Status of Tuberculosis Infection Control in Hospitals (의료기관의 결핵감염관리 현황)

  • Purpose: The purpose of this study was to determine the status of tuberculosis (TB) infection control in hospitals. Method: This study was a cross-sectional descriptive study in which self-administered questionnaires were used to survey 77 hospitals. Data were collected from August 12 to September 15, 2012. Results: Only 27 institutions (35.1%) were monitoring patients infected with or suspected of having TB. Most hospitals were conducting TB prevention education for patients' family and employees, and TB screening for employees along with follow-up examinations. However, private negative pressure rooms were more often available in wards and intensive care units than in outpatient departments, and in institutions with over 700 beds located in Seoul or GyeongGi Province. Most hospitals ensured that masks were placed on infected patients while transporting them. In addition, efforts to control TB infection such as placing respiratory etiquette posters in outpatient departments and airborne infection signs on the doors in the wards were also well established in most institutions. Conclusion: More efforts are needed to improve the status of TB infection control to ensure quality care, especially in terms of monitoring patients with TB infections and making private negative pressure rooms available in outpatient departments.

A case of Diphyllobothrium latum infection with a brief review of diphyllobothriasis in the Republic of Korea

  • A case of Diphyllobothrium latum infection in a 49-year old man is described, and diphyllobothriasis latum in the Republic of Korea is briefly reviewed. An incomplete strobila of a tapeworm, 95 cm in length, without scolex and neck, was spontaneously discharged in the feces of a patient. On the basis of morphologic characteristics of the worm and eggs, the worm was identified as D. latum. The patient was successfully treated with a single dose (15 mg/kg) of praziquantel. The most probable source of infection was salmon flesh according to the past history of the patient. The first case of D. latum infection was documented in 1971, and this is the 43rd recorded case in the Republic of Korea. The 43 cases were briefly reviewed. The patients' main complaints were gastrointestinal troubles, such as mild abdominal pain, indigestion, and diarrhea, and discharge of tapeworm segments in the feces. The suspected infection sources included raw or improperly cooked flesh of fresh or brackish water fish, including the perch, mullet, salmon, and trout.

  • The immnunogenicity of the possible non-essential component of the combined vaccine (KGCC-957) for the prophylaxis against Japanese encephalitis and Hantaan virus infection recently developed by Korea Green Cross Corporation was investigated using the Hartley guinea pigs. The KGCC-95Vl was administered to the guinea pigs subcutaneously to sensitize the animals. The guinea pigs did not induce any anaphylactic immune responses which could be detectable by the active systemic anaphylaxis (ASA), the passive systemic anaphylaxis (PSA), and the passive cutaneous anaphylaxis (PCA) tests. The KGCC-95Vl is considered not to induce any anaphylactic immune responses except the prophylatic immune effects of the vaccine.

Protection Against Salmonella Typhimurium, Salmonella Gallinarum, and Salmonella Enteritidis Infection in Layer Chickens Conferred by a Live Attenuated Salmonella Typhimurium Strain

  • In the present study, we investigated the protection conferred by a live attenuated Salmonella enterica serovar Typhimurium (ST) strain against Salmonella Typhimurium, Salmonella Gallinarum (SG), and Salmonella Enteritidis (SE) infection in layer chickens. Birds were orally primed with the attenuated ST strain at 7 days of age and then boosted at 4 weeks post prime immunization (PPI). Sequential monitoring of plasma IgG and mucosal secretory IgA (sIgA) levels revealed that inoculation with ST induced a significant antibody response to antigens against ST, SE, and SG. Moreover, significant lymphoproliferative responses to the 3 Salmonella serovars were observed in the immunized group. We also investigated protection against virulent ST, SE, and SG strain challenge. Upon virulent SG challenge, the immunized group showed significantly reduced mortality compared to the non-immunized group. The reduced persistence of the virulent ST and SE challenge strains in the liver, spleen, and cecal tissues of the immunized group suggests that immunization with the attenuated ST strain may not only protect against ST infection but can also confer cross protection against SE and SG infection.

