Herpes zoster is an acute infectious viral disease which affects the posterior spinal root ganglion of the spinal nerve. A single posterior spinal root ganglion or a small number of adjacent ones may be affected, usually on the same side. The corresponding ganglia of the cranial nerve may also be similarly affected. The causative virus, varicella zoster, belongs to the group of host-specific DNA viruses. Postherpetic neuralgia is a continuation of herpes zoster in older patients. Although spontaneous resolution of herpes zoster may be expected in most patients, a significant number experience intractable pain. Postherpetic neuralgia is one of the most difficult problems encountered by physicians. There are many methods for management of postherpetic neuralgia, but there is no method that results in complete remission. Laser has lately come into use to reduce several acute or chronic pains. In order to determine the degree of pain relief by laser, 27 patients of postherpetic neuralgia were irradiated with He Ne, Infrared, and $CO_2$ combine scan moded lasers two to three times per week. The results were as follows: 1) The most frequent site was thoracic vertebral nerve area. 2) Patients younger than 70 years of age showed an improvement rate of 57% vs 27% for those patients older than 70 years of age. 3) Laser therapy proved effective of those patients who received the laser treatment within one month of the onset of the disease. 4) For those patients who received treatment within one month of the disease and reflecting a 50% improvement rate, the average irradiation time was 5.7.
With the spread of Personal Computers(PC) in the 1980's, many people started to deal businesses with PC. From late 1990's, the Internet age with PC have started and many people have showed keen interest in cyber-space and now they are utilizing it. Since 2000's the use of cyber-space have skyrocketed and it caused significant changes to humans' life. There was a huge prosperity to us but the new kind of crime, cyber-crime, was raised. Unlike past physical type of crimes, those cyber-crimes take place in the cyber-space and they have special features of non-facing, anonymity, specialty, technologic, repetition, continuation. Those cyber-crimes are continually growing since 2003 and in 2010 it almost doubled compared to 2003. General cyber-crimes like phishing-scam pornography circulation was most of them and notably perpetrators of them are younger generation. Recently cyber-crimes are showing the trend of advancing more and more and cyber-bullying, fraud like phishing scam are on the rise. The police are responding by making 'Cyber Terror Response Center', but it does not work effectively with the problems of breakup of prevention and investigation unit, procedure of investigation and the system itself. So, I suggest practical use of private security to remedy our police's weakness and to prevent cyber-crimes. Preventing solutions of cyber-crime with private security are physical defense of large-scale servers and vital computers, building of Back-up system to prevent vital data loss, and building of cyber-crime preventing system combining software and hardware.
Background: Cervical cancer has been a leading female cancer in Thailand for decades, and has been second to breast cancer after 2007. The Ministry of Public Health (MoPH) has provided opportunistic screening with Pap smears for more than 30 years. In 2002, the MoPH and the National Health Security Office provided countrywide systematic screening of cervical cancer to all Thai women aged 35-60 years under universal health care coverage insurance scheme at 5-year intervals. Objectives: This study characterized the cervical cancer incidence trends in Songkhla in southern Thailand using joinpoint and age period cohort (APC) analysis to observe the effect of cervical cancer screening activities in the past decades, and to project cervical cancer rates in the province, to 2030. Materials and Methods: Invasive and in situ cervical cancer cases were extracted from the Songkhla Cancer Registry from 1990 through 2010. Age standardized incidence rates were estimated. Trends in incidences were evaluated by joinpoint and APC regression models. The Norpred package was modified for R and was used to project the future trends to 2030 using the power of 5 function and cut trend method. Results: Cervical cancer incidence in Songkhla peaked around 1998-2000 and then dropped by -4.7% per year. APC analysis demonstrated that in situ tumors caused an increase in incidence in early ages, younger cohorts, and in later years of diagnosis. Conclusions: Both joinpoint and APC analysis give the same conclusion in continuation of a declining trend of cervical cancer to 2030 but with different rates and the predicted goal of ASR below 10 or even 5 per 100,000 women by 2030 would be achieved. Thus, maintenance and improvement of the screening program should be continued. Other population based cancer registries in Thailand should analyze their data to confirm the success of cervical cancer screening policy of Thailand.

