MARGINAL SEALING OF AMALGAM RESTORATIONS USING DENTIN BONDING ADHESIVES (아말감수복시 상아질접착제가 변연봉쇄에 미치는 영향)

  • The purpose of this study was to evaluate microleakage of etched and non-etched amalgam restorations lined with dentin bonding system primer and adhesives. Class V amalgam preparations were made in 100 extracted human premolars divided into 10 groups with 10 teeth each. SEM was taken to assess the dentin/amalgam interface. The results were as follows; 1. At the occlusal and gingival margins, all experimental groups showed lower leakage value than unlined group(p<0.05) The $Copalite^{(R)}-lined$ group showed higher leakage value than other experimental groups(p<0.05). 2. In all experimental groups, the gingival margin showed higher leakage value than the occlusal margin. 3. There were no significant difference between non-etching and etching groups, primer only and primer & adhesive groups, primer & adhesive and self-priming adhesive groups(p>0.05). 4. On the SEM observation, continuous gaps were observed in the unlined and $Copalite^{(R)}-lined$ groups, but the gaps were filled with primer or adhesive layer in other experimental groups.

A STUDY ON THE EFFECT OF AMALGAM CAVITY LINER APPLICATION ON THE MARGINAL LEAKAGE AND RETENTION OF AMALGAM RESTORATIONS (Amalgambond Liner의 도포가 amalgam 수복재의 변연누출과 유지력에 미치는 영향에 관한 연구)

  • In this study, we tried to evaluate the effect of Amalgambond liner application on the degree of marginal leakage and retention of amalgam restoration by comparing with that of the Copalite and All-bond 2. The results obtained from this experiment were as follows; 1. Tensile strength representing the bond between amalgam and tooth structure was the highest in Amalgambond, and All-Bond 2, Copalite in descending order. There were statistically significant difference between each group(P<0.05). 2. The degree of microleakage in Amalgambond was lower than that of All-Bond 2, and Copalite, but no stastically significant difference could be found (P>0.05). 3. The liner such as Amalgambond or All-Bond 2 were evaluated to be superior to the conventional Copalite in bond strength as well as in microleakage. But the result of this study could not show the superiority of one material over the others; Amalgambond and All-bond 2. Besides the results of the study, other factors, such as practical convenience, should be considered in determining the selection of material. The support of welldesigned clinical studies on this subject are in demand.

THE EFFECT OF THE POLYSTYRENE LINER ON MICROLEAKAGE OF AMALGAM RESTORATION (Polystyrene 이장재가 아말감 변연부 미세누출에 미치는 영향)

  • Amalgam is one of the most commonly used dental restoration material because of its convenience, economic and physical properties. But microleakage in the tooth and amalgam interface has been its major problem, and many efforts have been made to overcome this shortcoming. The purpose of this study is to compare the effect of various liners on microleakage of amalgam restoration. Cavities were prepared on the buccal or lingual surface of ninety sound, extracted human premolars and six different liners (Tubulitec$^{(R)}$, Superbond D-liner II Plus$^{(R)}$, Superbond D-liner II Plus$^{(R)}$ with polymer, Scotchbond Multipurpose Plus$^{(R)}$, Copalite$^{(R)}$, No liner) were unapplied according to manufacturer's instructions and amalgam had been condensed immediately. The specimens were thermocycled by dipping in methylene blue dye at $5^{\circ}C$ and $55^{\circ}C$ for 1500 cycles. The amalgam-tooth interface was examined under stereobinocular microscope and the dye penetration was scored. The results were as follows : 1. The Tubulitec$^{(R)}$ group showed less microleakage than no liner or Copalite$^{(R)}$ group (p<0.01). 2. The Tubulitec$^{(R)}$, Superbond D-liner II Plus$^{(R)}$, Superbond D-liner II Plus$^{(R)}$ with polymer and Scotch bond Multipurpose Plus$^{(R)}$ groups were not significantly different. 3. The Copalite$^{(R)}$ and Scotchbond Multipurpose Plus$^{(R)}$ groups were not significantly different. Using the polystyrene liner and resin liners under admixed type of high-copper amalgam restoration significantly reduced microleakage in the tooth-amalgam interface. Further clinical studies on polystyrene liner are recommended.