Production of Hantaan Virus from Human Immortalized Retina Cell and Its Immunogenicity

  • Hantaan vims production, using human immortalized retina cell (PER. C6), was investigated to develop an inactivated virus vaccine. To infect Hantaan virus into PER. C6, two infection methods (medium-to-cell and cell-to-cell) were tried, and IFA results showed that the cell-to-cell infection method was very useful for producing Hantaan virus-infected PER, C6. Hantaan virus production was significantly affected by the growth rate of PER. C6 and the content of FBS in medium. Higher specific growth rate of infected PER. C6 and lower FBS content induced higher production of Hantaan virus. The inactivated human cell-culture vaccines with various EIA titers were prepared, their antibody responses were compared with those of inactivated suckling mouse brain vaccines ($Hantavax^처리불가$). and the result showed their immunogenicities were slightly higher than those of inactivated suckling mouse vaccines. Therefore, this study shows the possibility of the development of Hantaan virus vaccine from a human cell culture.

  • Objectives : The purpose of this study was to examine the relationship between the infection control education experiences of dental hygienists and the state of their infection control. Methods : The subjects in this study were the dental hygienists who worked in Seoul. The relationship of their infection control education experiences to their gender, marital status, academic credential, workplace, length of service, infection control implementation, experience of being exposed to infection, way of coping with it, hand washing, use of personal protection devices and equipment management was analyzed. Results : It is found that the variable to affect the state of infection control was educational experiences about handpiece water pipe management, ultrasonic scaler water pipe management and three-way syringe water pipe management. Conclusions : Dental hygienists who are one of major dental personnels should receive systematic education on infection control to acquire accurate knowledge to ensure the successful prevention of cross infection.

Cross-Infection of Flacherie and Densonucleosis Virus of the Silkworm, Bombyx mori, to Mulberry Pests. (누에 무름병 및 농핵병 바이러스의 주요 뽕나무 해충에 대한 교차감염)

  • Flacherie virus (FV) and Densonucleosis virus (DNV) of the silkworm, Bombyx mori, which give the most severest damage to the silk production in korea, were fed on the mulberry wild silkworm, Bombyx mori mandarina, the mulberry pyralid, Gryphodes phyloalis, and the American fall webworm, Hypantria cunea, to investigate cross infectivity by serological and histopathological at observation. By the Ouchterlony's double difusion test the mulberry wild silkworm was infected with both FV and DNV type 1 (DNV-1) and the mulberry pyralid with DNV-1, so those were confirmed the cross infection. But the American fall webworm was not recognized the cross infection by the same method. The infection and multiplication of the FV in the mulberry wild silkworm was observed in the cytoplasm of the goblet cell with the appearance of the virus-specific vesicle. In DNV-1 infection to the mulberry wild silkworm and the mulberry pyralid, the nuclei of columnar cell in the midgut of both insects was hypertrophied and the nuclei of midgut cell of the mulberry pyralid positively stained with the feulgen stain. Multiplication of DNV-1 in the midgut cell of the mulberry wild silkworm was replicated in two different patterens as linear arrays and large masses, while that of DNV-1 in the muberry pyralid was multiplied as virus masses in several portion of the nuclei of the midgut cell.

Effects of a Telephone Follow-up Program on Patient Compliance, Urea Kinetic Modeling and Incidence of Infection in Patients with Continuous Ambulatory Peritoneal Dialysis (CAPD) (전화추후관리 프로그램이 복막투석환자의 치료지속이행, 투석적절도와 감염발생도에 미치는 효과)

  • Purpose: The objective of this study was to identify the effects of a telephone follow-up program on patient compliance, urea kinetic modeling and incidence of infection in patients with continuous ambulatory peritoneal dialysis (CAPD). Method: A pretest-posttest design with a nonequivalent control group was used to examine the effects of a telephone follow-up program for 12 weeks in a group of 39 CAPD patients. The telephone follow-up group of 17 patients received telephone follow-up 10 min once a week for 1 month and 10 min twice a month for 2 months during 12 weeks compared to control group of 22 who patients received usual care. Results: The telephone follow-up group showed no significant changes in patient compliance and urea kinetic modeling, although patients with CAPD receiving a telephone follow-up showed reduced incidence of exit site infection during the intervention compared with the control group. Conclusion: Telephone follow-up program for CAPD patients may decrease the incidence of exit site infection. Future studies with a larger sample be done utilizing diet and exercise reinforcement program combined with telephone follow-up as a nursing intervention for patients with CAPD.

