Pediatric Cerebral Vascular Malformations : Current and Future Perspectives

  • Intracranial vascular malformations typically encountered by pediatric neurosurgeons include arteriovenous malformations, vein of Galen malformations and cavernous malformations. While these remain amongst some of the most challenging lesions faced by patients and caregivers, the past decade has produced marked advances in the understanding of the pathophysiology of these conditions, with concomitant innovations in treatment. This article will highlight present and future perspectives relevant to these diseases, with a focus on an emerging approach utilizing disease-specific mutations to develop a novel taxonomy for these conditions.

  • Malignant tumors located near the papilla of the duodenum can cause concomitant biliary and duodenal obstruction, which reduces the quality of life and increases the morbidity and mortality. Apart from traditional surgical treatment methods, various treatment methods such as endoscopic treatment and radiological interventions are used for the treatment. This study aimed to explore treatment methods according to the situation of patients with malignant biliary obstruction combined with duodenal obstruction.

Are Bladder Neoplasms More Aggresive in Patients with a Smoking-related Second Malignancy?

  • Background: Relationships between smoking and bladder neoplasms, one of the common malignancies, are well-known. Different smoking-related malignancies may occur together. In this study, we evaluated the stage and grade of bladder neoplasms in patients also featuring lung or larynx cancer. Materials and Methods: From January 2006 to February 2012, patients who underwent surgery for bladder neoplasms in our clinic were screened retrospectively. In the evaluation, 5 patients had larynx cancer and 20 patients have lung cancer in addition, all having been smoking for a long time. The bladder tumor stage and grade were investigated in these 25 cases. Results: Mean age of patients was 66.8 (49-78). In the evaulation, all of 5 patients who had larnyx cancer also had high grade urothelial cancer. One had T2 urothelial, and 3 T1 urothelial cancer. In the same way, all of the 20 patients with lung cancer also have high grade urothelial cancer, three T2, and 13 T1. Bladder cancer stage and grade were determined to be significantly increased in patients with concomitant bladder and lung or larynx cancer. Conclusions: In the patients who have smoking releated second malignancy, bladder cancer prognosis appears more aggressive. We now need a larger series and multi-center studies for understanding relevant pathophysiology.

Oviposition Patterns Associated with Prolactin Concentration in Domestic Chicken (Gallus domesticus)

  • Physiological mechanisms, involved in unusual ovulatory sequences in domestic hen are remaining undefined. One hundred individually caged white leghorn birds were divided into two equal groups viz. control and treatment, and 2-bromo-$\alpha$-ergocryptine, was administered to birds in the treatment group to modulate prolactin (PRL) secretion from anterior pituitary gland. The effect of modulation of PRL concentrations on egg production, sequence length and intersequence pause length were studied by analysis of oviposition records of the birds from 24 to 72 weeks of age. The surviving 48 birds in the control and treatment groups averaged $34.58{\pm}1.7$ and $25.67{\pm}1.15$ sequences of oviposition, with a mean sequence length of $9.92{\pm}0.63$ and ${\pm}1.12$ days respectively. Most of the birds had a single characteristically long sequence during the entire reproductive cycle, which averaged $46.04{\pm}3.09$ days in the control birds and $59.33{\pm}4.44$ days in the treated birds. 2-bromo-$\alpha$-ergocriptine treatments had significantly decreased (p$\leq$0.01) the circulating concentrations of PRL compared to the birds of the control group. This resulted in a significant increase (p$\leq$0.01) in the number of laying days in birds of the treatment group with a concomitant decrease in the intersequence pause length. The decreased PRL levels during prime sequences in birds of the both groups, reveals the negative role of the circulating PRL levels on egg production with concomitant shorter intersequence pause length. Hence, modulation of PRL with dopamine agonist may enhance the reproductive efficiency of hens later in life.

