의료진의 병원영양관리에 대한 인식조사 (The Medical Staffs' Perception about Nutrition Care)

  • This study was undertaken to identify medical staffs’ perception about nutrition care and dietitian's role in hospitals. Questionnaires were sent to 874 medical staffs at 50 general hospials size over 100 beds, located in Seoul and Pusan, A total of 649(74.3%) of medical staffs returned questionnaries from 47 general hospitals. The results of this study can be summarized as follows. 1. 89.6% of medical staffs responded that they required the help of the dietitian when the patients needed the nutrition acre and also most of them(87.0%) held positive perceptions about the necessity of the nutrition support team. 2. When the medical staffs prescribed the special diet of the patients' nutrition care, 90.0% of them preferred that diet would be order after consulting with dietitian. 3. Half of the medical staffs(52.1%) responded that they were scarced of the knowledge about the clinical nutrition and then 66.4% of them recognized the need of nutrition education at the medical school. 4. The medical staffs responded that dietitian must have the knowledge about food, nutrition and medical science(52.5%) and the competence of discussion with medical staffs about nutrition the patients(30.8%).

노인복지시설 사용자 중심의 영양·식생활관리 프로그램 요구 분석 (A study on the Needs for Nutrition Management Program for Elderly Who use Welfare Facilities)

  • Objectives: This study was performed to investigate the nutritional status, health conditions, eating habits and experience and demand for nutrition dietary management of senior citizens. And these data are formed foundation of development of nutrition dietary management education program and contents in welfare facilities for the aged. Methods: We visited 3 public health centers, 3 senior citizens centers, and 4 welfare centers in Seoul, Gyeonggi-do, Chungnam province, and Daejeon area and carried out interview by semi-structured questionnaire for senior citizens older than 65 years who use those facilities. Results: The study included 17%, 30.7% and 52.3% of senior citizens from public health centers, seniorcitizen centers and and welfare centers, respectively. The age range of 43.9% of the population was 65-74 years and and 56.1% were older than 75 years. We observed that 83.2% of subjects took some medicines due to diseases that they have and 58.0% took prescription medicines for hypertension. The thing that they considered the most when selecting food was 'the taste'(p<0.05). Regarding the level of practice to keep the dietary life, they answered more than 'average' for most of items but answered less than 'average' for lot of salt intake, drinking, exercise. For the experience of nutrition dietary life education, only 19.8% answered 'Yes' and the service for nutrition dietary life management showed the highest score in the demand for 'provide nutritious food'. For the size of consulting group for nutrition/dietary life education, the public health center and welfare center preferred a larger group size but the senior citizens center preferred a smaller group. With regard to who will carry out the consulting, the demand for dietitian was the highest and the operation type showed the high demand in the order of consulting, education. The contents that they want to have consultation in nutrition dietary life education were diet therapy for diseases and the ordinary diet therapy for health. Conclusions: This study suggested the management of nutrition dietry life necessitates qualitative measures according to the different types of welfare facilities. For these, it is in need of development of counseling and education program included therapy for disease. Above all, the policy to secure dietitian of welfare facilities for the aged to perform these should be achived.

인터넷에서 행동 수정 이론을 적용한 체중 감량 상담 방법 개발 (Development of Nutritional Counseling for Weight Reduction based on behavior modification through Internet)

