Perceptions of Quality of Patient-Centered Nursing Care among Women with Breast Cancer (유방암 여성이 인식한 환자중심 간호의 질)

  • Purpose: This study aimed to investigate the quality of patient-centered nursing care (PCNC) among women with breast cancer at a cancer center in Seoul, Korea. Methods: In a cross-sectional survey design, 223 women with breast cancer were recruited from the oncology surgery unit. The Korean version of the oncology patients' perceptions of the quality of nursing care scale, which is conceptualized in four sub-dimensions (individualization, proficiency, responsiveness, and coordination) was used for measurement. Data were analyzed using descriptive statistics and ANOVA. Results: The participants were all women, with a mean age of 51.3 years. The mean score of PCNC was high and significantly different from each other according to age group. Breast cancer women, who had mastectomy, were satisfied highly in terms of proficiency and responsiveness care, but less satisfied with individualization and coordination care than those of other women. Conclusion: The findings of this study show the quality of cancer nursing care, especially focusing on patient-centeredness, can be measured from the patients'perspective. The individualized and coordinated nursing care is considered to be the core of quality cancer care implying patient-centeredness. Based on the findings of this study, more research is necessary to explore the patients' view of quality cancer care and to test the effects of PCNC within the context of comparative effectiveness.

Clinical Nursing Survey of the Patients in the Intensive Care Unit (중환자실(I.C.U.)환자에 관한 임상 간호학적 관찰 -중환자실 임상 간호 교육을 위한 기초조사-)

  • The objectives of this study have been conducted to establish effective clinical teaching program to I.C.U in terms of proper assignment of the clients for the students, proper rotation schedule, priorities in critical nursing problems and selection of the teaching and learning. We have analyzed statistically 1,850 patients who have been admitted during a period from January 1977 to October 31 1979 in Ewha Woman's University Hospital. The results are as follows: 1. The proportion to the total inpatients number was 6.5% and mortality rate was 16.3%. 2. The average hospitalized days were 5.8 days in I.C.U and the total death was occured from 1 st hospital day to 5th hospital day. So it shows a certain difficulties for clinical experiences of the senior students in I.C.U. 3. In the age of the death, 41.3% of the patients were in the 41-60 year age group. It shows highest mortality rate in socially active and productive age groups. 4. The mortality rates of the departments of the medicine was 18.7%, general surgery 18.5%, and neurosurgery 14.7%. 5. The number of patients admitted to the department of neurosystem was 30.6%, cardiovascular system 22.6%, respiratory system 11.1 % and urinary system 2.9%. 6. On utilizing instruments and machine for diagnosis and client's assessment in I.C.U, they have utilized everything a usual. But they never utilize angiogram and cardiac catheterization in cardiovascular system, and retroperitoneal pneumography in the urologic system. Further more we would recommend as follows 1. In consideration of the average hospital days and the date of death, the rotation program for clincal experience need to be adjusted as continuing practice program in apposite to current alternative practice program for comprehensive nursing care. 2. Socioeconomic needs for the patient's families and himself should be emphasized by the students in addition to physical needs. 3. Course content for critical care might be built up in considering of core disease centered nursing problems. 4. The diagnostic procedures and client's assessment items which could not experience in our university hospital by the students might be considered and refilled as filled trips to another hospital and visual aids.

Effects of Breastfeeding Interventions on Breastfeeding Rates at 1, 3 and 6 Months Postpartum: A Systematic Review and Meta-Analysis (모유수유중재의 산후 1, 3, 6개월 모유수유율에 대한 효과: 체계적 문헌고찰 및 메타분석)

  • Purpose: This study was a systematic review and meta-analysis designed to evaluate the effects of breastfeeding intervention on breastfeeding rates. Methods: Based on the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA), a systematic search was conducted using eight core electronic databases and other sources including gray literature from January 9 to 19, 2017. Two reviewers independently select the studies and assessed methodological risk of bias of studies using the Cochrane criteria. The topics of breastfeeding interventions were analyzed using descriptive analysis and the effects of intervention were meta-analyzed using the Review Manager 5.2 software. Results: A total of 16 studies were included in the review and 15 were included for meta-analysis. The most frequently used intervention topics were the importance of good latch-on and frequency of feeding and determining adequate intake followed. The pooled total effect of breastfeeding intervention was 1.08 (95% CI 1.03~1.13). In the subgroup analysis, neither pre-nor post-childbirth intervention was effective on the breastfeeding rates at 1, 3, and 6 months, and neither group nor individual interventions had an effect. Only the 1 month breastfeeding rate was found to be affected by the individual intervention with the persistent strategies 1.21 (95% CI 1.04~1.40). Conclusion: Effective breastfeeding interventions are needed to help the mother to start breastfeeding after childbirth and continue for at least six months. It should be programmed such that individuals can acquire information and specific breastfeeding skills. After returning home, there should be continuous support strategies for breastfeeding as well as managing various difficulties related to childcare.

