Correction of a nasal soft triangle deficiency as a complication of augmentation rhinoplasty

  • The soft tissue triangle is an easily recognizable subunit of the nose. Therefore, deformities in this region resulting from trauma or complications after cosmetic surgery can have serious cosmetic impacts. Various reconstruction choices exist for deformities such as depression of the soft triangle but choosing the most appropriate treatment in each case remains a challenge. In the case described herein, a patient underwent augmentation rhinoplasty with a silastic implant and experienced implant exposure in the soft triangle area. After implant removal, the patient complained of depression in this area. The authors effectively solved this problem through a de-epithelialized composite tissue graft. In this report, we present this case and review similar cases of reconstruction of the soft triangle.

  • Purpose: This study evaluated the capability of bone formation with silk fibroin/nano-hydroxyapatite/corn starch composite scaffold as a bone defect replacement matrix when grafted in a calvarial bone defect of rabbits $in$ $vivo$. Methods: Ten New Zealand white rabbits were used for this study and bilateral round-shaped defects were formed in the parietal bone (diameter: 8.0 mm). The silk fibroin 10% nano-hydroxyapatite/30% corn starch/60% composite scaffold was grafted into the right parietal bone (experimental group). The left side (control group) was grafted with a nano-hydroxyapatite (30%)/corn starch (70%) scaffold. The animals were sacrificed at 4 weeks and 8 weeks. A micro-computerized tomography (${\mu}CT$) of each specimen was taken. Subsequently, the specimens were decalcified and stained with Masson's trichrome for histological and histomorphometric analysis. Results: The average ${\mu}CT$ and histomorphometric measures of bone formation were higher in the control group than in the experimental group at 4 weeks and 8 weeks after surgery though not statistically significant ($P$ >0.05). Conclusion: The rabbit calvarial defect was not successfully repaired by silk fibroin/nano-hydroxyapatite/corn starch composite scaffold and may have been due to an inflammatory reaction caused by silk powder. In the future, the development of composite bone graft material based on various components should be performed with caution.

고전적 및 삼각조각 기법을 이용한 단-측면 미세동맥 접합술의 실험적 연구 (The Experimental Study of the End-to-side Microarterial Anastomosis with the Longitudinal Slit and the Triangular Flap)

  • Problems of composite tissue transfer commonly arise when a single indispensable recipient vessel receives the graft vssel, and the graft vessel must be sutured in end-to-side fashion so as not todisturb the vascularity of the recipient vessel. The triangular flap in the recipient vessel wall gives an intact endothelial surface when the flow of blood stream is presented and may reduce the chance of anastomosis. We selected mature Wistar rats weighing over 450 grams to compare the conventional longitudinal slit from the triangular flap in the recipient carotid artery over bloood pressure and blood flow when the donor carotid artery was anastomosed in end-to-side fashion. In 30 minutes after anastomosis, maximum blood pressure measured in the donor carotid arterial side when the recipient arterial wall was fasioned with the longitudinal slit was recorded 114 mmHg and with the triangular flap 100mmHg. Minimum blood pressure with the longitudinal slit was 98mmHg and with the triangular flap 88mmHg. The amount of blood collected for 30 seconds in the conventional longitudinal slit was 1.18mg and in the triangular flap 0.78mg. Histology study in 30 minutes, the conventional longitudinal slit demonstrated the more hemorrhagic features around the suture material compared to that of the triangular. flap and, in the 7th day, the conventional longitudinal slit demonstrated the more prominent granulomatous reactions and vascular proliferations around the suture material compared to that of the triangular flap.

