The Cost Effectiveness Analysis of Health Promotion Policy: WHO-CHOICE Programme (WHO-CHOICE 프로그램: 건강증진정책의 비용효과분석 도구)

  • Objectives: Objectives: The objective of this study is to describe the WHO-CHOICE(World Health Organization- CHOosing Interventions that are Cost-Effective) programme, and to consider the application of WHO-CHOICE programme in Korea, especially on the health promotion policy. Methods: Literature review was conducted on the contents of WHO-CHOICE programme in the previous studies, guidebook, and software. We also contacted WHO-CHOICE team at WHO to identify the contents not clearly presented in the documents. Results: The WHO-CHOICE programme is a standardized tool for analyzing and comparing the cost effectiveness of health promotion policies. It is composed of PopMod to measure the health effect of intervention and of CostIt to measure the cost. The cost of tobacco control policy in Korea was analyzed with the cooperation of WHO-CHOICE team preliminary, and the results were different with the results of tobacco control policy on western pacific region of WHO. Conclusions: The cost effectiveness study based on WHO-CHOICE programme could help decide a priority of health promotion policy for settings with limited resources. For the improvement of health, the future work on WHO-CHOICE programme need to be considered.

  • We consider some dependability management systems(IEC300, NASA AHB5300.1 and DIN VDE 0801) with reference on IEC300. Part I of IEC300(IEC300-1) covers the essential features of a comprehensive dependability programme for the planning, organization, direction, and control of resources to produce products which will be reliable and maintainable. We consider the contents of IEC300-1; scope, normative references, definitions, management responsibilities, product or project independent programme elements and project specific programme elements.

Knowledge and Practices on Breast and Cervical Cancer Screening Methods among Female Health Care Workers: A Sri Lankan Experience

  • Breast and cervical cancer are the most common causes of cancer mortality among women worldwide, but they are largely preventable. There are limited data on knowledge and practices on screening methods of breast and cervical cancers among female health care workers in Sri Lanka, in spite of having an organized screening programme islandwide. A cross-sectional survey was conducted among 219 female health care workers including public health midwives (68.9%) selected from 6 districts in Sri Lanka using convenient sampling methods. A self-administered questionnaire was used as a pre-test in a capacity building training programme to collect the data. The mean (SD) duration of work experience of the respondents was 12 years and 52.5% were aged over 35 years. Most (76.7%) were married, and afamily history of cancer was reported by 24.2%. Over 98% knew about self breast examination. Even though 84.1% practiced it, only 47.9% practiced it on a monthly basis. Clinical breast examination and mammography were known by 94.1% and 64.3% respectively. Only 19.2% had undergone a clinical braest examination within one year and 3.6% had ever undergone a mamography. Only 76.3% knew that a Pap smear detects precancerous stage of cervical cancer. Among 169 married workers, 73.4% had never had a Pap smear and only 17.2% had got it done within the preceding 5 years. Among the reasons for not doing a pap smear within 5 years, 47.0% belived it as not nescessary, 17.3% due to fear/dislike, 23.2% as not having symptoms, 3% had not known about it and 3% not known about availability of services. The study findings suggest that the knowledge and practices on breast and cervical cancer screening methods among female health care workers need to be improved. Considering the role that health care workers play in communicating health behaviors to the general public, strengthening health education interventions for this group of females is essential.

Characteristics of the Proportional Pressure Control Valve for 4 Wheel Steering System on the Passenger Car (승용차 4륜 조향(4WS) 장치용 비례 압력 제어 밸브의 특성에 관한 연구)