  • The design and analysis of a novel photonic crystal electro-optic modulator are presented in this paper. The device incorporates an electro-optic (EO) polymer slot waveguide into the center of a silicon photonic crystal waveguide. In this device, strong optical confinement in the EO polymer core and small group velocity from the photonic crystal structure provide a surprise enhancement of the EO effect.

Effect of ascorbic acid, ethanol and acetone on adhesion between the treated fiber posts and composite resin cores

  • PURPOSE. The aim of the present study was to assess the effect of ascorbic acid, ethanol and acetone on microtensile bond strength between fiber posts pre-treated with hydrogen peroxide and composite resin cores. MATERIALS AND METHODS. Twenty four fiber posts were pre-treated with 24% hydrogen peroxide and divided into 4 groups as follows: G1: no treatment, as control group; G2: treatment with10% ascorbic acid solution for 5 minutes; G3: treatment with 70% ethanol solution for 5 minutes; and G4: treatment with 70% acetone solution for 5 minutes. Each fiber post was surrounded by a cylinder-shaped polyglass matrix which was subsequently filled with composite resin. Two sections from each sample were selected for microtensile test at a crosshead with speed of 0.5 mm/min. Statistical analyses were performed using one-way ANOVA and a post hoc Tukey HSD test. Fractured surfaces were observed under a stereomicroscope at ${\times}20$ magnification. The fractured surfaces of the specimens were observed and evaluated under a SEM. RESULTS. Means of microtensile bond strength values (MPa) and standard deviations in the groups were as follows: G1: $9.70{\pm}0.81$; G2: $12.62{\pm}1.80$; G3: $16.60{\pm}1.93$; and G4: $21.24{\pm}1.95$. G4 and G1 had the highest and the lowest bond strength values, respectively. A greater bond strength value was seen in G3 compared to G2. There were significant differences between all the groups (P<.001). All the failures were of the adhesive mode. CONCLUSION. Application of antioxidant agents may increase microtensile bond strength between fiber posts treated with hydrogen peroxide and composite cores. Acetone increased bond strength more than ascorbic acid and ethanol.

Effects of post surface conditioning before silanization on bond strength between fiber post and resin cement

  • PURPOSE. Post surface conditioning is necessary to expose the glass fibers to enable bonding between fiber post and resin cement. The purpose of the present study was to evaluate the effect of different surface conditioning on tensile bond strength (TBS) of a glass fiber reinforced post to resin cement. MATERIALS AND METHODS. In this in vitro study, 40 extracted single canal central incisors were endodontically treated and post spaces were prepared. The teeth were divided into four groups according to the methods of post surface treatment (n=10): 1) Silanization after etching with 20% $H_2O_2$, 2) Silanization after airborne-particle abrasion, 3) Silanization, and 4) No conditioning (Control). Adhesive resin cement (Panavia F 2.0) was used for cementation of the fiber posts to the root canal dentin. Three slices of 3 mm thick were obtained from each root. A universal testing machine was used with a cross-head speed of 1 mm/minute for performing the push-out tests. Two-way ANOVA and Tukey post hoc tests were used for analyzing data (${\alpha}$=0.05). RESULTS. It is revealed that different surface treatments and root dentin regions had significant effects on TBS, but the interaction between surface treatments and root canal regions had no significant effect on TBS. There was significant difference among $H_2O_2$ + Silane Group and other three groups. CONCLUSION. There were significant differences among the mean TBS values of different surface treatments. Application of hydrogen peroxide before silanization increased the bond strength between resin cements and fiber posts. The mean TBS mean values was significantly greater in the coronal region of root canal than the middle and apical thirds.