An Analytical Study of Mammalian Bite Wounds Requiring Inpatient Management

  • Background Mammalian bite injuries create a public health problem because of their frequency, potential severity, and increasing number. Some researchers have performed fragmentary analyses of bite wounds caused by certain mammalian species. However, little practical information is available concerning serious mammalian bite wounds that require hospitalization and intensive wound management. Therefore, the purpose of this study was to perform a general review of serious mammalian bite wounds. Methods We performed a retrospective review of the medical charts of 68 patients who were referred to our plastic surgery department for the treatment of bite wounds between January 2003 and October 2012. The cases were analyzed according to the species, patient demographics, environmental factors, injury characteristics, and clinical course. Results Among the 68 cases of mammalian bite injury, 58 (85%) were caused by dogs, 8 by humans, and 2 by cats. Most of those bitten by a human and both of those bitten by cats were male. Only one-third of all the patients were children or adolescents. The most frequent site of injury was the face, with 40 cases, followed by the hand, with 16 cases. Of the 68 patients, 7 were treated with secondary intention healing. Sixty-one patients underwent delayed procedures, including delayed direct closure, skin graft, composite graft, and local flap. Conclusions Based on overall findings from our review of the 68 cases of mammalian bites, we suggest practical guidelines for the management of mammalian bite injuries, which could be useful in the treatment of serious mammalian bite wounds.

Bentall 술식의 조기 성적 (Early Result of Bentall Operation)

  • 아산재단 서울중앙병원 흉부외과에서는 1993년 10월부터 1997년 4월 사이에 대동맥륜대동맥 확장환자에서 37례의 Bentall술식을 시행하였다. 환자군의 술전 호흡곤란 정도는 NYHA Class I이 4례, Class II가 10례, Class III가 20례, Class IV가 3례였다. 수술은 대동맥박리증이 동반된 경우 초저체온법(식도체온: 11.8$\pm$1.6!), 완전순환정지, 역행성 대뇌관류법을 시행하였다. 37례 모두 병변이 있는 대동맥벽을 대부분 제거하고 복합이식편이나 대동맥 인조혈관 및 인공판막을 이용하여 상행대동맥과 대동맥판막을 치환하였고, 관상동맥편을 인조혈관에 직접 연결하였다. 수술결과는 합병증은 7례 발생하였으며(18%), 수술사망은 없었으나 뇌혈관 발작으로 인한 1례의 만기 사망이 있었다(2.7%). 퇴원후 추적기간은 1개월에서 36개월까지였으며 평균 9.6개월였다. 결론적으로 대동맥륜대동맥 확장의 수술에 있어 Bentall술식은 안전하고 좋은 수술이라 할 수 있다.

Marfan 증후군에 동반된 우관상동맥 폐쇄증 - 1 례 보고 - (Right coronary artery atresia in Marfan's syndrome: A case report)

  • 우관상동맥 폐쇄증을 동반한 Marfan 증후군환자를 보고한다. 환자는 45세 여자로 약 1년 전에 Marfan 증후군으로 진단받고, 최근 흉통을 주소로 내원하였다. 환자는 손가락과 발가락이 긴 지주증과 돌출흉이 있고 몸통에 비해 사지가 긴 전형적인 외형을 갖고 있었다. 시력이 매우 약하고 수정체의 아탈구가 있었다. 심초음파상 대동맥판막 폐쇄부전증은 경도로 있었고, 좌심실의 중등도 확장과 심박출율의 중등도 저하가 있었다. 수술시에 우관상동맥의 폐쇄증을 발견하였다. 대동맥판막 폐쇄부전증과 대동맥 확장증에 대해서 SJM 27mm composite graft를 이용하여 좌관상동맥에는 Bentall 술식과 우관상동맥에는 PTFE 6mm를 이용하여 변형된 Piehler 식 방법으로 수술을 하였다. 원래의 우관상동맥구로 추정되는 부분부터 우관상동맥 원위부의 혈류가 있는 곳까지의 결손부위는 약 4cm 가량 되었다. 관상동맥 폐색증과 Marfan 증후군의 조합은 매우 드문 질환으로 양측 관상동맥의 혈류 재건을 위하여 각기 다른 술식으로 수술하였다.

  • 소아에 있어서 수부손상의 대부분은 가정이나 외부로부터의 위험인자에 대한 자기방어능력이 부족한데서 발생되는데, 핵가족화로 인해 자녀에 대한 부모의 관심이 줄어들면서 그 빈도가 더욱 증가되고 있는 추세이다. 본 영남대학교 의과대학 성형외과학교실에서는 1983년 6월부터 1991년 10월까지 수부손상으로 재건술을 받은 10세이하의 소아를 대상으로 조사를 시행하여, 주로 봄, 가을에 활동력이 주어지기 시작하는 2세-4세의 남아에서 수지 첨부와 zone II에 많이 발생하고, 문틈에 끼여서 손상을 입는 경우가 많았으며, 전충피부이식이나 복합조직이식에서 좋은 결과가 나타남을 알 수 있었다.

