Nurse Practitioner Roles and Curriculums in the United States (미국 전문간호사(NP)의 역할과 교육과정에 관한 고찰)

  • Based on literature, status and role of the NP in America was reviewed. The process of developing NP program in America suggests us many things. In America, nurse practitioners have sustained a mutually beneficial status with their patients for over thirty years. Excel fence in academic education and clinical training will enable nurse practitioners to continue to provide quality health care. The magnitude changes in the health care system of the United States, the challange of providing real access of health care continues. Lack of access to adequate primary care was the driving force in the initial 1965 Federal Involvement in developing the NP role. In 1993 President Bill Clinton's health care reform initiative provided policy support for NPs as primary care providers. The Institute of Medicine explicitly recognized NPs as an integral part of the primary care team. In addition, several national reports recognized NPs as affordable, accessible, high-quality care providers. The recent passage of direct Medicare reimbursement for NPs reflected public policy statements coincided with and likely contributed to a growth spurt in the NP workforce. From 1965 to 1977 NP programs offered traditional primary care clinical tracks(adult, family, woman's health, and pediatrics) for relatively small clusters of students in a variety of institutional settings. From 1978 to 1990 these educational programs were incorporated into graduate schools of nursing. By 1990 the majority of NPs received educational preparation in master's-level nursing programs. A new emphases was placed on postmaster's NP programs designed for master's prepared clinical nurse specialists and nurse managers. he the health care system shifted hospital nursing resources toward community-based care, these master's -level nurses sought additional NP preparation. NP educational programs are defined as the educational structure in which one or more NP clinical tracks are offered. NP clinical tracks, in turn, offer curriculum and supervised clinical experiences that match standards in specific practice areas such as family(FNP), adult(AUP), geriatrics(GNP), pediatrics(PNP), women's health (WHNP), neonatal (NNP), and acute care(ACNP). There were indications that NP practice was expanding into new clinical areas as evidenced by new types of tracks, particularly in acute care and psychiatry. The increase in acute care NP students likely reflects the increased demand from hospitals and other acute care settings. In Korea, change of nurse's role into nurse practitioner's role may have many difficulties. The need of health consumer, policy support of government, approval of medical care team are all essential component. Every nursing personnel make effort to planning the new health care delivery system.

Trend analysis of new health technology in dentistry (치의학 분야의 신의료기술 동향 분석)

  • Objectives : Currently, it is being reported that new health technology (HT) is introduced and spread throughout Korea at a very fast rate. However, the current status of new HT related to dentistry has not been known or studied. Therefore, this study aims to understand the present state and awareness on dentistry-related domestic new HT via surveys and research, and thus apply this as a reference to construct an effective new HT-related system in the field of dentistry. Methods : The assessment status of New Health Technology Assessment (nHTA) committee was searched on the website for any updates on new HT5). The cases were searched in the range between Jan 1 2012 and Jul 31 2016 by submission date and were analyzed by year, division of technology, progress and major operating practitioner. Among those, technologies utilized by dentists were additionally analyzed. The survey result was obtained by combining responses of 25 participants from the workshop for new HT application and 11 members of Korean Association of Oral and Maxillofacial Surgeons (KAOMS). The workshop was co-hosted by KAOMS and Clinical Translational Research Center for Dental Science (CTRC) under Seoul National University Dental Hospital (SNUDH). Results : Most of the respondents to the survey have heard of new health technology but were unaware of the application procedure. 'Safety and effectiveness' was selected the most for things to consider when introducing new HT, followed by 'scientific evidence of the technology.' For new dental HT status, submissions between 2012 and Jul 31 2016 were analyzed by major operating practitioner, including cases where there are multiple practitioners. Yet, only 19 cases (17 %) of dentists were reported. Among 385 cases approved by nHTA, only 2 cases included dentists. Conclusions : Active publicity by the government on new HT and its application procedure, and continued interest and research in the dental and medical community is likely to be required for the development of dentistry.