  • The purpose of this study was to compare the marginal leakage of resin liner which was used as a liner to amalgam restoration. Control group composed of no liner group, copalite group and experimental group composed of All-bond 2, Vitrebond, Superbond D-liner applied groups were evaluated. Cl.V preparation with a size of $3{\times}2{\times}1.5mm$ on 70 extracted primary molars were made and applied appropriate liners for each groups. After amalgam filling and polishing, polished specimens underwent temperature changed from $5^{\circ}C\;to\;55^{\circ}C$ a thousand times. After thermocycling, specimens were placed in 2% methylene blue dye solution for 72 hours in an incubator set at $37^{\circ}C$. The teeth were sectioned buccolingually and the degree of dye penetration was observed with a spectroscopy. (${\times}30$) The following results were obtained. 1. Vitrebond, All-bond 2, Superbond D-liner group exhibited a statistically significant lower degree of dye penetration than no liner and copalite group. (Kruskal-Wallis analysis, P<0.05) 2. No liner group and copalite group exhibited a similar degree of dye penetration. (Mann-Whitney analysis, P>0.05) 3. All-bond 2 group exhibited a statistically significant lower degree of dye penetration than Vitrebond group, (Mann-Whitney analysis, P<0.05) Superbond D-liner group also exhibited a lower degree of dye penetration than Vitrebond group but was statistically insignificant. A STUDY OF CAVITY VARNISH APPLICATION ON THE MARGINAL LEAKAGE OF THE VARIOUS DENTAL RESTORATIONS (치과용 Varnish가 충전재의 변연누출에 미치는 영향에 관한 연구)

  • This study was undertaken to evaluate the effects of varnish application and thermocycling on the marginal leakage. 240 cavities of Class V were prepared on the 120 extracted premolars, and the cavities were filled with amalgam, Heliosit$^{(R)}$, and Hipol$^{(R)}$ after application of Copalite$^{(R)}$ or Duraphat$^{(R)}$. All specimens were immersed in methylene blue solution for 24 hours after thermocycling at $4^{\circ}C$ and $60^{\circ}C$, embedded in acrylic resin, and sectioned with low speed saw into two parts. The sectioned specimens observed with the metallurgical microscope. The following results were obtained: 1. The marginal seal was more effective in the amalgam-filled cavities after application of Copalite$^{(R)}$ threetimes or an application of Duraphat$^{(R)}$ varnish than in those without varnish applications. 2. Of the composite resin-filled cavities, the leakage of the varnish applied cases showed much more than without application. 3. There was no significant difference in the effect of marginal sealing between Copalite$^{(R)}$ and Duraphat$^{(R)}$ applications. 4. There was a significant difference in the degree of the marginal leakage of amalgam-filled cavities between the groups of thermocycling times, but no significant difference in the resin-filled cavities. 5. There was no significant difference in the degree of the marginal leakage between the Heliosit$^{(R)}$ and the Hipol$^{(R)}$ resin-filled cavities.

  • The sealing effect of a few cavity liners upon the dentinal tubule were studied in vitro. The materials employed in this study were Silcot (SP$\'{E}$CIALIT$\'{E}$S SEPTODONT, M.-T. GENDRAULT, Pharmacien), Hypo-Cal(Ellman Dental Mfg.Co.Inc.), Cavity Lining(De Trey), and Copaute(Harry J. Bosworth Co.). Freshly extracted human teeth were devided into 5 groups by age-under twenty, twenties, thirties, forties, and over fifty. Class V cavities were prepared routinely. The cavity walls of eight teeth of each group were lined by Silcot, Copalite, Cavity Lining, and Hypo-Cal. Remaining eight were not lined as a control. These specimens were immersed in dye solution (2 gm eosin Y to 800 cc distilled water) for 48 hours to allow maximum dye penetration into dentinal tubules. Each specimen was sectioned longitudinally including Class V cavity floor under water spray. Dye penetration into dentinal tubules were examined and following results were obtained. 1. Liners used on this study showed more or less dye penetration into dentinal tubules. But compared with the teeth without lining, the dye penetration of lined specimens were decreased. 2. Of these liners tested, Silcot was the most effective sealer upon the dentinal tubules. Copalite was the moderate sealer and Cavity Lining showed a tendency similar to Copalite. Hypo-Cal revealed the greatest dye penetration. 3. As the age was increased, the more the dye penetration into the dentinal tubules was decreased.

