Experimental Study of Hwangginaetak-san on Anti-inflammatory Effect (黃기內托散의 소염 작용에 관한 실험적 연구)

  • Objective: Chronic otitis media is an inflammation and infection of the middle ear which is persistent. Chronic otitis media occurs when the eustachian tube becomes blocked repeatedly, multiple infections, ear trauma, or swelling of the adenoids. A chronic ear may be the result of an acute ear infection that does not clear completely, or the result of recurrent ear infections. A chronic ear infection may be more destructive than an acute ear infection because its effects are prolonged, and it may cause permanent damage to the ear. Methods : Experimental animals made use of 4-5 weeks age(weight 20-25g) ICR(male)mouse. In the breeding farm, the lighting time was controlled from 7:00 am until 7:00 pm, the temperature was controlled within $22{\pm}0.5{\circ}$ and water and food were not limited. The extracts which were extracted from Hwangginaetak-san devided low dose group( 1.0g/kg-HN) and high dose group(3.0g/kg-HN), they were intragastrically administered to the mouse of sample A and sample B prior to LPS I.P injection. Compared with inflammation induced group which were induced by LPS, we measured the WBC count, IL-6 level in plasma and TNF-${\alpha}$ level in plasma. Results: 1. Hwangginaetak-san decreased WBC count in inflammatory reaction induced by LPS 2. Hwangginaetak-san decreased IL-6 level in inflammatory reaction induced by LPS. 3. Hwangginaetak-san didn't decreased TNF-${\alpha}$ level in inflammatory reaction induced by LPS Conclusion: According to above results, Hwangginaetak-san was improved its suppression effect to the inflammatory reaction through WBC count and IL-6 level. So Hwangginaetak-san is considered to be used for treatment of chronic otitis media by controlling the WBC count and IL-6 level in plasma.

Subtypes of White Blood Cells in Patients with Prostate Cancer or Benign Prostatic Hyperplasia and Healthy Individuals

  • Background: This study aimed to evaluate the baseline white blood cell (WBC), neutrophil, lymphocyte, monocyte, basophil, eosinophil count, total prostate-specific antigen (TPSA), free PSA (FPSA) level, neutrophilto- lymphocyte and neutrophil-to-monocyte ratios among patients with prostate cancer and benign prostatic hyperplasia (BPH), as well as healthy individuals. Materials and Methods: 2005-2012 laboratory files of 160 patients with prostate cancer at Kayseri Training and Research Hospital, Oncology Outpatient Clinic, 285 patients who were pathologically diagnosed with BPH in Urology Outpatient Clinic and 200 healthy individuals who were admitted to Internal Medicine Outpatient Clinic were retrospectively analyzed. Baseline WBC, neutrophil, lymphocyte, monocyte, basophil, eosinophil count, TPSA, FPSA level, neutrophil-to-lymphocyte ratio and neutrophil-to-monocyte ratio were recorded and compared across groups. Results: Patients with prostate cancer had a lower lymphocyte level compared to the patients with BPH and healthy controls (p<0.001). The mean monocyte count, leukocyte-to-monocyte ratio, and leukocyte-to-lymphocyte ratio were higher in patients with prostate cancer, but without significance. The mean WBC and leukocyte count were lower in patients with prostate cancer, but again without statistical significance (p=0.130). The mean TPSA and FPSA were 39.4 and 5.67, respectively in patients with prostate cancer, while they were 5.78 and 1.28 in patients with BPH. There was a significant difference in the mean TPSA and FPSA levels between the patient groups (p<0.001). Conclusions: Our study results showed that patients with prostate cancer had a lower level of lymphocytes, neutrophils and WBCs and a higher level of monocytes with a significant difference in lymphocyte count, compared to healthy controls. We suggest that lymphocyte count may be used in combination with other parameters in the diagnosis of prostate cancer, thanks to its ease of assessment.

Methodology and Case Analysis of the Erroneously High Counts in Automated Hematology Analyzer(XE-2100) using WBC (자동 혈구분석기(XE-2100)를 이용한 WBC 과다측정의 확인방법 및 사례 분석)

  • During an 8 month period from January to August 2007, complete blood count (CBC) samples were taken from various wards and from the outpatient department at Samsung Medical Center. In order to determine whether or not the total white blood cells were over counted, results were obtained from both the 4Diff/channel and the white blood cell channel from the automated blood analysis equipment from S company for comparison. Among patients who were on long term treatments, the number of cases determined to be over counted by comparing the WBC counts during this 8 month period was 25. Clinical chemistry tests were also conducted on the same day on the 25 samples taken. 68% of the patients showed to exceed normal range of aspartate transaminase (AST) and alanine aminotransferase (ALT) indicating abnormal liver function, and the total bilirubin range were also in excess in 60% of the total samples taken. Further clinical information which was obtained from the patients showed 98% of the patients were administered with cefepime which is the 4th generation cephalosporin at the time when the WEC were over counted. It is assumed that a multiple of factors investigated caused the over count of the WEC rather than a single factor.

