소양인(少陽人) 신열두통망음증(身熱頭痛亡陰證)과 신한복통망음증(身寒腹痛亡陰證)의 소증(素證) 특성에 관한 단면연구 (The Cross-sectional Study on Characteristics between Soyangin Heat-related Diarrhea Accompanied by Headache and Soyangin Cold-related Diarrhea Accompanied by Abdominal Pain)

  • 1. Objectives: The purpose of this study was to survey the Original symptoms(素證) of Soyangin's and to compare the tendency of their symptoms between Soyangin Heat-related diarrhea accompanied by headache(身熱頭痛亡陰證) and Soyangin Cold-related diarrhea accompanied by abdominal pain(身寒腹痛亡陰證). 2. Methods: We recruited 56 Soyangin subjects in the Korean Medicine clinic and researched their original symptoms. Subjects were diagnosed by the Sasang Constitutional specialists and Sasang Constitutional Medication. Subjects were two groups, confirmed to Soyangin Heat-related diarrhea accompanied by headache and Soyangin Cold-related diarrhea accompanied by abdominal pain. All subjects answered the questionnaire about their Original symptoms. We found the differences between 2 groups using the Independent-Samples T-test. Analysis was conducted using SPSS 12.0 for Korean. 3. Results and Conclusions: In Original symptoms, Tendency of overeating(p=0.01), Condition of digestion(p=0.04), Perspiration after exercise (p=0.02), Usual perspiration(p=0.00), Tiredness after perspiration(p=0.05) Cold intolerance on extremities(p=0.04), General edema(p=0.01) etc. are different between 2 groups. We found the differences of heat and cold in symptoms between two Soyangin syndromes.

  • In order to research the pathological entity of Jueyin disease in ${\ll}Shanghanlun{\gg}$, some sharing concept of three-yin-three-yang used in ${\ll}Neijing{\gg}$ and ${\ll}Shanghanlun{\gg}$ were investigated first, and then the meaning of jueyin and jueyin disease were analyzed. In cold damage disease, time-space factor is important because the pathological change is rapid and the symptoms along path are similar, therefore three-yin-three-yang having complex meaning of time and space can be used as an appropriate pathological concept. So to speak, it is able to be interpreted as various modes like variations of yin-yang, qi-blood, change of pulse condition, theories of opening, closing, pivot or exuberance and debilitation of form and qi manifested in the six districts of the human body following disease process. Jueyin is between front taiyin and rear shaoyin, and it's attribution is inherent in qi stagnation and yin exuberance in relative to the location of flank and liver. Putting together above descriptions, pathological entity of jueyin disease is that the symptoms mingled with cold and stagnant heat competing each other when a subject having qi stagnation in flank with cold in extremities and lower abdomen in particular is seized with cold influenza.

Cold Hypersensitivity and Sleep Quality: A Cross-sectional Community-based Study

  • Background: Cold hypersensitivity (CH) and sleep disorders are not limited to patients with specific diseases, as they are also commonly observed in the general population. The objective of this study was to investigate the relationship between CH and sleep quality. Methods: This cross-sectional study analyzed data from a cohort of 5,052 individuals living in South Korea. Based on the results of a questionnaire on CH, the participants were classified into four groups: non-CH, CH in the hands (CHH), CH in the feet (CHF), and CH in the hands and feet (CHHF). Sleep quality was evaluated using the Pittsburgh Sleep Quality Index (PSQI). Multiple regression was used for groupwise comparisons of total PSQI scores and the scores for each component, and logistic regression was used to find the odds ratios of being a "poor sleeper" (PSQI≥8.5). Results: The CHF and CHHF groups showed significantly higher scores than the non-CH group on the overall PSQI as well as on the following sleep components: subjective sleep quality, sleep latency, habitual sleep efficiency, sleep disturbance, and sleep medication use. The odds ratios for being a "poor sleeper" were significant in the CHF (1.84; CI=1.38-2.43) and CHHF groups (1.54; CI=1.25-1.89), but the CHH group did not show any difference from the non-CH group. We identified a correlation between CH in the feet and poor sleep quality. Conclusions: These findings help to elucidate the relationship between sleep and CH and highlight the need to clarify the underlying mechanism.

