Among the ancient ideas of East Asia, there is a myth about the symbol of the Four Gods who commands all directions in the east, south, west, and north, and this is also reflected in the medical field. However, although there is discussion about Qinglong Decoction (青龍湯), Baihu Decoction (白虎湯) and Xuanwu Decoction (玄武湯) in the 『Shanghan Lun (傷寒論)』, there is no content focusing on Zhuniao decoction (朱鳥湯). Considering the consistent perspective of 『Shanghan Lun』, which recognizes the disease-syndrome and seeks prescriptions based on yin and yang thinking, this is very unusual. Therefore, in this study, it was revealed that the concept of 'Sishen Decoction (四神湯)' itself was a concept that emerged when the authors of 『Shanghan Lun』 established the 'eight-principles syndrome differentiation (八綱辨證)' system in their recognition and response to diseases-syndromes. Based on this, I considered the following possibilities: They were able to present Qinglong decoction, Baihu Decoction and Xuanwu decoction, as appropriate prescriptions for 'exterior cold excess syndrome (表寒實證)', 'interior heat deficiency syndrome (裏熱虛證)', and 'interior cold deficiency syndrome (裏熱實證)'. However, it is possible that the name of the prescription 'Zhuniao decoction' was not intentionally used, because it was not possible to provide an appropriate prescription for 'exterior cold excess syndrome (表熱實證)'.
Objective: This study was designed to investigate the possibility of quantification of the diagnosis of abdominal coldness (AC) in patients with functional dyspepsia (FD). Methods: Forty-four patients with FD were enrolled in this study. Three Korean medicine doctors each randomly examined all abdomens. Diagnosis of AC was made by consensus of at least two of the doctors. Body temperature (oral by digital oral thermometer) and skin temperature (by digital infrared thermal imaging [DITI]) were measured, followed by administration of the Cold and Heat questionnaire (CHQ) and the Instrument of Pattern Identification for Functional Dyspepsia (IPIFD). Results: Of the 44 patients with FD, 22 were assigned to the AC group and 22 to the non-AC group. The concordance rate of diagnosis among the three doctors was 63.6% (28/44), with a ${\kappa}$ of 0.504, indicating means moderate agreement). Neither the oral nor the skin temperatures showed statistically significant differences between the AC and non-AC groups. However, the CHQ scores and 'Simultaneous Occurrence of Cold and Heat Syndromes pattern' scores of the IPIFD were higher in AC group and showed statistically significant differences (p=0.010 and 0.009). Conclusions: This is the first study conducting quantitative measurements of abdominal coldness in patients with FD. Although oral and skin temperature showed no statistical significance between AC and non-AC groups, the concordance rate of diagnosis of AC among the three Korean Medicine doctors was moderate. The CHQ scores and 'Simultaneous Occurrence of Cold and Heat Syndromes pattern' scores of the IPIFD also suggest that diagnosis of AC is relevant to cold and heat patterns, and these questionnaires could be utilized as supportive data for the diagnosis of AC. Further studies should be conducted for the purpose of quantifying and standardizing abdominal examinations in Korean Medicine.

1. Objectives: The purpose of this study was to survey the Original symptoms(素證) of Soyangin's and to compare the tendency of their symptoms between Soyangin Heat-related diarrhea accompanied by headache(身熱頭痛亡陰證) and Soyangin Cold-related diarrhea accompanied by abdominal pain(身寒腹痛亡陰證). 2. Methods: We recruited 56 Soyangin subjects in the Korean Medicine clinic and researched their original symptoms. Subjects were diagnosed by the Sasang Constitutional specialists and Sasang Constitutional Medication. Subjects were two groups, confirmed to Soyangin Heat-related diarrhea accompanied by headache and Soyangin Cold-related diarrhea accompanied by abdominal pain. All subjects answered the questionnaire about their Original symptoms. We found the differences between 2 groups using the Independent-Samples T-test. Analysis was conducted using SPSS 12.0 for Korean. 3. Results and Conclusions: In Original symptoms, Tendency of overeating(p=0.01), Condition of digestion(p=0.04), Perspiration after exercise (p=0.02), Usual perspiration(p=0.00), Tiredness after perspiration(p=0.05) Cold intolerance on extremities(p=0.04), General edema(p=0.01) etc. are different between 2 groups. We found the differences of heat and cold in symptoms between two Soyangin syndromes.