  • The purpose of this study was to evaluate the marginal adaptability of the amalgam restorations in applying the cavity varnish (Copalite$^{(R)}$) and dentin bonding agent (Scotchbond 2$^{(R)}$) under the scanning electron microscope. For this study, eighteen sound extracted human molars were selected. Class I cavities in 12 teeth and class V cavities in 6 teeth were prepared using an air turbine with No. 701 tungsten carbide bur and finished using a low speed handpiece with No. 557 fissure bur. The prepared specimens were then divided into three groups including 4 class I cavities and 2 class V cavities in each group and restored as follows ; Group I. All the prepared cavities were restored with amalgam only (Control). Group II. Two layers of Copalite$^{(R)}$ cavity varnish were applied to the cavities with a gentle stream of air after each application and cavities were restored with amalgam. Group III. The enamel cavity margins were etched with 37% phosphoric acid gel for 60 sec., rinsed for 30 sec. and dried. One layer of visible lightcured Scotchbond Dental Adhesive$^{(R)}$ was applied and immediately cured for 20 seconds with visible light-cure unit and cavities were restored with amalgam. All the specimens were cut at the neck of the teeth and the occlusal halves of specimens were sectioned buccolingually in the longitudinal axis centering the amalgam restorations, using the disk. The cut specimens were ground with sandpapers (400, 600, 800, 1000 grit), and cleaned for 5 minutes in the ultrasonic cleaner (Brason Co. U.S.A.). In the cut surfaces, the amalgam - tooth interfaces were examined under the scanning electron microscope (JSM, 35C type, JEOL). The obtained results were as follows ; 1. The amalgam-tooth interfaces were reduced more significantly in the Copalite$^{(R)}$ and Scotchbond 2$^{(R)}$ application group than in the control group. 2. In the class I cavities, the Scotchbond 2$^{(R)}$ application group showed the findings similar to the Copalite$^{(R)}$ application group in the cavity floor, and the marginal adaptability was better in the side wall than in the cavity floor. 3. In the class I cavities, the Scotchbond 2$^{(R)}$ application group showed better marginal adaptability in the occlusal margin than in the gingival margin. 4. The marginal adaptability was in the order of the Scothbond 2$^{(R)}$ application group, the Copalite$^{(R)}$ application group and the control group.

A STUDY ON THE GALVANIC CURRENT BETWEEN GOLD AND AMALGAM (아말감과 금합금의 Galvanic 전류 측정에 관한 실험적 연구)

  • It was the purpose of this study to determine the galvanic current between a gold alloy inlay and four types of amalgam using the circuit through pulp chambers when the freshly extracted teeth with those restorations were brought into contact in a physiologic saline solution, and to investigate the effectiveness of cavity varnish or ammonated silver-nitrate on the surface of amalgam restoration in reducing galvanic current. The current was measured with current-to-voltage converter and recored on a physiograph 6630-257. The following results were obtained. 1. Generally, galvanic current decreased as the time elapsed. 2. Galvainc current decreased significantly in the first day and after then minimal change was observed until 30th day. 3. Initial galvanic current was 29.6 ${\mu}A$ in the cut amalgam and 24.5 ${\mu}A$ in Dispersalloy amalgam and after then the current was significantly decreased. 4. Initial galvanic current was 12.6 ${\mu}A$ in spherical amalgam (low copper amalgam) and 13.8 ${\mu}A$ in Tytin amalgam and the amount of change was lower in sperical amalgam and Tytin amalgam than that in lathe cut amalgam and Dispersalloy amalgam. 5. Painting ammoniated silver-nitrate or Copalite on the surface of amalgam resotration decreased initial galvanic current and ammoniated silver-nitrate is more effective in decreasing galvanic current than Copalite. 6. Galvanic current by contact between amalgam restoration and gold restoration increased abruptly and dropped rapidly becoming almost.

