Intestinal Lymphoma in a Halla Horse

  • A 21-year-old female Halla Horse weighing 248 kg was referred to the Jeju National University Equine Hospital with the chief complaint of anorexia accompanied by general weakness and depression for the previous three days suspected to be related to colic. Extensive diagnostic tests were performed for the following six days, including complete blood count (CBC), serum chemistry, gastroscopy, x-rays, and ultrasound imaging. The signalment, history, symptoms, and test results strongly suggested a chronic intestinal inflammatory disease with or without an alimentary tumor; hence, an exploratory laparotomy was performed. Almost the entire small intestine wall was severely thickened with diffuse ecchymosis on the serosa and protruded nodules on the mucosa. A presumptive diagnosis of an intestinal tumor was made, and the patient was euthanized, considering the patient's welfare with poor prognosis and low expectancy. A massive part of the small intestine was collected and submitted for macroscopic and microscopic pathology evaluations. The pathologic examination, including immunohistochemistry (IHC), indicated equine intestinal lymphoma showing strong positivity for T cell marker. This report describes the clinical signs, diagnosis and pathological features of intestinal lymphoma in a Halla Horse in detail.

Strangulating Large Colon Volvulus: A Diagnostic and Surgical Challenge in a Post-Partum Thoroughbred Mare

  • An 11-year-old thoroughbred post-partum broodmare presented with the symptoms of colic. The physical and clinical examinations indicated a large colon displacement or volvulus. Immediate surgical intervention was performed, and a strangulating large colon volvulus (LCV) was defined as a volvulus of >540° in the counterclockwise direction. After correcting and assessing the gut viability, approximately 80% of the entire large colon was resected and anastomosed using an end-to-end technique. With supportive care after surgery, the horse regained its appetite and vitality without significant clinical complications and was discharged on postoperative day 9. This report presents the first surgical correction using a large colon resection and anastomosis (LCRA) and the critical care for a strangulating LCV in a horse in Korea. This case enhances the current knowledge of clinical LCV and the related considerations for treatment.

Small intestinal epiploic foramen entrapment in a seven-month pregnant Thoroughbred mare: a case report

  • A seven-month pregnant four-year-old Thoroughbred mare with colic weighing 600 kg was referred to the Jeju National University Equine Hospital. A physical examination and ultrasound suggested epiploic foramen entrapment. The patient underwent ileum resection and end-to-side jejunocecostomy. After surgery, the horse showed a consistent pain when introduced to feeding. A repeat laparotomy was performed to enlarge the stoma size with side-to-side jejunocecostomy. The repeat laparotomy was successful, and the mare gave birth uneventfully. This report describes a case of small intestinal epiploic foramen entrapment and the clinical outcomes of the 2 consecutive laparotomies a Thoroughbred mare in late gestation.

고형물 삼킴장애로 내원한 환자 1례 (A Case of Metastatic Ampulla of Vater Cancer Achieving Cure)

  • We report a case with dysphagia for solids. A 51-year-old man with benign esophageal stricture was transferred for endoscopic treatment. He had lye ingestion history at 9 years old and underwent esophagectomy with right colonic interposition for the treatment of the benign esophageal stricture. But his symptom was acting up 2 years ago and lasted afterward even though he had underwent endoscopic treatments for dysphagia several times, including balloon dilation and stent insertion. He had polypoid enhancing wall thickening around anastomosis site of stomach with perigastric soft tissue density and suspicious nodular extension to omentum on the small bowel computed tomography. So he had a surgical resection of small bowel and jejunojejunostomy, and the pathological result was adenocarcinoma, intestinal type with soft tissue infiltration. Later he underwent total gastrectomy with segmental resection of interpositional colon and segmental resection of duodenum and ileo-colic anastomosis revision. And recently he has been on chemotherapy.

