Free Flap Reconstruction in Patients with Traumatic Injury of the Forefoot

  • Many techniques have been developed for reconstruction of the hand; however, less attention has been paid to foot reconstruction techniques. In particular, reconstruction of the forefoot and big toe has been considered a minor procedure despite the importance of these body parts for standing and walking. Most of the weight load on the foot is concentrated on the forefoot and big toe, whereas the other toes have a minor role in weight bearing. Moreover, the forefoot and big toe are important for maintaining balance and supporting the body when changing directions. Recently, attention has been focused on the aesthetic appearance and functional aspects of the body, which are important considerations in the field of reconstructive surgery. In patients for whom flap reconstruction in the forefoot and big toe is planned, clinicians should pay close attention to flap survival as well as functional and cosmetic outcomes of surgery. In particular, it is important to assess the ability of the flap to withstand functional weight bearing and maintain sufficient durability under shearing force. Recovery of protective sensation in the forefoot area can reduce the risk of flap loss and promote rapid rehabilitation and functional recovery. Here, we report our experience with two cases of successful reconstruction of the forefoot and big toe with a sensate anterolateral thigh flap, with a review of the relevant literature.

  • Vascular anomalies encompass a variety of malformations and tumors that can result in severe morbidity and mortality in both adults and children. Advances have been made in the classification and diagnosis of these anomalies, with the International Society for the Study of Vascular Anomalies establishing a widely recognized classification system. In recent years, notable progress has been made in genetic testing and imaging techniques, enhancing our ability to diagnose these conditions. The increasing sophistication of genetic testing has facilitated the identification of specific genetic mutations that help treatment decisions. Furthermore, imaging techniques such as magnetic resonance imaging and computed tomography have greatly improved our capacity to visualize and detect vascular abnormalities, enabling more accurate diagnoses. When considering reconstructive surgery for facial vascular anomalies, it is important to consider both functional and cosmetic results of the procedure. Therefore, a comprehensive multidisciplinary approach involving specialists from dermatology, radiology, and genetics is often required to ensure effective management of these conditions. Overall, the treatment approach for facial vascular anomalies depends on the type, size, location, and severity of the anomaly. A thorough evaluation by a team of specialists can determine the most appropriate and effective treatment plan.

  • Background The survival rate of newborns with severe congenital abdominal wall defects has increased. After successfully addressing life-threatening complications, it is necessary to focus on the cosmetic and functional outcomes of the abdominal wall. Methods We performed a chart review of five cases treated in our institution. Results Five patients, ranging from seven to 18 years of age, underwent the following surgical approaches: simple approximation of the rectus abdominis fascia, the rectus abdominis sheath turnover flap, the placement of submuscular tissue expanders, mesh repair, or a combination of these techniques depending on the characteristics of each individual case. Conclusions Patients with severe congenital abdominal wall defects require individualized surgical treatment to address both the aesthetic and functional issues related to the sequelae of their defects.

소포로리피드[미생물계면활성제]의 생산과 응용 (Production and Application of Sophorolipid, A Microbial Surfactant)

  • Microbial surfactants are more effective and environmentally friendly than many synthetic surfactants. Sophorolipid, a glycolipid type microbial surfactant, is produced from C. bombiocola. Cultivation techniques to increase the productivity have been developed using various carbon sources and reactor setup, reaching its concentration upto 100-300 g/L. Due to its high productivity and non-toxicity, sophorolipid became one of the most promising alternative to synthetic surfactants. Fermentative production of sophorolipid depends primarily on the carbon sources, such as glucose and vegetable oils, and nitrogen sources. Chemical modification of the sophorolipid produces various derivative with different physical properties including hydrophile-liphophilie balance(HLB), emulsion formation, surface tension and dispersing ability. Commercial potentials of sophorolipid in the cosmetic, health care and environment clean-up industries have been discussed.

  • Zygomatic fractures are the second most common fractures of the facial skeleton, after nasal bone fractures. Due to its uniqueness, the malar bone plays a very important role in maintaining appropriate facial contours. Zygomatic fractures can cause ocular and mandibular functional impairment, along with cosmetic defects. With the help of advanced imaging techniques and various treatment options, the management of zygomatic fractures has become more sophisticated and less invasive. This article discusses zygomatic fractures in detail: their clinical and radiographic features, and the various treatment options available.