사상체질에 따른 과체중 및 비만 환자의 소증(素證) 특성 비교 (Study on the Characteristics of Ordinary Symptoms in Overweight and Obesity Patients according to Sasang Constitution)

  • Objectives: This study was aimed to find out the characteristics of ordinary symptoms between the control and patient groups, diagnosed as overweight or obesity, in Soyang, Taeeum, and Soeum Constitutions and among those 3 groups. Methods: As a prospective cross-sectional study based on medical records, 9213 patients, who visited Kyung Hee University Medical Center from May in 2007 to June in 2010, were included. To diagnose the constitution, Revised Questionnaire for the Sasang Constitution Classification II and examinations by oriental medical doctors who majored in Sasang Constitution Medicine were performed. Based on the World Health Organization criteria for Asian obesity assessment, body mass index (BMI) was classified into 3 groups; more than 25 as obesity, 23 to 25 as overweight, and less than 23 as normal. Ordinary symptoms questionnaire, which consists of 46 questions in 8 categories, were given to the subjects. One-way ANOVA test in continuous variables and chi-square test in categorical variables were carried out to analyze statistical significance. Results and Conclusions: 1) In general characteristics, we could find out the significant differences in age, sex, height, weight, BMI, waist circumference, blood pressure, total cholesterol, triglyceride, low density lipoprotein cholesterol, high density lipoprotein cholesterol, fasting blood sugar, and hemoglobin A1c among the 3 Constitutional groups. 2) Soyang Constitution of overweight and obesity has tendency to have higher appetite, like cool or cold water, defecate more, swear more, dislike heat, and have warmer or hotter extremities in the ordinary symptoms, compared with the control group. 3) Taeeum Constitution of overweight and obesity has tendency to have higher appetite, like cool or cold water, defecate more, swear more, dislike heat, and have warmer or hotter extremities in the ordinary symptoms, compared with the control group. 4) Soeum Constitution of overweight and obesity has tendency to have higher appetite and less fatigue in the ordinary symptoms, compared with the control group. 5) Soeum Constitution showed the slightest increase in appetite, preference about room temperature water, less number of feces, and feeling of chill and warmer extremities in ordinary symptoms, compared with Soyang and Taeeum Constitutions. Taeeum Constitution showed the distinct increase in appetite and sweating in ordinary symptoms, compared with Soyang and Soeum Constitutions.

수부냉증 진단을 위한 조갑모세혈관 혈류속도 측정 예비연구 (The Role of Nail-fold Capillary Blood Velocity in Evaluating Cold Hypersensitivity in the Hands: A Pilot Study)

  • This study aims to compare nail-fold capillary blood velocity (CBV) according to the presence or absence of cold hypersensitivity in the hands (CHH) using nail-fold capillaroscopy under cold stress conditions. Eleven participants were taken from the cohort study for observing long-term health status of the faculty of Semyung university. Seven of the participants displayed CHH while four showed signs of non-CHH. CBV between 0 and 60 seconds (S1) and between 240 and 300 seconds (S2) was measured under cold stress for 5 minutes, respectively, and the average value was used to compare non-CHH group and CHH group. Body mass index, Nogung (PC8) temperature and temperature difference between Nogung (PC8) and Hyeopbaek (LU4) were significantly lower in CHH group compared to non-CHH group. The S2-S1 CBV difference was negatively correlated with PC8 temperature and PC8-LU4 temperature, and positively correlated with cold pattern score and cold hypersensitivity visual analog scale. The CHH group had a lower CBV overall than non-CHH group, and the S2-S1 CBV difference was significantly higher in the CHH group than in non-CHH group. This study suggests that CBV measured by nail-fold capillarosopy might be a useful indicator of cold hypersensitivity properties.