Comparative Analysis of Community Health Practitioner's Activities and Primary Health Post Management Before and After Officialization of Community Health practitioner (보건진료원의 정규직화 전과 후의 보건진료원 활동 및 보건진료소 관리운영체계의 비교 분석)

  • To provide better health care services to the rural population, the government has made the Community Health Practitioner(CHP) a regular government official from April 1, 1992. This study was carried out to study the impact of officialization of CHP on the activities and management system of Primary Health Post(PHP). Fifty PHPs were selected by two stage sampling, cluster and simple random, from 595 PHPs in Kyungnam and Kyungpook provinces. Data were collected by a personal interview with CHPs and review of records and reports kept in the PHPs. The study was done for the periods of January 1-March 31, 1992 (before officialization) and January 1-March 31, 1993 (after officialization). Ninety-six percent of the CHPs wanted to become a regular government official in the hope of better job security and higher salary. The proportion of CHPs who were proud of their iob was increased from 24% to 46% after officialization. Those CHPs who felt insecure for their job decreased from 30% to 10%. Monthly salary was increased by 34% from 802,600 Won to 1,076,000 Won and 90% of the CHPs were satisfied with their salary, also more CHPs responded that they have autonomy in their work planning, implementation of plan, management of the post, and evaluation of their activity. There were no appreciable changes in such CHPs' activities as assessment of local health resources, drawing map for the catchment area, utilization of community organization, grasping the current population structure in the catchment area, keeping the family health records, individual and group health education, and school health service. However, the number of home visits was increased from 13.6 times on the average per month per CHP to 27.5 times. More mothers and children were referred to other medical facilities for the immunization and family planning services. Average number of patients of hypertension, cancer, and diabetes in three months period was decreased from 12.7 to 11.6, from 1.5 to 1.2, and 4.3 to 3.4, respectively. Records for the patient care, drug management, and equipment were well kept but not for other records. The level of record keeping was not changed after officialization. The proportion of PHPs which had support from the health center was increased for drug supply from 14.0% to 30.0%, for consumable commodities from 22.0% to 52.0%, for maintenance of PHP from 54.0% to 68.0%, for supply of health education materials from 34.0% to 44.0%, and supply of equipment from 54.0% to 58.0%. Total monthly revenue of a PHP was increased by about 50,000 Won; increased by 22,000 Won in patient care and 34,700 Won in the government subsidy but decreased in the membership due and donation. However, there was no remarkable changes in the expenditure. The proportion of PHPs which had received official notes from the health center for the purpose of guidance and supervision of the CHPs was increased from 20% to 38% during three months period and the average number of telephone call for supervision from the health center per PHP was increased from 1.8 to 2.1 times(p<0.01). However, the proportion of PHPs that had supervisory visit and conference was reduced from 79% to 62%, and from 88% to 74%, respectively. The proportion of CHPs who maintained a cooperative relationship with Myun Health Workers was reduced from 42% to 36%, that with the director of health center from 46% to 24%, that with the chief of public health administration section from 56% to 36%, and that with the chairman of PHP management council from 62% to 38%. Most of the CHPs (92% before and 82% after officialization) stated that the PHP management council is not helpful for the PHP. CHPs who considered the PHP management council unnecessary increased from 4% to 16%(p<0.05). Suggestions made by the CHPs for the improvement of CHP program included emphasis on health education, assurance of autonomy for PHP management, increase of the kind of drugs that can be dispensed by CHPs, and appointment of an experienced CHP in the health center as the supervisor of CHPs. The results of this study revealed that the role and function of CHPs as reflected in their activities have not been changed after officialization. However, satisfaction in job security and salary was improved as well as the autonomy. Support of health center to the PHP was improved but more official notes were sent to the PHPs which required the CHPs more paper works. Number of telephone calls for supervision was increased but there was little administrative and technical guidance for the CHP activities.

