor site and the flexibility of the reconstruction with the use of muscle, bone, and skin. This flap has never been reported for the reconstruction of dorsum of foot. In our case, 20-year-old woman was referred with the 17 cm defect of 1st metatarsal bone and $16{\times}8cm$ sized soft tissue loss on the dorsum of the right foot. We reconstructed successfully the dorsum of foot with fibula-hemisoleus osteomusculocutaneous free flap and the patient can walk without crutches after 6 monthes.

오페라 "파우스트"의 무대의상 디자인 - 군중들을 중심으로 - (A Study on The Stage Costume Design of Opera $\lceil$Faust$\rfloor$ - Focused on The Crowd -)

  • Modernization movement has affected classical operas. Today, classical operas are often re-interpreted from the modern perspective and played differently only with the storyline maintained. Also, operas are increasingly performed outside traditional theaters as the boundaries between stage and audience become less obvious. Accordingly, stage costumes are being differently designed than before. New attempts are being made to look costumes in a harmony with increasingly streamlined stage machinery. This helps increase public attention on operas, consequently promoting the opera industry. This study examines modernization of opera costumes. For this study, the author worked as one of costume design staff for "Faust," which was played in Sungnam Art Center from November 24, 2005 to November 27, 2005 in commemoration of the opening of the center. The following outcomes of the crowd are drawn from this study. A variety of splendid pink dresses, ribbons, dolls, hand mirror, and hair bands were used to express haughty girls with "Princess Syndrome." Fashion models, the envy of all woman, wore fashionable clothes including luxury dresses, fur-coats, high heels, purses, and hats, Models also had big shopping bags and gift boxes to symbolize shopping lovers in a modern society. Gay men wore tight leather trousers and vest and sleeves shirts with deeply cut neckline to express their preference for feminie style. their clothes were splendid colors that normally women liked such as gold, purple, light green, scarlet, and silver. Soldiers were in combat uniform representing their participation in the war. In particular, clothes stained with dirt, iron helmet, crutches, and canteens were used to vividly express soldiers coming back home from the war. Modern opera costumes now help reduce the time and space between stage and audience, improve economic efficiency, and meet the audience's needs for various style.
