간호대학생의 임상 시뮬레이션 학습동기가 학습성취도에 미치는 영향: 학습몰입의 매개효과 (The impact of clinical simulation learning motivation on nursing student learning achievement: The mediating effect of learning immersion)

  • Purpose: This study aimed to examine the mediating effect of learning immersion in clinical simulations on the relationship between nursing student learning motivation and achievement in clinical simulation. Methods: This study was conducted using a cross-sectional survey with 184 nursing students from two universities who participated in clinical simulation between September and December 2022. The participants completed a self-administered questionnaire, and the collected data were analyzed using independent an independent t-test, Mann-Whitney U-test, one-way ANOVA, Pearson's correlation coefficient, and multiple regression analysis to identify the mediating effects of learning immersion on the relationship between nursing student learning motivation and achievement. Results: Among the subvariables of nursing student learning motivation, task value and self-efficacy for learning and performance had a significant effect on learning immersion (respectively, β=.36, p=.001; β=.31, p<.001) and learning achievement (respectively, β=.48, p<.001; β=.38, p<.001). With the input of learning motivation variables, the direct effect of learning immersion on learning achievement was significant (β=.20, p=.003), and the effects of learning motivation and task value and self-efficacy on learning achievement was reduced after controlling for learning immersion, which is a mediating variable (respectively, β=.41, p<.001; β=.32, p<.001). The bootstrapping test to confirm the mediating effect of learning immersion was also significant (task value 95% confidence interval [95% CI], 0.02~0.20; self-efficacy 95% CI, 0.01~0.12). Conclusion: The results of this study suggest that simulation educators should consider learners' motivation and immersion when organizing and operating clinical simulations.

Factors influencing the intention to engage in cervical cancer preventive behavior in human papillomavirus-infected women: a cross-sectional survey

  • Purpose: This study investigated the influence of cervical cancer knowledge, human papillomavirus (HPV) knowledge, self-efficacy, and uncertainty on the intention to engage in cervical cancer preventive behavior in HPV-infected women. Methods: This descriptive correlational study was conducted among 129 adult women aged 20 to 65 years who received positive HPV results at a general hospital in Changwon, Korea. The dataset was analyzed using descriptive statistics, the independent t-test, analysis of variance, the Pearson correlation coefficient, and multiple regression. Results: The mean score for the intention to engage in cervical cancer preventive behavior was high (4.43±0.65). This intention was significantly different according to age at first sexual intercourse (F=7.38, p=.001), HPV type (F=4.79, p=.010), vaccination (t=3.19, p=.002), and condom use (t=3.03, p=.003). The intention to engage in cervical cancer preventive behavior showed significant, weak-to-moderate positive correlations with HPV knowledge (r=.22, p=.012) and self-efficacy (r=.42, p<.001). Self-efficacy (β=.46, p<.001), first sexual intercourse at <20 years (β=.45, p<.001), first sexual intercourse at 20-24 years (β=.29, p=. 018), HPV high- and low-risk group infection (β=.26, p=.019), HPV high-risk group infection (β=.26, p=.026), and vaccination (β=.21, p=.007) significantly influenced the intention to engage in cervical cancer preventive behavior. These variables explained 34.6% of variance in intention. Conclusion: Study findings support the need to develop a program that effectively conveys accurate information about cervical cancer prevention to HPV-infected women and helps them enhance self-efficacy to boost the intention to engage in cervical cancer preventive behavior.

Factors associated with clinical nurses' preconception health behavior in Korea: a cross-sectional survey