  • Research objective:To exactly evaluate their functions of kojic acid, arbutin and vitamin C as a whitening agent, we performed experiments to compare their abilities to inhibit melanin synthesis.   • Strategies to make finished products more stable and efficacious can take advantage of formulation technologies- ingredients and techniques - to improve their delivery into the skin by both enhanced penetration and delayed release. Nanometer range particles and emulsion droplets can be formed with a selection of either silicone copolyol or conventional organic surfactants and dispersion shear rates sufficient to generate stable submicron droplets. By incorporating these into systems with biomimetic liquid crystalline gel networks in either simple or multiple phase emulsions the skin delivery can be enhanced as shown in tape stripping experiments. Such systems can stabilize labile actives, such as Vitamin C and retinol, and aid delivery. Laboratories in U.S and Europe investigated actives including green tea polyphenols, salicylic acid and methyl and benzyl nicotinate.

Correction of pixie ear: infraauricular skin redraping method

  • Patients with pixie ear have an attached, tapering, and low-set earlobe. Traditional methods usually describe excision of the caudal portion of the lobule and reattachment in a more superior position. The present report suggests a simplified skin redraping method for correction of pixie ear. The procedure provides easy method to design and perform, which only requires elevation and trimming of the skin. Other ancillary procedures, such as flap design, anchoring, plication, and subdermal fixation, are not required. This method produces satisfactory results. Postoperative scar is invisible because the incision is on the retroauricular region, and the corrected earlobe has a more natural appearance than the repositioned earlobe. Moreover, skin redraping avoids tension, which contributes to minimization of the postoperative scar.

천연색소의 과학과 기술 (Science and Technology of Natural Colorants)

  • Natural colorants are regarded as substitutes for synthetic colorants in the fields of fiber dyeing, hair dyeing, cosmetic, and food industries. The use of natural colorants has been increased in view of global environment and sustainability. However, scientific approach to the production is required because the supply of natural colorants depends on the climate, soil, and cultivation methods. This brief review contains the advances in science and technology related to natural colorants including characterization, analysis, CIEL*a*b* and Munsell color systems, and future prospects. For the characterization and the analysis of natural colorants, chromatography, some spectroscopic techniques (UV-Vis, FT-IR, NMR), mass spectrometry, and X-ray diffraction are generally used.

비골골절 시 골절정복과 동시에 시행된 융비술 (Simultaneous Augmentation Rhinoplasty with Bony Reduction in Nasal Bone Fracture)

  • Purpose: The nasal bones are the most common fracture sites of the facial bones, and a careful reduction may still result in secondary deformities, such as saddle nose, deviated nose, hump nose etc, requiring secondary cosmetic rhinoplasty. Therefore, this study examined the clinical characteristics of nasal bone fractures to propose guidelines for patient selection and surgical procedures to achieve more satisfactory results and to prevent secondary deformities with simultaneous augmentation rhinoplasty and bony reduction. Methods: The study was based on 26 out of 149 nasal bone fracture patients who underwent simultaneous augmentation rhinoplasty with bony reduction between May 2008 and April 2009. Retrospective analysis was performed according to the clinical data, surgical techniques and postoperative results. Results: Of the 26 patients, there were 15 males and 11 females. The incidence according to the Stranc's classification revealed that 62% of patients were injured by a frontal impact and 38% by a lateral impact. Frontal impact plane I (50%) was the most frequent type. At the follow up, 18 (81.2%) out of 22 patients were satisfied with their postoperative outcome, and the remaining 4 patients were fair. No one was dissatisfied. However, 5 cases in 3 patients (23%) had some complications; minimal implant deviation in 2 cases, minor irregularity on the nasal dorsum in 2 cases and palpable implant movement under palpation in 1 case. None of these cases required surgical correction. Conclusion: With the proper guidance, simultaneous augmentation rhinoplasty with bony reduction can prevent secondary deformities and satisfy the cosmetic outcomes.
