Priority-setting in Expanding the Basic Benefit Package in Korean Health Insurance Scheme (건강보험 기본급여의 우선순위)

  • Universal health insurance normally requires a basic benefit package, whose design intersects with almost all other aspects of the health insurance debate. Despite its central importance, basic benefit package has not received the analysis it deserves in Korea. The issue of how to decide which health services should be delivered and to whom has been a matter for consistent policy debate. Many industrialized countries observed in this study have been dealing explicitly or implicitly with the basic benefit package. The methods vary from having a specific positive list of services (Bismarkian countries) to the use of guidelines (Beveridgian countries). The purpose of this paper is to form the underlying principles and process for determining what is included or left out by getting accurate and representative responses from health-related personnel. Mail survey is used. Economic burden for treatment, seriousness of disease and urgency of treatment are ranked at the first three priorities. Services that had been suspended because of financial crisis in health insurance scheme in 2001 were selected as items which should firstly be expanded into coverage. Diagnostic test against heart disease and vaccination were also selected as items which should additionally belong to the list of covered services.

  • Two perspectives on developing better decision capabilities for a warranty system can be identified: one involving the inclusion of a 'learning' module and the other the inclusion of a 'prioritization' capability. This paper demonstrates how a warning process can be included in a warranty system by coupling with a neural network's learning capabilities. In addition to the neural network, a method is employed for assigning priorities to warning criteria by using the analytic hierarchy process (AHP). Thus, it is possible to construct an integrated system with three components: the warranty system, the AHP module, and the neural network system. A case study is provided to enhance the accuracy of warning/detection judgment in a warranty system for automobile companies, having many factors related to the warranty system.

  • This paper focuses on method of setting priorities for telecommunications Standardization in which usually includes prioritization, resource allocation, standardization schedule for each SWA is based on prioritization of SWAs. We mainly used the Analytic Hierarchy Process(AHP) and a modified Delphi technique as a method for planning process. Recently, rapid technology evolution in telecommunications fields increases the necessity of developing more and more new standards. Also increased complexity of technologies makes standards more complicated, more specified and more thick than the past ones. As a result, the number of SWAa(Standardization Work Areas) and SWIs(Standardization Work Iterms) is being increased continuously. Furthermore, rapid telecommunication environment changes such as deregulation, liberalization, privatization, regionalization, and globalization surrounding telecommunications standardization grows increasingly important. To cope with these challenges and to make the more efficient use of the limited standardization resources, i.e., time, financial, personal resources, we will suggest logical and rational approach that is a backbone of strategic plan for telecommunication standardization. The telecommunication standardization planning process involves a MCDM(Multi-Criteria Decision Making) process. The Analytic Hierarchy Process(AHP) is a kind of MCDM approach. Our method is based on a combination of a modified Delphi technique and an application of AHP(spreadsheet model). Analyzed data from Delphi technique is used as inputs to AHP. In addition we also focus on the technique how to combine group judgments and to handle a large number of comparisons.

Optimal monitoring instruments selection using innovative decision support system framework

  • Structural monitoring is the most important part of the construction and operation of the embankment dams. Appropriate instruments selection for dams is vital, as inappropriate selection causes irreparable loss in critical condition. Due to the lack of a systematic approach to determine adequate instruments, a framework based on three comparable Multi-Attribute Decision Making (MADM) methods, which are VIKOR, technique of order preference by similarity to ideal solution (TOPSIS) and Preference ranking organization method for enrichment evaluation (PROMETHEE), has been developed. MADM techniques have been widely used for optimizing priorities and determination of the most suitable alternatives. However, the results of the different methods of MADM have indicated inconsistency in ranking alternatives due to closeness of judgements from decision makers. In this study, 9 criteria and 42 geotechnical instruments have been applied. A new method has been developed to determine the decision makers' importance weights and an aggregation method has been introduced to optimally select the most suitable instruments. Consequently, the outcomes of the aggregation ranking correlate about 94% with TOPSIS and VIKOR, and 83% with PROMETHEE methods' results providing remarkably appropriate prioritisation of instruments for embankment dams.