  • Purpose: Nurses have been reported to be at an increased risk for miscarriage and preterm labor. However, there is limited knowledge regarding nurses' preconception health behaviors. Therefore, this study aimed to identify factors influencing these behaviors. Methods: One hundred sixty nurses, who were planning their first pregnancy within the upcoming year, participated in an online survey from August 11 to October 31, 2021. Data on preconception health behavior, perceived health status, pregnancy anxiety, nursing practice environment, and social support were analyzed using the t-test, Pearson correlation coefficients, and multiple regression analysis. Results: Age (p=.024), educational level (p=.010), marital status (p=.003), work experience (p=.003), satisfaction with the work department (p<.001), smoking status (p=. 039), and previous health problems related to pregnancy outcomes (p=.004) were significantly associated with nurses' preconception health behaviors. Furthermore, perceived health status (p<.001), pregnancy anxiety (p=.011), nursing practice environment (p=.003), and social support (p<.001) showed significant correlations with preconception health behaviors. Social support (β=. 28, p=.001), satisfaction with the work department (β=.23, p=.032), marital status (β=.22, p=.002), and perceived health status (β=.23, p=.002) were confirmed as factors associated with preconception health behaviors. These factors explained 40.9% of the variance in preconception health behaviors (F=6.64, p<.001). Conclusion: Clinical nurses' preconception health behaviors were influenced by social support, perceived health status, satisfaction with the work department, and marital status. Interventions to improve clinical nurses' preconception health behaviors should target social support and perceived health status. A preconception health behavior education program considering clinical nurses' marital status and satisfaction with the workplace can also be implemented.

Assessment of Questionnaire of Physical Activity at Workplace Based on the Social Cognitive Theory (PAWPQ-SCT): A Psychometric Study in Iranian Gas Refinery Workers

  • Background: This study aimed to develop and assess the psychometric features of the Physical Activity at Workplace Questionnaire (PAWPQ) based on the Social Cognitive Theory (SCT) to evaluate employees' physical activity (PA) behaviors at the workplace. Methods: This psychometric cross-sectional study was conducted on 455 employees working in one of the gas refineries in Iran. The participants were selected using the proportional stratified sampling method in 2019. The data collection tools were a demographic information questionnaire, the short form of the International Physical Activity Questionnaire (IPAQ), and a questionnaire developed based on the SCT, whose psychometric features were confirmed in terms of validity and reliability. Data were analyzed using SPSS22 and AMOS20 software. Results: The first version of PAWPQ-SCT had 74 items. After evaluating content and face validity, nine items were removed. The results of the content validity index (0.98), content validity ratio (0.86), and impact score (3.62) were acceptable for the whole instrument. In exploratory factor analysis, after removing seven items-58-item final version of the scale-six factors could explain 73.54% of the total variance. The results of structural equation modeling showed the acceptable fit of the model into the data (RMSEA = 0.052, CFI = 0.917, NFI = 0.878, TLI = 0.905, IFI = 0.917, CMIN/DF = 2.818). Cronbach's alpha coefficient and Intraclass Correlation were 0.90 and 0.86, respectively. Conclusion: This study confirmed that the psychometric features of the 58-item final version of PAWPQSCT constructs were acceptable in a sample of Iranian employees. This questionnaire can be used as a valid and reliable tool to evaluate Iranian employees' PA behaviors and develop effective educational interventions for workers and managers.

Using the Health Belief Model to Predict Tuberculosis Preventive Behaviors Among Tuberculosis Patients' Household Contacts During the COVID-19 Pandemic in the Border Areas of Northern Thailand

  • Objectives: The coronavirus disease 2019 pandemic has exacerbated the rate of tuberculosis (TB) infection among close contacts of TB patients in remote regions. However, research on preventive behaviors, guided by the Health Belief Model (HBM), among household contacts of TB cases is scarce. This study aimed to employ the HBM as a framework to predict TB preventive behaviors among household contacts of TB patients in the border areas of Northern Thailand. Methods: A cross-sectional study with multi-stage random sampling was conducted in Chiang Rai Province. The study included 422 TB patients' household contacts aged 18 years or older who had available chest X-ray (CXR) results. A self-administered questionnaire was used to conduct the survey. Results: The participants' mean age was 42.93 years. Pearson correlation analysis showed that TB preventive behavior scores were significantly correlated with TB knowledge (r=0.397), perceived susceptibility (r=0.565), perceived severity (r=0.452), perceived benefits (r=0.581), self-efficacy (r=0.526), and cues to action (r=0.179). Binary logistic regression revealed that the modeled odds of having an abnormal CXR decreased by 30.0% for each 1-point score increase in preventive behavior (odds ratio, 0.70; 95% confidence interval, 0.61 to 0.79). Conclusions: HBM constructs were able to explain preventive behaviors among TB patients' household contacts. The HBM could be used in health promotion programs to improve TB preventive behaviors and avoid negative outcomes.

