Cystadenocarcinoma of the salivary gland is a rare malignant tumor. It was first defined as papillary cystadenocarcinoma in the 1991 World Health Organization (WHO) classification, and it was reclassified as cystadenocarcinoma in the 2005 WHO classification. It is a low-grade neoplasm that features slow growing and predominantly cystic growth. We report a case of cystadenocarcinoma occurring on the parotid gland of a 61-year-old female patient presenting palpable mass on her left cheek. Preoperative examination may not reveal typical malignant characteristics. Such as in our case, the differential diagnosis between cystadenocarcinoma and benign lesion is difficult occasionally. We discuss the clinical and histopathological features of cystadenocarcinoma with the review of the literature.
This investigation was designed to categorize Angle's class II malocclusion groups through analyzing horizontal and vertical components of craniofacial skeleton in Angle's class II malocclusion. The material selected for this study consisted in standard lateral cephalogram of two hundred and twenteen children, eighty eight boys and one hundred twenty four girls, aged 6 through 18 years, having Angle's class II malocclusion. On the basis of findings of this study, the following results were obtained. 1. In horizontal skeletal classifications, 16 groups were classified according to FMN-A-B, SE-FMN-A, Ba-SE-Me, Ba-Se/Ra P. The sequences that have relatively high frequency are as follows: a) Horizontal Group 16 b) Horizontal Group 12 c) Horizontal Group 13 d) Horizontal Group 9 & 15 2. In vertical skeletal classification, 8 groups were classified according to the PMV/PP, PMV/Occ. P. PMV/Mn. P. The sequences that relatively high frequency are as follows; a) Vertical Group A b) Vertical Group D c) Vertical Group C d) Vertical Group H 3. In vertical and horizontal skeletal classifications, the sequence that relatively high frequency are as follows; a) Group13-A b) Group16-A & 9-A c) Group12-A & 15-A d) Group16-C
Background: Dermoid cysts of the auricular area are extremely rare. We report on six cases of auricular dermoid and epidermoid cyst, and differentiate dermoid cyst from epidermal cyst along with a review of the literature. Methods: Three cases involved a gradually enlarging mass of the superior and anterior aspect of the helix of their ear. Another two cases were located in the posterior aspect of the ear. Results: During the operation, a tumor was found just under the skin, not fixed mastoid or adjacent cartilage. Histologically, all specimens contained desquamated squamous epithelium and keratin in the lumen. However, two cases of posterior masses showed the presence of adnexal structures and three cases did not. Conclusion: A key in diagnosis of the dermoid cyst is the presence of adnexal structures. If the wall does not bear adnexal structures, the term epidermoid or keratin cyst is applied. Acquired cysts are most commonly of traumatic origin and result from an implantation or downward displacement of an epidermal fragment. Finally, the congenital epidermoid cyst grew at the upper part of the auricle; however, the dermoid cyst grew at the lower and posterior part of the auricle.

Here we report a case of a focal atypical proliferative nodule (PN) arising from a congenital melanocytic nevus (CMN). Diagnosis was challenging because it had both benign and malignant clinical features. Unusual histopathology, immunohistochemistry, and intraoperative findings of this atypical PN are discussed. A 5-year-old girl was admitted for a congenital $5{\times}5cm$ sized scalp mass. This hemangioma-like soft mass showed biphasic characteristics such as a slow, gradual, and benign increase in size but worrisome dural invasion with cranial bone defect. We removed the scalp mass with clear resection margins. Interoperatively, we found that the cranial bone defect had already filled. Histopathologic examination showed CMN with focal atypical PN. The nodule showed sharp demarcation and cellular pleomorphism. However, in immunohistochemical study, Ki-67 proliferation index and expression levels of protein S-100 and Melan-A were very low. These were unusual findings of atypical PNs. Despite her worrisome preoperative radiologic features, she showed an indolent clinical course compatible with previously reported biologic behavior. The patient underwent follow-up inspection with magnetic resonance imaging every 6 months for up to 3 years. The nodule appeared to be stationary at the last visit.
