Congenital Esophago-Bronchial Fistula without Esophageal Atresia (Report of 6 Cases) (성인의 선천성 식도.기관지루 6례 보고)

  • We had been experienced 6 cases of congenital esophagobronchial fistula which underwent surgical intervention in all cases. Of the 6 patients, 5 patients were male, one patient was female. The mean age of patients was 37.8 years old and clinical symptom free period was 27.1 years. According to Braimbridge`s classification, they were belonged to the type II[3 cases], type III[2 cases], <% type I[1 case]. Pre-operative diagnosis was available in 4 cases and the 2 cases could be diagnosed at operation field. The fistulectomy were performed in all cases, concomitant segmental resection[1 case], lobectomy[1 case], bi-lobectomy[1 case], and pneumonectomy[3 cases] were combined.

Bronchoplastic Procedures (기관지 성형술)

  • Bronchoplastic procedure has been considered as an appropriate surgery for traumatic bronchial disruption and occasionaly for primary bronchial tumors or tuberculosis because it can bring preservation of pulmonary tissue for patients without compromising the chance for cure. Nowadays bronchoplastic procedure is also applicable for the selected cases of bronchogenic carcinomas with favorable long term survival, when compared to standard pneumonectomy.Eighteen bronchoplastic procedures were performed with or without pulmonary resection at Department of Thoracic and Cardiovascular Surgery, Catholic University Medical College, between 1990 and 1994. The patients were 11 men and 7 wemen with average age of 57 years [range, 19 to 71 years . Tumor comprised 56% of the lesions, including 6 squamous cell carcinoma [33% , 2 bronchial adenoma [11% , 1 leiomyoma and 1 metastatic osteogenic sarcoma. Cicatrical stenosis secondary to endobronchial tuberculosis and traumatic disruption occurred in 6 [33% and 1 patient respectively.Applied bronchoplastic procedures were as follows ; sleeve lobectomy, 8 cases [right upper : 6, left upper : 1, right middle : 1 : bronchial segmental resection without pulmonary resection, 2 cases : sleeve bi-lobectomy, 1 cases :patch dilating bronchoplasty with or without concomitant lobectomy in 7. There was no perioperative mortality. Morbidity in 4 patients included 1 transient recurrent laryngeal nerve palsy, 1 unstability of bronchial patch resulting atelectasis of afftected lung and 2 bronchial stenosis of anastomotic site.Throughout our experiences, we feel strongly that bronchoplastic procedure is a safe and effective surgical method preserving normal pulmonary tissue below affected bronchus for the wide range of various bronchial lesion including selected cases of bronchogenic carcinoma with acceptable complication and mortality.

  • In the highly competitive world, there has been a concomitant increase in the need for the research and planning methodology, which can perform an advanced assessment of technological opportunities and an early Perception of threats and possibilities of the emerging technology according to the nation's economic and social status. This research is aiming to provide indicators and visualization methods to measure the latest research trend and aspect underlying scientific and technological documents to researchers and policy planners using 'Co-word Analysis' Organic light emitting diodes(OLED) is an emerging technology in various fields of display and which has a highly prospective market value. In this paper, we presented an analysis on OLED. Co-word analysis was employed to reveal patterns and trends in the OLED fields by measuring the association strength of terms representatives of relevant publications or other texts produced in the OLED field. Data were collected from SCI and the critical keywords could De extracted from the author keywords. These extracted keywords were further standardized. In order to trace the dynamic changes in the OLED field, we presented a variety of technology mapping. The results showed that the OLED field has some established research theme and also rapidly transforms to embrace new themes.

Kraft Lignin Biodegradation by Dysgonomonas sp. WJDL-Y1, a New Anaerobic Bacterial Strain Isolated from Sludge of a Pulp and Paper MillS

  • Wastewater containing kraft lignin (KL) discharged from pulp and paper industries could cause serious environmental contamination. Appropriate effluent treatment is required to reduce the pollution. Investigations on anaerobic bacteria capable of degrading KL are beneficial to both lignin removal and biofuel regeneration from the effluent. In this paper, an anaerobic strain capable of degrading KL was isolated from the sludge of a pulp and paper mill and identified as Dysgonomonas sp. WJDL-Y1 by 16S rRNA analysis. Optimum conditions for KL degradation by strain WJDL-Y1 were obtained at initial pH of 6.8, C:N ratio of 6 and temperature of 33℃, based on statistical analyses by response surface methodology. For a 1.2 g/l KL solution, a COD removal rate of 20.7% concomitant with biomass increase of 17.6% was achieved after 4 days of incubation under the optimum conditions. After the treatment by strain WJDL-Y1, KL was modified and degraded.

  • A method to determine the trace level of arsenic in scalp hair by the hydride generation(HG)-inductively coupled plasma(ICP) spectrometry is described. The optimum conditions for the generation of arsine($AsH_3$), and the interference effects from the concomitant ions were studied. Severe interference effect from Ni(II) is circumvented by the coprecipitation of arsenic with $La(OH)_3$. The detection limit of arsenic is 0.3ppm and the arsenic contents in scalp hair ranged 10~20ppb.