중환자실 입원환자 가족의 삶의 질 영향요인 (Factors Affecting Quality of Life in Family Caregivers of Patients in Intensive Care Units)

  • Purpose : This study aimed to identify factors influencing the quality of life of family caregivers of intensive care unit (ICU) patients. Methods : We conducted a study using a cross-sectional design. The study involved 109 family caregivers of ICU patients at a university-affiliated hospital in Gyeonggi-do, South Korea. Data were collected through self-report questionnaires between July 2020 and April 2021 and analyzed using descriptive statistics, independent t-tests, one-way ANOVA, Pearson's correlation coefficients, and multiple regression analysis. Results : The study revealed significant differences in quality of life based on economic status (F=11.63, p<.001), cohabitation with patients (t=-2.04, p=.044), sleep duration after patient's admission to the ICU (t=-2.48, p =.025), and subjective health status (F=30.06, p<.001). There were significant negative correlations observed between quality of life and post-traumatic stress symptoms (r=-.38, p<.001) as well as caregiver burden (r=-.46, p<.001). Factors affecting quality of life were subjective health status, economic status, and caregiver burden (adj. R2=0.52, F=15.64, p<.001). Conclusion : These findings underscore the need to develop and implement intervention programs tailored to the health conditions and economic status of family caregivers, with a focus on alleviating caregiver burden. Such initiatives are essential to ultimately improve the quality of life for family caregivers of ICU patients.

Association between Transfusion-Related Iron Overload and Liver Fibrosis in Survivors of Pediatric Leukemia: A Cross-Sectional Study

  • Purpose: Patients who receive frequent blood transfusions are at an elevated risk of developing hepatic fibrosis due to iron overload in the liver. In this study, we evaluated the effectiveness of transient elastography (TE) (FibroScan®) for assessing liver fibrosis in patients with pediatric cancer. Methods: We enrolled 106 consecutive cases of acute leukemia in individuals under 21 years of age. The participants were followed for 2 years. Based on their serum ferritin (SF) levels, the patients were divided into two groups: group 1 (SF≥300 ng/mL) and group 2 (SF<300 ng/mL). A liver FibroScan® was performed, and a p-value of less than 0.05 was considered statistically significant. Results: Among the various parameters in the liver function test (LFT), alkaline phosphatase was significantly higher in a subgroup of patients aged 5-8 years in group 2 compared to those in group 1. The indices of liver fibrosis determined by TE, including the FibroScan score, controlled attenuation parameter score, steatosis percentage, and meta-analysis of histological data in viral hepatitis score, as well as indirect serum markers of liver fibrosis such as the aminotransferase (AST)/alanine aminotransferase (ALT) ratio, Fibrosis 4 score, and AST to platelet ratio index, did not differ significantly between the two groups. The association between the TE results and LFT parameters was only significant for ALT. Conclusion: Transfusion-associated iron overload does not have a significant correlation with severe liver fibrosis. FibroScan® is not a sensitive tool for detecting early stages of fibrosis in survivors of pediatric leukemia.

  • 콘크리트 도로 상판 또는 터널 라이닝의 배면 공동은 구조물의 함몰 또는 붕괴로까지 이어질 가능성이 있다. 이 공동을 비파괴로 검출하기 위한 방법 중의 하나가 저주파수대 레이더(GPR)의 사용이다. 그러나 이와 같은 토목구조물에는 분해능이 좋지 않은 저주파수대의 레이더를 사용할 수밖에 없기 때문에 작은 공동의 크기나 두께까지 탐사하는 일은 현재 기술로는 거의 불가능하다. 본 연구는 이와 같은 문제와 한계를 극복하기 위해 다양한 공동의 크기와 두께, 깊이를 대상으로 많은 량의 실측 실험을 통해, 저주파 레이더 측정치와 공동 간의 상관관계를 분석함으로 해서 공동의 깊이, 크기와 두께까지를 추정하기 위한 새로운 해석방법을 제안하고 그 적용성을 검토하였다. 또한 본 검토에서는 공동만을 그 크기(횡폭)에 맞게 화상처리에 의해 나타낼 수 있는 역치를 산정하였다. 그 결과, 본 연구에서 제안된 방법에 의해 실험실 수준에서는 정도 좋게 공동의 깊이와 크기, 두께를 추정하는 것이 가능하였다.

