Air leakage due to the cuff hanging on the vocal cords during nasotracheal intubation: a case report

  • Nasotracheal intubation is commonly performed under general anesthesia in oral and maxillofacial surgery. For the convenience of surgery, nasal Ring-Adair-Elwyn (RAE) tubes are mainly used. Because the nasal RAE tubes were bent in an "L" shape, the insertion depth was limited. Particularly, it is necessary to accurately determine the appropriate depth of the RAE tubes in children. Several types of nasal RAE tubes are used in the medical market, which vary in material and length. We performed endotracheal intubation using a nasal RAE tube for double-jaw surgery, but air leakage persisted even when the air pressure in the cuff was increased. When checked with a laryngoscope, it was confirmed that the tube was pushed out, and the cuff was caught on the vocal cords, causing air leakage. Since inserting the tube deeply did not solve the problem, replacing it with a nasal RAE tube (PolarTM, Preformed Tracheal Tube, Smith Medical, Inc., USA) did not cause air leakage; thus, we reported this case.

Effect of Cytoskeletal Manual Therapy, a Novel Soft Tissue Mobilization Technique, on Axillary Web Syndrome after Axillary Lymph Node Dissection: A Case Report

  • Objective: Axillary web syndrome (AWS) is a condition comprising fibrous band-like cords that appear in the axilla of patients after axillary lymph node dissection (ALND) during breast cancer surgery and result in pain and reduced mobility. The cords appearing with AWS are hardened veins or lymphatic vessels. Manual therapy and stretching are recommended for pain control and mobility improvement. Therefore, this study investigated the effect of cytoskeletal manual therapy (CMT), which is a new soft tissue mobilization technique. Design: A case report Methods: A 41-year-old woman with AWS after breast cancer surgery and ALND visited a physical therapy clinic because of shoulder pain, decreased function, and decreased mobility. The cords were palpable and pain occurred 2 weeks after surgery. CMT was performed three times per week for a total of 6 weeks. Her pain intensity, range of motion (ROM), and shoulder function were measured. Results: Measurements were performed after 2 weeks and 6 weeks of CMT and evaluated using the numeric pain rating scale (NPRS). Her pain intensity largely decreased after 2 weeks (4-point score reduction) and after 6 weeks (5-point score reduction) of CMT. After CMT, her full ROM was restored and her shoulder function was improved (7-point score reduction). Conclusions: CMT is effective for pain control, mobility improvement, and functional improvement of patients with AWS.

A Study on the Short Circuit Current and Molten Mark Analysis of Polyvinyl Chloride Insulated Flexible Cords (비닐코드의 단락전류 및 용융흔 분석에 관한 연구)

  • In this paper, we studied the short circuit current and molten mark of polyvinyl chloride insulated flexible cords(VFF). The calory decreased remarkably with increase of current. The surface structure and composition of a strand wire were analyzed by stereoscope, scanning electron microscope (SEM), and energy dispersive x-ray spectroscopy(EDX). The surface of a strand wire showed columnar and void. The intensity of CuL spectrum increased in melted mark.

  • Though normal vocal cords show regular vibration, pathologic vocal cords show irregularity between peaks. Jitter means fluctuation in the time interval between peaks, and Shimmer means cycle to cycle variation in the amplitude of the peaks. We investigated the vocal vibration of Korean normal persons objectively. The fundamental frequency, Jitter, Shimmer and SNR(signal to noise ratio) of normal persons were compared with that of vocal Polyp Patients with CSpeech Program for the possibility of distinguishing the pathologic vocal vibration from normal. The results were as follows ; Comparing the fundamental frequency of vocal Polyp Patients with normal persons, great change was noted only in female cases. But the Jitter and Shimmer of vocal polyp patients were greater than normal significantly in both male and female cases. SNR was lower than normal in vocal polyp patients. In the conclusion, fundamental frequency, Jitter, Shimmer and SNR might be meaningful parameters distinguisuing pathologic vibration from normal.

