Inflammatory Cytokines and Dietary Factors in Korean Elderly with Chronic Disease (만성 질환 노인에서의 면역 성분 양상과 식이예방인자)

  • The purpose of this study was to investigate the association between inflammatory cytokines and chronic disease status in Korean elderly. The subjects were 248 elderly people aged over 65 years recruited from Health Center in Seoul. The subjects were classified into 3 groups based on their disease (diabetes, hypertension, and hyperlipidemia) status: subjects with one diagnosed disease of diabetes, hypertension, and hyperlipidemia fall into singular group (n=89), subjects with more than 2 disease into multiple group (n=39), and those with free of the diseases into normal (n=122). Anthropometric and biochemical characteristics, and dietary intakes were assessed. Dietary intakes were surveyed by 24-recall method. The means of IL-2, IL-6, MCP-1 and C3 were not differ among 3 groups. However, when subjects classified into tertiles of IL-6, MCP-1, TNF-$\alpha$ and C3 and frequencies of each fertile were compared, the multiple group showed significantly lower frequencies in lowest fertile than normal group (p<0.05), suggesting higher tendency of inflammatory responses. For hematological values, blood pressure, triglycerides, fasting blood glucose levels were highest in multiple group (p<0.05) compared to other 2 groups. BMI, body fat(kg), and triceps skinfold thickness were also significantly higher in multiple group than in 2 other groups(p<0.05). Moreover, the concentrations of IL-2, IL-6 and C3 were significantly correlated with hematologic values of fasting blood glucose, total cholesterol, LDL-cholesterol, and triglycerides or obesity factors such as triceps skinfold thickness, BMI, and body fat(%). Among singular and multiple group, the subjects with higher intakes for vitamins A, C, and E showed the higher level of IL-2 and the lower level of MCP-1, and C3. In conclusion, blood concentrations of triglycerides and proinflammatory cytokines, blood pressure, obesity parameters (BMI, body fat, triceps skinfold thickness) were higher in multiple group than in normal, but this result strongly suggest that the increasement of the vitamin A, C, and E intakes would modify the cytokine levels to reduce the inflammatory response in the elderly people with chronic diseases.

A Study on Nutritional Status during Dialysis in Patients Undergoing Continuous Ambulatory Peritoneal Dialysis (지속성 외래 복막투석 환자의 투석기간에 따른 영양상태)

  • Patients undergoing peritoneal dialysis are at risk for protein-energy malnutrition because of nutrient losses during dialysis. This study determined the nutritional status of patients undergoing continuous ambulatory peritoneal dialysis (CAPD). Forty-four patients receiving CAPD were divided into two groups according to dialysis period. We investigated the nutritional status of the patients by measuring anthropometric and biochemical parameters, as well as food intake, self-appetite, dietary habits, a subjective global assessment, and a total nutritional status assessment. Group I subjects (7 males, 13 females) had received dialysis for < 2 years, whereas the group II subjects (18 males, 6 females) received dialysis for ${\geq}$ 2 years. Energy intake with added dextrose in the dialysate per kg of body weight was $30.3{\pm}5.8$ kcal in group I and $29.0{\pm}8.1$ kcal in group II. The average protein intake per kg of weight was $1.0{\pm}0.3$ g in group I and $1.0{\pm}0.4$ g in group II, which were less than the recommended protein intake for patients undergoing CAPD (1.2-1.5 g/kg). Mean serum albumin level was significantly lower in group II than that in group I (p < 0.05). A recent self-appetite score was significantly higher in group II than that in group I (p < 0.01). The dietary habits score was significantly lower in group II than that in group I (p < 0.05). The subjective global assessment was significantly higher in group I (85.0%) than that in group II (54.2%) under normal nutrition status (p < 0.05). The dialysis period was significantly and negatively correlated with the subjective global assessment (r = -0.502, p < 0.01) and the total nutritional status assessment (r = -0.575, p < 0.01). These results demonstrated that patients undergoing CAPD for ${\geq}$ 2 years had worse nutritional status than those who had been undergoing dialysis for < 2 years. Good nutritional status can predict the long-term survival of patients undergoing peritoneal dialysis. Additionally, the exact evaluation of nutritional status before 2 years will be important to maintain long-term dialysis therapy in patients undergoing CAPD.