Glycochenodeoxycholic Acid Induces Cell Death in Primary Cultured Rat Hepatocyte: Apoptosis and Necrosis

  • Intracellular accumulation of bile acids in the hepatocytes during cholestasis is thought to be pathogenic in cholestatic liver injury. Due to the detergent-like effect of the hydrophobic bile acids, hepatocellular injury has been attributed to direct membrane damage. However histological findings of cholestatic liver diseases suggest apoptosis can be a mechanism of cell death during cholestatic liver diseases instead of necrosis. To determine the pattern of hepatocellular toxicity induced by bile acid, we incubated primary cultured rat hepatocytes with a hydrophobic bile acid, Glycochenodeoxycholate (GCDC), up to 5 hours. After 5 hours incubation with $400\;{\mu}M$ GCDC, lactate dehydrogenase released significantly. Cell viability, quantitated in propidium iodide stained cells concomitant with fluoresceindiacetate was decreased time- and dose-dependently. Most nuclei with condensed chromatin and shrunk cytoplasm were heavily labelled time- and dose-dependently by a positive TUNEL reaction. These findings suggest that both apoptosis and necrosis are involved in hepatocytes injury caused by GCDC.

Familial hyperkalemic periodic paralysis caused by a de novo mutation in the sodium channel gene SCN4A

  • Familial hyperkalemic periodic paralysis (HYPP) is an autosomal-dominant channelopathy characterized by transient and recurrent episodes of paralysis with concomitant hyperkalemia. Mutations in the skeletal muscle voltage-gated sodium channel gene $SCN4A$ have been reported to be responsible for this disease. Here, we report the case of a 16-year-old girl with HYPP whose mutational analysis revealed a heterozygous c.2111C>T substitution in the $SCN4A$ gene leading to a Thr704Met mutation in the protein sequence. The parents were clinically unaffected and did not have a mutation in the $SCN4A$ gene. A $de$ $novo$ $SCN4A$ mutation for familial HYPP has not previously been reported. The patient did not respond to acetazolamide, but showed a marked improvement in paralytic symptoms upon treatment with hydrochlorothiazide. The findings in this case indicate that a $de$ $novo$ mutation needs to be considered when an isolated family member is found to have a HYPP phenotype.

Hypokalemic periodic paralysis: two different genes responsible for similar clinical manifestations

  • Primary hypokalemic periodic paralysis (HOKPP) is an autosomal dominant disorder manifesting as recurrent periodic flaccid paralysis and concomitant hypokalemia. HOKPP is divided into type 1 and type 2 based on the causative gene. Although 2 different ion channels have been identified as the molecular genetic cause of HOKPP, the clinical manifestations between the 2 groups are similar. We report the cases of 2 patients with HOKPP who both presented with typical clinical manifestations, but with mutations in 2 different genes ($CACNA1S$ p.Arg528His and $SCN4A$ p.Arg672His). Despite the similar clinical manifestations, there were differences in the response to acetazolamide treatment between certain genotypes of $SCN4A$ mutations and $CACNA1S$ mutations. We identified p.Arg672His in the $SCN4A$ gene of patient 2 immediately after the first attack through a molecular genetic testing strategy. Molecular genetic diagnosis is important for genetic counseling and selecting preventive treatment.

Effect of Soybean Oil Supplementation on the Contents of Plasma Cholesterol and Cis9, trans11-CLA of the Fat Tissues in Sheep

  • A feeding trial was conducted with 10 sheep for 12 weeks to examine the effect of soybean oil (SBO) supplementation on long-chain fatty acids composition, especially cis9,trans11-conjugated linoleic acid (c9,t11-CLA) in fat tissues. Sheep were fed either a SBO supplemented diet (5%, DM basis) or a control diet without SBO. Chopped rye grass hay was fed as roughage. Concomitant increases in contents of total cholesterol (T-C) and HDL-cholesterol (HDL-C) in the plasma of sheep were observed from the SBO supplementation. The supplementation of SBO reduced (p<0.05) the proportions of $C_{16:1}$, $C_{17:0}$ and $C_{17:1}$ but increased (p<0.05) the proportions of $C_{18:0}$ and octadecenoic acid (t11-$C_{18:1}$) in the intramuscular fat. The $C_{18:0}$ proportion only in the subcutaneous fat was increased (p<0.05) by the SBO supplementation. The SBO supplementation slightly increased CLA proportion in the intramuscular fat and subcutaneous fat.

Spontaneous Contractions Augmented by Cholinergic and Adrenergic Systems in the Human Ureter

  • Interstitial cells of Cajal (ICC) evoke pacemaker activities in many tissues. The purpose of this study was to investigate the relationship between interstitial cell and pacemaker activity in the human ureter through the recording of spontaneous contractions. Spontaneous contractions of eight circular and longitudinal smooth muscle strips of the human ureter to acetylcholine (ACh) and/or norepinephrine (NE) were observed. Human ureteral strips were divided into proximal and distal groups, and each group was subdivided into circular and longitudinal groups. The proximal group showed spontaneous activities of 3~4 times within 5 minutes in the longitudinal group. ACh ($10^{-4}\;M$) augmented the frequency of the spontaneous contractions. The cumulative application of NE also augmented the frequency in a dose-dependent manner. The effects of NE application were inhibited by concomitant application of $10^{-5}\;M$ glibenclamide. Receptor tyrosine kinase (c-kit) staining revealed abundant ICCs only in proximal tissues. Therefore, spontaneous contractions of the human ureter might be modulated by ICC in the proximal region, and the actions might be related with the activation of cholinergic and/or adrenergic system mediated by a glibenclamide-sensitive pathway.
