고려인삼의 주요 효능과 그 임상적 응용 (Clinical Applications and Efficacy of Korean Ginseng)

  • 본 총설은 1980년대부터 최근까지 국내외 학술잡지나 심포지움 등에 발표된 인삼의 주요 임상효능 연구결과를 요약 고찰하고, 그 임상 적응증과 안전성 등에 대한 검토를 통해 금후 인삼의 임상적 활용성 제고와 인삼의 진정한 약용가치 평가를 위한 임상연구의 발전에 도움을 주고자 하였다. 당뇨병을 비롯한 동맥경화성 질환, 고혈압, 악성질환, 성기능 장애 등의 만성질환에 대한 그 동안의 연구결과는 인삼의 치료효과보다는 예방 및 회복제로서의 효용성을 보여 주었다. 특히 이들 질환의 각종 자각적 장해증상과 장기 약물치료의 부작용으로 인한 QOL의 악화에 에 대한 개선효과가 관찰되었다. 그러나 인삼의 효과는 일반적으로 mild 하여 일차적 치료보다는 관행적 약물요법과 병용할 때 보조요법제로서 또는 부작용을 보다 적게 하는 효과가 기대된다. 또한 주요 강장효능과 관련하여 작업수행능력에 미치는 임상연구 결과는 인삼복용이 각종 스트레스 상태하의 신체적 조건에 대한 적응능력을 개선시켜 육체적 정신적 기능저하를 회복시키는 효과를 보였다. 이러한 임상시험에서 얻어진 결과가 그대로 인삼의 적응증(indication)이라고 단정할 수는 없으며, 그 효능의 과학적 증거들에 대해서는 아직도 논란이 많고, 임상실험의 유효성 평가와 관련된 방법론적 문제점도 많이 지적되고 있다. 보다 확실한 적응증 제시를 위해서는 표준화된 인삼시료를 이용하여 보다 체계적인 시험설계에 의한 객관적 효능평가가 필요하다. 한편 인삼(제품)복용에 의한 부작용(adverse effects)의 발생 가능성에 대한 사례보고들도 대부분 인삼의 과량복용이나 품질관리 미흡에서 기인되는 것으로 여겨지고 있다. 최근 해외 시장에서 유통되는 인삼제품 품질검사에서 사포닌 성분의 불검출 또는 함량 미달과 유해성분의 오염가능성 등 부정적 견해들이 다수 보고되었다. 그러나 표준화된 인삼제품의 추천 복용량을 사용한 대부분의 임상실험에서는 거의 유의할만한 부작용은 인정되지 않았다. 금후 연구와 관련하여, 품질표준의 지표성분으로 간주되는 진세노사이드의 절대함량과 그 성분조성 차이에 따른 임상효과의 차별성이 있는지에 대한 검토와, 특히 최근 실험적으로 밝혀지고 있는 사포닌 성분의 장내 세균에 의한 생물전환체의 인체 실험을 통한 효과 검정이 필요하다. 나아가서는 적정 복용량의 설정과 이와 관련되는 생체내 동태 및 생체이용율(bioavilability)에 관한 정보가 거의 없으므로 이것도 금후 검토해야 할 과제로 사료된다. 인삼은 전통약물로서 오랜 역사성과 그동안의 연구결과에 의한 과학성을 가지고 있으므로 건강유지와 병의 예방 및 회복촉진을 위한 보조요법제 또는 기능성 식품으로써의 유용성이 있는 것으로 판단된다. 앞으로 인삼의 활용성 증대를 위해서는 보다 과학적인 임상평가에 의한 안전성 및 유효성 입증과 제품의 엄격한 품질관리의 필요성이 더욱 강조되어야 할 것이다.

과맥의 주요형질에 대한 조합능력 및 유전에 관한 연구 (Studies on Combining Ability and Inheritance of Major Agronomic Characters in Naked Barley)