"Byun Jeung Rok(辨證錄)" written by Jinsatak(陳士鐸) is composed of 14 volumes. In relation to the contents, it is organized into 126 gates(門) and 770 remaining syndromes(餘證) where internal medicine, external medicine, pediatrics, gynecology(內科 外科 小兒 婦人), etc. are divided into sub-sections of cold damage, cold stroke, wind stroke(傷寒 中寒 中風), etc. He explained the symptom, cause of disease, method of treatment, prescription, construction of prescription, instruction of medicine and prognosis(症狀 病因 治法 處方 處方構成 服用法 預後) thoroughly. This study, as an inquiry of the sec 14 volume pediatrics(小兒), deals with febrile fit cacotrophy vomiting diarrhea(驚疳吐瀉), coprozoic parasite(便蟲), smallpox(痘瘡), eruption(疹症), eating mud(喫泥), fetal toxin(胎毒). It was written very logically so it is easy to understand. The analysis of the symptoms are brief and appropriate. Therefore, it is considered to have significant clinical value for future generations and is applied by them. Finally, this topic was studied in hopes of helping later pediatrics clinical treatment.

Seop-Gye divided up the constitution's strength and weakness, and so he cured disease by the 'cold and heat'. 'deficiency and excessiveness' means on the basis of the YINㆍYANG of constitution. He classified constitution with pathogenesis grounding in body formal distinction, flesh hardness and softness, face color and form, skin color. Therefore he grasped the rule of constitution connected with physiological function and pathological distinction, applied the rule of constitution in the diagnosis and cure of the chronic & epidemic febrile disease.
Stellate ganglion block has been used to treat diseases such as peripheral vascular disease, sympathetic dystrophy, and various pain syndromes involving the head or arm. Raynaud's disease is a syndrome manifested by attacks of pallor, cyanosis, numbness and pain of the digits in response to cold or emotional change. I report one case who was given Stellate ganglion block using 18G teflon Catheter(4.5 cm in length) for Raynaud's disease. Continuous stellate ganglion block is more convinient to inpatient than repeated needle punctures and may reduce major complications and more useful to patient who needs continuous sympathetic block about one week duration.

Objectives: Bi Jeung (비증) is a pain syndrome derivative of pathological patterns of East Asian medicine. The purpose of this study was to develop a self-rated questionnaire for Bi Jeung. Methods: Twelve questionnaire items for Bi Jeung (3 items for the Wind Bi, 5 items for the Cold Bi, and 4 items for the Dampness Bi) were extracted through the literature review. These items were presented to the 18 Korean medical doctors who specialized in pain medicine to conduct two sessions of the Delphi method. The Korean medical doctors were asked to rate the importance of each item for the corresponding Bi syndromes, using a Likert 7-point scale, and were asked to propose which item should be additionally included to increase determinant power to each Bi syndrome. We determined the 4 points of the importance as a cut-off point of each item. Results: Through two sessions of the Delphi method, two items were deleted because their mean values of the importance were below 4 point. Korean medical doctors proposed to add three items for the Bi Jeung. However, the mean values of only two among the three items were over 4 points, so finally 12 items (3 items for the Wind Bi, 5 items for the Cold Bi, and 4 items for the Damp Bi) were determined for the Bi Jeung questionnaire. Conclusions: It is concluded that a 12 item-Bi Jeung questionnaire comprising the three subcategories of the Bi Jeung (Wind, Cold, and Dampness Bis) possesses contents validity through literature review and Delphi methods.
Sometimes, spinal cord injury (SCI) results in various chronic neuropathic pain syndromes that occur diffusely below the level of the injury. It has been reported that behavioral signs of neuropathic pain are expressed in the animal models of contusive SCI. However, the observation period is relatively short considering the natural course of pain in human SCI patients. Therefore, this study was undertaken to examine the time course of mechanical and cold allodynia in the hindpaw after a spinal cord contusion in rats for a long period of time (30 weeks). The hindpaw withdrawal threshold to mechanical stimulation was applied to the plantar surface of the hindpaw, and the withdrawal frequency to the application of acetone was measured before and after a spinal contusion. The spinal cord contusion was produced by dropping a 10 g weight from a 6.25 and 12.5 mm height using a NYU impactor. After the injury, rats showed a decreased withdrawal threshold to von Frey stimulation, indicating the development of mechanical allodynia which persisted for 30 weeks. The withdrawal threshold between the two experimental groups was similar. The response frequencies to acetone increased after the SCI, but they were developed slowly. Cold allodynia persisted for 30 weeks in 12.5 mm group. The sham animals did not show any significant behavioral changes. These results provide behavioral evidence to indicate that the below-level pain was well developed and maintained in the contusion model for a long time, suggesting a model suitable for pain research, especially in the late stage of SCI or for long term effects of analgesic intervention.