Statistical Analysis of 1,000 Cases of Kawasaki Disease Patients Diagnosed at a Single Institute (단일 기관에서 진단받은 가와사끼병 환아 1,000례의 통계학적 분석)

  • Purpose : To find the risk factors associated with coronory artery lesions, non-responsiveness to intravenous immunoglobulin(IVIG) treatment, and recurrences in Kawasaki disease patients. Methods : We retrospectively analyzed 1,000 Kawasaki disease patients who were admitted to Yonsei University Medical Center from September 1990 to December 2003. We compared between responder and non-responder groups to IVIG treatment as well as between relapsed and non-relapsed groups, and as to the relapsed group, we also compared variables between patients in their first and second attack states. Finally, factors associated with longer-fever duration from disease onset were evaluated. Results : Longer fever durations before and after IVIG treatment, male sex, lower Hgb and Hct level, higher WBC count and segmented WBC proportion, and higher CRP and Harada's score were related with coronary artery lesions. Non-responsiveness was related to higher WBC count, segmented WBC proportion, CRP, SGPT, Harada's score, and pyuria. Moderate-to-severe coronary artery dilatations and recurrences were more commonly seen among the non-responder group. No significant predictive factors for recurrence were found. In the relapsed group, lower WBC count, CRP, and shorter fever duration from disease onset were observed in their second attack state. Fever duration from disease onset showed positive correlation with WBC count, CRP, and Harada's score and negative correlation with Hgb levels. Conclusion : Higher WBC count, CRP, and higher Harada's score were related to both higher incidences of coronary artery lesions and non-responsiveness to IVIG treatment, and these factors were also related with longer fever duration. Non-responders to IVIG treatment showed higher recurrence rate and more moderate-to-severe coronary artery dilatations than responders.

Study of Effects of Harpagophyti Radix on Edema and Blood of Rats with Arthritis (관절염 백선의 부종과 혈액에 미치는 천수근의 효능연구)

  • Objectives: In order to study the effects of Harpagophyti Radix in rats with arthritis induced by Freund's complete adjuvant. Methods : In this study, rats with arthritis induced by Freund's complete adjuvant were divided into four groups; Normal, Control, Sample A(Harpagophyti Radix 6mg/200g) and Sample B(Harpagophyti Radix 12mg/200g). After 1 week, separated searches were made on the change of edema rate, the count of WBC, the quantity of total protein, total bilirubin, GOT and GPT in the serum. Results: In the right plantar edema, both of Sample groups showed a decrease in comparison with Control group. In the count of WBC, the quantity of serum total protein and SGOT, Sample B showed a decrease with statistical significance in comparison with Control group. In the quantity of serum total bilirubin and SGPT, both of Sample groups did not show any harmful change in in comparison with Control group. Conclusion : Reviewing these experimental results, it appeared that Harpagophyti Radix showed the therapeutic effect of anti-inflammatation on adjuvant arthritis.

The Case-Control Study on Complete Blood Count as a Risk Factor of Stroke (뇌졸중 위험지표로서의 일반혈액검사 소견에 대한 환자;대조군 연구)

  • Objective : Stroke is one of the most common causes of death in Korea. This study was done to evaluate the association of complete blood count (CBC) with the risk of hemorrhagic stroke and ischemic stroke. Methods : In 217-case patients with ischemic stroke or hemorrhagic stroke and 146 healthy control subjects without stroke, hypertension, diabetes mellitus, hyperlipidemia, or ischemic heart disease and 160 controls without ischemic stroke or hemorrhagic stroke, we tested and compared white blood cell count (WBC), red blood cell count (RBC), hemoglobin (Hgb), hematocrit (Hct) and platelet. These data were statically analyzed by general linear models and binary logistic regression analysis to get each adjusted odds ratio. Results :The level of WBC was significantly higher in all cases. The level of RBC, Hct and Hgb was significantly lower in patients of ischemic stroke. The level of platelet was significantly higher in patients of ischemic stroke. Conclusion : These results suggest high WBC may be a risk factor of hemorrhagic stroke and ischemic stroke and low RBC, low Hct, low Hgb and high platelet may be risk factors of ischemic stroke in Koreans.

