Replantation of a Distal Lower Leg in an Elderly Patient After Retrieval of the Limb from Seawater Immersion (바다에서 건진 절단된 하지 원위부의 재접합)

  • A case of distal lower leg replantation, in an elderly patient, was successfully performed after retrieval of the amputated limb from 3 hours of immersion in seawater. Due to the patient's age, bony union was delayed and functional nerve recovery was unsatisfactory. After massive saline irrigation of the amputated part, employing a catheter through the main artery, as well as, prophylactic antibiotics to prevent infection by Aeromonas, there were no postoperative complications. Despite the issue of age, immersion of an amputated part into seawater is not a contraindication of replantation.

  • Objective: It will be examined in this research whether Lacquer poison can be used as an distinguished treatment to cure incurable diseases by considering literature existing and various papers. Method: I studies origin, alias, species, toxicity, effect, treatment, component, medical action and contraindication of Rhus vemiciflua stokes through various kinds literatures. Results: Sap of Rhus vemiciflua stokes that is used for medical purposes, has an effect on anti-tumor, anti-oxidation, hangover cure, and gastritis suppression. Even though urushiol and fIavonoids, the main ingredient of lacquer, has medical cure effects. but urushiol results in a dermatropic allergy. Sincc xylem of a Rhus vemiciflua stokes, however. does not induce the allergy but has medical efficacy, research on this topic is needed.

  • A 45-year-old male was admitted to Department of Thoracic Surgery, Korea University Hospital with the chief complaints of cough and high fever of two months duration. His chest roentgenogram revealed homogenous ill-defined increased density in right lower lung, and bronchogram showed the abrupt cut-off sign of the proximal portion of the right intermediate bronchus. Preoperative evaluation of this patient revealed no contraindication of the pulmonary surgery. So Radical Right pneumonectomy was performed under the preoperative impression of lung cancer. And post-operative course was uneventful. Pathologic examination of the resected lung revealed Carcinosarcoma without regional lymphnode metastasis.

Supratentorial Intraparenchymal Haemorrhages during Spine Surgery

  • Intracranial haemorrhages are rare but potentially life-threatening complications of spine surgery. Most reported cases involved subdural or cerebellar haemorrhages; supratentorial parenchymal bleeding is very uncommon. We report a 28-year-old woman who underwent resection of a thoracic Ewing's sarcoma, and developed fatal haemorrhages around her cerebral metastases during surgery. The clinical presentations, possible pathogenesis and potential preventive measures are discussed. Patients with disseminated metastases within the neural axis are at risks of intracranial complications during spine surgery. The presence of intracranial mass lesions should be considered as a relative contraindication to intradural spine surgery.

  • Hyperbaric oxygen therapy (HBOT) is used to treat carbon monoxide (CO) poisoning. However, untreated pneumothorax is an absolute contraindication for HBOT. More caution is needed with regard to monoplace hyperbaric chambers, as patient monitoring and life-saving procedures are impossible inside these chambers. Central catheterization is frequently used for various conditions, but unnecessary catheterization must be avoided because of the risk of infection and mechanical complications. Herein, we describe a case of CO poisoning in which iatrogenic pneumothorax developed after unnecessary subclavian central catheterization. The patient did not need to be catheterized, and HBOT could not be performed because of the pneumothorax. Hence, this case reminds us of basic-but nonetheless important-principles of catheterization.

  • Coronary artery disease has historically been a contraindication to lung transplantation. We report a successful combined bilateral lung transplantation and off-pump coronary artery bypass in a 62-year-old man. The patient had a progressive decline in lung function due to idiopathic pulmonary fibrosis and a history of severe occlusive coronary artery disease.

  • As the cholera was spread over the Joseon dynasty at 1821, Hwang Doyeon investigated the symptoms like diarrhea, vomiting, muscle cramp and so on, and he presented the cause of cholera as the damage of Primordial-gi caused by abnormal climate and Damp-heat made by taking inadequate foods. He regarded as of great importance the ordinary health condition by guessing the prognosis of the disease, and proposed how to make a diagnosis of dehydration by observing nails and toenails. To treat cholera, he presented the methods of Sipseon-bloodletting and Singwol-moxacautery, and mentioned compound herb remedies and single herbs like garlic etc. He wrote down Mulberry leaves and Argyi wormwood leaves as the preventor of cholera to emphasize the importance of prevention, and mentioned food contraindication in addition to keep from getting worse.

  • In this paper, the types of tea mentioned in DongUiBoGam and the usage of tea were studied under these four main themes: tea as a folk medicine, types of tea, various uses, contraindication of tea. Tea used in mid Chosun. medicine, including in DongUiBoGam, was folk medicine. DongUiBoGam specialized the use of tea according to type. Also, it viewed tea as a medicinal ingredient that could exhaust the patient's EumBlood.

  • Segmental amputation of the digits and hand has been described as a contraindication for replantation because of poor results. We report the results of replantation for a patient who experienced multi-segment amputation of the hand. A 39-year-old man presented six hours after an accident, while using a straw cutter, that caused a multi-segment amputation of the entire palm and digits. The replantation surgery took 18 hours. We observed the patient gain satisfactory function of the hand. For replantation of a multi-segment amputation, connecting as many blood vessels as possible without tension is most important.

A Study of Acupuncture or Moxibustion Contraindications in Taipingshenghuifang and Tongrenzhenjiujing (태평성혜방과 동인침구경의 침구금기혈에 대한 고찰)

  • Objectives : This research aimed to verify the validities of contraindicated acupoints in acupuncture or moxibustion treatments in Taipingshenghuifang and Tongrenzhenjiujing. Methods : We investigated contraindicated acupoints when performing needling or moxibustion in Taipingshenghuifang and Tongrenzhenjiujing, then verified them in various literatures and today's medical knowledge. Results : In Taipingshenghuifang and Tongrenzhenjiujing, 15 acupoints were described to need a careful approach in acupuncture treatment, and 24 acupoints were described so in moxibustion treatment in common. And additionally GB1 was described to need a careful moxibustion treatment in Taipingshenghuifang. Most of the descriptions for the contradindicated acupoints can be explained based on medical knowledge. Conclusions : The contraindicated acupoints in acupuncture or moxibustion treatments in Taipingshenghuifang and Tongrenzhenjiujing seem to have been classified based on the experience of the doctors at that time. Therefore they may be referred to for safe acupuncture or moxibustion treatments at these points.