  • Saccharin, the first artificial sweetener, was discovered in 1879 that do not have any calories and is approximately 200~700 times sweeter than sugar. Saccharin was the most common domestically produced sweetener in Korea in 2010, and it has been used as an alternative to sugar in many products. The interaction between artificial sweeteners and drugs may affect the drug metabolism in patients with diabetes, cancer, and liver damage, this interaction has not been clarified thus far. Here, we examined the effects of the potential saccharin-drug interaction on the activities of 5 cytochrome P450 (CYPs) in male ICR mice; further, we examined the effects of saccharin (4,000 mg/kg) on the pharmacokinetics of bupropion after pretreatment of mice with saccharin for 7 days and after concomitant administration of bupropion and saccharin. Our results showed saccharin did not have a significant effect on the 5 CYPs in the S9 fractions obtained from the liver of mice. In addition, we observed no differences in the pharmacokinetic parameters of bupropion between the control group and the groups pretreated with saccharin and that receiving concomitant administration of saccharin. Thus, our results showed that saccharin is safe and the risk of saccharin-drug interaction is very low.

  • Tsutsugamushi disease (Scrub typhus) is an acute, febrile illness caused by Orientia tsutsugamushi, which is transmitted to humans through chigger bites. Leptospirosis, a febrile disease caused by various pathogenic Leptospira, and is acquired by exposure to contaminated water and soil. Both diseases have been the most common acute febrile diseases in the autumn in Korea for many years. Concomitant leptospirosis and scrub typhus is quite rare. We report a case of a coinfection with leptospirosis and scrub typhus in a 51-year-old male who presented with fever, abdominal pain and acute dyspnea. The patient was diagnosed with as acalculous cholecystitis, acute respiratory distress syndrome, and septic shock caused by the infection. This is the first case report of a coinfection with leptospirosis and scrub typhus in Korea.

A Clinical Study about Factors and Improvement Score on Sasang Constitution of Bell's Palsy Patients. (특발성 구완와사로 입원한 환자에 대한 사상체질별 유형분석)

  • Objectives : This study was conducted in order to analyze the admission patients of facial nerve paralysis by Sasang constitution. Methods: A clinical study was done on 61 admission patients who were diagnosed and treated as facial nerve paralysis (Bell's palsy) from September, 2005 to May 2006 at the Dept. of Acupuncture and Moxibution, Chung-ju Hospital, college of Daejeon Oriental Medicine. We classified 61 patients according to Sasang constitution by QSCC II, attack side, sex, cause factor, concomitant symptom and Yanagihara' s score at the admission and discharge, and analyzed the improvement of Yanagihara's score between Sasang constitution groups. Results: 1. By sex, the ratio of Taeumin was higher than that of Soyangin in female. 2. By attack side, there is no significant difference between constitution groups In concomitant symptoms and cause factors. 3. By improvement of Yanagihara's scores between admission and discharge, the improvement score in Soyangin was a little bit higher than that in Soeumin, but there is no significant difference. Conclusion : The results suggest that there is no significant difference in all aspects except sex in Sasang constitution. But it is considered that we need to keep on studying afterwards because we had limits in getting enough patients.

Tonsillectomy as prevention and treatment of sleep-disordered breathing: a report of 23 cases

