Purpose: The purpose of this study was to examine the effects of inhalation and exhalation exercise combined with upper extremity proprioceptive neuromuscular facilitation pattern on two spirometry values: forced volume vital (FVC) and peak expiratory flow (PEF). Methods: Thirty-two healthy adults were divided into two groups: 1) a combined group, which performed upper extremity D2 flexion pattern (shoulder flexed/abducted/external rotated, forearm supinated, wrist radial deviated, and finger extended) during exhalation and D2 extension pattern (shoulder extended/adducted/internal rotated, forearm pronated, wrist ulnar deviated, and finger flexed) during inhalation; and 2) reverse combined group, which performed the D2 flexion pattern during inhalation and the D2 extension pattern during exhalation. The inverse application of upper extremity movements during inhalation and exhalation induced selective resistance or assistance on respiration. FVC and PEF were measured at two time points, before and after four weeks. Results: In both groups, the pre-post intervention comparison showed significant increases in FVC and PEF (p < .05). In the between-groups comparison, the reverse combined group showed a significantly higher PEF than the combined group at four weeks post intervention (p < 0.05). Conclusion: The combined respiration exercise with reverse PNF upper extremity patterns using selective resistance showed an effective increase in PEF in healthy adults. Clinicians and researchers might consider using selective resistance as a widely applicable and cost-effective option for respiratory rehabilitation planning.
The influence of various fermenting conditions using Saccharomyces cerevisiae, alone (Control, Single) and in combination with mixed lactic acid-producing bacteria (Combined 1, Mixed, Combined 2), including Bifidobacterium longum, Enterococcus faecium, and Lactobacillus acidophilus on flour sourdough preparation was examined. For the Combined 2 method, starters were incubated separately for 15 h, combined, and then further incubated for 10 h. Fermentation using Combined 2 improved the growth of mixed lactic acid-producing bacteria, but inhibited that of S. cerevisiae. This was also reflected in the extent of the pH reduction in sourdough produced in the Combined 2 step by these organisms. Among biochemical activities, $CO_2$ production and titratable acidity were increased by Combined 2, although the viable yeast counts were decreased. Aroma compounds in sourdough markedly varied according to fermentation conditions.
BACKGROUND/OBJECTIVES: The aim of this study was to evaluate the biological and sleep-promoting effects of combined ${\gamma}$-aminobutyric acid (GABA) and 5-hydroxytryptophan (5-HTP) using caffeine-induced sleepless fruit flies, ICR mice, and Sprague-Dawley rats. MATERIALS/METHODS: Video-tracking analysis was applied to investigate behavioral changes of Drosophila melanogaster. Pentobarbital-induced sleep test and electroencephalogram (EEG) patterns were used for analysis of sleep latency, duration, and quantity and quality of sleep in vertebrate models. RESULTS: Administration of combined GABA/5-HTP could significantly reverse the caffeine induced total distance of flies (P < 0.001). Also, individually administered and combined GABA/5-HTP significantly increased the total sleeping time in the caffeine-induced sleepless ICR mice (P < 0.001). In the caffeine-induced sleepless SD-rats, combined GABA/5-HTP showed significant differences in sleep quality between individual amino acid administrations (P < 0.05). CONCLUSIONS: Taken together, we identified inhibitory effects of combined GABA/5-HTP in locomotor activity, sleep quantity and quality in caffeine-induced sleepless models, indicating that combined GABA/5-HTP may be effective in patients with insomnia by providing sufficient sleep.

This study analyzes the design performance of a combined system of a recuperated cycle micro gas turbine (MGT) and a bottoming organic Rankine cycle (ORC) adopting refrigerant (R123) as a working fluid. In contrast to the steam bottoming Rankine cycle, the ORC optimizes the combined system efficiency at a higher evaporating pressure. The ORC recovers much greater MGT exhaust heat than the steam Rankine cycle (much lower stack temperature), resulting in a greater bottoming cycle power and thus a higher combined system efficiency. The optimum MGT pressure ratio of the combined system is very close to the optimum pressure ratio of the MGT itself. The ORC's power amounts to about $25\%$ of MGT power. For the MGT turbine inlet temperature of $950^{\circ}C$ or higher, the combined system efficiency, based on shaft power, can be higher than $45\%$.