  • The factors involved in controlling the vocal pitch are the tension, the mass, the length of the vocal cords and the subglottic pressure. Vocal pitch can be elevated surgically by increasing the stiffness or reducing the vibrating mass of the vocal cords. Recently, we experienced a case of 25 year-old female whose vocal pitch was elevated with radiofrequency surgery of right vocal cord on suspension laryngoscopy. Elevation of vocal pitch(134.11 to 144.73) was noted two months after operation. So we report this case with review of literature.

  • Laryngeal tuberculosis is very rare, but the most common sites of laryngeal lesions include true vocal cords and false vocal cords. The major symptoms of laryngeal tuberculosis are hoarseness, dysphagia, and cough. Epiglottic tuberculosis has rarely been reported. We experienced one case of epiglottic tuberculosis that presented as acute epiglottitis and report it with reviews of literature.

  • The purpose of this study was to investigate the Digital Paradigm cords depicted on the Digital Clothing by borrowing nearby fields. Therefore, it will offer not only theoretical data but also databases for product development on functional and aesthetic views even though it is not systematically set up the idea vet. The Digital Paradigm cords of Interactivity. Nonlinearity and Combination in the Digital Clothing were determined. This study found that Interactivity Paradigm cord was expressed by the interaction of clothing and computer, the techniques of built in·attach·detach·fragment of the Digital Clothing at the external indicators, and maintained this personalizing that was pursued comprehension and the shape and silhouette of the existed clothing, that Nonlinearity Paradigm cord was able to overcome the limitation of a successively approach, in other words the discontinuous moving possibilities of time and space, that Combination Paradigm cord was contained creative concept without fixed ideas through coordination of Digital Clothing that was combination. continuous creation, changes and process between heterogeneous factors. In conclusion, introduction of the new Digital Paradigm is going to accelerate on lasting development and research.

  • Laryngeal framework surgery to improve change the voice is a challenging development in phoniatric surgery. Basically two categories can be distinguished : (1) attempted medialization of the vocal fold, as for the treatment of paralytic dysphonias (2) adjustment of the vocal fold's tension of transsexuals or mutational dysphonia. Vocal pitch can be elevated by various surgical technique 1) cricothyroid approximation 2) A-P expansion of the thyroid ala 3) longitudinal incision in the cords 4) intrachondral injection of the steroid, and 5) evaporation of the cords by $CO_2$ laser. We have experienced a case of pitch elevation procedure after transsexual operation. After transsexual operation, he had received anterior commissure laryngoplasty modified from Le Jeune with no change in voice pitch(Fo=110Hz). 8 monthes later, he had received cricothyroid approximation resulting in pitch elevation(Fo=160Hz).

Electroglottographic Spectral Tilt in Frequency Ranges of Vowel Sound (모음 주파수 범위에 따른 성문전도 스펙트럼 기울기)

  • In this study, electroglottographic spectral tilt (EST) was investigated for characterization of vocal cords vibration. EST was analyzed from the power spectrum of electroglottographic signals by dividing frequency analysis range as full range (0~4 octave), low range (0~2 octave), and high range (2~4 octave). EST of all ranges in female were greater than those in male. In female and male groups, EST of high range was higher than that of low range. This result suggests that EST has at least two components and dividing frequency range in analysis of EST is effective for investigating characteristics of vocal cords vibration.

A Case of Pyogenic Granuloma of Vocal Cords (성대에 발생한 화농성 육아종 1예)

  • Pyogenic granuloma is very uncommon disease. It is a benign, elevated, and capillary-rich lesion occupying on the skin and mucous membranes, and is a reactive lesion, an overgrowth of granulation tissue. And this lesion may grow rapidly and can recur frequently. Pyogenic granuloma usually occurs on the lip, tongue, oral mucosa, and nasal mucosa. But, pyogenic granuloma of vocal cords is very rare. Recently, we experienced a case of pyogenic granuloma of a 48-year-old man who had been presented with hoarseness for 3 months. He was diagnosed pyogenic granuloma after laryngeal microscopic surgery. So we report this rare case with review of literatures.