환경온도 -10℃에서 Clo값에 따른 인체 생리반응 및 주관적 감각 (Physiological Responses and Subjective Sensations by Clo Values at -10℃)

  • The purpose of this study was to review physiological responses and subjective sensations in the cold environment when the subjects wore ensemble with different clo values. Seven healthy male subjects participated in this experiment. This experiment was conducted in a climatic chamber with $-10^sC$ and 50%RH. Subjects wore five different kinds of ensemble[C1 (4.453 clo), C2 (3.452 clo), C3 (2.865 clo), C4 (2.387 clo), and C5 (2.280 clo)]. The experiment was composed of 20 min of rest period, 20min of treadmill exercise(6 km/h) period, 30 min of recovery period. We monitored skin temperature on 7 sites, clothing microclimate and subjective sensations. The clo value had positive correlations with mean skin temperature and clothing microclimate. The subjects feel more warm and humid as the clo value goes up. The subjects reported comfort when they wore C1 and C2 ensemble having over 3 clo value. However, they felt less comfortable during the exercise period since there was high humidity. Skin temperature on the extremities were more dramatically changed by the exercise rather than clo value. Thus it seems that in the cold environment, heat balance can mostly be controlled by the choice of clothing, and the clothes with high clo values can provide higher insulation. In conclusion, our findings suggest that it would be more effective to control clo value depending on the activity level for maintaining comfort level in the cold environment.

사상체질에 따른 대사증후군과 수족냉증 분포 차이와 역상관관계 (Inverse Association and Differences in the Distribution of Metabolic Syndrome and Cold Hypersensitivity in the Hands and Feet According to Sasang Constitution)

  • Objectives This study aimed to examine the differences in the distribution of metabolic syndrome(MetS) and cold hypersensitivity in the hands and feet(CHHF) according to Sasang constitution, and to determine whether CHHF and MetS have an inverse association. Methods MetS and its components, CHHF, Sasang constitution data from 1,998 participants in the Korean medicine Daejeon Citizen Cohort study(KDCC) were obtained. The participants were divided into a non-CHHF(n = 1,270, 63.6%), intermediate(n = 220, 11.0%) and CHHF(n = 508, 25.4%) group according to the thermal sensitivity questionnaire. Sasang constitution was diagnosed by Korea Sasang Constitutional Diagnostic Questionnaire(KS-15). One-way ANOVA and the chi-square test were used for participants' general characteristics and thermal sensitivity and MetS related factors. ANCOVA and logistic regression were used to compare the differences and the odds ratios(ORs) for MetS and its components. Results The MetS and CHHF prevalence rates of the Taeeumin, Soeumin, and Soyangin were 27.6%, 3.8%, 7.7%, and 18.3%, 42.3%, 26.4% respectively. The ANCOVA for MetS components showed that the waist circumference was significantly lower in the CHHF group as compared to the non-CHHF group in total and Soyangin. The logistic regression for MetS prevalence showed that CHHF had a significant inverse association in total(OR = 0.611) and Taeeumin(OR = 0.521). Conclusions The MetS prevalence had the highest in Taeeumin, followed by Soyangin and Soeumin, while the prevalence of CHHF was highest in Soeumin, followed by Soyangin and Taeeumin. In addition, it was confirmed that CHHF and MetS had an inverse association independently.

  • 'On the Morbid Pulse of Abdominal Fullness, Cold Mounting, and Abiding Food' in the Chapter 10 of Synopsis of Golden Chamber that enunciates Abdominal Fullness, Cold Mounting, and Abiding Food is related to the stomach and intestines respectively, and is similar to the region and symptom of disease in the light of both abdominal fullness and symptoms of pain. This chapter was united into one because the formula mentioned in this chapter can be applied to three disease patterns. Abdominal fullness shows the symptoms of distention and unease, but judging from the text as well as a specific formula and symptoms, it follows pain. Accordingly Abdominal fullness dealt with in this chapter is the first consideration and it is a kind of a disease pattern attendant on abdominal pain. Cold Mounting does not mean mounting gi disease but means the abdominal pain. The cause of cold mounting is mainly due to insufficiency of yang gi and oversufficiency of cold evil. And the main symptoms of cold mounting follow the severe pain around the naval and sweating, cold of the extremities, pulse deep and tight. Abiding food is of the same meaning such as damage of food today. Abiding food is now referred to as damage of food. Principles which have set forth in this chapter are put to use of the method of ejection in case that abiding food places in the upper part, precipitation in the lower part. The symptoms of abiding food show that the pulse is slight and slippery and the wrist pulse is both superficial and large and rough in applying the pressure, and the cubit pulse is also slight and superficial as well, and that have diarrhea and have little appetite.