  • 과맥의 조숙단간다수성 품종육성에 관한 기초자요를 얻고저 고맥에서 주로 교배모본으로 많이 사용되어온 애원과 001, 사국과 4002, 산수과, 영성과, 향천과 001, 장주백과, 백동, 청맥, 세도하다가, 목포 4002등 10품종을 1974연에 diallel cross하여 1975연부터 1976연까지 작물시험장 목포지장 시험포장에서 양친, F_1과 F_2를 재배하여 출수기, 간장 및 주당수량을 조사하여 그들의 유전율, 조합능역과 유전에 대하여 분석검토한 결과를 요약하면 다음과 같다. 1. 광의의 유전율은 출수기, 간장 및 수량이 각각 0.7831, 0.7599, 0.6061로서 비교적 높았고 협의의 유전율은 출수기가 F_1에서는 0.3972, F_2에서는 0.7789였으며 간장은 F_1에서 0.6567, F_2에서 0.6414로 매우 높아서 초기세대에서 선발이 유효할 것이나 주당수량은 F_1이 0.3776, F_2에서 0.4170으로 비교적 낮았다. 2. 출수기에 대한 GCA는 F_1 및 F_2에서 애원과 001, 사국과 4002, 산수과, 영성과는 조생방향으로 기타품종은 만생방향으로 높았으며 F_1의 GCA는 F_2 보다 높았다. SCA는 사국과 4002$\times$목포 애원과 001\times산수과, 사국과 4002\times산수과, 사국과 4002\times영성과 조합은 F_1과 F_2에서 모두 조생 방향으로 높았다. 간장의 GCA는 F_1 및 F_2에서 모두 목포 4002, 영성과, 산수과, 사국과 4002는 단간방향으로 높았으며 기타품종은 장간방향으로 높았고 F_1의 GCA는 F_2보다 높았다. SCA는 가장 단간인 목포 4002와 애완과 001, 산수과 및 영성과와의 조합에서 단간방향으로 높았다. 4. GCA는 F_1 주당수량이 높은 백동, 향천과 001 산수과, 영성과, 사국과 4002등이 다수방향으로 높았고 기타품종은 저수방향으로 높았으며 F_1의 GCA는 F_2보다 다소켰다. SCA가 가장 큰 조합은 목포 4002를 편친으로 한 사국과 4002, 세도하다가, 청맥과의 조합이었다. 5. F_1 및 F_2의 평균출수기는 mid-parent보다 조생이였으며 F_1이 F_2보다 조생이었고 이러한 경향은 조생품종일수록 높고 만생품종일수록 낮았다. 6. 출수기의 유전은 조생방향으로 부분우성이며 대립유전자의 작용이 크고 품종간의 우열관계는 애원과 001, 사국과 42경, 향천과 001, 목포 4002가 다른 품종에 대하여 열성으로 나타났다. 7. F_2에서 출수기유전은 45조함중 33조함은 소수 유전자 개념으로는 설명할 수 없고 1개조함은 조생 대 만생이 3 : 1, 1개 조합은 만생 대 조생이 3 : 1 로 분리하며 4개 조합은 조생 대 만생이 9 : 7, 6개 조합은 만생 대 조생이 9 : 7로 분리하는 것으로 나타났다. 8. F_2에서 출수기에 대하여 조생개체의 초월분리가 많은 조합은 영성과\times백동, 애원과 001\times세도하다가. 산수과\times향천과 001, 애원과 001\times향천과 001, 사국과 4002, \times향천과 001, 향천과 001\times세도하다가, 애원과 001\times사국과 4003, 애원관 001\times영성과 등이였다. 9. F_1 및 F_2의 평균간장은 양친의 간장 범위내에서 mid-parent보다 F_1 및 F_2가 다소 큰 영향을 보였으며 이러한 경향은 단간품종에서 크고 장간품종에서 적었다. 10. 간장유전은 부분우성이며 대립유전자의 지배를 받고 상가적 유전자의 작용이 큰 것으로 나타났으며 극단간인 목포 4002는 열성, 장간인 백동은 우성인 것으로 나타났다. 11. 간長에 대한 $F_2에서의$ 분리양상을 보면 대부분의 조함에서 정규분포를 보여 유전자의 상가적작용이 클 것으로 보이며 단간 또는 장간으로 초월분리를 보인 조합이 많았다. 애원과 001, 사국과 4002, 영성과, 세도하다가 등의 중간\times중간조합에서 단간개체의 분리가 많았다. 12. F_1 및 F_2의 평균주당수양은 mid-paren에 비하여 F_1에서 6% F_2에서 5% 높았으며 주당수량이 많은 품종에서는 증수율이 낮고 주당수량이 적은 품종에서는 증수율이 높았다. 13. 주당수량에 대하여는 Wr, Vr graph에서 비대입유전자작용이 컷고 초월우성을 보였으며 품종간의 우열관계는 목포 4002의 저수성이 열성을 나타났으나 유전현상의 정확한 추정은 국난하였다.

창업보육서비스에 따른 입주기업의 창업보육센터 의존도에 관한 연구 (A Study on Startups' Dependence on Business Incubation Centers)

  • 창업보육센터는 창업을 준비하는 예비창업자 및 신규 창업자에게 사업을 영위하는데 필요한 자원 및 서비스를 제공하고, 사업을 수행하는 과정에서 발생되는 다양한 문제의 해결에 도움을 주고 있다. 그러나 이러한 지원은 상황에 따라 기업의 자생력을 높이기보다는 창업보육센터에 대한 의존도를 높이는 경향이 있다. 본 연구는 창업보육서비스를 제공형태에 따라 인프라 지원, 네트워크 연계 지원, 직접 지원으로 분류한 후에 이들 서비스의 제공이 입주기업의 창업보육센터 의존도에 미치는 영향관계를 살펴보고, 아울러 입주기간에 따른 조절효과를 살펴보았다. 연구결과 세 가지 보육서비스 모두 의존도에 영향을 미치는 것으로 파악되었으며, 입주기간에 따른 조절효과는 기간이 늘어날수록 네트워크 연계 지원서비스에 정의 효과가 있는 반면 직접 지원서비스는 부의 효과가 있는 것으로 파악되었다. 이러한 결과는 입주기간이 늘어감에 따라 직접 지원서비스의 경우에는 입주기업이 센터의 자원을 흡수하여 기업의 역량으로 만들어 가지만, 네트워크 연계 지원서비스는 기업의 센터에 대한 의존도를 높이고 있다는 것을 보여준다. 따라서 신생기업이 자생력을 갖기 위해서는 보육서비스를 보육기간에 따라 차별적으로 제공해야 한다는 시사점을 제시하고 있다.