Computed Tomographic Features of Gallbladder Agenesis in a Golden Retriever

  • A two-year-old, intact female Golden Retriever had previously been diagnosed with a portosystemic shunt (PSS) during an ultrasonographic examination at a local animal hospital. The serum biochemistry revealed elevated liver enzymes and bile acid levels. The abdominal radiographic examination revealed mild serosal detail loss and microhepatica, while abdominal ultrasonography revealed mild ascites and high-velocity flow to the caudal vena cava (CVC) suspected as a PSS. The gallbladder was not observed within the hepatic parenchyma during ultrasonography. Computed tomography (CT) revealed an absent gallbladder and dilation of the common bile duct (CBD). Dilations of the gastroduodenal, splenic, colic and renal veins were also observed. A dilated left phrenico-abdominal vein that entered the CVC was previously misinterpreted as a PSS on the ultrasound examination. Based on the imaging examinations, the dog was diagnosed with congenital gallbladder agenesis associated CBD dilation.

Large Mesenteric Pedunculated Lipoma in a Horse

  • A seven-month pregnant 15-year-old Thoroughbred mare presented with acute abdominal pain to Jeju National University Equine Hospital. At presentation, a nasogastric intubation revealed 10 L of gastric reflux; rectal palpation and ultrasound revealed dilated loops and thickening of the walls of the small intestine. An exploratory laparotomy revealed strangulation of the small intestine due to a large abdominal mass. The mass was double-ligated and resected blindly due to the short pedicle. An enterectomy was not performed as intestinal motility was detected following the mass removal. Histopathological examination confirmed that the mass was a lipoma, measuring 24 cm × 16 cm × 16 cm in size. On day 8 post-surgery, the mare was discharged without complications. This case report describes the diagnosis and treatment of strangulation of the small intestine by a pedunculated lipoma, thus providing useful information on lipoma in horses.

Successful Removal of a Difficult Common Bile Duct Stone by Percutaneous Transcholecystic Cholangioscopy

  • Common bile duct (CBD) stones are prevalent in 11% to 21% of patients with gallstones and can cause various clinical manifestations, from biliary colic to biliary sepsis. The treatment of choice is endoscopic retrograde cholangiopancreatography, but approximately 5% to 10% of CBD stones are difficult to remove using these conventional endoscopic methods. Although percutaneous transhepatic cholangioscopy and lithotripsy can be used as an alternative, it can be technically demanding and risky if the intrahepatic duct is not dilated. We report a case of a large CBD stone that was successfully removed using percutaneous transcholecystic cholangioscopy.

Growth and Tolerance Assessment of a Lutein-fortified Infant Formula

  • Purpose: To evaluate safety, gastrointestinal tolerance, and growth of a new experimental starter formula (NESF) fortified with lutein, prebiotics, probiotics, nucleotides and beta-carotene, fed to infants within the first months of life. Methods: This was a non-randomized, open, uncontrolled study conducted from December 2010 to May 2011. Twenty-three healthy infants aged 10 days to 2 months old were enrolled. Outcomes included gastrointestinal tolerance, physical growth and safety. Prior to the initiation of the trial, the Scientific Research Institute of Nutrition of the Russian Academy of Medical Sciences confirmed that the NESF met all safety and nutritional parameters. Results: NESF was well tolerated. The majority of infants fed this formula passed semi-liquid, yellow or yellow-brown. The mean stool frequency/day was $2.5{\pm}0.4$ on study-day 14 and $1.8{\pm}0.5$ on study-day 28. The mean daily weight gain was $30.9{\pm}3.8$ grams and the mean length gain during the 28 days of follow up was $3.1{\pm}0.8cm$, corresponding to the average physical growth normally seen in the first months of life in Russian infants. Six children left the study: one refused to drink the formula, one left the study as parents changed residence; and one child's parents have recalled their informed consent due to adverse event unrelated to the product. Three infants presented adverse events possibly related to the product (rash; colic and abdominal pain; constipation). Seventeen infants completed the trial. Conclusion: This study demonstrated that lutein-fortified formula is safe, well-tolerated and supported physical growth of evaluated infants.