Fibrofolliculoma is a benign tumor characterized by a smooth, dome-shaped papule of size 2-4 mm. Most fibrofolliculomas occur as multiple lesions, and very rarely, they are solitary. Herein, we report a case of solitary fibrofolliculoma found in the alar rim, without the typical characteristics of a fibrofolliculoma. A 42-year-old man visited the hospital with a protruding lesion that had occurred 1 year previously. A mass of size 5×7 mm was observed on the left alar rim. The tumor was dome-shaped and palpable. The patient did not have any similar lesions elsewhere. No family member was known to have such a lesion. An incisional biopsy was performed before surgery, and pathological examination revealed hyperkeratosis and dyskeratosis; however, an accurate diagnosis was not made. Complete resection was planned for the mass on the alar rim. The resected mass was subjected to permanent biopsy, and the pathological examination results led to the diagnosis of fibrofolliculoma. Therefore, when diagnosing a dome-shaped mass in the alar rim, despite the suspicion of a very rare disease, it is necessary to suspect fibrofolliculoma and consider the process from diagnostic examination to treatment.
Background: This study was designed to evaluate the clinical safety of Bee Venom (BV) pharmacopuncture at craniofacial acupuncture points. Methods: This was a retrospective study of 108 patients diagnosed with peripheral facial paralysis, trigeminal neuralgia, or facial spasm who were admitted to Kyung Hee University Korean Medicine Hospital at Gangdong, from April 1st, 2017 to August 30th, 2017. Patients were allocated into either, Group 1 (the non-allergy group of patients who did not have an allergic reaction to BV) or Group 2, the group who had allergic reactions to BV. To evaluate the clinical safety of BV pharmacopuncture after each treatment, several criteria were used to measure any side effects: outcome, Common Terminology Criteria for Adverse Events scale, Mueller HL scale, treatment decision after adverse reaction, causality, measures performed for patients with adverse reactions, and efficacy assessment. Results: BV pharmacopuncture delivered in 0.1-0.2 mL at a concentration of 1:30,000 at the craniofacial acupuncture points, showed no statistically significant differences in baseline characteristics between non-allergy Group 1 and allergy Group 2. Amongst the 108 patients, 11 reported side effects after BV pharmacopuncture treatment. These adverse events included rash (n = 7), pruritus (n = 5), swelling (n = 1), vesicles (n = 1), erythema (n = 1), and hives (n = 1). All side effects resolved without sequelae. Conclusion: In this study, BV pharmacopuncture delivered at low doses at the craniofacial acupuncture points, resulted in 10% of patients experiencing non serious side effects suggesting that BV pharmacopuncture was clinically well tolerated.
Skeletal class III had been classified by the position of the maxilla, the mandible, the maxillary alveolus, the mandibular alveolus and vertical development. This morphologic approach is simple and useful for clinical use, but it is insufficient to permit understanding of the pathophysiology of dysmorphoses. The author hypothesizes that there are different patterns of mutual relation of the skeletal components which have contributed pathologic equilibrium of skeletal class III. The purpose of this study are threefold: 1) to classify skeletal class III in subgroups, which can show the architectural characteristics of the deformity, 2) to analyse the craniofacial architecture of each subgroup on etio-pathogenic basis, and 3) to characterize and visualize the pattern as a prototype. Materials used in this study were lateral cephalograms of 106 skeletal class III adults, which were analysed with modified Delaire's architectural and structural analysis. Linear and angular measurements of the individual subject were obtained and cluster analysis was used for the subgrouping. Data were evaluated for verification of the statistical significances. The following results were obtained. 1. By the modified Delaire's architectural and structural analysis and cluster analysis, skeletal class III adults were classified into 7 clusters and presented as prototypes, which could show the pathophysiology of the skeletal architecture 2. There was significant relationship in measurement variables of each cluster, which could reflect characteristics of the skeletal pattern of growth. 3. The flexure of cranial base had a close relationship to the anterior rotational growth of the maxilla and contributes to understand the etio-pathology of skeletal class III. 4. The proportion of craniospinal area in cranial depth, craniocervical angle and vertical position of point Om had a close relationship to rotational growth of the mandible and direction of condylar growth. They contribute to understand the etio-pathology of skeletal class III. In summary, the cranium and the craniocervical area must be considered in diagnosis and treatment planning of dentofacial deformity. And the occlusal plane can be considered as a representative which shows the mutual relationships of the skeletal components.