A gravity wall combined with an anchoring lattice frame (a combined retaining structure) is adopted at a typical engineering site at Dali-Ruili Railway Line China. Where, the combined retaining structure supports a soil deposit covering on different inclined rock slopes. With an aim to investigate and compare the effects of inclined rock slopes on the response of combined retaining structure under seismic excitation, three groups of shaking table tests are conducted. The rock slopes are shaped as planar surfaces inclined at angles of 20°, 30°, and 40° with the horizontal, respectively. The shaking table tests are supplemented by dynamic numerical simulations. The results regarding the horizontal acceleration response, vertical acceleration response, permanent displacement mode, and axial anchor force are comparatively examined. The acceleration response is more susceptible to outer structural profile of combined retaining structure than to inclined angle of rock slope. The permanent displacement decreases when the inclined angle of the rock slope increases within a range of 20°-40°. A critical inclined angle of rock slope exists within a range of 20°-40°, and induces the largest axial anchor force in the combined retaining structure.

PURPOSE: The purpose of this study was to examine the effectiveness of a low-intensity combined exercise program on daily physical fitness parameters such as balance, flexibility, muscle strength, and fear of falling of elderly women in the community. METHODS: This study assigned 30 elderly women randomly into two groups: The control and experimental groups. The control group (n = 15) underwent routine gait. The experimental group (n = 15) underwent the low-intensity combined exercise. The exercise program in this study comprising combined exercise, including balance, flexibility, muscle strength training, and the exercise program using props was conducted twice a week for 8 weeks. RESULTS: The average age of the control and experimental groups was 77.27 years and 78.33 years, respectively. There were significant differences in static balance (t = -4.167, p < .001), dynamic balance (t = 2.463, p < .001), (t = -3.870, p < .001), (t = -2.262, p < .001), (t = -5.732, p < .001), (t = -6.573, p < .001), and fear of falling (t = -5.129, p < .001). CONCLUSION: The results show that low-intensity combined exercise is an effective intervention that improves physical health fear of falling in older women. The combined exercise program was found to be more effective in terms of physical function and fall-related psychological function compared to the control group that only walked.
The purpose of this study was to determined the converge effects of combined exercise rehabilitation on lower limb muscular function and cytokines in taekwondo player with patellofemoral pain syndrome. In this study, combined exercise rehabilitation(aquatic exercise 3 times a week, resistance exercise 2 times a week) was performed and 20 collegiate taekwondo player with patellofemoral pain syndrome(control group: n=10, combined exercise rehabilitation group: n=10) were participated for 8 weeks. Statistical significance verification was carried out by a two - way ANOVA repeated measures design as a mixture using the SPSS 18.0 program. The statistically significant level was set at 0.05. The results of this study were as following. Peak torque was higher significantly in extensor(p<.001) and flexor(p<.01) at $60^{\circ}/sec$ in the combined exercise rehabilitation group than the control group over combined exercise treatment period. Average power was higher significantly in extensor muscles at $180^{\circ}/sec$ in the combined exercise rehabilitation group than the control group over combined exercise treatment period.(p<.01). The variable of TNF-a was higher significantly in the combined exercise rehabilitation group than the control group as over combined exercise treatment period. Therefore, it can be concluded that a combined exercise rehabilitation program is effective in improving the lower limb muscular function and TNF-a in Patellofemoral Pain Syndrome patient.