  • The purpose of the study was to develop an internet nutritional counseling program using an expert system to assist obese people to lose weight through behavior modification. The internet counseling program for weight loss was developed by the accumulation of knowledge dealing with eating habits and exercising behaviors in expert system tool, Knowledge Engineering Agent (KEA) by a dietitian without any help of computer expert. To accumulate knowledge into KEA, survey was performed in 150 obese people, dietitians reviewed and consulted each survey case, and the consulted contents were learned and accumulated into KEA. Survey questionnaire was the same as that of the internet consulting program, and it included general characteristics, dietary habits, lifestyle, and exercise patterns related to obesity. Also, the dietitian selected proper factors inferred from the survey questionnaire of each case, and added the conclusions for them. Conclusions were made for helping clients to correct bad eating behaviors and accumulate good behaviors to lose weight. Counseling was divided into two parts; a two-week part and a daily part. Two-week counseling was performed based on 4 step questionnaires, and daily counseling was done for daily food consumption and physical activity. When clients answered survey questionnaires in a counseling internet program, the recommendations on how to eat, to exercise and to deal with stress in a real time for each case, was given. In conclusion, a counseling internet program for weight reduction can be used to give advices how to deal with obesity in a man-to-man way in a real time using KEA where nutritional knowledge based on behavior modification for weight loss was accumulated.

전문가 시스템을 이용한 인터넷 체중 감량 상담 프로그램 개발 (Development of Nutritional Counseling Internet Program for Weight Reduction Using Expert System)

  • 본 연구에서는 인터넷상에서 일반 대중들이 행동 수정 방법을 통해 올바른 식습관을 비롯한 생활 습관을 가짐으로 체중 감량을 통해 정상적인 체중을 장기간 유지할 수 있도록 하기 위하여 전문가 시스템 툴인 KEA를 사용하여 실시간 상담을 해줄 수 있는 프로그램을 개발하였다. 전문가 시스템인 KEA는 MCRDR 이론을 사용하여 특정 분야의 전문가가 전문가 시스템에 관한 전문지식 없이도 시스템을 직접 구축 운용할 수 있는 환경을 구축하여 지식의 전문가가 직접 지식데이터 베이스를 구축하고 유지.보수하여 저렴한 비용으로 장기간 운영이 가능하다는 장점이 있다. 처음에 KEA로 상담할 수 있도록 지식을 학습시키기 위하여 체중 감량에 관련된 설문지를 작성하여 과체중 또는 비만한 사람 150명을 대상으로 설문을 받아 전문 영양사가 이에 대한 상담을 하고 이것을 KEA에 학습시켰다. 설문지는 인터넷 상담 프로그램과 같은 설문지를 사용하여 각 대상으 비만의 원인, 식습관, 생활습관, 운동 습관을 조사하였다. 체중 감량 상담은 행동 수정 이론을 적용하여 내담자가 스스로 잘못된 식습관 및 생활습관을 수정함으로써 점차 체중을 감량하여 평생 적절한 체중을 유지할 수 있는 방법을 터득할 수 있도록 상담하는 것을 원칙으로 하였다. 지식을 KEA에 학습시키는 방법은 조사한 설문을 바탕으로 case에서 적절한 항목값으로 추출하고 KEA에 지식을 만들기 위한 조건으로 추가한 후 이러한 조건에 맞는 상담 결론을 선택하고 추가하는 방식으로 KEA에 학습시켰다. 이 프로그램은 인터넷을 통해 일반인이 사용할 수 있도록 인터페이스를 만들었고 사용자가 프로그램에 들어가서 실시간으로 전문가의 상담을 받을 수 있다. 사용자가 인터넷 체중감량 상담 프로그램을 사용하기 위하여 화면에 있는 설문에 답을 기입하면 이미 축적된 KEA의 지식으로 실시간으로 상담의 결론을 내려주었다. 만약 사용자의 상황이 KEA에 축적되어 있는 지식으로 결론을 내릴 수 없으면 '결론없음'이라는 말이 뜨게 되고 이때는 전문가각 직접 이 상담 case를 분석하여 KEA에 지식을 축적시켜 점차 KEA의 지식을 확충시켰다. 결론적으로 전문가 시스템인 KEA를 이용하여 체중 감량의 원칙을 행동 수정 방법에 따라서 사용자의 개인에 맞는 방법을 체중 감량을 할 수 있도록 전문가가 일대일로 실시간으로 상담할 수 있도록 하여 건강하게 체중 감량을 할 수 있도록 하였다.