  • The proportional pressure control valve(PCV) is an essential component in the open loop controlled rear wheel steering gear of the four wheel steering(4WS) system on the passenger car. The valve should have versatile functions and higher performance. But, it is hard to find the proportional pressure control valve suitable for the 4WS system. In this paper, the determination of the valve parameters was studied by the stability discrimination and the characteristic analysis for the purpose of the development of a new PCV for the 4WS. The mathematical model of the valve was derived from the valve-cylinder system and the programme for numerical computation was developed. The transfer function of the system was obtained from the mathematical model. The characteristics of the valve were inspected through the experiment and compared to those obtained by numerical method. And then the stability discrimination of the system was done by root locus and the analysis of characteristics was done by the developed programme. From the experiment and the analysis of characteristics was done by the developed programme. From the experiment and the inspection, the appropriation of mathematical model and the usefulness of the programme were confirmed. And the parameters which might affect the performance of the valve can be determined by considering the stability discrimination, the characteristics analysis and required functions.

Evaluation of Acute Respiratory Infections(ARI) Control Programme in a Korean Rural Community -The Patterns of Antibiotic Prescription- (한 농촌지역에서 실시한 소아 급성호흡기감염 관리사업의 평가 -항생제 사용을 중심으로-)

  • The purpose of this study was to evaluate the program for the control of acute respiratory infections(ARI) in children in a Korean rural area(Yonchon county). Evaluating the program, we focused on the pattern of prescription and appropriateness of antibiotics prescribed by the health personnel who had participated in the ARI Control Program. It was implemented at the primary health care setting in rural area, such as district hospital, health subcenters, and health posts. During six-months programme monitoring period, medical records were reviewed and collected data were analysed by the pediatrician, research coordinator of this study. The baseline data were collected from medical records of the same period(six months) of one year before the implementation of the ARI programme. The study results were as follow : 1. Common cold was the most prevalent disease(78.7%. 594 cases) among the all ARI cases (755 cases). The less frequent cases were bronchitis(11.9%), acute pharyngitis(5.2%), and pneumonia(1.8%). 2. Significant reduction in the use of antibiotics was observed after the programme implementation. Ninety three(15.7%) of 594 common cold cases were received antibiotics compared with 282(35.2%) of 802 in the baseline period. In the cases of bronchitis and acute pharyngitis, the reduction rates were 15.1% and 23.2% respectively compared to the baseline period. 3. Mean duration of antibiotics prescription was 1.81-1.75 days, similar to the baseline data. 4. The appropriateness rate of antibiotics prescriptions were 84.3%(common cold), 35.6% (bronchitis) and 28.2%(acute pharyngitis). In the case of pneumonia, the antibiotics prescription was compatible to the criteria developed. 5. Pediatrician prescribed antibiotics more appropriately for all cases than general practitioners in health sub-center, and nurse practitioners in health posts. 6. Antibiotics therapy was shown to be of no effect in the treatment of the all ARI cases. At the 5 and 10 days check-up of common cold cases after visits, proportion of improved patients were 58.3% in the antibiotics-used group and 51.4% in the control group. In the other cases of ARI, the patterns of response were similar to common cold. None of the differences in outcome between the antibiotics-used and control group was statistically significant. This ARI programme may have substantial a substantial impact on antibiotics use at the public health institutions(district hospital, health subcenters, health posts) which are of major domain for primary health care in Korean rural areas.

Effects of Integrated Dementia Managing Programme for the Elderly with Mild Dementia in Nursing Home (요양시설 입소 경증치매노인의 통합 치매관리 프로그램의 적용 효과)

  • Purpose: The purpose of this study was to verify the effects of the integrated dementia managing programme for the elderly with mild dementia in which the physiological, cognitive, emotional, sociological and spiritual aspects have considered. Methods: This study employed non-equivalent control group pre-post tests with the repeated measure design. The subjects of the study were 39 elderly with mild dementia (20 in the experimental group and 19 in the control group) who have admitted to two nursing homes in B city. The data were analyzed by repeated measures ANOVA. Results: There were significant interactions between two groups and times in self-efficacy (p=.011), cognitive function (p<.001), depression (p=.005), spiritual well-being (p=.002), and quality of life (p=.037). The integrated dementia managing programme of this study showed significant positive effects on the quality of life for the elderly with mild dementia. Conclusion: On considering the current trends of aging and increasing number of the elderly with dementia, this integrated programme would be highly recommended to be used in nursing homes for the elderly with mild dementia, and contribute to improving their quality of life, and saving the social and medical expenses as well.