The BMPs expression and histomorphometric study of ${\beta}-TCP$ / rhBMP-2 Grafting on the rabbit cranial bone defects

  • Objective: The Purpose of the study was to investigate the bone morphogenic protein expression of rhBMP-2(recombinant human bone morphogenic protein-2) as singnaling molecule and ${\beta}-TCP$(Tricalcium phosphate) as a bone substitute and carrier medium of rhBMP-2. Materials and Methods: 16 rabbits divided into 2 group of each 8 rabbit. Two standardized bone defect, round bilateral defect was made in the cranium of the 8 rabbit of first group, and was grafted with $150{\sim}500{\mu}m$ diameter ${\beta}-TCP$ 0.25g in one side, which was soaked with rhBMP-2, and autogenous bone was grafted on another side as a positive control. Second group of 8 rabbit, only ${\beta}-TCP$ was grafted with same size and same manner. After 2, 4, 8, and 12 weeks, specimen was taken for microscopic immunohiostochemical and histomorphometric analysis. Result: Grafting ${\beta}-TCP$ with rhBMP show the early formation of the bone regenerative factor (BMP-4) and more quantity of new bone formation than only use of ${\beta}-TCP$ (8,12 week), even show less new bone formation than autogenous bone. Conclusion : The experimental study result that ${\beta}-TCP$ graft combination with rhBMP-2 as a delivery system is an effective with osteoinductive capacity and biodegradable properties, so that provide clinical availibility of composite use in reconstruction of bony defect.

  • 영상기술과 수술 술식의 발전으로 골반골의 악성종양에서 내 골반골 절제술이 보편화되었으나 절제 후 골 결손의 기능적인 재건 방법에 대하여는 아직 논란의 여지가 많다. 골반환 재건술식 중 열처리한 자가골과 대퇴골 전치환물을 조합하는 방법은 생역학적이며 해부학적인 적합성이 장점으로 적절한 증례를 선택하면 좋은 결과를 얻을 수 있다. 그러나 이 술식 후 감염이나 기계적인 파괴가 발생 시 내고정물과 자가골 전부를 제거하는 것이 불가피하다. 이런 경우 가관절을 형성하더라도 심한 하지 단축이 문제가 된다. 저자들은 이런 문제점을 해결하기 위해 대퇴골두를 제거하지 않고 골반골만 열처리 후 재삽입 한 1례를 보고하고자 한다.

Comparison on Mechanical Properties of SSBR Composites Reinforced by Modified Carbon black, Silica, and Starch

  • Solution-styrene-butadiene rubber (SSBR) composites were manufactured using four kinds of fillers: silica-silane coated carbon black (SC-CB) hybrid, starch-SC-CB hybrid, pure silica, and pure starch. The influence of filler type on the mechanical properties of the rubber matrix was studied in this work. SC-CB was prepared by silane-graft-coating using vinyl triethoxy silane and carbon black, which enhanced the dispersion effect between the rubber matrix and the filler, and improved the mechanical properties of the compounds. The morphology of the composites was observed by field-emission scanning electron microscopy (FE-SEM). The thermal decomposition behavior of the composites was determined by thermogravimetric analysis (TGA), and the crosslinking behavior of the composites was tested using a rubber process analyzer (RPA). The hardness, tensile strength, swelling ratio, and gas transmittance rate of the composites were evaluated according to ASTM. The test results revealed that with the addition of SC-CB, the hybrid fillers, especially those blended with silica, showed a better reinforcement effect, the highest hardness and tensile strength, and stable thermal decomposition behavior. This implies that the silica-SC-CB hybrid filler has a notable mechanical reinforcement effect on the SSBR matrix. Because of self-crosslinking during its synthesis, the starch-SC-CB hybrid filler produced the most dense matrix, which improved the anti-gas transmittance property. The composites with the hybrid fillers had better anti-swelling properties as compared to the neat SSBR composite, which was due to the hydrophilicity of silica and starch.