Factors Related to the Medical Service Performance of Rural Health Sub-centers (농어촌 보건지소 진료실적과 관련 요인)

  • Objectives: This study was conducted to identify factors that affect medical service performance in rural health sub-centers and to develop a method to improve health sub-center performance that takes advantage of these insights. Methods: This study included 1,242 South Korean health sub-centers that had been in operation at least since December 31, 2009 as units of analysis. After adjusting for population differences between areas, the performance of medical service among health sub-centers was analyzed according to medical services performed per person. We performed t-tests, ANOVA, Scheffe's tests and multiple regression analyses. Results: The following were significant variables affecting the medical service performance of rural health sub-centers: number of hospitals and clinics, presence of community health practitioner posts, distance from health sub-centers to main public health centers, distance from health sub-centers to the nearest emergency medical facilities, and proportion of the local population aged 65 and over. In contrast, the proportion of the local population between ages 0-4 and the placement of public health doctors that had already completed their internship were not significant variables. Conclusions: The medical service performance of health sub-centers located in rural areas is significantly affected by local population and health care environment characteristics, and therefore, it is imperative to develop strategies to provide differentiated service based on these factors.

A Study on the Projected Workforce for Public Health Nurses in Korea (보건간호사 인력수요 추계에 관한 연구)

  • Purpose: Recently there have been many changes in health care environments in Korea. To perform public health programs effectively and efficiently, it is necessary to analyze and identify the demand and supply for the public health nurses. Method: The study analyzed experts' opinions regarding the supply of public health nurses, as well as national and foreign statistical data on workforce supply of public health nurses. Two methods for estimating the amount of demand for public health nurses were used: one was applying the indicators of developed countries for public health nurses based on population: the other was to refer to regulations and/or recommended guidelines in Korea. Result: 1) The number of public health centers, public health sub-centers, and primary health care posts have decreased between 1990 and 2001, from 260 to 242, 1318 to 1270, 2038 to 1907, respectively. 2) Between 1997 and 2002, the number of public health nurses has also decreased from 5572 to 5112. 3) In the case of applying regulations, the number (5112) of existing public health nurses falls shortly by 942. 4) In 2001, the Korean population per one public health nurse was 9262. 5) In the case of applying regulations, the number of public health nurses required to meet the demand for health services in 2001 and 2020 is estimated at 5932 and 6347, respectively. 6) In the case of applying the indicators of developed countries, the number of public health nurses required to meet the demand for health service in 2001 and 2020 is estimated at 9.469 and 10.310, respectively. Conclusion and suggestions: Because of the importance of public health industry, public health nurses have been approved as a field specialist and specialized nurse practitioner by the newly revised legal regulation, there have been absence of approval of their role differentiation and capability. In addition, organizational activity and insufficient number of the public health nurses have contributed to the inactive utilization of them. As community public health is focused on caring individuals as well as organizations, it requires more autonomy and special skills than other fields. Therefore, public health nurses need to enhance the capability as health educator, consultant, and information management persons through advanced education course for public health nurses. Public health nurses need to be prepared as advanced nurse practitioners by receiving advanced education courses and field experiences.

The Effect on the Pain, Discomfort in Daily living and Life Satisfaction of Flexibility Exercise and Local Heat in Rural Elderly with Osteoarthritis (퇴행성관절염 노인의 유연성운동과 온열요법이 통증과 일상활동장애 정도 및 생활만족도에 미치는 영향)

  • Muscle stiffness, pain and discomfort in dally activities are cardinal symtoms on arthritis. To reduce these symtoms, a nonequivalent one-group pre & posttest experiment was carried out at a rural community health post area in Chonbuk province during eight weeks (from 25th of January to 20th of March, 1999). The subjects were sixteen rural old women(65-89yrs) with osteoarthritis. And to reduce the level of pain, discomfort in daily living and to increase that of life satisfaction, 30 minutes of flexibility exercise followed by $15{\sim}20\;minutes$ of local heat therapy were done three times a week. Pain scale from Arthritis Impact Measurement Scales developed by Meenan(1980) for pain, Jette's Daily Activity Discomfort Scales revised by Lee(1988) for discomfort and Life Satisfaction Scales developed by Youn(1982) for life satisfaction were used as research instruments. The data were collected by face-to-face interviews with Investigator and the experiments were done under demonstration and supervision of investigator in community health post. The data were analyzed in percentage and Wilcoxon sign rank sum test by SPSS/PC+ program. The result could be summarized as follows ; 1. The scores of experiment group who were applied hot bag followed by flexibility exercise were tended to be significant lower in the level of pain statistically(Z=4.949, P=0.0001). 2. The scores of experiment group who was applied hot bag followed by flexibility exercise were tended to be significant lower in the level of discomfort in daily living statistically(Z=2.7226, P=0.0279). 3. The scores of experiment group who was applied hot bag followed by flexibility exercise were tended to be higher in the level of life satisfaction, but there was no significant statistical difference(Z=-1.2087, P=0.2171). In conclusion, flexibility exercise with local heat therapy could be identified as an effective therapy to reduce pain and discomfort in daily living of the rural elderly with osteoarthritis. And as a community health practitioner, the investigator concluded that this exercise with local heat therapy could be developed as a health promotion program for the rural aged people.