A Study on the Introduction of Linear Programming Model into the Management of Korean Coastal and Offshore Fisheries (한국 연근해어업의 합리적 관리를 위한 LP모형의 도입방안에 관한 연구)

  • Many studies to cope with the present problems of Korean coastal and offshore fisheries has been performed, but these were done partly in necessities and general studies for Korean inshore and offshore fisheries are in early stage. Most of these studies adopted analytical way of approach for each fishery individually and they could not reflect the effect of correlated interaction among fisheries on the several common species/stocks, and thus optimal effort allocation was impossible. To consider general fisheries and optimal effort allocation among competing mixed species, a linear programming (LP) approach is applied in this study and introduced into 16 important inshore and offshore fisheries with 13 constraining species which were chosen by annual yield order. This study is not based on the biological interaction among species (i.e., prey - predator system) but the technological interaction between species and fishing efforts. For the application of LP model in these fisheries, the standardization of fishing efforts through different fishing gears could not be successful and a new way of effort standardization through CPUE for vessel tonnage was originated. Total standardized fishing effort on a particular species i, Ei, is computed as the linear summation of standardized fishing effort generated by each fishery j. That is, (equation omitted) where $f_{j}$ is the total vessel tonnage of fishery j and aij is the coefficients contributing to the standardized fishing effort per ton for species i taken in fishery j. The total fishing effort level on species i due to both directed fishing and by - catch can thus be accounted in the aij's. Optimal effort allocation among the j fisheries may be considered a minimizing problem (minimize $\Sigma$ $f_{j}$), subject to the constraints that standardized fishing effort levels on particular species are maintained at, above, and below certain predefined levels. Fishing effort goals for individual species can be based on various biological and/or economic criteria, i.e., fishing effort level generating maximum sustainable yield and/or maximum economic yield. But in this study the $F_{0.1}$ criteria which was accepted as an approximate level for $F_{mey}$ by Outland and Boerema's (1973) study. The findings of this study are, (1) LP model can be applied to the Korean inshore and offshore fisheries giobally. (2) Through a new way of combining multiple different fisheries' efforts for a particular species together generating standardized fishing effort, Schaefer curve could be applied to the complex system successfully. (3) The results of this study for total reduction scale were mostly the same as those of prior studies, but different much from the individual scales of reduction. This study showed the necessities for exploitation of more concrete parameters to put into consideration of profitability of fisheries and social factors, and this model can be modified according to the actual constraints. Also, considering the age structure of stocks, this model can be developed into better one for better fisheries management.ent.

Associations among Physical Activity, Comorbidity, Functional Capacity, Peripheral Muscle Strength and Depression in Breast Cancer Survivors

  • Physical inactivity may an important outcome in the prognosis of breast cancer. Physical activity levels decrease significantly for breast cancer patients following the treatment and remain low after oncology treatment is completed. The aim of this study was to determine physical activity levels and to examine associations among physical activity, comorbidity, functional capacity, peripheral muscle strength, and psychosocial status in breast cancer patients. Forty breast cancer survivors were included in this study. Demographic and clinical characteristics were recorded. Physical activity was assessed with a short form of the International Physical Activity Questionnaire (IPAQ). We defined comorbid diseases according to the Charlson Comorbidity Index (CCI). Functional capacity was evaluated with a six-minute walk test (6MWT). Peripheral muscle strength was measured for quadriceps femoris muscle with a hand-held dynamometer. Psychosocial status was measured with the Hospital Anxiety Depression Scale (HADS). Forty percent of patients were inactive. The IPAQ total score was significantly related with quadriceps muscle strength (r=0.492; p<0.001) and HADS depression score (r=0.341; p<0.05). Marked correlations were also observed between IPAQ walking score and quadriceps muscle strength (r=0.449; p<0.001), HADS depression score (r=0.341; p<0.05), and CCI (r=-0.433; p<0.001). The CCI score was also markedly associated with quadriceps muscle strength (r=-0.413; p<0.001). 6MWT distance was not significantly correlated with any of the parameters. Regression analyses revealed that psychosocial status and peripheral muscle strength were significant predictors of physical activity estimated with the IPAQ short form and when combined, they explained 35% of the variance. Comorbidities, peripheral muscle strength and psychosocial status partially explain the variability of physical activity level in breast cancer survivors. These results suggest that physical inactivity contributes to worsening health in breast cancer survivors.