Background: There have been many reports of nasal bone fractures in adults, but there are few clinical reports of them in adolescents, although adolescence is the main growth period of the nasal bone. In addition, previous studies have tended to classify and describe child and adolescent nasal fractures in the same category. The aim of this study was to identify the clinical aspects of nasal fractures in adolescents, and to evaluate the characteristics of nasal fractures in the growth period. Methods: Our institution's database was reviewed to extract data on adolescent patients between the age of 13 to 18 who had isolated nasal bone fractures from March 2012 to February 2020. The study excluded patients with other facial fractures, previous facial fractures, or congenital deformities. Results: This study included 243 boys and 26 girls. Interpersonal violence and sports-related accidents accounted for 85 and 79 cases, respectively. There were 128 frontal impact injuries and 132 lateral impact injuries, and 97 patients had accompanying septal fractures. An accompanying septal fracture was present in 36.06% of all patients, but in 51.20% of those who underwent surgery. Conclusion: The prevalence of adolescent nasal fractures was significantly higher in boys, to a greater extent than observed for pediatric or adult fractures. The main causes of fractures were interpersonal violence and sports-related accidents. An explanation for these results is that adolescent boys are more prone to have violent accidents or companionship with active sports than other age or sex groups, leading to substantially more fractures.
정상치열을 가진 성인 84명 (남자 44명, 여자 40명)과 밀집 치열을 가진 성인 61명 (남자 33명, 여자 28명)을 대상으로 측모두부방사선 규격사진상에서 정상치열인과 밀집치 열인간의 두개 안면골의 형태학적 차이를 분석하여 다음과 같은 결론을 얻었다. 정상군과 밀집군에서 남녀별 각 계측항목의 평균치, 표준편차 및 Facial polygon들을 얻었다. 정상군에 비해 밀집군에서 두개기저골에 대해 구개평면, 교합평면 및 하악하연이 하방으로 경사되었고 구개평면에 대해 하악하연도 하방으로 경사되었으며 하악우각도 크게 나타났다. 정상군에 비해 밀집군에서 상하악골이 두개기저골에 대해 후방위치하였다. 정상군에 비해 밀집군에서 상악전치는 순측으로 경사되 었으며 하악전치는 설측으로 경사되었다. 정상군의 남자에 비해 밀집군의 남자에서 후안면고경과 하악지길이가 더 짧으며 정상군의 여자에 비해 밀집군의 여자에서 후안면 고경, 하악골체장 및 후두개저 장경이 더 짧으며 전안면고경은 더 길게 나타났다.
조선대학교 치과대학 학생 중 안모형태와 교합상태가 양호하고, 교정치료의 경험이 없는 정상교합자 74명 (남자 38명, 여자 36명)을 대조군으로 하였고 조선대학교 부속 치과병원 교정과에 내원하여 악교정 수술을 통한 교정치료가 필요하다가 진단된 골격성 III급 부정교합자 성인 45명 (남자 23명, 여자 22명)을 실험군으로 하여 두부규격방사선 사진을 촬영하고 Burstone & Legan방법에 의해 각각의 계측항목별로 남여를 비교 분석하여 악교정 수술이 필요한 골격성 III급 부정교합환자의 악안면 골격 특성에 대해 다음과 같은 결론을 얻었다. 1. 골격성 III급 부정교합 환자는 함몰된 안모를 지니고 있었고 (P<0.001), 여자에서 뚜렷한 상악골 열성장 (P<0.nt)과 하악골 과성장 (P<0.01)을 보인 반면, 남자에서는 통계학적인 유의성이 존재하지 않았고 상,하악골 연조직의 전,후방적인 비교에서는 특징적으로 남자는 하악골 연조직의 과다한 전방돌출 (P<0.001), 여자는 상악골 연조직의 과다한 후퇴 (P<0.001)을 보였는데 이는 골격성 III급 부정교합이 남자는 하악골 과성장, 여자는 상악골 열성장에 의해 주로 기인됨을 알 수 있다. 2. 골격성 III급 부정 교합을 지닌 남여 모두에서 특징적으로 MP-HP 각과 genial angle이 커서 하악골의 전하방 과성장을 보인 반면, ramus height는 남여 모두에서 아무런 영향도 미치지 않았으며, 상악중절치는 순측경사, 하악전치는 설측경사 되어 있었는데 이는 악교정 수술을 위한 술전교정 치료시 이러한 치성보상을 정상으로 회복시켜 각 골격구조내에서 치아들이 적절하게 위치되도록 하는 것이 중요하다고 것을 의미한다. 3. 여자에서 상안면고경에 비해 하안면고경이 특징적으로 짧았고, 남자의 lower facial throat angle이 특징적으로 작았다. 이는 골격성 III급 부정교합이 남자에서는 하악골의 과다한 하방 성장및 긴 목, 여자에서는 하악골의 전방으로의 과다성장및 짧은 목에 기인하는 것으로 보인다.