  • In this county, the gap between the urban 'haves' and the rural 'have-nots' continues to be an increasing problem. WHO and UNICEF see primary health care(PHC) as the key to achieving an acceptable level of health throughout the world as a community development. PHC is essential health care made accessible to individuals and families in the community by means acceptable to them. It is the first level of contact of individual, the family, and community with the national health system. It includes at least education on health system. It includes at least education on health problems, promotion of food supply, MCH including family planning, immunization against infectious diseases, control of endemic diseases, treatment of common diseases and injuries, promotion of mental health, and provision of essential drugs. However, of the aboves, education concerning of mental health problems and the methods to identify, prevent, and control them is the principal step of establishment. In Korea, the category of PHC worker includes the physician as public doctor and nurse as primary health care practitioner and community health leader as village health worker. PHC workers of the aboves will thus function best if they are appropriately trained to respond to the health needs of the community. However in this country, since the national PHC service project launched in 1980, the government has not developed and performed appropriate and enough education and training activities. In light of above reasons, several categories of health education activities had been planned and performed being aimed at above specific target groups and the main focus was on the village health workers for about one year from July 1991 to July 1992 in Yeoju Kun of Kyonki Province. At the end of the period, evaluation of education input was carried out to measure the improvement of healthful life of people in terms of awareness, attitude, and practice. The totals of 80 village health workers, 13 public health practitioners and 9 public docters took in the course of health education for a few hours at every month and the evaluation works of educational effect were taken. The results the study were as follows. 1) Number of persons who realized the maxim "health care of the people is a duty of the government" increased after the education course, On the other hand, the rate of satisfaction on the effort of government for health promotion of the people decreased. 2) Public doctors and primary health care practitioners(nurses) liked and enjoyed the education schedule as a meeting of peer group. It provided chances of communication with staffs of Korea University Hospital. It was said that lectures covered great deal of knowledge and technic they urgently needed in the field. 3) After finishing the education course, more of village health workers(VHW) thought they adapted themselves to their roles and functions showing increased number of home visit and contact with primary health care practitioners by month. 4) In case of patient refer, VHW preferred primary health care practitioners to public doctors. 5) Capability of VHWs in most of their functions increased dramatically after when the education course finished except tuberculosis control.

The Effect of a Self-Regulation Program for Hypertensives in Rural Areas (일부 농촌지역 고혈압 환자의 자가간호 증진과 혈압조절을 위한 자기조절 프로그램 효과)

  • This study was conducted to identify a effect of the comprehensive self-regulation program for hypertensives as a nursing intervention on self-care performance and the physiologic parameters in rural areas. For this purpose, a nonequivalent control group and a pre and post-test quasi- experimental design was used. Fifty-four were evaluated subjects from two Community Health Practitioner Posts in the suburbs of Taejon City. The subjects of the control and experimental groups were matched for age and sex. The self-regulation program developed by author given to the experimental group. The program consisted of group education on hypertension and self-care, self-regulation including the blood pressure self-monitoring, recording of self-care activities, and encouraging and reinforcing self-efficacy. The whole program was carried out from September to November of 1999. The data were analyzed by repeated measure ANCOVA, t-test, and ANCOVA. The results were as follows ; There was significant improvement in the scores on knowledge (F=.68, P=.004), perceived self-efficacy (F=26.39, P=.000), self-care performance (F=26.11, P=.000) of the experimental group compared with those of the control group. There was no significant change on the score of locus of control, perceived benefits and perceived barrier, blood cholesterol level, body weight between two groups (P>.05). From these results, it can be concluded that the self-regulation program is an effective nursing strategy to promote self-care performance of hypertensives in rural area.