Purpose : Arterial blood gas analysis is frequently performed in neonatal intensive care unit (NICU) to evaluate ventilation and the metabolic state of critically ill infants. In occasions when umbilical arterial catheterization is not available, frequent arterial puncture is mandatory. This requires some technical skill and may occasionally have side effects. So we studied the validity of capillary blood gas analysis which can be performed conveniently compared with arterial blood. Methods : Twenty-four neonates admitted to NICU during April to Aug. 2001 were studied. They were more than two weeks old without indwelling arterial catheters. Thirty-six times, simultaneous arterial, and capillary blood gases were drawn by puncture and the pH, $pCO_2$ and $pO_2$ of each sample was measured. Blood pressure and body temperature was checked before sampling to rule out impaired peripheral circulation. Capillary blood was collected from warmed heels. Results : There was a strong correlation between capillary and arterial pH(r=0.91, P<0.05). The absolute value of the difference between arterial and capillary pH was less than 0.05. Also capillary $pCO_2$ showed correlation with arterial $pCO_2$(r=0.77, P<0.05). Despite a statistically significant correlation between capillary and arterial $pO_2$(r=0.68, P<0.05), the absolute value of the difference was more than 10 mmHg in 92% of cases. Conclusion : Capillary blood gases accurately reflected arterial pH and $pCO_2$ and showed a relative correlation with $pO_2$. Capillary blood gas analysis can be a useful alternative to arterial blood when continuation of the umbilical arterial catheter is no longer available.
This study is an attempt to evaluate the use of an oral contraceptive by: 1. Deforming the acceptance rate of the oral contraceptive according to socio-demographic characteristics 5. Estimating the duration of the use of the contraceptive f. Discovering reasons why the contraceptive was discontinued 4. Computing the cumulative continuation and discontinuation rate according to socio-demographic characteristics, history of pregnancies, and attitude of husband at the time of selection of the contraceptive. One hundred sixty-eight women of child bearing age (14-49 years), residing in Yonhee Dong, Seoul, Korea and registered as oral contraceptive accepters at Yonsei Community Health Center, were interviewed during a three week period in 1973. The questionnaire was designed to determine the socio-demographic characteristics of the oral contraceptive accepters, the current status of their family planning practice, medical reactions if any, discontinuation practice, and the duration of the use of the contraceptive. The major findings of the study were as follows; 1. The acceptance rate was highest for women between 30 and 34 years of age, the mean age being 34.4 years. 2. Fifty six point five percent of all respondents said that thirty years of age was the ideal age to stop having children. 3. The average number of living children was 3.5 and fifty four point eight percent of respondents felt 2 boys and I girl was the ideal number of children. 4. The average number of pregnancies was 5.2 and the average number of induced abortion was I. 4. 5. Fifty eight point eight percent of the respondents had experience in the use of contraceptive methods in the past. 6. Forty one point seven percent of the respondents replied that they chose the oral contraceptive because, "it was not complicated to use." 7. Sixty four point eight percent of the women said their husband approved of their taking the pill. 8. The cumulative discontinuation rate was sixty point five percent of the total respondents for one year in first segment. 9. Sixty six point nine percent of the respondents had experienced side-effect while using the pill. The side effects for 68.9 percent of these women were castro-intestinal upsets. Twenty point eight percent of the women who had side effects consulted with medical personnel about them. Women who had more education had more side effects. 10. Seventy three point two percent of the women who discontinued the pill did so because of medical reasons. Women who were younger discontinued the pill for personal reasons more-often than older women. Among personal reasons listed for discontinuing the pill was the attitude of the husband. 11. The average duration of continuance of the contraceptive was 5.3 cycles under 29 years of age; 7.4 cycles between 30 and 34 years of age; and 8.4 cycles over 40 years of age. 12. The discontinuation rate was seem to increase sharply in the early cycle and increase more slowly in later cycle. Conclusions and suggestions: Since the attitude of the husband was shown to be important, the current family planning program should be expended to include approach to husband. For women who must use the oral contraceptive, education and support must be enhanced so that the discontinuation rate due to side effects will be decreased.