사상(四象) 체질별(體質別) 약재(藥材) 분류(分類) 관한 연구(硏究)(태음인(太陰人) 및 태양인편(太陽人編) (A Research on the Classification of Herbal Medicines based on the Sasang Constitution (Taeumin and Taeyangin Part))

  • We analyzed 44 and 16 types of herbal medicines for Taeumin and Taeyangin, clinically applied in Dongyisoosebowon. In order to discover the standard of herbal classification for the Sasang constitutions, four concepts of Sasang Medicine were applied. These included 'Sadangron (theory of four groups)', 'Seungganggaehap (ascending-descending and gathering-dispersing)', 'Pyorihanyoul (exterior-interior and hot-cold)',and 'Hyungchiaekmi (fragrance, smell, bodily fluid and taste)'. According to these analyzing methods of herbal properties, we have reached the following conclusions: Herbal medicines for 'Taeumin' are characterized by opening energetics that reinforce dispersing Qi. The 'Exterior cold disease' is treated with herbs that ventilate the lungs and disperse dampness in three ways: by moistening the lungs, by releasing exterior, and by resolving dampness. The 'Interior heat disease' is treated with herbs that clear the liver and disperse heat in two ways. One by clearing liver heat through dispersing damp-heat of the small intestine and the other by opening orifices. Herbal medicines for Taeyangin are characterized by those that gather energy inward. They include herbs that treat beriberi, dystrophy of the extremities and vomiting, as well as, fish, shellfish, fruits and vegetable.

소양인처방(少陽人處方) 활용(活用)에 대한 임상적(臨床的) 연구(硏究) (A Clinical Study on Soyangin Prescription)

  • 1. Objectives Due to a lack of detail guidelines for prescriptions to ordinary and pathological symptoms in ${\ulcorner}$DongyiSuseBowon${\lrcorner}$, doctors depend on their own decision when give a prescription. This study aimed to compare ordinary symptoms and pathological symptoms in regards to six prescriptions being applied to Soyangin to utilize them as guidelines in practices. 2. Methods Date were collected from September, 2003 to August, 2004 at the Sasang Constitutional clinic of Pohang Oriental Medicine Hospital. Patients were first asked to fill Out a questionnaire of symptoms that they had at the time of visiting. Data from 273 Soyangin patients whose chief complains got alleviated after taking the prescribed drug were analyzed to examine if there were differences in prescriptions according to ordinary or pathological symptoms complained. 3. Results and Conclusions 1. Hyungbangpaedog-san(Paedog) was given more to the patients having good digestive function than Hyungbangjihwang-tang(Hyungji) and Hyungbangdojeok-san (Dojeok) ; Dojeok was given for bloating symptom compared to Hyungbangsabaek-san(Sabaek). Yangkyuksanhwa-tang(Yangkyuk) and Dojeok were more like for patients with stomachache compared to Paedog. Doghwaljihwang-tang(Dogji) was prescribed to patients complaining frequent voiding compared to Sabaek while Sabaek was given for patients who prefer cold water and feel their body warm compared to other prescriptions. Yangkyuk and Dojeok were prescribed more likely to patients with feeling of suffocation than Sabaek. 2. In symptom comparisons between one prescription and the group of the rest of 5 prescriptions, Paedog was found to be prescribed when patients had good digestive function while Dojeok was given in symptoms cases of indigestion, bloating, constipation and dizziness. Sabaek was applied more for patients having tendency of preferring cold water and feeling of heat body as well as good digestive function. Hyungji was used for patients complaining a symptom of fatigue, in addition Yangkyuk was given to patients who were nervous, sensitive, or impatient when compared to Other prescriptions. Dogji was applied to frequent voiding, insomnia, tingling of extremities, backache or leg pain, or in case of tendency of catching a common cold.