  • This study suggested cooperative governance cases and improvement strategies between local welfare centers and urban regeneration support centers, centered on urban revitalization areas around Seoul Station, by applying FGI. Based on the analysis of the contents of FGI participating in the cooperative governance process, it was proved that the common role and identity between the local welfare center and the urban regeneration support center were regional based activities. And the advantage of constructing collaborative governance was analyzed by four ways. In addition, this study presented improvement strategies from a microscopic and macro perspective. The results of this study suggest that the change in the curriculum of social workers' education and the expansion of the scope of the practitioner in the Urban Regeneration Business Act propose.

  • 인간 궁극의 목적은 ''행복한 생활''이며 이를 위한 가장 중요한 조건이 ''건강''이라는데 이론을 제기할 사람이 없다. 그간 우리나라는 1970년대를 거치면서 절대빈곤을 해결하느라고 경제개발계획을 수립하는 등 최선의 노력을 해 왔으며, 따라서 보건정책은 등한시 해 왔던 것이 사실이며 이는 극히 당연한 일이라고 생각한다. 최근 경제 개발 계획의 성공적인 시행으로 중진국의 대열에 서게 되었다. 산업 전략이 빚는 인구 이동의 불가피성, 인구의 도시집중 현상과 사회 구조의 변화는 생활방식 뿐 아니라 의식구조의 변화와 가치관의 변화를 가져왔으며 질병 양상과 건강 문제의 양상을 변화시켰다. 이로 말미암은 경제적, 지역적, 심리적 및 문화적 불균형 상태는 건강관호와 보건의료제도에도 불균형 상태를 갖고 와 불가피하게 된다. 이러한 불균형은 해소되어야 하는 시점에 온 것이며 정부가 목표로 하는 복지사회 건설을 위한 가장 중요한 보건 정책에 정부가 역점을 두게 된 것은 극히 당연하다. 이에 1976년 발족한 한국 보건 개발 연구원이 1977년부터 시작한 시법 사업을 실시한 결과, 지역 주민의 반응, 수용성, 의료 이용도, 의료비 절감 등을 분석하고 그 효율성을 인정받아 1981년 12월 31일 농어촌 보건의료를 위하여 특별 조치법을 제정하고 의료시설, 요원의 도시편중 교통의 불편, 고가의 의료 수가로 소외되어 오던 보건의료 취약지역 주민에게 기본권으로서의 기초 건강 서비스를 제공하기에 이르렀다. 이 건강 관호 제도에 그 바탕을 두며 보건 진료원이 그 척추의 역할을 담당한다. 1981년도와 1982년도에 선발되어 교육을 받고 배치된 738명의 보건 진료원은 38만명의 벽오지주민에게 현재 의료의 손길을 펴고, 질병의 예방을 위한 조치를 취하며 건강의 유지, 증진을 위하여 활동하고 있다. 건강관호는 시설이나 장비가 하는 것이 아니고 건물이 하는 것은 더욱 아니며, 지식과 기술을 갖춘 자격있는 의료인이 소명의식을 갖고 임할 때만 가능하다. 오랜숙원이었던 보건의료의 지역간 경제, 사회적, 문화적 계층간의 불균형을 해소하고 온 국민에게 기본권으로서의 건강을 갖도록 하는 이 새로운 제도는 패기에 넘치는 열정을 지닌 많은 젊은 간호학도들의 참여없이는 성공을 기대할 수 없다. 어떠한 제도이건 새로운 제도가 사회에 정착되기까지는 여러 해 동안의 시행착오와 고난이 반드시 수반되어 왔다는 사실을 우리는 역사를 통해 알고 있다. 그러나 그 제도가 다수를 위해 정의롭고 바람직한 제도일 때 반드시 성공을 거두었다는 사실도 알고 있다.