  • Cloud-computing is in the limelight with expectation for cost reduction, because it alleviates the burden of initial investment and maintenance cost and based on pay-as-you-use billing policy. However, many suppliers of Cloud-computing service are suggesting diverse and complicated billing policies without consideration for setting reasonable service billing policy and definite criteria of properties to determine service billing system. So companies willing to use Cloud-computing service are hard to understand the billing system and often spend more expensive cost than necessary. Therefore, this study invested billing system properties of four representative suppliers. Based on these properties of billing system, this study found priorities using AHP survey which conducted to experts who are able to make decisions for adopting Cloud-computing in the company using or willing to use Cloud-computing service. We expect that this study can suggest basic guideline for comparing and analyzing properties of Cloud-computing service with standardized and objective method.

  • The environment of Information and Telecommunication industry has rapidly changed. This is leading the growth of the world economy. Because of the characteristic of the information and communication, the standardization plays a important role in this market. The company that owns the standardization in its industry has dominant power like Microsoft in personal computer software, and Qualcomm in digital cellular phone system. Therefore, to take advantage of this standardization power, it is important to find which technology will be set up as a new standard. This paper presents how to capitalize the standardization as a market strategy. To perform this strategy, we take two steps. The first step is to strategically choose the most important technology or service which would lead the information and telecommunication market. We used modified AHP(Analytic Hierarchy Process) to set the priorities among the various standard related technologies and services, We interviewed with the information and telecommunication experts to find out some criteria and the weights of them. The second step is to select the standard to realize the technology. Actually, this is very hard to decide which technology we should select, because there are lots of alternative standards to develop the technology. In this step, we use a strategic framework for choosing the standard. This framework provides how the research institution and company do to get a maximum benefit from the standardization strategy, Finally, we applied these to Korea's situation simply in order to show how to take advantage of the framework.

Process Design of Isothermal Forging for Three-Dimensional Ti-6Al-4V Wing-Shape (Ti-6Al-4V 합금 3D 날개형상의 항온단조 공정설계)

  • The isothermal forging design of a Ti-6Al-4V wing shape was performed by 3D FE simulation. The design focuses on near-net shape forming by the single stage. The process variables such as the die design, pre-form shape and size, ram speed and forging temperature were investigated. The main design priorities were to minimize forging loads and to distribute strain uniformly in a given forging condition. The FE simulation results for the final process design were compared with the isothermal forging tests. The instability of deformation was evaluated using a processing map based on the dynamic materials model(DMM), including flow stability criteria. Finally, a modified process design for producing a uniform Ti-6Al-4V wing product without forming defects was suggested.

  • To visualize a strategic plan including resource allocations and standardization schedules, it is important to set priorities among information and communication technologies(ICTs) standardization work areas(SWAs). This ranking process involves decision making process with multiple criteria which results a complex selection problem. In this study, analytic hierarchy process(AHP) is employed and applied to solve this decision making problem. The processes and results are illustrated for an effective strategic plan in standardization decision processes.

  • This study was carried out to show how the Analytic Hierarchy Process technique could be used in setting the priority among selected diseases to increase the range of health insurance benefit. Thirty experts, including doctors (group1), experts for preventive medicine or public health(group2), and representatives of the insured(group 3), participated in the study panel that is conducted for priority setting. They were asked to evaluate the priorities among 6 selected criteria and then 42 selected diseases. The results were as follows; First, representatives of the insured think that the magnitude of out-of-pocket payment should have high priority while doctors think that effectiveness of treatment should have high priority. Second, all experts think that catastrophic diseases such as malignant neoplasm, major heart disease, and cerebral vascular disease should have high priority in health insurance coverage even though there is little difference among groups. These results can be useful to establish a systematic strategy for expanding health insurance benefit package.

  • The article assesses the functioning of the DE system or a separate educational institution, where it can be carried out on the basis of developed criteria or on a regulatory basis. The assessment was carried out on the basis of a qualitative and quantitative nature by comparing the actual state of affairs with a certain "ideal" (educational standard), which must be defined and used as a kind of benchmark against which the assessment is made. Conducted an assessment based on a regulatory framework that represents an alternative approach. It has been emphasized that the exceptional difficulty in determining the ideal indicators (norms) of the activities of universities, it has been found that the normative approach, in which the activities of traditional and open universities are compared, taking into account the differences in social, cultural and economic conditions, is the most acceptable.