Effects of liposomal-curcumin on five opportunistic bacterial strains found in the equine hindgut - preliminary study

  • Background: The horse intestinal tract is sensitive and contains a highly complex microbial population. A shift in the microbial population can lead to various issues such as inflammation and colic. The use of nutraceuticals in the equine industry is on the rise and curcumin is thought to possess antimicrobial properties that may help to minimize the proliferation of opportunistic bacteria. Methods: Four cecally-cannulated horses were utilized to determine the optimal dose of liposomal-curcumin (LIPC) on reducing Streptococcus bovis/equinus complex (SBEC), Escherichia coli K-12, Escherichia coli general, Clostridium difficile, and Clostridium perfringens in the equine hindgut without adversely affecting cecal characteristics. In the first study cecal fluid was collected from each horse and composited for an in vitro, 24 h batch culture to examine LIPC at four different dosages (15, 20, 25, and 30 g) in a completely randomized design. A subsequent in vivo $4{\times}4$ Latin square design study was conducted to evaluate no LIPC (control, CON) or LIPC dosed at 15, 25, and 35 g per day (dosages determined from in vitro results) for 9 days on the efficacy of LIPC on selected bacterial strains, pH, and volatile fatty acids. Each period was 14 days with 9 d for acclimation and 5 d withdrawal period. Results: In the in vitro study dosage had no effect ($P{\geq}0.42$) on Clostridium strains, but as the dose increased SBEC concentrations increased (P = 0.001). Concentrations of the E. coli strain varied with dose. In vivo, LIPC's antimicrobial properties, at 15 g, significantly decreased (P = 0.02) SBEC when compared to 25 and 35 g dosages. C. perfringens decreased linearly (P = 0.03) as LIPC dose increased. Butyrate decreased linearly (P = 0.01) as LIPC dose increased. Conclusion: Further studies should be conducted with a longer dosing period to examine the antimicrobial properties of curcumin without adversely affecting cecal characteristics.

소아 추나에 대한 국내·외 연구 동향 (Domestic and Foreign Research Trend on the Pediatric Chuna Treatment)

  • Objectives The purpose of this review is to investigate the domestic and foreign studies of pediatric Chuna treatment and propose the directions of future studies and clinical applications. Methods We searched for the study at RISS, KISS, DBPIA, Pubmed, CNKI by keywords, '추나', 'Osteopathic', 'Chiropractic', 'Manipulation', '推拿', '導引', '按摩', After 2010. Results 1. Selected 3 domestic studies were categorized as 1 survey study and 2 case reports. Selected 41 foreign studies from Pubmed were categorized as 15 systemic reviews, 8 survey studies, 12 case reports and 6 control studies. Selected 82 foreign studies from CNKI were categorized as 10 systemic reviews, 22 case reports and 50 control studies. 2. 2 clinical domestic studies researched on idiopathic Scoliosis. The foreign clinical studies from Pubmed are 18 cases, and those studies were categorized into Premature baby care (3), Infant colic (2), ADHD (2), Congenital talipes equinovarus (1), Somatic dysfuntion (1), Nonsynostotic occipital plagiocephaly (1), Conversion disorder (1), Lower back pain (1), Chronic bilateral dorsal foot pain and stiffness (1), plantar fasciitis (1), Migraine headaches (1), Cyclic vomiting syndrome (1), Acute otitis media (1) and Cerebral palsy (1). The other 72 foreign clinical studies were from CNKI, and they studied 39 different diseases. Systematically, they studied about digestive diseases (25), respiratory diseases (20), fever (6), musculoskeletal diseases (5), nervous system diseases (5), dermatology diseases (2) and other disease states. The Chuna treatment was used in variety of studies. 3. 2 clinical domestic studies adopted techniques of Osteopathy Chuna. The foreign clinical studies from Pubmed adopted techniques of Osteopathic manipulation (10) and Chiropractic manipulation (8). The other foreign clinical studies from CNKI adopted techniques of Acupressure (69), Abdominal manipulation (23), Spinal manipulation (21), Thoracic manipulation (11), Traction manipulation (2), Muscular manipulation (2), Squeezing Sha manipulation (1), Spine correction (1), Joint manipulation (1) and Fascia manipulation (1). Conclusions In addition to musculoskeletal disorders, variety of pediatric diseases could be treated with Chuna treatment instead of acupuncture.