Developing a Competency-based Continuing Education Program for Library and Information Professions based on the National Competency Standards (NCS) (역량기반 사서계속교육을 위한 핵심역량지표 및 콘텐츠개발 - NCS적용 타당성 분석 및 보완방안 연구 -)

  • The purpose of this study was to develop a competency-based continuing professional development (CPD) model using the National Competency Standards (NCS)-Library & Information Management. This study specifically examined various competency standards for librarians developed in and out of the country, conducted a survey research and focused group interviews to obtain professional librarians' needs for continuing education and their opinions on the NCS for library professions. It analyzed the contents of the NCS elements and mapped with elements in the five other competency standards for library and information professions. It also scrutinized the current training programs at the National Library of Korea in order to identify their strengths and weaknesses. The results of the study informed the areas to enhance the current NCS by identifying underdeveloped competencies and identified the course contents that should be developed with priorities. The study also developed a modified National Competency Standards for Continuing Professional Development (NCS-CPD). Continuing education institutions can utilize this scheme as a useful guideline to develop balanced continuing professional development curriculums for the practitioners working in various library and information centers.

  • Objectives: The main objectives of the article are to review roles and prospective of the Health Education Specialist on the globalized World, and develop the new positions and jobs of the Health Education Specialists on health promotion. Results and Conclusion: There are many rapid changes, these days, in the Medical and Health Environments, because of Globalization. Modern society needs health professionals who are equipped with new knowledge and information to correspondence with various Health Problems, such as the appearing and disappearing of new contagious diseases, problems of improper health barriers for foreign products, health problems from poverty and also health inequalities are known to be rising. Globalization has induced new needs for Health Professional manpower. After the Ottawa Charter, international society is training and utilizing Health Education Specialists as the propulsive core member of the Health Promotion Era. And also society now expects and requests the activities of Health Education Professionals as a group effort, not only in their own countries, but also across the barriers of international society. Health Professionals are working in WHO, UNICEF KOICA or other international organizations. Especially England and USA are utilizing Health Education Professionals in Health Planning and Education Work to keep up with Health Promotion Era. Now, we need to establish ideal and proper strategies in Health Promotion Work, as a one of the pioneer countries to lead Internationalization. To accomplish this task, Health Education Specialists should be well utilized in the field of Health Promotion Work, such as communities, schools, industrial sites and international health organizations.

The influence of anthropogenic disturbances and watershed morphological characteristics on Hg dynamics in Northern Quebec large boreal lakes

  • Mercury (Hg) dynamics in the boreal environment have been a subject of concern in recent decades, due to the exposure of local populations to the contaminant. Land use, because of its impact on mercury inputs, has been highlighted as a key player in the sources and eventual concentrations of the heavy metal. In order to evaluate the impact of watershed disturbances on Hg dynamics in frequently fished, large boreal lakes, we studied sediment cores retrieved at the focal point of eight large lakes of Qu$\acute{e}$bec (Canada), six with watersheds affected by land uses such as logging and/or mining, and two with pristine watersheds, considered as reference lakes. Using a Geographical Information System (GIS), we correlated the recent evolution of land uses (e.g., logging and mining activities) and morphological characteristics of the watershed (e.g., mean slope of the drainage area, vegetation cover) to total Hg concentrations (THg) in sedimentary records. In each core, THg gradually increased over recent years with maximum values between 70 and 370 ng/g, the lowest mercury concentrations corresponding to the pristine lake cores. The Hg Anthropogenic Sedimentary Enrichment Factor (ASEF) values range from 2 to 15. Surprisingly, we noticed that the presence of intense land uses in the watershed does not necessarily correspond to noticeable increases of THg in lake sediments, beyond the normal increment that can be attributed to Hg atmospheric deposition since the beginning of the industrial era. Rather, the terrestrial Hg inputs of boreal lakes appear to be influenced by watershed characteristics such as mean slopes and vegetation cover.

  • In this paper, we propose the classification scheme and retrieval mechanism which can apply to many application domains in order to construct the software reuse library. Classification scheme which is the core of the accessibility in the reusability, is defined by the hierarchical structure using the agglomerative clusters. Agglomerative cluster means the group of the reuse component by the functional relationships. Functional relationships are measured by the HACM which is the representation method about software components to calculate the similarities among the classes in the particular domain. And clustering informations are added to the library structure which determines the functionality and accuracy of the retrieval system. And the system stores the classification results such as the index information with the weights, the similarity matrix, the hierarchical structure. Therefore users can retrieve the software component using the query which is the natural language. The thesis is studied to focus on the findability of software components in the reuse library. As a result, the part of the construction process of the reuse library was automated, and we can construct the object-oriented reuse library with the extendibility and relationship about the reuse components. Also the our process is visualized through the browse hierarchy of the retrieval environment, and the retrieval system is integrated to the reuse system CARS 2.1.