MICROLEAKAGE EVALUATION IN AMALGAM RESTORATIONS USED WITH BONDING AGENTS (수종의 접착제를 사용한 아말감 수복시의 미세 변연 누출에 관한 연구)

  • The purpose of this study was to evaluate the effectiveness of copal varnish and five bonding agents(Scotchbond Multi-Purpose Plus Dental Adhesive System, Panavia 21, All Bond 2, Superbond D-Liner II plus, Fuji Duet) in reducing microleakage under amalgam restorations. Class V cavity were prepared on both buccal and lingual surface of sixty extracted human molars with cementum margin and were filled with different kinds of liners and amalgam. All teeth were stored at $37^{\circ}C$ in physiologic saline solution. After 1 week they were thermally stressed for 500 cycles between 5 and $55^{\circ}C$ in baths containing 0.5 % basic fuchsin dye. The dye penetrations were observed with a stereomicroscope. Scanning electron micrographs were taken of representative tooth sections from each group to assess the nature of the tooth/liner/amalgam interface. The statistical test were applied to the results using a one way analysis variance (ANOVA) and Duncan's multiple range test. The results were as follows ; 1. In all groups, the enamel margin showed significantly lower leakage value than the cementum margin (p<0.05). 2. At the dentin and enamel margins, the leakage value of Copalite-lined group showed significantly higher than that of no liner group (p<0.05), but showed significantly lower than that of bonding agent lined-groups (p<0.05). 3. There was no significant difference between the bonding agent lined-groups (p>0.05). 4. On the backscattered scanning electron microscopy observation, discontinuous gaps were observed between amalgam and dentin in the bonding agent-lined amalgam restorations and the bonding agents appeared to fill the gap space and were mixed with amalgam particles. At the amalgam/tooth interface, unsealed dentin and continuous gaps were found in both unlined and Copalite-lined restorations.

  • The purpose of this study was to evaluate the effects of bleaching agent through the dentinal tubules of cervical area in the intracoronal bleaching of pulpless teeth on cutured fibroblast cells. Extracted human incisors were enlarged to # 40 K-file and obturated with gutta-perella and AH 26 sealer. The gutta-percha was removed to 2mm below the cementoenamel junction of the root The teeth were divided into 3 experimental and control groups. Experimental groups; Experimental group 1: Temporary inlay wax filld with 30% $H_2O_2$ in pulp cavity. Experimental group 2: Temporary inlay wax filld with 30% $H_2O_2$ in pulp cavity after placement of ZOE cement to cementoenamel junction. Experimental group 3: Temporary inlay wax filld with 30% $H_2O_2$ in pulp cavity after application of Copalite to cementoenamel junction. Control group: Temporary inlay wax filled without 30% $H_2O_2$ in pulp cavity under the same condition at each experimental group. Each tooth was immersed in well of multidish cultured fibroblast cell for 48 hours. The cellular multiplication and cell viability were calculated at the interval of 1, 3, 5. 7 hours and the morphological changes in well were observed and their photographs were taken with inverted microscope. The obtained results were as follows : CD The cellurar multiplicaton and cell viability decreased in all experimental groups at 1 hour after experiment and the morphology of fibroblast cell was changed from star shape to round (2) The cell viability was lowered to 34 % in experemental group 1, 44 % in experimental group 2, and 38 % in experemental group 3 at 3 hours after experiment (3) The cell multiplication was decreased to 54% in experemental group 1. 47% in experimental group 2, and 40% in experemental group 3 at 7 hours after experiment. (4) The decrease of cell number and morphological changes of fibroblast cell were remarkable in experimental group 1, group 3 and 2 in order. These results suggest that the fibroblast cells receive severe damage by 30% $H_2O_2$ solution leaked through the dentinal tubules and the dentinal tubules are able to be obturated better by ZOE cement than by Copalite.