  • Background: The paradigm of tonsillectomy has shifted from a treatment of recurrent throat infection to one of multi-discipline management modalities of sleep-disordered breathing (SDB). While tonsillectomy as a treatment for throat problems has been performed almost exclusively by otorhinolaryngologists, tonsillectomy as a part of the armamentarium for the multifactorial, multidisciplinary therapy of sleep-disordered breathing needs a new introduction to those involved in treating SDB patients. This study has its purpose in sharing a series of tonsillectomies performed at the Seoul National University Dental Hospital for the treatment and prevention of SDB in adult patients. Methods: Total of 78 patients underwent tonsillectomy at the Seoul National University Dental Hospital from 1996 to 2015, and 23 of them who were operated by a single surgeon (Prof. Jin-Young Choi) were included in the study. Through retrospective chart review, the purpose of tonsillectomy, concomitant procedures, grade of tonsillar hypertrophy, surgical outcome, and complications were evaluated. Results: Twenty-one patients diagnosed with SDB received multiple surgical procedures (uvulopalatal flap, uvulopalatopharyngoplasty, genioglossus advancement genioplasty, tongue base reduction, etc.) along with tonsillectomy. Two patients received mandibular setback orthognathic surgery with concomitant tonsillectomy in anticipation of postoperative airway compromise. All patients showed improvement in symptoms such as snoring and apneic events during sleep. Conclusions: When only throat infections were considered, tonsillectomy was a procedure rather unfamiliar to oral and maxillofacial surgeons. With a shift of primary indication from recurrent throat infections to SDB and emerging technological and procedural breakthroughs, simpler and safer tonsillectomy has become a major tool in the multidisciplinary treatment modality for SDB.

  • The treatment of adults with dentofacial deformities is frequently complicated by existence of discrepancies in transverse dimension. Control of the transverse dimension through surgical technique is now a valuable treatment consideration in non-growing patients. Proper diagnosis of relative and absolute maxillary transverse deficiency is imperative prior to surgical intervention. Therapeutic purpose should be implemented accordingly to facilitate correction of transverse discrepancies concomitant with treatment objectives in sagittal dimension in order to provide the patient with the most stable and functional result possible. Concomitant surgical correction of narrow palate require less treatment time, treatment cost and post-treatment relapse is less than to do only orthodontic therapy. This report is concerned with rapid palatal expansion by a combination of corticotomy and orthodontic treatment.

Antibiotic Susceptibility of Helicobacter pylori and the Combination Effect of Antibiotics on the Antibiotic-Resistant H. pylori Strains (위암환자에서 분리된 Helicobacter pylori의 항생물질에 대한 감수성과 저항성 균에 대한 항생제 병합요법의 효과)

  • The purpose of this study was to evaluate the existence of amoxicillin, clarithromycin, and metronidazole resistance Helicobacter pylori and to determine the in-vitro MIC of two and three kinds of antibiotic concominant administration in the isolates. The antimicrobial agents tested against 169 H. pylori included metronidazole, amoxicillin, ciprofloxacin, clarithromycin, omeprazole, josamycin, erythromycin, and tetracycline. MIC of each antimicrobial agents was determined by broth microdilution method. The 169 strains of H. pylori were isolated from biopsy specimens of patients with gastric cancer. $MIC_{50}$ of clarithromycin, amoxicillin, metronidazole, omeprazole, erythromycin, josamycin, tetracycline, and ciprofloxacin was 2.0, 1.0, 4.0, 8.0, 0.5, 0.5, and $0.5\;{\mu}g/ml$, respectively. $MIC_{90}$ of clarithromycin, amoxicillin, metronidazole, omeprazole, erythromycin, josamycin, tetracycline, and ciprofloxacin was 64.0, 64.0, 32.0, 16.0, 8.0, 2.0, and $1.0\;{\mu}g/ml$, respectively. H. pylori isolates were detected in the following resistaince rates: 34.3% to clarithromycin, 31.9% to metronidazole, 20.7% to amoxicillin, 12.4% to erythromycin, and 10.1% to josamycin. The prevalence of the antibiotic resistant strains of H. pylori were detected 18.1% for two kind of antibiotics and 9.6% for three kind of antibiotics, and 3.9% for four kind of antibiotics. The $MIC_{90}$ of clarithromycin-, metronidazole-, and amoxicillin-resistant H. pylori was decreased under the $1\;{\mu}g/ml$ by the two or three kind of antibiotic concomitant administration in-vitro. These results suggest that two or three antibiotics concomitant administration could be more effective for the treatment of clarithromycin-, amoxicillin-, metronidazole-, and josamycin-resistant H. pylori strains.