Perception of Importance and Performance in the Infection Control of Visiting Nursing Personnel in Public Health Centers (보건소 방문간호요원의 감염관리에 대한 중요성 인지도 및 실천도)

  • The work of visiting nursing personnel consists mostly of direct nursing and it is absolutely necessary that training of the nursing personnel for infection control be emphasized because most of patients are composed of elderly people with chronic diseases and malnutrition who are highly susceptible to infection. Accordingly. this study was designed to provide basic data for the development of an infection control program in public health centers as well as the promotion of perception and performance of infection control by analysing the perception of importance and performance in the infection control of visiting nursing personnel who carry out visiting nursing services. The subjects of this study consisted of a number of visiting nursing personnel working for 15 public health centers. 15 health-center branches. and 16 health clinics. The collection of data was grounded on a questionnaire method. The questionnaire breaks down to 8 questions about general characteristics and 108 questions in connection with the perception and performance of infection control. The results of this study are as follows: 1. 82 subjects (78.1%) responded that it is necessary to control infection. 79 subjects(75.3%) responded that education is necessary for infection control. 50 respondents (47.6%) answered in the affirmative in regard to the role of infection carriers. 2. The mean score for the perception of importance in infection control was 4.55 and that performance accounts for 4.03. this turns out to be very high. 3. In terms of the perception of importance per each field of infection control. medication. the management of contamination and sterilization, the control of environments and instruments. washing hands and individual hygiene as well as the prevention of cross-infection and the supervision of infection stand at 4.72. 4.64. 4.60. 4.58. 4.52 and 4.21 respectively. In terms of performance. the supervision of infection. the control of environments and instruments, individual hygiene and the prevention of cross infection. medication. washing hands, and the management of contamination and sterilization was 4.21. 4.12. 4.10. 4.03. 3.91 and 3.81 respectively. 4. In terms of the correlation between the perception of importance and the performance of infection control. the score stands high in all fields: this means the high perception of importance leads to high performance statistically (P< .05). 5. In terms of the perception of importance in general characteristics. there is no significant difference while the form of working places. working career and the career as visiting nursing personnel show a significant difference in terms of performance(P< .05). From the results. it is suggested that the visiting nursing personnel should be constantly educated with the aid of the development of infection control programs and that instructions and instruments for the management of infection should be worked out so as to enhance the performance of infection control on the part of nurses in applicable communities.

Research on Cognition of Infection Control by Dental Hygienics Student's in some Areas (일부 지역 치위생학과 학생들의 감염방지에 관한 인식 조사)

  • A dental treatment room is always exposed to diverse kinds of pathogenic bacteria, and may be a mediating place of cross-infection given being contaminated the interior of a room through several routes in the form of patient's secretion and aerosol. The main agent of preventing cross-infection is a dental hygienist in the dental treatment and the dental treatment room where are scattered about a risk of cross-infection. A dental hygienist needs to have right recognition on infection control before being active as a clinical expert. This infection-control recognition level is influenced from the clinical practice. Accordingly, to survey recognition of infection control, a self-administered questionnaire research was conducted targeting 314 students who are fixed the clinical practice as regular subject in the junior course out of curriculum for the Department of Dental Hygiene at some of 4-year universities. Data collection was performed from December 9, 2011 to February 22, 2012. Except 11 copies of questionnaire with insincere response among the collected materials, 303 copies were finally analyzed by using SPSS WIN 20.0. The following conclusions were obtained. In the infection disease section, both on and off campus showed 4.89 points from 'the importance of recognizing the infections prevention', 4.65 points from 'recognizing the compulsory preventative injection for hepatitis type B', 4.77 points from 'recognizing the necessity of the preventative injection for hepatitis type B', 4.71 points from 'whether practice the prevention in reality or not', and 4.76 points from 'the educational helps to the prevention'. In other words, the section recorded the highest and meaningful points. It is considered to be needed the development in systematic and diverse infection-control educational programs and the differentiated education depending on school year for dental hygiene students.