Evaluating the Reliability of Short-Form Berg Balance Scales and Short-Form Postural Assessment Scales in Chronic Stroke Survivors

  • Objective: This study aims to assess the test-retest reproducibility of the Short Form Berg Balance Scale (SF-BBS) and the Short Form Postural Assessment Scale for Stroke (SF-PASS) among chronic stroke survivors, focusing on their reliability for consistent measurements over time. Design: A cross-sectional study design Methods: Thirty chronic stroke survivors participated in this study, undergoing evaluations with SF-BBS and SF-PASS scales at two different points, separated by a seven-day interval. The analysis focused on test-retest reliability, employing statistical measures such as the Intra-Class Coefficient (ICC2,1), Standard Error of Measurement (SEM), Minimal Detectable Change (MDC), and MDC%, the Bland-Altman plot to assess the limits of agreement and the extent of random measurement error. Results: The study found notable test-retest reproducibility for both SF-BBS and SF-PASS, with ICC values demonstrating strong reliability (0.932 to 0.941, with a confidence interval of 0.889 to 0.973). SEM values for SF-BBS and SF-PASS were reported as 1.34 and 0.61, respectively, indicating low measurement error. MDC values of 3.71 for SF-BBS and 1.69 for SF-PASS suggest that the scales have an acceptable level of sensitivity to change, with reliability metrics falling below 20% of the maximum possible score. Conclusions: The findings suggest that both SF-BBS and SF-PASS exhibit high intra-class correlation coefficients, indicating strong test-retest reliability. The SEM and MDC values further support the scales' reproducibility and reliability as tools for evaluating mobility and dynamic balance in chronic stroke survivors. Therefore, these scales are recommended for clinical use in this population, providing reliable measures for assessing progress in rehabilitation.

재활전문병원 간호사의 환자안전문화인식, 간호전문직관, 간호근무환경이 환자안전간호활동에 미치는 영향 (Effects of Rehabilitation Hospital Nurses' Perception of Patient Safety Culture, Nursing Professionalism, and Nursing Work Environment on Patient Safety Nursing Activities)

  • Purpose: This study aimed to investigate the effects of rehabilitation hospital nurses' perception of patient safety culture, nursing professionalism, and nursing work environment on patient safety nursing activities. Methods: A cross-sectional design was used with a convenience sample of 230 nurses with more than six months of experience working in rehabilitation hospitals located in D Metropolitan City, South Korea. Data were collected from October 23 to 31, 2023 through a self-administered questionnaire and analyzed by descriptive statistics, independent t-test, one-way ANOVA with post-verification of Scheffé test, Pearson's correlation coefficient, and multiple stepwise regression analysis using SPSS 28.0. Results: The average scores were 3.73±0.35 for perception of patient safety culture, 3.54±0.47 for nursing professionalism, 2.67±0.49 for nursing work environment, and 4.68±0.45 for patient safety nursing activities. Patient safety nursing activities was significantly positively correlated with perception of patient safety culture, nursing professionalism, and nursing work environment. Experience of reporting incidents (β=.19, p=.002), communication and procedure (β=.18, p=.003), frequency of reporting (β=.18, p=.002), total clinical experience (β=.17, p=.004), patient safety (β=.17, p=.005), and direct supervisor/manager (β=.17, p=.008) affected patient safety nursing activities in rehabilitation hospitals. Conclusion: Perception of patient safety culture may increase rehabilitation hospital nurses' patient safety nursing activities.