Predictive Factors for Severe Thrombocytopenia and Classification of Causes of Thrombocytopenia in Premature Infants

  • Purpose: This study investigated predictive factors for severe neonatal thrombocytopenia, which greatly increases the need for intensive care and is associated with a high mortality rate in premature infants. Factors adopted for prompt identification of at-risk newborns include blood test results and birth history. This study analyzed the relationship between the presence of severe neonatal thrombocytopenia and the mortality rate. The causes of thrombocytopenia in premature infants were also examined. Methods: This retrospective study evaluated 625 premature infants admitted to the neonatal intensive care unit (NICU) at Chung-Ang University Medical Center. The neonates were classified into 3 groups according to the severity of thrombocytopenia: mild ($100{\times}10^9/L{\leq}platelet<150{\times}10^9/L$), moderate ($50{\times}10^9/L{\leq}platelet<100{\times}10^9/L$), or severe (platelet<$50{\times}10^9/L$). Analysis of blood samples obtained at the onset of thrombocytopenia included platelet count, white blood cell (WBC) count, hemoglobin level, hematocrit level, absolute neutrophil count, and high-sensitivity C-reactive protein level. Results: Of the 625 premature infants admitted to our NICU, 214 were detected with thrombocytopenia. The mortality rate in thrombocytopenic neonates was 18.2% (39/214), whereas a mortality rate of only 1.0% was observed in non-thrombocytopenic neonates. The major causes of thrombocytopenia were perinatal insufficiency and sepsis in premature infants. Severe thrombocytopenia was noted more frequently in premature infants with higher WBC counts and in those with a younger gestational age. Conclusion: Platelet count, WBC count, and gestational age are reliable predictors for severe neonatal thrombocytopenia. The major causes of thrombocytopenia were perinatal insufficiency and sepsis in premature infants.

  • DW2282,(S)-(+)-4-phenyl-1-[1-(4-aminobenzoyl)-indoline-5-sulfonyl]-4,5-dihydro-2-imidazolone hydrochloride, is a new anticancer agent which is thought to exhibit a characteristic mechanism of action in the inhibition of tumor growth. In this study, we estimated the toxicities of DW2282 in mice. When mice were orally dosed for five consecutive days at the dosages of 50, 100 and 150 mg/kg, DW2282 did not induced methemoglobinemia and hypoglycemia at any of these doses. However, increased ALT and AST values were observed in the 150 mg/kg dosing group, and white blood cells (WBC) were significantly decreased at all doses. However, the changes in WBC count, ALT and AST immediately reversed after the cessation of drug administration. In addition, we found that DW2282 did not cause an increase in hemolysis in human blood. Taken together, these data suggested that DW2282 may have a relatively low level of toxicity, and that there may be a quick recovery from any toxicity it dose produce.

A Comparison of Complete Blood Cell Count in Canine Blood Samples Obtained from the Jugular Vein, Cephalic Vein and Lateral Saphenous Vein

  • The purpose of this study was to compare the results of complete blood cell count (CBC) of blood samples collected from the jugular vein, cephalic vein and lateral saphenous vein and to find out if there were clinically significant differences. Total of 40 dogs were tested. CBC tests were conducted with blood samples obtained from the jugular vein, cephalic vein and lateral saphenous vein and manual differential count was performed to accurately distinguish the white blood cell (WBC) types. The results were analyzed using Repeated Measures ANOVA and posthoc test was conducted using the least significant difference method. As a result, there was a statistically significant difference (P < 0.05) in the total WBC and monocyte count. The post-hoc test of total WBC counts revealed a significant difference between the jugular vein and cephalic vein, and the jugular vein and lateral saphenous vein. For monocyte counts, a significant difference was observed between the jugular vein and lateral saphenous vein.

The effect of regular exercise on immune response and hormone (규칙적인 운동이 면역반응 및 호르몬에 미치는 영향)

  • This study has measured the pulmonary function by treadmill test for 6 young women who were in twenties, and analyzed the respiratory-circulatory function and the change of hormone and immune response after performing the exercise program (60% severity) for 10 weeks. The results are as follows; 1. 10 weeks regular exercise made a decrease in weight and body fat proportion, and improved the respiratory-circular function by increasing the maximum oxygen absorption and ventilation. 2. 10 weeks regular exercise made a significant increase in count of WBC, lymphocyte, and T lymphocyte, but a significant decrease in B lymphocyte. NK cell also showed an increase in counts, but insignificant. 3. 10 weeks regular exercise made a significant increase in blood norepinephrine level. Epinephrine and cortisol also showed an increase in count, but insignificant. In summary, it suggested that 10 weeks regular exercise improves the immune function by decrease in body fat, increase in respiratory-circular function and metabolic efficiency, and also by raising Th/Ts ratio (an increase in count of WBC, lymphocyte, and T lymphocyte, but a decrease in suppressor T lymphocyte).