Background: The performance of combined testing visual inspection with acetic acid (VIA) and cervical cytology tests might differ from one setting to another. The average estimate of the testing accuracy across studies is informative, but no meta-analysis has been carried out to assess this combined method. Objective: The objective of this study was to estimate the average sensitivity and specificity of the combined VIA and cervical cytology tests for the detection of cervical precancerous lesions. Materials and Methods: We conducted a systematic review and a meta-analysis, according to the Cochrane Handbook for Systematic Review of Diagnostic Test Accuracy. We considered two cases. In the either-positive result case, a positive result implies positivity in at least one of the tests. A negative result implies negativity in both tests. In the both-positive case, a positive result implies having both tests positive. Eligible studies were identified using Pubmed, Embase, Website of Science, CINHAL and COCRANE databases. True positive, false positive, false negative and true negative values were extracted. Estimates of sensitivity and specificity, positive and negative likelihood (LR) and diagnostic odds ratios (DOR) were pooled using a hierarchical random effect model. Hierarchical summary receiver operating characteristics (HSROC) were generated and heterogeneity was verified through covariates potentially influencing the diagnostic odds ratio. Findings: Nine studies fulfilled inclusion criteria and were included in the analysis. Pooled estimates of the sensitivities of the combined tests in either-positive and both-positive cases were 0.87 (95% CI: 0.83-0.90) and 0.38 (95% CI: 0.29-0.48), respectively. Corresponding specificities were 0.79 (95% CI: 0.63-0.89) and 0.98 (95% CI: 0.96-0.99) respectively. The DORs of the combined tests in either-positive or both-positive result cases were 27.7 (95% CI: 12.5-61.5) and 52 (95% CI: 22.1-122.2), respectively. When including only articles without partial verification bias and also a high-grade cervical intraepithelial neoplasia as a threshold of the disease, DOR of combined test in both-positive result cases remained the highest. However, DORs decreased to 12.1 (95% CI: 6.05-24.1) and 13.8 (95% CI: 7.92-23.9) in studies without partial verification bias for the combined tests in the either-positive and both-positive result cases, respectively. The screener, the place of study and the size of the population significantly influenced the DOR of combined tests in the both-positive result case in restriction analyses that considered only articles with CIN2+ as disease threshold. Conclusions: The combined test in the either-positive result case has a high sensitivity, but a low specificity. These results suggest that the combined test should be considered in developing countries as a primary screening test if facilities exist to confirm, through colposcopy and biopsy, a positive result.

The objective of this study was to analyze the characteristics of combined 1D/2D inundation simulation of riverside farmland using the Hydrologic Engineering Center - River Analysis System (HEC-RAS). We compared and analyzed inundation simulation results between 1D and combined 1D/2D hydraulic simulation using HEC-RAS. Calibration and validation of stream stage were performed using three rainfall events. The coefficient of determination ($R^2$) and root mean square error (RMSE) between simulated and observed stream stage were 0.935 - 0.957 and 0.250 m - 0.283 m in calibration and validation, respectively. The inundation area showed no significant difference in 1D and combined 1D/2D simulation ($8.48km^2$ in 1D simulation, $8.75km^2$ in combined 1D/2D simulation). The average inundation depth by 1D simulation was 1.4 m deeper than combined 1D/2D simulation. In the lower inundation depth, the inundation area by combined 1D/2D simulation was larger than inundation area by 1D simulation. As the inundation depth increased, the inundation area by 1D simulation became wider. In the case of the 1D/2D combined simulation, low elevation areas along the river bank were inundated widely. Compared to 1D/2D combined simulation, the flood radius in some sections was longer in 1D simulation. In the 1D analysis, because the low altitude riverside farmlands are also assumed to stream, it is calculated that riverside farmlands have the same stage as the mainstream when the stream is overflowed. Therefore, the inundation area seems to be overestimated in those sections. In other regions, the inundation areas tend to be broken depending on overflow by each stream cross-section. In the case of river flooding, the overflow is expected to flow to the lower area depending on the terrain, such as the results of the combined 1D/2D simulation. It is concluded that the results of combined 1D/2D inundation simulation reflected the topographical characteristics of low-lying farmland.