Morphological Factors and Cardiac Doses in Whole Breast Radiation for Left-sided Breast Cancer

  • Background: To investigate the impact of the breast size, shape, maximum heart depth (MDH), and chest wall hypotenuse (the distance connecting middle point of the sternum and the length of lung draw on the selected transverse CT slice) on the volumetric dose to heart with whole breast irradiation (WBI) of left-sided breast cancer patients. Materials and Methods: Fifty-three patients with left-sided breast cancer undergoing adjuvant intensity-modulated radiotherapy (IMRT) were enrolled in the study. The primary breast size and shape, MHD and DCWH (chest wall hypotenuse) were contoured on radiotherapy (RT) planning CT slices. The dose data of hearts were obtained from the dose-volume histograms (DVHs). Data were analyzed by one-way analysis of variance (ANOVA), Student's t-test and linear regression analysis. Results: Breast size was independent of heart dose, whereas breast shape, MHD and DCWH were correlated with heart dose. The shapes of breasts were divided into four types, as the flap type, hemisphere type, cone type and pendulous type with heart mean dose being $491.8{\pm}234.6cGy$, $752.7{\pm}219.0cGy$, $620.2{\pm}275.7cGy$, and $666.1{\pm}238.0cGy$, respectively. The flap type of breasts shows a strong statistically reduction in heart dose, compared to others (p=0.008 for V30 of heart). DCWH and MHD were found to be the most important parameters correlating with heart dose in WBI. Conclusions: More attention should be paid to the heart dose of non-flap type patients. The MHD was found to be the most important parameter to correlate with heart dose in tangential WBI, closely followed by the DCWH, which could help radiation oncologists and physicsts evaluate heart dose and design RT plan in advance.

The relationship between of Uncertainty, Depression, Physiologic Index and Basic Psychological Need of Hemodialysis Patients (혈액투석환자의 질병 불확실성, 우울, 생리지표와 기본심리욕구와의 관계)

  • This purpose of study was to identify variables predicting basic psychological need in hemodialysis patients. The participants were 134 patients from two major general hospitals and two dialysis center located in J city. Data were collected using self-report questionnaires and physiological index. Data analysis was done by using SPSS WIN 18.0 program for one-way ANOVA, independent t-test, Pearson correlation coefficients, and multiple regression. This study showed a negative correlation between basic psychological need and uncertainty(r=--.464, p<.001), depression(r=-.422, p<.001). In addition, relationships and physiological index were Positively correlated. The uncertainty(${\beta}=-.345$), depression(${\beta}=-.279$), physiological index(${\beta}=-.117$) have a 29% explanatory power for the basic psychological need in hemodialysis patients. Physiological index, uncertainty and depression in turn influenced the basic psychological needs of hemodialysis patients. It is necessary to develop nursing strategies and programs to reduce disease uncertainty and depression in order to increase self-deterministic health behavior through autonomy, competence and relationship satisfaction.

Study on the Use of Bracketed Summations of the Peak Ground-motion Acceleration Per Second for Rapid Earthquake Alert Notifications (신속 지진피해통보를 위한 지반가속도의 초당 최대값 구간적산 방법의 활용에 관한 연구)

  • In an effort to further exploit the peak ground-motion acceleration (PGA) information per second available in real time by the enacted law, bracketed summations of the PGA per second ($BSPGA_k$) for 30 seconds based on the records with a rate of 100 samples were compared with the cumulative absolute velocity (CAV) and earthquake intensities based on a worldwide database of records from small-to-large earthquakes. The CAV, currently in use as an earthquake damage indicator for nuclear power plants due to its strong correlation with the earthquake intensity, has the disadvantage of requiring a massive amount of digital data with a rate of more than 100 samples per second. The comparative study shows that the $BSPGA_k$ is well correlated with the CAV over the wide range of strong ground-motion levels, which suggests that the $BSPGA_k$ is one of the new promising ground-motion parameters especially useful for rapid earthquake alert notifications through an earthquake monitoring network. Based on the domestic database of records from small-to-moderate earthquakes with felt reports, it is also observed that the $BSPGA_k$ is comparable to the CAV and better than the PGA in predicting the intensity by using the correlation relation.

