『동의수세보원』의 비약(脾約)에 관하여 (A Study on the Splenic Constipation(脾約) in Donguisusebowon)

  • Objective The purpose of this study is to understand the meaning of Splenic Constipation(脾約) in "Donguisusebowon(東醫壽世保元)". Methods We investigated the provisions associated with splenic constipation written in "Donguisusebowon", "Donguibogam(東醫寶鑑)", "Shanghanlun(傷寒論)" and several papers which related to splenic constipation. Results and Conclusions 1. Splenic constipation in "Donguisusebowon" falls into the category of Soeumin Yang Depletion Symptomatology(少陰人 亡陽病). The symptoms of splenic constipation include sweating and smooth urination. 2. Splenic constipation in "Donguisusebowon", "Shanghanlun", "Donguibogam" is based on fluid deficiency, but splenic constipation in "Donguisusebowon" is focused on sweating unlike "Shanghanlun" and sweating was quoted in "Donguibogam". 3. Splenic constipation in "Shanghanlun" and "Donguibogam" is focused primarily on constipation. The difference is that splenic constipation in "Donguibogam" relates to sweating. 4. Therefore, even if they use the same term, we need to understand that the meaning of splenic constipation in "Donguisusebowon" is different from that of "Shanghanlun" and "Donguibogam".

  • Chronic constipation is a very common disease in children. Successful treatment of constipation can be achieved not only with medication but also with lifestyle changes, including a proper diet. Diets including fruits, fluids, and probiotics are good for constipation. Some dietary components are helpful for constipation, and some are harmful. In this study, we present diets related to constipation from the literature, and propose some perspectives regarding diets related to constipation.

시설노인의 변비실태와 일상생활 수행능력 (Constipation and Activities of Daily Living of the Institutionalized Elderly)

  • Purpose: This study attempts to develop the basic data of Constipation reduction program for institutionalized elderly by surveying the constipation and the ADL (Activities of Daily Living) of them. Method: Data are collected by nurses caring for four hundreds five institutionalized elderly during May through July in 2004. Questionnaire contains a bowel assessment, constipation control intervention, and ADL. Result: The constipation rate was found to be 43%. The 82.5% of the study sample was treated with laxatives, and a 59.9% of the sample was practicing enema. The average score of the ADL was 38.32(range: 0 - 100). According to surveying on the ADL in relation to the function of the constipation, ADL is found to be higher in the non-constipation group than the constipation group (t=-2.786, p=.006). Conclusions: The institutionalized elderly tended to have a high rate of constipation. The elderly having constipation was almost dependent on laxatives and enema for relaxing constipation. The high rate of constipation was related to the low performance of the ADL.

만성변비환자의 한열변증에 따른 심박변이도에 관한 연구 (Relationship between Heart Rate Variability and Cold-Heat Patternization in Patient with Chronic Constipation)

  • Objectives : The aim of this study was to investigate the relationship between the autonomic nervous system and the cold-heat pattern in patients with chronic constipation. Methods : Subjects with chronic constipation (n=30) and without chronic constipation (n=20) were recruited, interviewed and measured for heart rate variability (HRV). Chronic constipation was defined as functional constipation and Irritable Bowel Syndrome (IBS) with constipation based on Rome III criteria. We surveyed patients' general characteristics, categorized by cold-heat pattern, and took measurement of HRV for 5 minutes. Analysis was conducted among these three indices. Results : Women accounted for most of the subjects with chronic constipation (96.7%). In groups with or without constipation, the number of cold patterns was more than of heat patterns. The mean heart rate of subjects without chronic constipation was significantly higher than that of subjects with chronic constipation. No other statistical significance was noted among indices of HRV and cold-heat pattern. Conclusions : Through this study, there were few relationships between autonomic nervous system measured by HRV and cold-heat pattern in chronic constipation.

강원지역 직장여성들의 변비 관련 생활습관 요인 (A Study on Life Style and Eating Habits Correlated with Constipation of Working Women in Kangwon Provicne)

  • In this study we included 298 working women in Kangwon province to investigate the life style and eating habits related to constipation. The subjects were divided into 3 groups according to Roman II criteria as follows : normal, mild constipation and constipation groups. For the result, 48.4%, 42.7%, 27.2% and 28.6% of the subjects had constipation in the order of 20's, 30's, 40's, and 50's, respectively. Constipation group had low frequency of bowel movement per week and irregular visit to toilet compared to normal and mild constipation groups. There were higher percentage of people with reading habit while sitting on a toilet, in the constipation group compare to other groups. Constipation groups had difficulty having bowel movement when sleeping outside of their home. There was a significant difference in water consumption per day among the 3 groups. But there was no significant difference in the amount of meals consumed and the number of meals with snacks per day. For the food frequency, there was lower frequency of yogurt and vegetables consumption in the constipation group. But there was no difference in how to eat fruits. From these results, we concluded that in order to treat the constipation, it is recommended to take enough water and dietary fiber, and to have physical activities for the working women.