  • Medium size rotorcraft consist of approximately 5,000 key components. The components must satisfy the target weight and performance requirement through optimization. Therefore a rotorcraft component has approximately 100 times higher unit cost for mass production than the equivalent of an automobile's, along with huge development cost. Configuration management consists of four main parts : configuration identification, configuration control, configuration audit, and data management. In development programme, configuration management is one of the methods to save development cost, and setting a well-structured configuration management system according to the Military Standard is an important factor in success of the programme. The paper proposes a configuration management system suitable for development of various multi-use components development and presents that the configuration management was performed successfully during dual-use component development programme using the established system.

Aortic Remodelling in Chronic Nicotine-Administered Rat

  • Vascular remodelling is an adaptive mechanism, which counteracts pressure changes in blood circulation. Nicotine content in cigarette increases the risk of hypertension. The exact relationship between nicotine and vascular remodelling still remain unknown. Current study was aimed to determine the effect of clinically relevant dosage of nicotine (equivalent to light smoker) on aortic reactivity, oxidative stress markers and histomorphological changes. Twelve age-matched male Sprague-Dawley rats were randomly divided into two groups, i.e.: normal saline as control or 0.6 mg/kg nicotine for 28 days (i.p., n=6 per group). On day-29, the rats were sacrificed and the thoracic aorta was dissected immediately for further studies. Mean arterial pressure (MAP) and pulse pressure (PP) of nicotine-treated vs. control were significantly increased (p<0.05). Nicotine-treated group showed significant (p<0.05) increase tunica media thickness, and decrease in lumen diameter, suggesting vascular remodelling which lead to prior hypertension state. The phenylephrine (PE)-induced contractile response in nicotine group was significantly higher than control group ($ED_{50}=1.44{\times}10^5M$ vs. $4.9{\times}10^6M$) (p<0.05~0.001). However, nicotine-treated rat showed significantly lower endothelium-dependent relaxation response to acetylcholine (ACh) than in control group ($ED_{50}=6.17{\times}10^7M$ vs. $2.82{\times}10^7M$) (p<0.05), indicating loss of primary vascular function. Malondialdehyde (MDA), a lipid peroxidation marker was significantly higher in nicotine group. Superoxide dismutase (SOD) enzymatic activity and glutathione (GSH) were all reduced in nicotine group (p<0.05) vs. control, suggesting nicotine induces oxidative imbalance. In short, chronic nicotine administration impaired aortic reactivity, probably via redox imbalance and vascular remodelling mechanism.

A Study on the Improvement of On-board Training Program through the Analysis of Satisfaction Level

  • The educational process and result of onboard training should be evaluated according to the 1995 Amendments to the International Convention on Standards of Training, Certification and Watch-keeping for seafarers(STCW), 1978. In particular, the revised Convention requires that a trainee's seagoing service must be recorded in each cadet's Training Record Book approved by the maritime administration responsible for the issuance of certificates of competency. Trainees for certification under regulation III/1 of the STCW Convention are required to complete an approved on-board training programme. The purpose of this paper is to understand the compliance of the education for an approved on-board training programme. The questionnaire was distributed among 110 cadets being trained on board the training ship of the maritime college of the Mokpo National Maritime University. In this study, we conducted the questionnaire survey which is related to the on-board training programme such as marine engineering; controlling the operation of the ship and care for persons on board; electrical, electronic and control engineering; etc. The survey revealed that onboard training program was normally satisfactory, however, lack of practical training tools and time have accounted for most of the reasons for dissatisfaction. Therefore, it is our goal to enhance the satisfactory value of onboard training education by analyzing the reason of the dissatisfaction.