Efficient Occurrence Monitoring by Yellow Sticky Traps for Major Flying Pests in Strawberry Greenhouses (시설딸기 주요 비행해충의 황색끈끈이트랩 이용 효율적인 발생예찰)

  • Sampling plan using yellow sticky traps for the major strawberry flying pests - western flower thrips Frankliniella occidentalis adults, cotton aphid Aphis gossypii alate and greenhouse whitefly Trialeurodes vaporarium adults was developed to determine the initial occurrence time. The analyzed trap data were obtained from three commercial strawberry greenhouses for the whole growing season (September to May of the following year) during 2013 to 2017 in Jeju province. Three flying pests showed the aggregated distribution patterns resulted from commonly used regression techniques - Taylor's power law and Iwao's patchiness regression. Taylor's power law was better description of mean-variance relationship of the western flower thrips and the cotton aphid than Iwao's patchiness regression, otherwise greenhouse whitefly was better described by Iwao's patchiness regression. There were highly significant correlated among mean density per trap, maximum density and proportion of traps with more than 10 individuals. To estimate 4.0 heads of mean density per trap, the minimum number of traps were required 13 traps for western flower thrips, 11 traps for cotton aphid and 10 traps for greenhouse whitefly. The sequential sampling plans at the fixed precision level 0.25 were developed using parameters of Taylor's power law for western flower thrips and cotton aphid, and of Iwao's patchiness regression for greenhouse whitefly.

Studies of Solvolyses of Di-n-butyl Phosphorochloridate by Extended Grunwald-Winstein Equation (확장된 Grunwald-Winstein 식에 의한 Di-n-butyl Phosphorochloridate의 가용매 분해반응 연구)

  • The solvolysis rate constants of di-n-butyl phosphorochloridate ((CH3CH2CH2CH2O)2POCI, 1) in 28 different solvents are well correlated with the extended Grunwald-Winstein equation, using the NT solvent nucleophilicity scale and YCl solvent ionizing scale, with the sensitivities values of 1.40 and 0.42 for l and m, respectively. These l and m values can be considered to support an SN2 reaction pathway. This interpretation is further supported by the activation parameters, i.e., relatively small positive ΔH (8.0 to 15.9 kcal·mol−1 ) values and large negative ΔS (−25.8 to −53.1 cal·mol−1 ·K−1 ) values, the Kivinen’s n values (0.9~1.7), and the solvent kinetic isotope effect (1.62).

Impact of iron deficiency anemia on chronic periodontitis and superoxide dismutase activity: a cross-sectional study

  • Purpose: Both chronic periodontitis (CP) and iron deficiency anemia (IDA) induce oxidative stress in the body and cause an imbalance between reactive oxygen species and antioxidants, such as superoxide dismutase (SOD). This study explored the SOD enzyme activity of saliva and serum in CP patients with and without IDA and analyzed the impact of IDA on CP. Methods: A total of 82 patients were divided into four groups: control group (CG, 22), periodontally healthy IDA patients (IDA-PH, 20), CP patients (CP, 20), and IDA patients with CP (IDA-CP, 20). After clinical measurements and samplings, serum and salivary SOD levels were determined using an SOD assay kit. Results: IDA-CP patients exhibited a higher gingival index, bleeding on probing, probing pocket depth, and percentage (%) of sites with a clinical attachment loss (CAL) of ${\geq}6mm$ (P<0.008) than CP patients. The mean salivary and serum SOD levels were significantly lower in the IDA-PH, CP, and IDA-CP patients than in the CG group (P<0.008). A significant positive correlation between salivary and serum SOD activity was observed in IDA (P<0.05). Furthermore, serum and salivary SOD levels were significantly and negatively correlated with all periodontal parameters including the percentage of sites with CAL of 4-5 and ${\geq}6mm$ (P<0.05) except the significant correlation between salivary SOD activity and mean CAL and the percentage of sites with CAL of 4-5 mm (P>0.05) in these patients. Conclusions: Within the limits of this study, it may be suggested that IDA patients with chronic periodontitis have more periodontal breakdowns than patients with chronic periodontitis. Serum and salivary SOD activity levels were lower in the IDA-PH, CP and IDA-CP groups than in the CG. Iron deficiency anemia influenced the serum SOD activity but did not seem to affect the salivary SOD activity in these patients.