폐암 세포에서 기저 상태와 TNF-${\alpha}$ 자극 시 NF-${\kappa}B$의 활성화 (Activation of NF-${\kappa}B$ in Lung Cancer Cell Lines in Basal and TNF-${\alpha}$ Stimulated States)

  • 연구배경 : Nuclear factor ${\kappa}B$ (NF-${\kappa}B$)는 면역기능, 급성기 반응, 세포주기 조절 등 다양한 세포활동을 조절하는 전사인자로서 외부 자극에 의해 세포질에 존재하던 NF-${\kappa}B$가 핵 속으로 이동되어 여러 유전자의 ${\kappa}B$ element에 결합하여 그 유전자의 전사를 가져온다. 최근 들어 암의 발생과 증식 및 전이에 있어 NF-${\kappa}B$의 역할이 주목받고 있다. 즉, 여러 종류의 암세포에서 NF-${\kappa}B$의 과발현 및 지속적인 활성화가 알려져 NF-${\kappa}B$와 암의 발생 및 증식과의 관련성이 제시되고 있고, NF-${\kappa}B$의 항 아포프토시스 기능은 암세포의 생존에서 중요한 역할을 하는 것으로 이해되고 있다. 또한 ICAM-l, VCAM-l 등 세포 부착물질의 발현에 영향을 끼쳐 암 전이와의 관련성도 제시되고 있다. 폐암에서 NF-${\kappa}B$의 역할에 관한 연구는 많지 않은 상태로 폐암 조직 및 폐암 세포주에서 p50과 c-Rel의 과발현이 보고된 바 있다. 그러나 NF-${\kappa}B$의 과발현이 NF-${\kappa}B$의 활성화를 의미하는 것은 아니며 현재까지 폐암 세포에서 NF-${\kappa}B$의 활성화 유무에 관한 연구는 없는 실정이다. 방 법 : 본 연구에서는 정상 기관지 세포주와 폐암 세포주에서 기저 상태와 외부 자극에 의한 NF-${\kappa}B$의 활성화를 비교하여 폐암 세포에서 NF-${\kappa}B$의 활성도를 평가하였다. 정상 기관지 상피세포로는 BEAS-2B 세포주를 사용하였고 폐암 세포주로는 A549, NCI-H358, NCI-H441, NCI-H522, NCI-H2009, NCI-H460, NCI-H1229, NCI-H1703, NCI-H157, NCI-H187, NCI-H417, NCI-H526 등 12종을 실험에 사용하였다. NF-${\kappa}B$의 활성화는 p65와 p50의 핵내 발현과 electrophoretic mobility shift assay (EMSA)를 이용한 NF-${\kappa}B$ DNA binding activity로 평가하였다. 결 과 : NCI-H358과 NCI-H460 세포를 제외한 모든 폐암 세포주와 BEAS-2B 세포의 기저상태에서 핵 단백질내에 p65와 p50의 발현이 관찰되었다. TNF-$\alpha$로 자극하고 30분이 경과한 후에는 핵 내 p65와 p50의 발현이 증가하였다. NCI-H358과 NCI-H460 세포에서는 기저 상태와 TNF-${\alpha}$ 자극 시 핵 단백질 내의 p65의 발현이 관찰되지 않았고 TNF-${\alpha}$ 자극했을 때에도 p65의 발현은 증가하지 않았다. 그러나 이 두 세포주에서는 TNF-${\alpha}$로 자극 시 p50보다 분자량이 작은 두 종의 단백질의 발현이 증가되어 p50의 변형된 형태로 생각되었다. 기저 상태에서의 NF-${\kappa}B$의 DNA 결합능은 실험에 사용한 모든 세포주에서 거의 관찰되지 않았고 TNF-${\alpha}$ 자극 시 유의하게 증가하였다. TNF-${\alpha}$ 자극으로 활성화된 NF-${\kappa}B$ complex는 NCI-H358 과 NCI-H460을 제외한 모든 세포주에서는 p50/p65 heterodimer로 확인되었고 NCI-H358과 NCI-H460에서는 변형된 p50/p50 homodimer가 활성화되었다. 결 론 : 이상의 결과로 일부 폐암 세포주에서 외부 자극으로 활성화된 NF-${\kappa}B$ complex의 구성에 차이를 보였지만 전체적으로는 정상 기관지 세포주와 비교해 폐암 세포해서 NF-${\kappa}B$ 활성화에 있어 큰 차이가 없었다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.