The relationship between primary headache and constipation in children and adolescents

  • Purpose: Many patients presenting with headache also complain of constipation; the relationship between these two symptoms has not been explored in detail. The aim of this study was to investigate the association between primary headache and constipation. Methods: This retrospective study included all children who attended the Inje University Sanggye Paik Hospital complaining of headache, and who had been followed up for at least 100 days. Patients were divided into 2 groups: group A, in whom the headache improved after treatment for constipation, and group B, in whom headache was not associated with constipation. Results: Of the 96 patients with primary headache, 24 (25.0%) also had constipation (group A). All 24 received treatment for constipation. Follow-up revealed an improvement in both headache and constipation in all patients. Group B contained the remaining 72 children. Comparison of groups A and B indicated a significant difference in sex ratio (P=0.009, chi-square test). Patients with probable tension-type headache were more likely to be in Group A (P=0.006, chi-square test). Conclusion: Resolution of constipation improves headache in many patients diagnosed with primary headache, especially those with probable tension-type headache. We suggest that either constipation plays a key role in triggering headache, or that both constipation and headache share a common pathophysiology.

노인의 변비관련 요인 - 요양시설 거주 노인을 중심으로- (The Factors Affecting the Constipation of the Elderly at Nursing Homes)

  • Purpose: The purpose of this study is to identify factors affecting the constipation of the elderly at nursing homes. Method: A total of 184 elders at 7 nursing homes in P City participated. Information about constipation status (Constipation Assessment Scale). functional status (Katz' Activities of Daily Living). nutritional status (Mini-Nutritional Assessment). and depressive symptoms (Geriatric Depression Scale Short Form Korea version) were collected through interviews. Descriptive statistics. Chi-square test. and t-test were used to describe and compare the non constipation group and the constipation group. Multiple logistic regression analysis was used to determine factors affecting constipation status. Results: The prevalence of constipation was 47.5% (Men 37%. women 51%). Elders suffering from a disease (P=.021) and having low sleep satisfaction (P= .000) were more likely to be constipated than those who did not. The factors affecting the constipation of the elderly at nursing homes were gender (OR=3.558), sleep satisfaction (OR=.580), and depressive symptoms (OR=1.132). Conclusion: According to the result, to reduce the constipation rate of old women at nursing homes, we have to develop guidelines that can solve problems related to constipation cause by sleep-satisfaction and depressive symptoms.

자의적 변비 분류에 의한 정상군과 변비군 여대생의 식사관련 요인 비교 (Comparison of the Dietary Factors between Normal and Constipation Groups by Self-reported Constipation in Female College Students)

  • The purpose of this study was to obtain baseline data on dietary management of women's constipation. We estimated the prevalence of self reported constipation, bowel habits and foods & nutrients intake. The subjects were 169 female college students (normal = 92, constipation group = 77) aged 19 to 23 years. We conducted anthropometric measurements, an evacuation habits survey, and a dietary intake assessment for three days by a 24 hour recall method. The prevalence of self reported constipation was 46% (n = 77). Results showed that bowel habits (the difficulty of evacuation, the duration of evacuation and the feeling after evacuation) were significantly different between the two groups. Over 65% of constipation group used laxatives for constipation relief. Frequency of stress and deep sleeping were related with constipation. In daily food consumption and nutrients intake, there was not a significant difference between the groups. However in relation to bowel habits which factors influence constipation, the study showed that the intake of water, potatoes, kimchi, and fruits correlated with evacuation facility. Further, there were no findings in the difference of foods consumption and nutrients intake between the two groups. But some life style changes and food intakes (potato and kimchi) may be useful to improve constipation symptoms in young women.

  • Constipation is very common phenomenon during childhood that accounts approximately 3% of general pediatric outpatient visits and up to 25% of visit to the pediatric gastroenterologist. The diagnosis of functional constipation is based on the historical features and absence of physical abnormalities. Most infants and children with chronic constipation require no diagnostic tests. Constipation and subsequent fecal retention behavior often begins soon after experience of painful defecation. The effective management of childhood chronic constipation consist of education, prolonged support by physicians and parents, medications, and long term follow up.

뇌손상 환자의 재활단계 변비발생과 영향요인 (Occurrence of Constipation during the Rehabilitation Stage in Patient with Cerebral Vascular Disease)

  • Purpose: The purpose of this study was to investigate the occurrence of first onset of constipation during the rehabilitation stage and risk factors for constipation in patients with cerebral vascular disease (CVD). Methods: Participants for this retrospective study were 214 CVD patients admitted to rehabilitation units. First onset of constipation and factors influencing constipation such as dependency level were recorded for 14 days during the rehabilitation stage. Survival analysis with Cox proportional hazard model and descriptive statistics were conducted. Results: Age, patients' severity, types of diet, and dependency level were different between constipation and non-constipation groups. In survival analysis, 99% of participants developed constipation within 14 days after admission to rehabilitation units. Median constipation occurrence time was 7.4 days. Patients who were immobile and had tube feedings were more likely to develop constipation 4.07 times (95% CI: 1.018~16.301, p=.047) and 2.09 times (95% CI: 1.001~4.377, p=.050) respectively compared to patients who were independent and had a regular diet, respectively. Conclusion: Most CVD patients experienced constipation within 2 weeks after entering the rehabilitation stage. Constipation was linked to types of diet and dependency level. These factors associated with constipation should be considered when caring for patients in rehabilitation.