Objective : To provide background information for strengthening cervical cancer prevention in the Pacific by mapping current human papillomavirus (HPV) vaccination and cervical cancer screening practices, as well as intent and barriers to the introduction and maintenance of national HPV vaccination programmes in the region. Materials and Methods: A cross-sectional questionnaire-based survey among ministry of health officials from 21 Pacific Island countries and territories (n=21). Results: Cervical cancer prevention was rated as highly important, but implementation of prevention programs were insufficient, with only two of 21 countries and territories having achieved coverage of cervical cancer screening above 40%. Ten of 21 countries and territories had included HPV vaccination in their immunization schedule, but only two countries reported coverage of HPV vaccination above 60% among the targeted population. Key barriers to the introduction and continuation of HPV vaccination were reported to be: (i) Lack of sustainable financing for HPV vaccine programs; (ii) Lack of visible government endorsement; (iii) Critical public perception of the value and safety of the HPV vaccine; and (iv) Lack of clear guidelines and policies for HPV vaccination. Conclusion: Current practices to prevent cervical cancer in the Pacific Region do not match the high burden of disease from cervical cancer. A regional approach, including reducing vaccine prices by bulk purchase of vaccine, technical support for implementation of prevention programs, operational research and advocacy could strengthen political momentum for cervical cancer prevention and avoid risking the lives of many women in the Pacific.
Road safety audit is the preventive enhancement strategy for safety. : it gets rid of beforehand the potential factor of a traffic accident in the stage of road planning and design and it evaluates the appropriation for road geometric structure or safety facility to prevent traffic accident in the stage of operation after the construction. Since this strategy is introduced to our country in the early 2000s, various projects have been processed and it was legislated recently. And now, the evaluation of past project for its continuation is needed. Therefore, in this study the evaluation of road safety audit on existing freeway is performed. The spatial extent of this study is Yong-dong line on which the safety treatment was executed in 2005 and 2006. And, the temporal range of this study is each 2-year of before and after from 2005 and 2006. The empirical bayes method of observational evaluation studies is applied to analyze. As a result, there is an effect of improvement on most of treated sections. But there is ineffective or negligible on some sections. Compared with the detail of treatment on each section, the effect of multiple or various treatments is good for that section. On the other hand, the section on which effect doesn't appear is the result of single or unimportant treatments. Throughout these results, the concrete analysis can be performed and the countermeasures designed for the section on which effect doesn't appear. Also it is used as reference to the future plan and direction of road safety audit on existing freeway.

This study investigates the environmental factors of endemic species in Korea in order to understand their ecological characteristics, and to investigate the target species of their natural habitats to find similar sites. The purpose is to restore and follow suitable growth methods for the appropriate highway facility of target species to establish a management system via monitoring. This study endeavors to restore the target species near highway facilities on the basis of monitoring data and restore sites have similar natural characteristics of the target species. After restoring the target species, a restoration strategy and management plan will be established for breeding and continuation. The restoration strategy and management plan of the target species is divided into breeding, restoring, maintaining and monitoring plans. Specially management plans include several divisions such as soil, water, non-point pollution source reduction and naturalized plants. The results of this study can be used as reference materials for the restoration of endemic Korean plants in the future of highway routes, and for systematic management measures in habitats.

Background: Guidelines for esophagogastroduodenoscopy (EGD) in the West allow the continued use of warfarin under therapeutic international normalized ratio (INR) level. In Korea, no guidelines have been issued regarding warfarin treatment before EGD. The authors surveyed Korean cardiac surgeons about how Korean cardiac surgeons handle warfarin therapy before EGD using a questionnaire. Participants were requested to make decisions regarding the continuation of warfarin therapy in two hypothetical cases. Methods: The questionnaire was administered to cardiac surgeons and consisted of eight questions, including two case scenarios. Results: Thirty- six cardiac surgeons at 28 hospitals participated in the survey, and 52.7% of the participants chose to stop warfarin before EGD in aortic valve replacement patients without risk factors for thromboembolism. When the patient's INR level was 2, 31% of the participants indicated that they would choose to continue warfarin therapy. For EGD with biopsy, 72.2% of the participants chose warfarin withdrawal, and 25% of the participants chose heparin replacement. In mitral valve replacement patients, 47.2% of the participants chose to discontinue warfarin, and 22.2% of the participants chose heparin replacement. For EGD with biopsy in patients with a mitral valve replacement, 58.3% of the participants chose to stop warfarin, and 41.7% of the participants chose heparin replacement. Conclusion: This study demonstrated that attitudes regarding warfarin treatment for EGD are very different among Korean surgeons. Guidelines specific to the Korean population are required.
